1.Impact Study of County-Level Public Hospital Comprehensive Reform on New Rural Cooperative Medical Fund Payment

Objective: As the breakthrough for public hospital reform in China, County public hospital comprehensive reform has become the main part of the reform, the reasonable use of New Rural Cooperative Medical System(NCMS) would strongly push the process of new medical reform. Methods: The method of mathematical modeling was applied in policy imitate of hospital operation historical data, to estimate the increasing of decreasing status of NCMS hospital compensation expenditure after the implementation of county-level public hospital comprehensive reform measurement in 10 county-level public hospital. Results: Through analyzing the operation data of applying county-level public hospital comprehensive reform measurement in 10 county-level public hospital from 2010 to 2012, there was 66.67% annual time, the NCMS compensation expenditure for public hospitals increased to 23.075 6 million yuan, which increased by 1.691 7 yuan averagely. Conclusion: After the implementation of comprehensive reform policies, the influencing factors of NCMS funds on the decreasing degree of county-level public hospital expenditure were mainly the proportion of pre drug addition ratio and basic drug sales in the total drug sales.
2.External iliac-popliteal artery artificial graft cross bypass for the treatment of arterial occlusion in lilac-femoral arteriosclerosis

Objective To study the curative effectiveness of external iliac-popliteal artery artificial graft cross bypass in treating extensive arterial occlusion in unilateral lilac-femoral arteriosclerosis. Methods From Sep. 1999 to Oct. 2007, 39 elder patients were diagnosed as having extensive arterial occlusion in unilateral lilac-femoral arteriosclerosis by color Doppler ultrasonography, CTA or DSA. 25 patients (61.5%) had resting pain, 14 had ulcers or gangrenes in the toes. The average ankle-brachial index was 0. 19. Contralateral external iliac-popliteal artery artificial graft cross bypass surgeries were performed on 22 patients from left to right, the other 17 were from right to left. Results There were no perioperative death or amputation. The ankle-brachial index increased from 0 ~ 0. 41 ( preoperative ) to 0. 85 ~ 1.02(postoperative) ; the claudication distance increased from 15 ~ 60 m (preoperative) to 350 ~ 500 m or more than 500 m [ postoperatively at the speed of (100 ~ 120) m/min] ; The postoperative color Doppler uhrasonography showed the average velocity of the popliteal artery was 45 cm/s. In the anterior tibial artery or the posterior tibial artery, little blood flow was showed in preoperative color Doppler ultrasonography examination, the postoperative average blood flow velocity was 41 cm/s. 35 patients (89. 7% ) were followed up for an average of 3.4 years, with an accumulative patency rate of 85.7%. Four patients had amputation, the limb salvage rate is 88.6%. Conclusion External iliac-popliteal artery artificial graft cross bypass is effective in treating unilateral lilac-femoral arterial occlusion, particularly for aged patients with vital organ incompetence.
3.Research progress of ovarian cancer apoptosis related factor-Survivin and Smac

Survivin is the strongest member of the inhibitor of apoptosis protein(IAP)family. Smac is the second mitochondria-derived activator of cysteine proteases,which can promote apoptosis by combining with IAP. Abnormal expression of them is closely related with occurrence,development,treatment tolerance and prognosis of ovarian cancer. It is prompted that Survivin and Smac are expected to play important roles in the early diagnosis and targeted therapy of ovarian cancer.

6.Effects of arsenic trioxide and imatinib mesylate on proliferation and apoptosis in lymphoblastoid Raji cell line

