Objective To summerize the experience in diagnosis and surgical treatment for hilar cholangiocarcinoma in recent years.Methods 86 consecutive cases of hilar cholangiocarcinoma were surgically treated in this hospital from Jan. 1992 to Oct. 1998. Results The tumor resection rate was 37%(32/86) in this series. The postoperative morbidity and mortality rate were 47% and 7% respectively. The median survival time in palliative resection group was 16 months and the 1-year, 3- year, 5-year survival rate was 63%,21%,15% respectively. For curative resection,the median survival time was 19 months and the 1-year, 3- year, 5-year survival rate was 80%,35%,25% respectively( P =0 038). For drainage group, the median survival time was 5 months and the 1-year, 3- year, 5-year survival rate was 28%,8 4%,5% respectively.Conclusion Early diagnosis, improvement of patients′ general preoperative condition and the surgical expertise may help in increasing the curative resection rate and reducing postoperative morbidity and mortality rate, which are keys to prolong patients′ survival.

METHODS: Eosinophils (EOS) from the peripheral blood of normal subjects were isolated. The hypodense eosinopil (HE) and normodense eosinophil (NE) were studied with electron microscopy. The effects of PAF on eosinophil activation and the action of dexamethasone or theophylline during the above process were measured. By researching the relevant papers at home and abroad, the principles of preparation, drug release mechanism, existing problems and so on were analysed and summarized on prodrugs triggered by colon bacteria-releasing enzymes (PTCBE). Compared with ordinary drugs, PTCBE was degraded by colon bacteria-releasing enzymes on colon-targeted treatment. PTCBE have the drug release characters of specificity, accuracy and so on for the colon-targeted treatment and can reduce the adverse drug reactions. Thus PTCBE have the good prospects of applications.

Aim:To develop a method for rapid and sensitive simultaneous determination of the hydrophilic and lipophilic active components in Salvia miltiorrhiza tablets. Methods:RP-HPLC condition was established to separate danshensu,salvianolic acid B,cryptotanshinone and tanshinone ⅡA from each other on the Diamonsil C18(250 mm×4.6 mm ID,5 μm)column with the mobile phase consisted of methanol and 0.5% formic acid in gradient elution. Results:The four active components had good linearity. The average recoveries ranged between 96.63% to 100.3% with RSDs of less than 2%. Conclusion:The method appeared to be precise and accurate for the simultaneous determination of the four active components in Salvia miltiorrhiza Bunge tablets within 35 min.

Objective To evaluate the efficacy and safety of Liqifuwei oral liquid for prophylaxis of chemotherapy induced-constipation through a randomized controlled trial.Methods Cancer patients who met the inclusion criterias were randomly assigned to the treatment group and the control group by random number table.Liqifuwei oral liquid was prescribed 20 ml per tid from the first day of chemotherapy in the treatment group,the control group accepted the conventional treatment.Incidence and degree of chemotherapy inducedconstipation,degree and duration time of abdominal distension,anorexia and diarrhea were compared by criteria of NCI-CTC 3.0.Results A total of 100 patients were eligible,including 50 patients in the treatment group and 50 patients in the control group respectively.Incidence rate of chemotherapy induced-constipation was 48 ％ (24/50) and 98 ％ (49/50) (P ＜ 0.000 1),all patients were grade 1-3 constipation,no grade 4 and grade 5 constipation (P =0.036).Incidence rate of abdominal distension was 36 ％ (18/50) and 70 ％ (35/50)(P =0.001 3),abdominal distension degree (P =0.033),duration time of abdominal distension was (2.22±1.48)days and (3.43±1.60) days (P =0.01).Incidence rate of anorexia was 56 ％ (28/50) and 90 ％ (45/50)(P ＜ 0.000 1),there was no significance different of anorexia degree (P =0.818).One patient in treatment group underwent mild diarrhea.Conclusion The prophylaxis of Liqifuwei oral liquid for chemotherapy induced-constipation and abdominal distension is effective,which deserves large-sample studies and clinic application.

Objective To detect the expression pattern of microRNA in A549 cells treated by lipopolysaccharide, study the expression of miRNA-1247-3P in A549 cells under LPS treatment and explore the possible mechanism of miRNA-1247-3P in A549 cells under LPS treatment. Methods A549 cells were divided into experimental and control groups. Immunocytochemical method and RT-PCR were used to detect the changes of SP-A and SP-C. The expression of miRNAs were detected using miRNAs array in different groups. The key miR-1247-3P was collected to detect the changes of miR-1247-3P in all groups using quantitative real-time polymerase chain reaction. Results Compared with control group, the expressions of SP-A and SP-C were significantly decreased in the experimental groups (P < 0.05). MiRNA array showed that 31 miRNAs were up-regulated and 3 miRNAs were down-regulated. Compared with control group, the expression of miR-1247-3P was significantly increased in the experimental groups (P < 0.05). Conclusion The increased expression of miR-1247-3P may play an important role in the pathogenesis of ARDS.

