1.Study on the relationship between Acute Coronary Syndrome and serum homocysteine

ObjectiveTo discuss the relationship between serum homocysteine (Hcy) level and severity of acute coronary syndrome(ACS) through measuring the level of serum Hey in the patients with ACS.Methods95 patients who were diagnosed ACS were divided into two groups:40 cases of unstable angina(UA) and 55 cases of a cute myocardial infarction(AMI).30 individuals were chosen as normal control(NC).In addition,according to the results of coronary angiography,the same 95 patients were further divided into three subgroups which were single-vessel disease( 28cases),double-vessel disease ( 36cases ) and multi-vessel disease ( 31 cases ).Level of serum Hcy was measured in all groups.ResultsSerum Hcy level of ACS patients was significantly higher than NC group( P < 0.01 ).The level of serum Hcy in AMI group was significantly higher than that in UA group( P < 0.05 ).The Hcy level in the multi-vessel disease group,double-vessel disease group and single-vessel disease group were increased in turn,and the difference of pairwise comparison was statistically significant ( P < 0.05 ).ConclusionThe serum Hcy level has rela tionship with the severity of coronary lesions and is directly proportional to the number of coronary artery lesions.The result suggests that the level of serum Hcy maybe play an important role in the prevention,diagnosis,treatment and monitoring of ACS.
2.Impact of nursing intervention on the secondary infection in chemotherapy of elderly patients with lung cancer

Objective To investigate the effect of intensive nursing intervention on the secondary infection in chemotherapy of elderly patients with lung cancer.Methods 168 cases of elderly lung cancer patients whose neutrophil ≤ 1.0×109/L during chemotherapy were randomly divided into two groups,namely,the intensive care group and the control group,with 84 cases in each group.The control group was given routine nursing of infection prevention,while the intensive care group was given intensified nursing for infection prevention.A contrastive analysis was conducted in terms of the incidence,clinical characteristics and the duration of antibiotic treatment of the two groups of patients with secondary infection.Results The secondary infection rate of the intensive care group was lower than that of the control group during chemotherapy.What's more,the duration of fever and treatment of the intensive care group was shorter than that of the control group,hence the patients were safe during infection period.The difference was statistically significant.Conclusions Strengthening the nursing intervention can reduce the incidence rate of secondary infection in aged patients with lung cancer,increase the tolerance of patients to chemotherapy and ensure the successful completion of the chemotherapy cycle,prolong the life span of the patients with an improved quality of life.
5.Clinical application of key-way attachment in fixed restoration of mandibular first and second molar dele-tion

Objective:To evaluate the clinical effects of the key-way attachment in fixed restoration of mandibular first and second molar deletion.Methods:A 1 -5-year follow-up study was conducted on 1 6 patients with the reconstruction of mandibular first and second molar deletion.The key-way attachment was used as a connector in the fixed partial denture,with the third molar and 2 sec-ond premolars as the abutments.Results:During 5-year follow-up,1 3 of the fixed partial denture were good-looking,stable and functioning.The abutments were stable,without endodontic and periodontal disease.No absorption of alveolar bone was observed by X-ray examination.3 cases were failure.Bleeding on probing of third molar was observed in 1 patient 1 year after restoration.1 re-tainer of a denture on the third molar was loosing 1 .5 years after restoration.1 patient had dental caries on the third molar 5 years af-ter restoration.Conclusion:The key-way attachment as a connector is useful in fixed partial denture in patients with mandibular first and second molar deletion.
6.Association between exhaled nitric oxide and other noninvasive methods in assessing asthma in children

Measurement of Fractional exhaled nitric oxide (FeNO) is a noninvasive,simple,and reproducible procedure to assist in assessment of disease severity and evaluation of response to treatment in asthma children.This review discusses the value of FeNO in children with asthma,and the association between FeNO and other noninvasive methods.
7.Curative effect of small incision thyroidectomy on thyroid nodule