Objective To investigate the effect of arsenic trioxide (As2O3) and imatinib mesylate (imatinib) or in combination with imatinib with different concentration on Raji cells and the mechanisms.Methods The inhibition of cell proliferation was measured by MTT assay to assess the cells survival after As2O3 and imatinib or in combination with imatinib treatment with different concentration at indicated time.Caspase-3 activity changes and the relative number of cells in different phases and the percentages of cells calculated in G1 and S phase of the cell cycle were assessed by FCM. The expression of p16 protein was analyzed by SABC immunohistochemical method. Results The results of MTT assay showed that As2O3 and imatinib or in combination with imatinib could inhibit Raji cells growth. There was clear statistical significance between the union groups and the single-drug group (P<0.05). As2O3 inducing apoptosis was accompanied by up-regulation and activation of apoptosis protein Caspase-3. The mean percentage of apoptosis cells was both in time-and dosage-dependent form (P<0.0001). While Raji cell lines were less sensitive to imatinib than arsenic trioxide (P >0.05). Further more, Combination of imatinib with As2O3 had not a synergistic inducing apoptosis effect on Raji cells (P >0.05). The optical density of p16 protein increased both in time-and dosage-dependent form with As2O3 by immunohistochemical method (P<0.05). While at higher concentration and for a longer time, imatinib could up-regulated the optical density of p16 protein. There was no obvious statistic significance for p16 protein in Raji cells with using As2O3 only compared with the unions group(P >0.05). The cell cycle was arrested at G1 phase, the number of cells G1 period increased significantly,and S phase decreased on Raji cells after As2O3 treatment. The relationship between the cellular DNA contents and the concentration of As2O3 showed a dose-and time-dependent manner (P<0.0001). But it was found that imatinib had no effect on Raji cell cycle. There was no the obvious difference (P >0.05) between two drugs unions group with only using As2O3 group. Conclusion The results showed that As2O3 exerted variable and definite effects on lymphoma Raji cells, and suggested that As2O3 might induce apoptosis and up-regulate the optical density of p16 protein and arrest cell cycle. As for Raji cells, imatinib might affect the expression of p16 protein at higher concentration. Two drugs unions had no effective and synergistic effect.

Radiotherapy is an important treatment for malignant tumors, but it will have a negative impact on the reproductive function of young female patients.In recent years, the measures to protect female fertility are increasing, including clinical mature technologies such as embryo cryopreservation and mature oocyte cryopreservation; highly experimental technologies such as immature oocyte in vitro fertilization and ovarian tissue cryopreservation and transplantation.In addition, the application of stem cells and the proposal of artificial ovary also show great prospects in the protection of female fertility.This paper reviews the research progress of female fertility protection strategies in radiotherapy.
8.Explore the breadth and depth of clinical research and applications of fundus autofluorescence

Fundus autofluorescence (FAF) relies primarily on the presence of accumulated lipofuscin in the retinal pigment epithelium (PRE) cells. It has emerged as a valuable tool to detect and evaluate the viability and structural changes of the RPE in live. As a non-invasive, repeatable, simple and efficient means of detection. FAF imaging can provide information of RPE structure and function to assistant the diagnosis of many retinal diseases with other conventional fundus imaging technologies. With quantitative analysis and complementary analysis with other fundus imaging technologies, the FAF features of different retinal diseases will be further understood. This knowledge will not only extend the reasonable and unique clinical applications of FAF, but also will contribute to the understanding the pathogenesis and improving the treatment of many retinal diseases.
9.Clinical research of prophylactic antifungal treatment in severe acute pancreatitis

Objective To investigate whether prophylactic antifungal treatment will reduce the incidence of fungal infection(FI)and the mortality in severe acute pancreatitis(SAP).Methods Forty consecutive patients with SAP admitted from January 2008 to December 2009 were divided into prevention group and control group with 20 cases each by random digits table.The patients in prevention group underwent routine therapy plus intravenous fluconazol as prophylactic antifungal treatment,while routine therapy only in control group.The length of hospital stay,the time when FI occurred,the incidence of FI and mortality were compared with each other.Results Compared with control group,the length of hospital stay wag shorter[(40.3±15.4)d vs.(46.5±18.5)d],the time when FI occurred was slewer[(24.5±14.3)d vs. (14.5±12.7)d],the incidence of FI[15.0%(3/20)vs.35.0%(7/20)]and the mortality [10.0%(2/20)vs. 25.0%(5/20)]were lower in prevention group,there were significant differences between the two groups(P<0.05).Conclusion Prophylactic antifungal treatment can shorten the hospital stay,delay the FI occurrence,reduce the incidence of FI and the mortality in patients with SAP.
10.STAT1 mutations associated with primary immunodeficiency diseases

STAT1 plays a central role in multiple intracellular signal transduction pathways.STAT1 gene mutations have led to four types primary immunodeficiency disease,including.autosomal recessive (AR) complete STAT1 deficiency,AR partial STAT1 deficiency,autosomal dominant (AD) STAT1 deficiency,and AD gain of STAT1 activity.The first three diseases due principally to the impairment of IFN-γ-mediated and/or IFN-α/β-mediated immunity.Different from common primary immunodeficiency diseases,AD gain of STAT1 function probably due to an enhancement of IFN-a/b-mediated immunity.This article reviews the pathogenesis,clinical manifestations,diagnosis and treatments of inborn errors of human STAT1 immunity.
