1.The Forensic determination of Post-Traumatic Epilepsy in 21 Patients

To evaluate the forensic determination of post cerebral traumatic epilepsy. Methods In 21 patients, traumatic history and previous history were analysied combined with the demonstrations of electroencephalogram(EEG), X-ray, CT and MRI. Results Post-traumatic epilepsy,manily in late stage, usually occurred following serious cerebral trauma. The type of traumatic epilepsy was determined by the traumatic location and extent. Abnormal epileptic wave in scalp EEG and 24h dynamic EEG and medical image examinations were helpful for qualitative analysis. Conclusion The forensic determination of post traumatic epilepsy must be on the basis of traumatic and previous history combined with EEG, CT and MRI analysis.

4.Effect of prone-position ventilation on oxygenation in patients with acute respiratory distress syndrome

Objective To determine the effect of prone position (PP) ventilation on oxygenation in patients with acute respiratory distress syndrome (ARDS) .Methods Twenty-three patients of both sexes (14 males, 9 females) with ARDS (within 72 h after onset of ARDS) were included in this study. The patients ranged in age from 43-78 years. Ten minutes before the patients were placed in prone position, midazolam 2-5 mg was given i. v. followed by continuous infusion at 2-3 mg?h-1 for sedation and a bolus of vecuronium 4 mg per hour for muscle relaxation. The patients were mechanically ventilated (VT 6-8 ml?kg-1, inspiration-time 1.0-1.2 s, RR 12-20 bpm, FiO2 0.4-1.0, PEEP 6-18 cm H2O) . The patients were placed in prone position for 2 h. Radial artery was cannulated and S-G catheter placed. MAP, CVP, MPAP, PAWP, cardiac output, PaO2 , PaCO2 peak inspiratory pressure (PIP) and airway resistance (Raw) were measured and recorded immediately before (T0) and 0.5, 2 h after the patients were placed in prone position (T1 , T2) and 2 h after supine position was resumed(T3) . Objective To explore the role of halofuginone in immune regulation,observe the effects of halofuginone on maturation of dendritic cells and immune tolerance during homograft heart transplantation of rats,in order to provide new ways and means to treat transplantation tolerance and to explore its possible mechanism.Methods Rat bone marrow derived dendritic cells were isolated and the effects of halofuginone in LPS-treated DCs were investigated by flow cytometry;heart transplantation model of rats were established and the effect of halofuginone stimulated dendritic cells in the immune tolerance by pathological analysis and mixed lymphocyte reaction were observed.Results Flow cytometry results show that the expression of cell surface CD80 CD86,OX62 antigens were not high in control group,their expression increased significantly when stimulated by LPS for 24h,and decreased when stimulated by both Changshan ketone and LPS for 24h,and even lower than their expression levels in blank group,which showed that Changshan ketone has the ability to inhibit the maturation of dendritic cells induced by LPS.In rat heart transplantation model,dendritic cells stimulated with Changshan were injected into recipient rats in vivo,and receptor of rat transplantation rejection decreased significantly,which showed that dendritic cells stimulated with Changshan ketone may be used as a new method for cell therapy in treatment of transplant rejection.Conclusion This study systematically observes that halofuginone can inhibit the maturation of rat bone marrow-derived dendritic cells and lymphocyte proliferation for the first time in vivo and in vitro.
6.Surveillance for Distribution and Resistance Rate of Clinical Strains from Neonatal Sepsis

OBJECTIVE To investigate the distribution and resistance rate of clinical isolates from neonatal sepsis in Hubei hospitals.METHODS The antimicrobial susceptibility of clinical isolates from(neonates) blood culture derived from sixteen hospitals in Hubei from 2000 to 2004,was retrospectively analyzed and tested by KirbyBauer(method).RESULTS A total of 1 019 strains were isolated from blood specimens,Gram-positive and Gram(-negative)(bacteria) accounted for 93% and 7% of these isolates,respectively.The main isolates were coagulase(-negative) staphylococci(CNS,79.8%),Enterococcus spp(7.0%),Staphylococcus aureus(SAU4.5%),Klebsiella spp(1.7%),Stenotrophomonas maltophilia(1.2%),and Escherichia coli(0.9%).71.3% CNS and 19.6% SAU were meticillin resistant,46.5% enterococci resistant to ampicillin and almost strains were susceptible to(vancomycin)(except E.gallinarum).ESBLs were detected in isolates of 56% E.coli and 35% K.pneumoniae.S.maltophilia was susceptible to trimethoprim(-sulfamethoxazole) and levofloxacin with resistance rate of 8.3% and 0%.In strains of other Gram-negative rods,resistance rate to imipenem,amikacin and fluoroquinolones were detected between 0% to 15%,and to cephalosporins Ⅰ-Ⅳ were between 20% to 70%.CONCLUSIONS Gram-positive cocci are important pathogens in neonatal sepsis,antimicrobials should be used on the result of(antimicrobial) susceptibility test.It is more important to reduce health care-associated(nosocomial) infections.
7.Integration of MOOC and information literacy education in academic libraries