Objective To discuss the curative effect of small incision thyroidectomy on thyroid nodule . Methods 94 patients with thyroid nodule were selected and divided into the small incision group and control group according to different modes of operation ,47 cases in each group .The patients in small incision group were given small incision thyroidectomy ,while the patients in control group were given the traditional big incision thyroidectomy .The curative effect and security in the two groups were observed and compared .Results The operation time,intraoperative amount of bleeding ,length of incision and length of hospital stay in the small incision group were (48.76 ±10.37) min, (39.57 ±7.68)mL,(3.57 ±0.68)cm(4.89 ±1.13)d,which were significantly less than those in the control group [(54.07 ±11.64)min,(64.16 ±7.98)mL,(6.88 ±0.65)cm,(5.47 ±1.32)d](t=2.13,3.12,3.96,2.14,P<0.05 or P<0.01).The incidence rate of adverse reactions in small incision group was 10.6%,which was significant-ly lower than 29.8%in control group (χ2 =5.34,P<0.05).Conclusion Compared with traditional mode of opera-tion,small incision thyroidectomy has following advantages on thyroid nodule ,smaller operation wound ,less intraopera-tive amount of bleeding ,quicker postoperative recovery ,better cosmetic effect ,less adverse reactions ,etc.
8.Development of the integrated TCM hepatology with western medicine

The treatment for severe acute respiratory syndrome (SARS) that happened epidemically this spring confirmed again that the integrated traditional Chinese and western medicine had its clinical advantage and vitality, and indicated that this integration could benefit the pathologic understanding and therapeutic strategies for the diseases. Based on the experimental research, western medicine focuses on the histological and structural changes of the diseases. In order to probe the key mechanisms of the disease's occurrence and development, it tries to develop the therapy targeting the mechanism. Materialism is the academic characteristic of western medicine. Based on the differentiation of individual patient and comprehensive analysis of all manifestations, with the academic characteristic of dynamics and dialectics, traditional Chinese medicine (TCM) focuses on the regulation and recovery of the body functions. Theoretically, materialism and dialectics are complementary to each other. Their combination and unity is the key point for the integration of TCM with western medicine in order to improve the clinical effectiveness and TCM development. Therefore, we must adhere to the principles of the evidence-based medicine, strengthen the integrated study on "disease" of western medicine and "syndrome" of TCM, insist on "materialism" and improve the understanding of the mechanism of Chinese herb formula, pay efforts on the clinical study of difficult and common hepatic diseases, in order to develop the integrated traditional and western medicine on liver diseases.
9.Strategy for treating liver fibrosis by integration of the disease and its syndromes in traditional Chinese medicine

The strategy for treating liver fibrosis in chronic hepatitis patients includes dispelling the etiological factors, inhibiting the inflammatory reaction, decreasing the sedimentation of extracellular matrix, accelerating the degradation of extracellular matrix, improving the microcirculatory and metabolic disturbance, and ameliorating the complicating diseases, etc. Researchers should pay attention to the liver function indexes in chronic liver disease in evaluating the therapeutic effects of anti-fibrosis. Effective etiological treatment should be considered as the first step in treating liver fibrosis in chronic hepatitis patients, and inhibiting the inflammatory reaction is one of the most important tactics for suppressing the development of fibrosis and for decreasing the incidence rate of liver cancer in chronic hepatitis patients. Treatment based on syndrome differentiation, a dynamic therapy aimed at the holistic pathological state, can improve the pathological state of the disease. It is especially important to take the advantages of the integration of traditional Chinese and western medicine in the clinical diagnostic and therapeutic procedure for increasing the therapeutic effect of live fibrosis.
10.Diagnosis value of CTA on posterior inferior cerebellar artery aneurysm(report of 2 cases)

Objective To explore the diagnosis value of CTA on posterior inferior cerebellar artery ( PICA ) aneurysm.Methods The clinical data of 2 patients with PICA aneurysms were analyzed retrospectively .Results Two patients were elderly,acute onset,CT confirmed subarachnoid hemorrhage ,CTA of the first time were negative within 3 h after the onset .After the treatments of preventing cerebral vasospasm , increasing blood volume , protecting brain cells and etc for 3-7 d, one case improved ,one case were invalid .Given CTA examination again then found 3 aneurysms, one case with 2 aneurysms located on the left side of the PICA , accompanying with hydrocephalus ,and the other case with 1 aneurysm located on the left side of the PICA .Two patients obtained a good image evaluation and rich information .Conclusions In view of the particularity of the PICA aneurysms , CTA can be used as the preferred method of examination , which can provide comprehensive imaging data .When CTA examination of the first time is negative , giving CTA again remains an ideal choice after short-term treatment .
