1.Analysis of influencing factors for clinical outcomes of patients with chronic hepatitis B

Objective To investigate the clinical outcomes of patients with chronic hepatitis B and its influencing factors,and provide reference for the prognosis of patients with hepatitis B.Methods 102 patients with chronic hepatitis B from January 2009 to June 2014 were selected and divided into the case group of 32 patients and the control group of 70 patients according to whether to occur with cirrhosis or liver cancer.The two groups of patients were analyzed and compared for general information,HBV DNA,serum ALT and other indicators.Results The cumulative cases of negative HBsAg,chronical hepatitis B,liver cirrhosis and liver cancer were 5,65 and 32,respectively.Univariate Logistic regression analysis showed that the differences between the two groups in age,sex,drinking history,HBV DNA levels and serum ALT levels were statistically significant (all P < 0.05).Whether to cirrhosis or liver cancer was the dependent variable,the statistically significant single factors between the two groups were the independent variable,multivariate Logistic regression analysis showed that older age,male sex,alcohol abuse,high level of HBV DNA and high level of serum ALT were the risk factors of leading to cirrhosis,liver cancer for the patients with chronic hepatitis B (OR values were 9.94,14.36,8.25,13.01,120.32,respectively).Conclusions The older age,male,alcohol history,high level of HBV DNA and high level of serum ALT are higher risk factors for patients with chronic hepatitis B to development of cirrhosis and liver cancer.Patients with risk factors should be gave systematic interventions to reduce progression to cirrhosis,liver cancer and improve the quality of life.

3.Safety of infliximab during perioperative period in Crohn disease patients

4.Fundamental laws of information therapy.

The author thinks that acupuncture-and-moxibustion is a form of information therapy (or informotherapy), which heals by means of feeding negative feedback information into disordered meridians at optimum information entry points (loci or acupoints). There is an essential difference among information threapy, physical therspy and chemical therapy. Informotherapy is a communication or dialogue between doctor (or informotherapy machine) and autonomic nervous system. Acupuncture, moxibustion, cupping, applying plaster and massotherapy etc. all are general acupuncture, special informotherapy. The article also gives a brief explanation on general informotherapy including Qigong (a system of deep breathing exercise), biofeedback, hypnotism and suggestion therapy ect.
The article discusses the deficiency and excess of meridian states, reinforcing and reducing of information, and the fundamental laws of information processing in meridians, as such the relationship of YIN (negative) and YANG (positive) meridians, exterior and interior meridians, left and right side of a meridian, hand and foot meridians, Fron-Mu and Back-Shu loci, the Five Elements and interpromoting and interacting relations, the isohypse loci, and the bio-holographic law. It introduces five Juci-puncture, the opposite side puncturing methods, and one quasi-Juci-puncture method.
The author treats acupuncture from the view point of cybernetics. He deems that the research of informotherapy may be developed to form a completely new discipline of technical science--Meridian Cybernetics. It will produce important impact to artificial intelligence and computer science.
6.Research progress of laparoscopic-assisted lymphadenectomy in patients with advanced gastric cancer

The diagnosis rate of early gastric cancer in China is low,most patients seeing the doctor are already in advanced period.Lymphadenectomy is the key factor affecting the tumor treatment and prognosis.With the depth of the research on tumor immunology,the function of lymph nodes in tumor immunity has been gradually paid more and more attention.The role of lymphadenectomy according to the rule of the lymph nodes metastasis has become the research point of the radical gastrectomy.Gastrectomy with D2 lymphadenectomy is the standard surgical treatment for advanced cancer.Laparoscopic-assisted gastrectomy with D3 lymphadenectomy is also in active exploring.In this article,the application of laparoscopy in advanced gastric cancer of current status,near-term efficacy,long-term outcome and complications are reviewed.
7.Analysis of stenosis application of micro minimally invasive technique for the treatment of lumbar spinal

