2.Effectiveness of filling ex vivo mandibular central incisor using bypassing root canal preparation with two kinds of warm gutter fillings and cold lateral condensation technique

BACKGROUND: The filling compactness can not be guaranteed due to apparatus retention in traditional lateral condensation.The warm gutter filling can achieve good results because of the easy changeable of gutta-percha in softening states.OBJECTIVE: To compare the warm gutta-percha fillings (Obtura Ⅱ and BeeFill 2 in1 filling) and cold lateral compacted gutta-percha with the bypassing root canal preparation in the intracanal separated instrument of oval canal.METHODS: Firstlythe 60 oval root canals of fiat mandibular central incisors were randomly divided into three groups, with 20 teeth in each group. After that, the root canal was filled with BeeFill 2 in1, Obtura Ⅱ condensation, and lateral condensation.The percentage of void at 5-mm to root tip versus cress section area was observed under a scanning electron microscope.RESULTS AND CONCLUSION: Under in vitro condition, the difference of percentage of void area versus root canal area between the Obtura Ⅱ condensation group and the lateral condensation group had significance (P=0.013), and the similar difference could be found between the BeeFill 2 in1 condensation group and the lateral condensation group (P=0.014), but the difference had no significance between the BeeFill 2 in1 condensation group and the Obtura Ⅱ condensation group (P=0.088).The results revealed that the root canal can be sealed accurately when establish hypass root next to the separated instrument and filled with BeeFill 2 in1 warm gutter filling.
3.Correlation between motor function and activity of daily living in hemiplegic patient

BACKGROUND: The recovery of activity of daily living (ADL) in hemiplegic patient is related to the recovery of motor function to certain extent. But the correlation between two aspects has not been determined yet over the past study.OBJECTIVE: To probe into the correlation between the motor function of upper and lower limbs and ADL of hemiplegic patients after cerebral apoplexy.DESIGN: Case analysis.SETTING: Department of Rehabilitation of First Affiliated Hospital of Nanjing Medical University.PARTICIPANTS: Thirty cases of hemiplegia of cerebral apoplexy were selected from inpatients of Department of Rehabilitation of Jiangsu People's Hospital from August 2002 to June 2003, of which, 20 cases were male and 10 cases were female, aged varied from 39 to 76 years.METHODS: Simplified Fugl-Meyer assessment of motor recovery (FMA)was used to evaluate the motor function of patient. Modified Barthel index assessment was used to evaluate ADL.MAIN OUTCOME MEASURES:① Simplified Fugl-Meyer assessment and modified Barthel index assessment.② Correlation between the motor function and ADL of hemiplegic patients after cerebral apoplexy.RESULTS: All of 30 cases entered result analysis.①The average result of general FMA was 40.96±32.04.The average result of motor function of upper limb was 21.78±22.87 and of low limb was 18.43±10.29.The average result of modified Barthel index assessment was 39.46±25.94. ② The correlative coefficient of general FMA and modified Barthel index assessment was 0.65, in which, the correlative coefficient of upper limb assessment and modified Barthel index assessment was 0.62 and that of lower limb assessment was 0.61.CONCLUSION: A certain correlation presents between motor function and ADL in hemiplegic patient and the influences of upper and lower limb motor functions are almost same to daily life activity.
4.THE EFFERENT PROJECTIONS OF THE ROSTRAL VENTROLATERAL MEDULLA TO THE HYPOTHALAMIC PARAVENTRICULAR NUCLEUS AND THE THORACIC CORD IN THE CAT

The efferent projections of the rostral ventrolateral medulla(RVL) to the hypothalamic paraventricular nucleus(Pa) and the thoracic cord were studied in the adult cat by using WGA-HRP or fluorescent retrograde tract-tracing method. After injection of WGA-HRP or fluorescent tracer Fast blue(FB) into one side of the Pa, retrogradely labelled cells were found in bilateral RVL, with an ipsilateral predominance. The labelled cells decreased in number from the caudal to the rostral level. After injection of FB into one side of the thoracic cord at T_2-T_3 segments, retrogradely labelled cells in the RVL were observed which increased in number from the caudal to the rostral level and reached the peak at 1.0-1.5mm caudal to the trapezoid body. Most of these cells were distributed in the ipsilateral RVL, and clustered in the region 0.0-1.0mm from the ventral surface of the medulla. After Diamino yellow 2HC1 and FB were injected into the Pa and the thoracic cord respectively, only single labelled cells were detected in the RVL, no double labelled cells were found. The above results suggest that the Pa and the thoracic cord receive separate fiber projections from different cells of the RVL.
5.The mechanisms of carbapenem resistance inKlebsiella pneumoniae