The factors influencing the integration of MOOC and information literacy education in academic libraries were divided into objective factor, subjective factor and social factor according to their connotation and significance. Certain measures for their integration were proposed, such as strengthening network construction, perfecting literacy education systems, and carrying out systemic training.Integration of MOOC and information literacy education in academic libraries can improve information service, promote wide application of MOOC and information literacy ed-ucation in academic libraries.
8.Clinical research in controlling infusion rate during nasal feeding by infusion pump

Objective To investigate and improve the methods of nasal feeding to reduce the occurrence of complication such as diarrhea and vomiting.Methods We separated 140 patients with heavy craniocerebral injury into the control group (50 patients) and the experimental group (90 patients) according to different nasal feeding methods. For the control group traditional method was used. For the experimental group infusion pump was used to control infusion rate. The incidence of complication was compared between two groups. And within experimental group, the same nutrient solutions in respect of type and volume were infused during 10 hours, 16 hours, and 18 hours respectively. The relationship between infusion rate and the occurrence of complication was observed, analyzed and compared to seek for the most suitable enteral nutrition route which incurred the least complication.Results The incidence of diarrhea and vomiting in the experimental group was lower than that of the control group. And there were no significant differences in the occurrence of complication by using infusion pump at three different infusion rates within the experimental group. Conclusions The application of infusion pump in nasal feeding can reduce the incidence of complication and infusing nutrient solution at a slow rate was not necessary.
9. Evaluation of the efficacy of anti-platelet aggregation drugs in patients using thromboelastograph after percutaneous transluminal angioplasty and stenting

Objective: To detect the platelet inhibition of aspirin and Clopidogrel in patients after percutaneous transluminal angioplasty and stenting (PTAS) using a thromboelastograph (TEG) instrument in order to guide the individualized adjustment of anti-platelet aggregation drugs after PTAS. Methods: The clinical data of 65 patients with ischemic stroke or transient ischemic attack included from Nanjing Stroke Registry Program were analyzed retrospectively. Venous blood samples were collected at day 3 after PTAS. A TEG instrument was used to detect arachidonic acid (AA) -induced inhibition rate of platelet aggregation and adenosine diphosphate (ADP) receptor-induced inhibition rate of platelet aggregation. The AA pathway and ADP receptor-induced inhibition rate of platelet aggregation, as well as the response differences of the patients between aspirin and clopidogrel therapy were compared. Results: Circled digit oneThe inhibition rate (80 ± 28%) of aspirin for the AA pathway was significantly higher than that (53 ± 31%) of clopidogrel for the ADP receptor pathway (P < 0.01). Circled digit twoOf the 65 patients, the therapeutic effects in 45 (69.2%), 8 (12.3%), 7 (10.8%) and 5 (7.7%) patients were good, effective, low response, and ineffective in the aspirin group, and those in 19 (29.2%), 14 (21.5%), 23 (35.4%) and 9 (13.8%) were good, effective, low response, and ineffective in the clopidogrel group. Of those who had a good response to aspirin, 3 had no response to clopidogrel and 14 had low response to clopidogrel; of those who had a good response to clopidogrel, all responded well or effective to aspirin. Of those who had low response to clopidogrel, 4 had low response to aspirin, 5 had a good response to aspirin, and 14 had good results. The two efficacies had some relevance (χ2 = 33.311, P < 0.01). Circled digit threeA total of 53 patients had a good + effective response to aspirin, and 12 had low response + ineffective to aspirin, while 33 and 32 patients had good + effective and low response + ineffective to clopidogrel. There was significant difference in efficacy between the two drugs (χ2 = 15. 042, P < 0.01). Conclusion: Using TEG instrument to detect the inhibition rate of platelet aggregation in patients after PTAS is beneficial to guide the development of therapeutic scheme for individualized anti-platelet aggregation in clinical practice. The inhibitory effect of platelet aggregation of aspirin after PTAS is stronger than that of clopidogrel. The patients show differential responses to aspirin and clopidogrel therapy. Some patients who have low response to clopidogrel may respond well or may be effective to aspirin.
10.Analysis on prescription evaluation and drug use in a hospital

Objective To promote rational drug use by investigating quality of the outpatient and emergency prescriptions in the hospital .Methods We statistically summarized the evaluations of prescriptions and analyzed the drug use from January to Novem-ber ,2012 .Results The outpatient and emergency prescriptions were compliant to relevant writing standards ,without missing i-tems .The inaptitude of prescriptions were mainly in forms of inappropriate indication ,usage ,and dosage of drug and menstruum . The irrational prescriptions accounted for 5 .5% of those evaluated .Conclusion Rational drug use has been basically ensured through implementation of prescription evaluations and interventions .Irrational prescriptions exist in relatively small scale and are to be eliminated .