Objective To explore the effects of minimally invasive technique in the treatment of lumbar spinal stenosis.Methods 80 lumbar spinal stenosis patients were divided into the observation group and control group,40 cases in each group,who were informed consent under the premise of the stochastic averaging grouping.The two groups were treated with microscopic invasive technology and traditional posterior bilateral fenestration decompression,and the therapeutic effect of two groups were compared.Results The average operation time of the observation group (36.2 ± 4.1) rain was obviously shorter than that of the control group (41.4 ± 5.7) min (t =2.719,P < 0.05) ; the average amount of bleeding of the observation group (51.6 ± 7.3) ml was significantly lower than that of the control group(101 ± 11.7) ml(P < 0.05) ; the two groups of patients before treatment JOA score had no significant difference after treatment,follow-up,JOA scores of the two groups increased significantly,the observation group increased significantly compared with the control group (P < 0.05) ; the treatment group 19 cases of excellent,good in 17 cases,the excellent and good rate was 90%,control group 13 cases of excellent,good in 16 cases,the excellent and good rate was 72.5 %,the treatment group was better than the control group (x2 =6.594,P < 0.05) ; the observation group did not appear obvious postoperative complications occurred in 3 cases,the control group of nerve root injury,1 cases of intervertebral space infection,2 cases of incision leakage,the incidence of complications was 15%,observation complication rate was significantly lower than that of the control group (x2 =3.357,P < 0.05).Conclusion The microscopic invasive technique for the treatment of lumbar spinal stenosis has a good curative effect,small trauma,high safety,which is conducive to improve the prognosis of patients,can gradually replace the traditional posterior bilateral fenestration decompression.
8.Investigation on the efficacy and safety of omeprazole combined itopride in the treatment of functional dyspepsia

Objective To study the effect of omeprazole combined itopride in treatment of functional dyspepsia and the safety.Methods 246 patients with functional dyspepsia were randomly divided into three groups,each group of 82 cases.Group A was treated with omeprazole,20mg each time,twice every day.Group B was treated with itopride,50mg each time,three times every day.Group C was treated with omeprazole combined itopride.All groups were treated for four weeks.Improvement of symptoms,clinical curative effect and safety were observed.Results The effective rate of early enough and after dinner full bilge symptoms of group C were 95.1% and 95.3%,which were significantly higher than 64.6% and 63.6% of group A.The effective rate of abdominal pain and epigastric voiding symptoms of group C were 93.1% and 90.8%,which were significantly higher than 67.2% and 65.1% of group B.The total effective rate of group C was 91.5%,which was significantly higher than 69.5% of group A and 73.2% of group B.All differences were statistically significant (P < 0.05).Conclusion Omeprazole combined itopride in treatment of functional dyspepsia has definite curative effect,which is safety and worth of clinical promotion and popularization.
9.Intestinal rehabilitation in the management of acute gastrointestinal injury

Acute gastrointestinal injury (AGI) is a malfunctioning of the gastrointestinal tract in intensive care patients due to their acute illness.AGI could be divided into 4 types,including AGI grade Ⅰ:increased risk of developing gastrointestinal dysfunction or failure; AGI grade Ⅱ:gastrointestinal dysfunction; AGI grade Ⅲ:gastrointestinal failure; AGI grade Ⅳ:gastrointestinal failure complicated with distal organ dysfunction.The symptoms of AGI include vomiting and regurgitation,gastric retention,diarrhea,gastrointestinal bleeding,paralytic intestinal obstruction,bowel dilatation and abnormal bowel sounds.Intestinal rehabilitation therapy was used to promote the patients with short bowel syndrome to restore enteral or oral feeding,and it could be used to improve the gastrointestinal function in patients with AGI.The procedure of intestinal rehabilitation therapy include total parenteral nutrition,parenteral and enteral nutrition,total enteral nutrition and oral feeding.The specific therapy could be implemented according to the severity of AGI and early enteral nutrition should be tried frequently.Intestinal tissue specific nutrients should also be provided either through enteral or parenteral approaches.
10.The research of the clinical effect for high-dose ulinastatin small on the SIRS and patients with MODS

Objective To study the clinical effect of large dose ulinastatin on patients combined SIRS and MODS.Methods 120 patients in ICU were randomly divided into two groups,each 60 cases,the control group using conventional standard treatment,the observation group in the control group on the basis of the 12h after the operation started on intravenous injection of large dose ulinastatin,observe two groups of patients after treatment with cytokine content and duration of hospital stay,mortality rate and APACHE Ⅱ score after treatment.Results The two groups before treatment TNFα,IL-6,IL-8 and APACHE Ⅱ score difference is not big,after treatment,48 h,72 h,two groups of patients with TNFα,IL-6,IL-8 were decreased,and the observation group decreased greater than the control group (t =8.46,8.72,8.68,10.38,7.83,9.26,all P < 0.05),after treatment the observation group APACHE Ⅱ score,mortality,average time of hospitalization were significantly lower than those of the control group,there was statistical significance(t =9.58,x2 =10.28,t =9.17,all P < 0.05).Conclusion High dose ulinastatin on inflammatory cytokines have obvious inhibition effect,can reduce the surgical risk in patients with APACHE Ⅱ score and mortality,shorten the treatment time of patients.
