Objective To study the advantage and disadvantage of different treatments of unstable distal radial fractures in elderly patients. Methods In the period from August 2005 to January 2010, 55 elderly patients with unstable d istal radial fractures were treated with manual diaplasis and external plaster splint fixation, operation and internal plate fixation, or external fixation. Clinical outcomes were evaluated by Gartland-Werley wrist scoring systems and questionnaire of the disabilities of the Arm, Shoulder and Hand (DASH). We used SPSS13.0 software package for statistical analysis. Results Compared with the manual diaplasis group, the operation and internal plate fixation group or external fixation group was better in Gartland-Werley scores, the differences were statistically significant (P<0.05), but there were no differences in DASH scores among the 3 groups. Conclusions The treatment of unstable distal radial fractures in elderly patients should be chosen based on the characteristics of the fractare and patients desire.

Objective To explore the role of apoptosis in the pathogenesis of acute pancreatitis(AP)and to (investigate) the effect of ligustrazin(LGT) on the apoptosis in acute pancreatitis .Methods Experimental animals were divided into three groups randomly: (1) AP group, (2) comparison group,and (3) therapy group. Pathologic score and TUNEL method were used to detect the pathologic mophology of pancreatic tissue and the pancreatic cell apoptosis index in the 3 groups. Results In the early stage of AP,the majority of cells are apototic and the degree of AP is slight.In the late stage of AP,the majority of cells show necrosis,and the degree of AP is severe.The (induction) of cell apoptosis can lessen the pathologic state of AP. Conclusions The degree of pathologic changes in AP can be lessened by the induction of cell apoptosis,but there is a definte time limit.

目的观察苁蓉通便汤治疗老年性便秘的临床疗效。方法将 62例老年性便秘患者分为两组。治疗组(32例)内服苁蓉通便汤,对照组(30例)内服麻仁润肠丸。结果治疗组疗效优于对照组(P<0.05)。结论苁蓉通便汤对老年性便秘疗效显著。

Objective To study a simple surgical method for continuous rapid tissue expansion and its clinical application. Methods The operation was accomplished in two-stage procedures. First, expanders were placed in the subcutaneous pocket through interlesional perpendicular short incisions. Continuous expansion by trace syringe pump began from the second day after expander placement. The speed of water injection was set according to patient's algesthesia reaction and vascularity of skin. The second operation was carried out at the third to fifth day after the expansion completed.Results The method was put into clinical practice in 22 cases (16 cases of scar, 2 cases of capillary angioma, and 4 cases of black nevus), and 35 expanders were placed. The speed of water injection was set in 0.2～0.8ml/hr/100ml.The mean time of water injection was 15.3 days. In the process of expansion, all cases were satisfactory except 1 case, in which the expander exposed in the incision. After the second operation, all flaps survived except one showing distal skin necrosis. The result was satisfactory after 3 months to 1 year follow-up. Conclusion The procedure consists of expander placement through interlesional perpendicular short incisions and continuous expansion in advance, which maximally shortens the time of expansion and hospitalization. This expansion method is a simple, safe, reliable, and ideal rapid tissue expansion technique.

Objective Previously, there was an argument on the surgery way of unilateral papillary thyroid microcarcinoma(PTMC).The purpose of this study was to investigate the predictive factors of occult thyroid carcinoma in unilateral thyroid papillary microcarcinoma, and to compare the surgical methods of unilateral thyroid papillary microcarcinoma.Methods We retrospective analyzed 327 cases of unilateral PTMC patients confirmed by preoperative color Doppler ultrasound examination from January 2014 to January 2016 in Department of Breast Surgery, First Affiliated Hospital of Harbin Medical University.According to the presence of occult cancer, patients were divided into unilateral glandular foci (n=278) and bilateral glandular foci(n=49);according to whether carry out the prevention of central lymph node dissection, patients were divided into lymph node dissection(n=117) and without lymph node dissection(n=210) to further analyze the predictive factors of contralateral gland occult cancer and postoperative complications of preventive central lymph node dissection.Results 55 cases had central lymph node metastasis in 117 cases with lymph node dissection, the metastasis rate was 47.0%.The multiple foci in the ipsilateral gland is a risk factor for the presence of occult foci in the contralateral gland (OR=4.924, 95%CI: 1.605~15.107).There was no significant difference in postoperative complications between lymph node dissection and without lymph node dissection(P>0.05).Conclusion multiple foci in the unilateral thyroid papillary thyroid microcarcinoma can help us to predict the contralateral gland occult cancer, and the central lymph node metastasis rate is higher, therefore, for the unilateral glands of multiple tumors in the papillary thyroid microcarcinoma, we recommended the total thyroidectomy+prophylactic central lymph node dissection surgery way.

Objective To propose a method & Implementation for automatic blood ingredient separating and transferring.Methods Whole blood could be layered after centrifugal processing,and the color of the layers appeared different certainly.So color sensors could be used to receive different color signals of ingredient blood to control the process of separating whole blood and transfer different blood ingredients to different blood bags.When plasma was extruded out of the whole blood,resistance-strain pressure sensor was used to get the weight,and the air was cleaned out through extrusion movement.Air cleaning automatic control was realized through examining the intensity of pressure in the blood bag by photoconductive resistance.When the process of separating blood finished completely,the glue pipes could be heated automatically.The device adopted main-subsidiary configuration,the master was mostly in charge of separating and transferring blood ingredients of whole blood and the assistor was used to extrude out the air remains in the plasma bag and get the weight of the plasma.Results The assistor was made actually by using a PC as a virtual master to conduct experiments debugging and improve the assistor,and then the precision of electronic weighing reached 0.5g while the air in blood bag was almost cleaned absolutely.A communication protocol was developed and the serial communication between the master and the assistor were realized by VC6.0.Conclusion This method and device can automatically separate and transfer different blood components fast and efficiently,thus meeting the automation needs of the blood station.