BACKGROUND:Until now, musculoskeletal bioengineering used in the orthopedic surgery and regenerative medicine are developing. OBJECTIVE:To summarize the construction strategies for tissue-engineered knee ligaments. METHODS:A computer-based online search was conducted in PubMed to screen the relevant literatures using the terms“knee ligament”AND“tissue engineering”OR“regenerative medicine”. The literature retrieval, data assessment and extraction were performed by both authors independently. The relevant literatures were included through reviewing preclinical and clinical studies to display the preclinical and clinical medicine progresses in the construction strategies for tissue-engineered ligaments. RESULTS AND CONCLUSION:Included 224 literatures focus on the construction strategies for tissue-engineered ligaments, including stem cel therapy, growth factors, biomaterials and scaffolds, and mechanical stimulation pattern. Construction techniques for tissue-engineered ligaments have some potential advantages, such as rapid function recovery, good tenacity, and reduced relapse. However, rigorous and high-level randomized control ed clinical trials are needed for the translation of preclinical results into successful clinical trials.

OBJECTIVE:To provide reference for better improving the risk perception of Omeprazole sodium for injection in nurses in a third-grade class-A hospital,strengthening risk management of ward drugs and ensuring safety of patients. METHODS:Nurses in the clinical departments in a third-grade class-A hospital where usage frequency of Omeprazole sodium for injection was relatively more were adopted as study subjects and a self-designed questionnaire was used to investigate on related knowledge on medication risk in nurses in emergency department,gastroenterology department,ICU and CCU in 2012 and 2014. RESULTS:51 and 81 questionnaires were distributed in 2012 and 2014,and 51 and 81 were effectively received with effective recovery of 100%and 100%,respectively. In both surveys,100% surveyed nurses could correctly choose one solvent at least,1.96% and 3.70%nurses could correctly choose 2 solvents;correct answering rates of intravenous administration time were 94.12% and 96.30%,re-spectively;correct answering rates of stable duration after preparation were both lower than 9%;correct answering rates of adverse reaction of this drug were 3.92% and 1.23%,respectively;correct answering rates of interaction of this drug were 1.96% and 6.17%,respectively;correct answering rates of stable formulation of this drug were 17.65%and 22.22%,respectively;the propor-tion of nurses observing ADRs were 25.49% and 13.58%,respectively;the proportion of nurses finding inappropriate compatibility were 15.69% and 22.22%,respectively,Ornidazole and sodium chloride injection was the most used incompatible drug;45% or more nurses hoping to strengthen the continuing education of nursing personnel. CONCLUSIONS:Nurses in this third-grade class-A hospital lack certain knowledge on the medication risk of Omeprazole sodium for injection,the drug risk management is not sufficient,and awareness prevention of drug risk is relative weak. It is suggested that the hospital should strengthen drug risk management system,nurses should strengthen the learning about drug risk and play the role of clinical pharmacists in the drug risk management to reduce the related drug risk and incidence of adverse reactions.

Objective To investigate the effect of serum containing different concentrations of traditional Chinese medicine Jinmaitong on β-catenin, GSK-3β and P0 in Schwann cells cultured in high glucose medium.MethodsTwenty-five male Sprague-Dawley rats were randomly treated with 5, 10, 15 and 20 times of Jinmaitong and distilled water.Schwann cells were divided into six groups, which are control group, high glucose group, 5 times group, 10 times group, 15 times group, 20 times group.72 hours later, the proliferative activity of SCs cells were detected by CCK, the mRNA and protein expression of β-catenin, P0 and GSK-3β was detectived by rt-PCR and Western blot.Results High glucose medium could inhibit the proliferation of Schwann cells, down-regulate the expression of β-catenin and P0(P<0.01), and up-regulate the expression of GSK-3β(P<0.05) mRNA significantly.But Jinmaitong can invert the results (P<0.01, P<0.05).Conclusions High glucose medium will injure the proliferation of Schwann cells, but Jinmaitongcan increase the proliferation activity of Schwann cells, and promotes the secretion of P0 partially dependent on up-regulating the activity of β-catenin and down-regulating the activity of GSK-3β.

AIM: To investigate the angiogenesis status,the expression of vascular endothelial growth factor(VEGF),angiopoietin-1(ANG-1),angiopoietin-2(ANG-2),thrombospondin-1(TSP-1) in cholangiocellular carcinoma(CCC) and relationship with tumor angiogenesis,differentiation,invasion and metastasis.METHODS: 33 specimen of surgically resected CCC were investigated.Immunohistochemical staining of CD34,VEGF,ANG-1,ANG-2 and TSP-1 was carried out.RESULTS: The mean MVD was(87.2?52.6)/mm2.VEGF positive expression was found in 75.6% cases;ANG-1 positive expression was observed in 36% cases;ANG-2 positive was detected in 57.6% cases and 45.5% cases exhibited positive TSP-1 expression.VEGF and ANG-2 expressions were found to be associated with significant higher level of MVD(P