Klebsiella pneumoniae is well known as a causative agent of both community and nosocomial infections and is generally believed to be the cause of 10% of the nosocomial infection. Recently, with the widespread using of carbapenem, the isolation of carbapenem-resistant strains has been greatly increased, which bring great dififculties and challenges in clinical treat-ment. In this article, the progresses in the mechanisms of carbapenem, resistance inKlebsiella pneumoniae such as the acquisition of carbapenemases, hyperproduction of AmpC cephalosporinases or extended spectrum beta-lactamases (ESBLs) in combination with loss of the outer membrane protein, eflfux pump system, and bioiflm were reviewed.
6.Clinical effect of acyclovir combined with prednisone in treatment of herpes zoster

Objective To observe the clinical efficacy of acyclovir combined with prednisone in the treatment of herpes zoster.Methods 128 patients with herpes zoster were selected as study objects,and they were divided into the treatment group and the control group by using the method of complete random distribution,64 cases in each group.The patients in the treatment group were treated with oral acyclovir and prednisone,while those in the control group received oral acyclovir alone.The clinical efficacy of the two groups was observed.Results The blistering,crusting and healing time had no significant differences between the two groups(t=1.76,1.79,1.41,all P>0.05).The analgesic time of the treatment group was (3.2±1.2)d,which was significantly shorter than (7.4±2.6)d of the control group (t=11.73,P<0.01).The total effective rate of the treatment group was 95.3%,which was significantly higher than 73.4% of the control group,the difference was significant(χ2=16.13,P<0.01).In the treatment group,there were 2 cases of postherpetic neuralgia.In the control group,8 cases had postherpetic neuralgia.The incidence rate of adverse events in the treatment group was significantly lower than that in the control group(χ2=6.01,P<0.05).Conclusion Acyclovir combined with prednisone in the treatment of herpes zoster has significant clinical efficacy,it can shorten the analgesic time,and is worthy of clinical application.
7.Value of NT-proBNP in predicting severity of coronary artery disease and prognosis in acute coronary syndrome

Objective To assess the in-hospital prognostic value of NT-proBNP in patients with acute coronary syndrome (ACS) and its relation to the severity and prognosis of coronary artery disease .Methods Totally 132 consecutive patients with ACS in Affiliated Hospital of Nantong University from January 2013 to December 2014 were collected in this study ,in which 64 pa-tients with unstable angina(UA) ,46 patients with non-ST segment elevation myocardial infarction(NSTEMI) ,and 22 patients with ST segment elevation myocardial infarction(STEMI) .Patients were divided into two high NT-proBNP group(NT-proBNP ≥ 474 pg/mL) and low NT-proBNP group(NT-proBNP < 474 pg/mL ) .Ejection fraction were detected and compared between the two groups ,coronary artery disease were detected by coronary angiography .cTnI ,CK-MB ,CRP and NT-proBNP were detected by sero-logical ,and the average length of hospital stay and heart failure ,cardiac shock incidence were calculated .Results There was a sig-nificant negative correlation between NT-proBNP and ejection fraction .Incidence of heart failure and duration of hospital stay in high NT-proBNP group were significantly higher than in low NT-proBNP group ;The number of coronary vessels affected ,severity of stenosis and proximal left anterior descending artery(LAD) disease in high NT-proBNP group were higher than in low NT-proB-NP group ;TIMI flow grade was significantly lower in high NT-proBNP group than in low NT-proBNP group ,the difference was statistically significant(P< 0 .05) .Conclusion NT-proBNP is a valuable marker for predicting prognosis and severity of coronary artery disease in patients with acute coronary syndrome .
8.Revision to origin of northern Artemisia argyi in Compendium of Materia Medica (Bencao gangmu).

The origin of northern Artemisia argyi recorded in Compendium of Materia Medica(Bencao gangmu) is Fudao(Chinese characters) in Tangyin county, While there is only Fudao(Chinese characters) instead of Fudao(Chinese characters). Whether indeed Fudao(Chinese characters) is Fudao(Chinese characters)? By reviewing the genuine evolution of A. argyi, doing textual research on Fudao(Chinese characters) and combing with field survey data of national census of Chinese Materia Medica resources, this paper concluded that the word Fudao(Chinese characters) firstly emerged in Figure Canon of Chinese Materia Medica(Bencao tujing) of Susong in Song dynasty and was applied in later generations, but the implication was not clear, then emerged both Tangyin and Fudao(Chinese characters) in Compendium of Materia Medica(Bencao gangmu). The place Fudao(Chinese characters) is one of the graves of Bianque, that existed from Shang and Zhou dynasty and never changed until now, the A. argyi of Tangyin was famous from the grave of Bianque in Fudao(Chinese characters), which could infer that Lishizhen considered Fudao (Chinese characters) was Fudao(Chinese characters) indeed, and the origin of northern A. argyi was Fudao(Chinese characters) in Tangyin county.
