The widespread application of adjuvant therapy has shown clear benefits in the treatment of operable breast cancer. It is essential for the clinicians to select appropriate adjuvant regimens for this heterogeneous group of patients. Recent data from clinical trials evaluating the utility of adjuvant therapy were reviewed.

Objective To explore the difference and clinical effect in pediatric regional anesthesia with ketamine and fentanyl.MethodsThe clinical data of 60 cases of children with surgical treatment from June 2014 to December 2015 in Longde People's Hospital were retrospectively analyzed and randomly divided into two groups,with 30 cases in each group.The control group was given simple ketamine anesthesia;The intervention group was given ketamine combined with fentanyl anesthesia, of children with breathing,After anesthesia,the heart rate, respiratory,arterial pressure and other indicators and the anesthetic effect of the two groups were comprehensively evaluated.ResultsIn the intervention group after anesthesia, heart rate, respiratory rate, mean arterial pressure and pulse oxygen saturation was significantly better than the control group, there was statistical significance (P<0.05);and wake-up time in the intervention group, the dosage of ketamine and ketamine bolus times was significantly lower than the control group,there was statistical significance (P<0.05).ConclusionThe regional anesthesia operation were given ketamine and fentanyl assisted anesthesia, can stabilize children's heart rate, respiratory rate, shorten postoperative awake time, reduce anesthetic dosage, higher safety, anesthetic effect, can be widely applied in clinical.

Objective To explore the clinical efficacy of compound Xueshuantong capsule combined with compound anisodine injection in the treatment of non-proliferative diabetic retinopathy.MethodsSixty patients with non-proliferative diabetic retinopathy were enrolled in this study.All patients were enrolled from March 2014 to May 2016 and were divided into two groups by simple randomization.The control group were treated with salvia miltiorrhiza injection, and the patients were treated with compound Xueshuantong capsule combined with compound anisodine injection.The clinical efficacy and the level of hemorheology were compared.ResultsThe clinical improvement rate was 93.33% in the observation group, which was significantly higher than that in the control group (P<0.05).The PV, HCT, KAI and Fg levels in the observation group were significantly lower than those in the control group (P<0.05).ConclusionCompound Xueshuantong Capsule combined with compound anisodine injection for local injection of non-proliferative diabetic retinopathy has a significant clinical effect and is widely used.

Objective To investigate the superiority of proximal femoral nail anlirotafion(PFNA) fixation in the treatment of intertrochanteric fractures.Methods 106 patients with intertrochanteric fracture were included in the study.According to different intemal fixation methods,they were divided into PFNA group and 2 dynamic hip screw (DHS) group,53 cases in each group.The intraoperative conditions,the postoperative Harris score and the incidence of complications of the two groups were observed and compared.Results The length of incision in the PFNA group was significantly shorter than that in the DHS group [(4.50 ± 1.35) cm vs.(12.63 ± 2.50) cm],the operative time in the PFNA group was significantly shorter than that in theDHS group [(68.50 ± 10.22) min vs.(102.36 ± 17.55) min],the amount of blood loss in the PFNA group was significantly less than that in the DHS group[(152.40 ±35.08) mL vs.(298.33 ± 70.11) mL],there were statistically significant differences between the two groups (t =20.832,12.138,13.551,all P＜0.05).The fracture healing time in the PFNA group was slightly shorter than that in the DHS group[(9.70 ± 2.45)weeks vs.(10.22 ± 2.38)weeks],but there was no statistically significant difference between the two groups(t =1.108,P ＞ 0.05).The excellent and good rate of Harris score in the PFNA group was slightly higher than that in theDHS group (94.34％ vs.86.79％),the incidence rate of postoperative complications in the PFNA group was slightly less than that in the DHS group(5.66％ vs.9.43％),but there were no statistically significant differences between the two groups (x2 =1.767,0.541,all P ＞ 0.05).Conclusion PFNA internal fixation has characteristics of less surgical trauma,less bleeding,shorter operation time and less complications.It is easy to fracture healing and functional recovery of bone and joint.

Objective To analyze the proprietary Chinese medicine adverse reaction(ADR),thus to promote rational drug use in clinical.Methods According to clinical physicians,nurses and pharmacists fill in drug ADR reports,collected 48 cases of proprietary Chinese medicine ADR reports in our hospital during 2012-2015.The patients'age,sex,adverse reactions and drug varieties and aspects and so on,involved organs or systems,the prognosis were analyzed.Results According to the age distribution,children under the age of 10 years had 24 cases (50.00％).According to the varieties of drug,Reduning injection caused adverse reaction was up to 25 cases (52.08％).Damage to one of the most common clinical manifestations in the skin and accessories,a total of 29 cases (60.42％).Conclusion We should pay attention to the ADR of Chinese medicines,strengthen the monitoring analysis of daily work,avoid or reduce ADR,the clinical use of Chinese patent medicine is more safe and reasonable.

Objective: To study correlation among blood lipids and main inflammatory factors levels in patients with coronary heart disease (CHD) complicated hyperlipidemia.Methods: A total of 130 CHD patients treated in our hospital were selected.According to blood lipid level, they were divided into hyperlipidemia group (n=90) and non-hyperlipidemia group (n=40).Levels of total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), high sensitive C reactive protein (hsCRP), interleukin (IL)-6 and tumor necrosis factor (TNF)-α were measured and compared between two groups.Results: Compared with non-hyperlipidemia group, there were significant rise in levels of hsCRP [(1.52±0.89) mg/L vs.(9.03±1.04) mg/L], IL-6 [(10.52±2.41) mg/L vs.(35.63±3.08) mg/L] and TNF-α [(1.04±0.04) μg/L vs.(10.08±0.25) μg/L] in hyperlipidemia group, P=0.001 all.Pearson correlation analysis indicated that levels of TC, TG and LDL-C were significant positively correlated with levels of hsCRP, IL-6 and TNF-α (r=0.603~0.843, P<0.05 or <0.01), and HDL-C level was significant inversely correlated with levels of hsCRP, IL-6 and TNF-α (r=-0.853~-0.742, P<0.01 all) in hyperlipidemia group.Conclusion: Blood lipid levels are significantly correlated with inflammatory factors levels in patients with coronary heart disease complicated hyperlipidemia.Regulation of blood lipid levels can control inflammatory factors levels, which may have an active effect on prognosis.
