1.MSCTA features for blood supply artery of non-small cell lung cancer and its value for differential diagnosis

Objective:To observe the clinical features ofblood supply arterial of non-small cell lung cancer(NSCLC)by the multi-slice spiral CT angiography(MSCTA).Methods:92 patients with confirmed NSCLC conducted blood supply arterial examination by the MSCTA,38 patients with chronic lung inflammation of the same period as control group.Two experienced physicians analyzed the results though double-blind method.Results:In 92 NSCLC patients,only 78 patients(84.8%)supplied by the bronchial arteries;9 patients (9.8%)supplied by he extra-pulmonary arterial;5 patients(5.4%)supplied by mixed blood vessels.MSCTA performance for all the tumor feeding artery were distorted,and tumor area showed chaotic vascular shadow.And chronic lung inflammation were easier to have extra-pulmonary circulation arterial blood supply compared with NSCLC(?2=14.981,P=0.000 1),the rates were 47.4%and 15.2%, respectively;the number of blood supply arteries in patients with chronic lung inflammation and NSCLC were quite(t=1.542,P=0.126); but the blood supply artery diameter in patients with chronic inflammation significantly thicker than that of the NSCLC group(t=5.805, P=0.000 1).And,MSCTA also showed that vascular often coincide with each other to form a broad network in chronic lung inflammatory group.Conclusion:The multi-slice spiral CT angiography have a good identification for non-small cell lung cancer arterial blood supply, which can provide the basis for their interventional therapy;and,the blood supply arteries become more evident,more to stimulate the outside pulmonary arterial in chronic lung inflammation,so that,it can provide references for non-small cell lung cancer differential diagnosis.
2.THE EXPRESSION OF CYCLIN DEPENDENT KINASE 11 FOLLOWING TRANSECTED SPINAL CORD INJURY IN RATS

Objective To explore the expression and distribution of cyclin dependent kinase 11(CDK11) after transected spinal cord injury(tSCI). Methods tSCI was performed at T9 in adult Sprague-Dawley(SD) rats.The rats were sacrificed 1day,3days,5days,7days,14days after operation respectively.Western blotting was used to detect the changes of CDK11 protein expression after tSCI,and the distribution and localization of CDK11 in spinal cord were investigated by immunohistochemical staining and immunofluorescence double staining. Results Western blotting analysis showed the expressions of CDK11~(p58) and CDK11~(p110),which were the two major isoforms of CDK11,increased at first and then had a decrease tendency in both broken ends of the transected spinal cord.CDK11~(p58) was enhanced on the 3rd day and remained high level until the 7th day; while CDK11~(p110) was also increased,on the 3rd day but went down rapidly after the 5th day.Immunohistochemistry staining was indicated that CDK11 was distributed evenly in the normal spinal cord.On the 3rd day,the staining was elevated obviously in both white and grey matter.Immunofluorescence double staining suggested that CDK11 was colocalized with neuronal neclei(NeuN),cyclic nucleotide 3′phosphohydrolase(CNPase) and glial fibrillary acidic protein(GFAP).Conclusion The expression of CDK11 undergoes spatiotemporal changes after tSCI.These alterations may be involved in secondary spinal cord lesion such as neuronal apoptosis and proliferation of glial cells.
4.Preliminary application of nursing home quality evaluation system

Objective To check the maneuverability and sensitivity of nursing home quality assessment system. Methods Taking convenience sampling method, a nursing home of subsidiary of general hospital in Chongqing, a nursing home affiliated specialist hospital in Chengdu and a Red Cross nursing home in Shanghai were chosen for this study. In the basis of this nursing home quality evaluation system, a nursing home quality evaluation list was formed. Literature data, expert scoring method, field investigation, institutional self-evaluation methods were used to score each indicator, finally the quality of nursing homes score value was drawn. Results The self-assessment score of the nursing home affiliated general hospital in Chongqing was 6.2168 and good quality level, the self-assessment score of the nursing home affiliated specialist hospital in Chengdu was 5.7014 and good quality level, the self-assessment score of a Red Cross nursing home in Shanghai is 6.1103 and good quality level, consistent with the findings of the field. Conclusions The index system has more comprehensive, better operability and better sensitivity, it can effectively distinguish between good and bad of the same content in different nursing homes and have a better sensitivity and can be used to evaluate the quality of nursing homes.
5.Correlation between serum vitamin D level and prognosis in patients with acute cerebral infarction

