4.A Clinical and Pathological Study of Dysfunctional Uterine Bleeding.

OBJECTIVES: Dysfunctional uterine bleeding(DUB) is defined as abnormal bleeding from the uterine endometrium unrelated to anatomic lesions of the uterus, and its incidence is 10-15% among gynecologic diseases. We conducted this study for understanding correlation between clinical aspecets and pathological findings of DUB. Methods: We conducted a retrospective review of 599 women with DUB who underwent endometrial biopsy with special regard to the relation between pathologic findings and presenting symtoms or complaints from Jan. 1988 to Dec. 1997. RESULTS: Age distribution of DUB was mainly 5th decade, mean age was 44.1years, among various bleeding patterns, intermenstrual bleeding was the most common pattern(31.6%) and the next was menorrhagia(25.0%). Histologic findings of endometrium were proliferative phase, 327 cases(54.6%), hyperplasia, 139 cases(23.2%), secretory phase, 74 cases(12.4%) in order of frequency, and there was no difference in distribution of histologic findings among various bleeding patterns. CONCLUSION: Compared to other previoius studies, our study showed more incidence of endometrial hyperplasia, especially at age group of 40 or more. So patients aged more than this age with abnormal uterine bleeding must undergo emdometrial biopsy for pathologic diagnosis. Patients who are diagnosed endometrial hyperplasia must be carefully followed up because there are possibilities of progression to endometrial carcinoma.

5.A Clinical Study of the Patella Fracture

Vertical and marginal fractures of the patella are uncommon injuries compared with other types of patellar fracture. It's possible that they are more common than supposed, but often remain undiagnosed as acute injuries. Vertical and marginal fractures often results in less acute disability than stellate or transverse fracture and the routine radiographs are often unhelpful. Failure to diagnose acute vertical and marginal fractures of the patella, especially when the fragments are displaced, may result in prolonged disability of the knee and possible development of degenarative changes in the patello-femoral joint later. Twelve patients of the vertical and the marginal patellar fractures treated at St. Mary's Hospital from Feb. 1982 to Dec. 1987 were analyzed in clinical and radiological aspect. The results obtained from this study were as followings. 1. Vertical and marginal fractures had 15.4% of injury rate in all patellar fractures. 2. The fracture and its displacement were always present on the tangential view of the patella. 3. The mechanism of the fracture was s direct blow to the patella in which the affected knee was flexed. 4. The location of the fracture line was related to the degree of the flexed knee at the time of fracture. 5. Large separated fragments of the patella should be accurately reduced with rigid fixation in the rationale of the intraarticular fracture.

7.Production and Gene Cloning of Monoclonal Antibodies Directed Against S Antigen of Hepatitis B virus.

We produced two murine monoclonal antibodies designated S2E1 and S2C11, which recognize S antigen of hepatitis B virus (HBsAg). S2E1 could bind to denatured form of recombinant HBsAg as well as native form of HBsAg, but S2C11 could bind only to native form of HBsAg. Both antibodies reacted with HBsAg in the hepatocyte of patient infected with hepatitis B virus. Analyses of the nucleotide sequences encoding the variable regions of these antibodies revealed that S2E1 and S2C11 utilize variable gene segment which belong to V4/5 gene family and utilize the J5 and Jk4 gene segments, respectively. In addition, the heavy chain of S2E1 express a member of V14 gene family and a member of DSP2.9 and Jh3 gene families. S2C11 is related to the V1 gene family and expresses DFL16.1 gene regions in conjunction with the Jh3 gene segment.

8.Effects of Balance Taping Therapy for Elders with Leg Pain.

PURPOSE: The purpose of this study was to examine the effects of balance taping for elders suffering from leg pain. METHOD: This study was a nonequivalent pretest-posttest design, quasi-experimental study. There were 25 elders in the experimental group and 25 in the control group. The degree of leg pain was measured three times. RESULTS: In the experimental group, the leg pain score before taping was 6.28, but 1 hour after the taping was applied it was 3.24, and at 24 hours after the taping was applied, 2.16. The leg pain score for elders in the experimental group decreased significantly but for those in the control group, it hardly changed. CONCLUSION: The findings of this study support the conclusion that balance taping may benefit individuals with leg pain. Also, balance taping therapy can be used as an independent nursing intervention.

9.Correlations among Emotional Labor, Servant Leadership, and Communication Competence in Hospital Nurses.

PURPOSE: The purpose of this study was to identify correlations among emotional labor, servant leadership, and communication competence in hospital nurses. METHODS: A self-administrated survey was conducted with a convenient sample of 210 nurses, using a structured questionnaire. The data were analyzed by descriptive statistics, t-test, ANOVA, and Pearson's correlation coefficient using SPSS/WIN 20.0 program. RESULTS: The mean emotional labor score was 3.4+/-0.60. There was very strong and negative correlation between emotional labor and servant leadership (r=-.896, p<.001). Communication competence was positively correlated with servant leadership (r=.298, p<.001) and moderately and negatively correlated with both frequency of emotional labor (r=-.166, p=.016) and the degree of attention to emotional expression (r=-.143, p=.039). CONCLUSION: As a strategy to reduce emotional labor of hospital nurses, the findings suggest training programs for nurses to enhance servant leadership and communication competence.

10.Diffuse Pigmented Villonodular Synovitis of the Hip Joint

In 1941, Jaffe and coworkers studied a lesion with histologic appearance of fibrous stroma, pigmented deposition and histiocytic infiltration as well as giant cell, for which they named pigmented villonodullar synovitis, bursitis and tenosynovitis. Thereafter many authors have disscussed etiology, clinical and radiological features, pathology and treatment regarding the disease. Recently we experienced a case of pigmented villonodular synovitis involving an hip joint which was treated by synovectomy and total hip replacement with good result.