Objective To investigate the relationship between serum vitamin D level and prognosis in patients with acute cerebral infarction. Methods One hundred patients with acute cerebral infarction within 48 h and 60 healthy subjects were selected. The serum 25-hydroxy-vitamin D level was measured. The patients with acute cerebral infarction were divided into vitamin D sufficient group, vitamin D insufficient group and vitamin D deficiency group according to the serum 25- hydroxy- vitamin D level. The National Institutes of Health stroke scale (NIHSS) before treatment and 14 d after treatment was evaluated, and this result reflected short- term prognosis; the long- term prognosis was evaluated by modified Rankin scale (mRS) 3 months after treatment. Results The serum 25-hydroxy-vitamin D level in patients with acute cerebral infarction was significantly lower than that in healthy subjects:(14.21 ± 0.98)μg/L vs. (22.43 ± 1.01)μg/L, and there was statistical difference (t=3.95, P=0.012). The patients with acute cerebral infarction were divided into 3 groups according to the serum 25- hydroxy-vitamin D level:vitamin D sufficient group (19 cases), vitamin D insufficient group (28 cases) and vitamin D deficiency group (53 cases). The NIHSS before treatment and 14 d after treatment in vitamin D sufficient group was significantly lower than that in vitamin D insufficient group and vitamin D deficiency group: (7.03 ± 1.82) scores vs. (10.21 ± 2.03) and (14.35 ± 2.96) scores, (2.04 ± 1.86) scores vs. (5.21 ± 2.28) and (10.38 ± 2.34) scores, and that in vitamin D insufficient group was significantly lower than that in vitamin D deficiency group, and there were statistical differences (P<0.01). The Pearson correlation analysis result showed that the serum 25- hydroxy- vitamin D level was negatively correlated with NIHSS scores (r = -1.738, P = 0.031). The short- term total effective rate in vitamin D sufficient group was significantly higher than that in vitamin D insufficient group and vitamin D deficiency group:17/19 vs. 67.9% (19/28) and 47.2% (25/53), and that in vitamin D insufficient group was significantly higher than that in vitamin D deficiency group, and there were statistical differences (P<0.05). The long- term good prognosis rate in vitamin D sufficient group was significantly higher than that in vitamin D insufficient group and vitamin D deficiency group:18/19 vs. 75.0%(21/28) and 50.9%(27/53), and that in vitamin D insufficient group was significantly higher than that in vitamin D deficiency group, and there were statistical differences (P<0.05). Conclusions The serum vitamin D level is significantly decreased in patients with acute cerebral infarction. It is negatively correlated with NIHSS scores. And it is important to judge prognosis.

7.The postoperative analgesic activity of flurbiprofen to intracranial aneurysm patients who went under endovascular treatment

ObjectiveTo investigate the postoperative analgesic activity offlurbiprofen to intracranial aneurysm patients who went under endovascular treatment with propofol and remifentanil anesthesia.Methods Totally 40 ASA physical status Ⅰ - Ⅱ intracranial aneurysm patients were randomly scheduled flurbiprofen group (group F, 20 cases) and control group (group C, 20 cases). The patients in group F were given flurbiprofen 1 mg/kg respectively at 10 min before operation and at the end of the operation; while the patients in group C were given fat milk at the same time. The patients' circulation of the two groups before and after operation,the headache visual analog scores (VAS) and body comfort score (BCS) were observed.ResultsThe mean arterial pressure(MAP) in group C after operation 1,2 and 3 h were significantly higher than that before operation (P < 0.05 ); the heart rate in group C after operation had no significantly difference compared with before operation (P >0.05). The MAP and the beart rate in group F had no significantly differences before and after operation(P > 0.05 ). The VAS of postoperation headache after operation 1,2 and 3 h were (0.30 ± 0.65 ), (0.30 ± 0.57) and (0.25 ± 0.44) scores in group F, which were significantly lower than those in group C [ (3.25 ± 1.58), (3.00 ± 1.56) and (2.90 ± 1.48 ) scores](P < 0.05 ) ; the BCS after operation in group F after operation 1,2 and 3 h were (3.30 ±0.86), (3.45 ±0.86) and(3.62 ±0.86)scores, which were significantly higher than those in group C [ ( 1.20 ± 1.00), ( 1.45 ± 0.94) and ( 1.50 ±1.00)scores](P< 0.05 ). ConclusionFlurbiprofen can reduce intracranial aneurysm patient's postoperative headache under endovascular treatment.
8.Efficacy of integrative medicine on 134 patients of post-stroke depression

Objective To investigate the curative effect of combined traditional Chinese and Western medicine treatment of de-pression after stroke.Methods Patients with depression in 134 cases of cerebral stroke from July 2011 to August 2012 in Tongli-ang People′s hospital were treated in department of internal medicine,and according to the different treatment methods were divided into experimental group and control group,each group of 67 cases.The control group was given paroxetine hydrochloride orally,the experimental group was based on the use of miananning granule in the treatment,obered curative effect of two groups.Results In the experimental group,the control group total effective rate were 95.5%,77.6%.The experimental group total effective rate was significantly better than the control group and has a statistically significance(P<0.05).Conclusion The combination of traditional Chinese and Western medicine treatment of depression after stroke is exact,it is worthy of popularization and application.
9.Field research on psychological needs of independent old people in nursing homes

Objective To understand the psychological needs of old people in nursing homes,so that more humanistic services will be supplied to old people,and hopefully to facilitate the management of the nursing home facilities.Methods Field research method was used in this paper in four nursing homes in Chongqing City.Data were collected by observation and interview.Results Data were coded and translated into three themes:wishing to be listened,especially the new resident; eager to be more respected and the feeling of being at home; the most important thing was happiness.Conclusions To concern the psychological needs of old people in nursing homes and supply corresponding service have a significant meaning for improving the survival state and their life quality,as well as increase service level.
10.Development of epigenetic treatment in myelodysplastic syndromes

It have been known that many epigenetic alterations palyed an important role in development of myelodysplastic syndromes (MDS). In contrast to genetic alterations, epigenetic alterations could be in principle pharmacologically reversed. Application of epigenetic drugs can reactivate inactivated suppressor genes. Epigenetic drugs mainly include demethylating agents and histone deacylase (HDAC) inhibitors, which are available in treatment. 5-AZA and decitabine as DNA demethylation agents have been approved by FDAof treatment in intermediate or high risk MDS, especially those old patients who are resistant to chemotherapy.HDAC inhibitors such as valproic acid are mostly employed in phase I trial, probably effective in treating low risk MDS, but treatment schedules and curative effects still have to be evaluated. The combination of demethylation agents and HDAC inhibitors may result in synergistic activity, but its therapeutic effect seems not to be superior to monotherapy of demethylation agents in current clinical trials, and it still need new clinical trials containing more cases and rational treatment schedules to identify safety and effect of combination.
