1.The use of Tacrine (THA) and succinylcholine compared with alcuronium during laparoscopy

Either tacrine (THA) with succinylcholine or alcuronium was used on a randomized basis for laparoscopic procedures in twenty young females. The technique using THA with succinylcholine was found to be more suitable and predictable for this procedure and gave a smoother anaesthetic course, brighter recovery and minimal postoperative complications.

2.Effects of Lacto-ovo Vegetarian Diet on Nutrient Intakes and Serum Lipid Levels in Non-smoking Non-alcohol Drinking College Male Students.

In order to elucidate the effects of a lacto-ovo vegetarian diet on serum lipid levels, 91 male collegians residing in the Seoul area were recruited. The vegetarians did not smoke tobacco nor drink alcohol. Their anthropometric measurements, dietary intakes, and blood lipid concentrations were compared to age-matched omnivorous controls. The subjects were divided into four groups. The four groups were: 1) NSNDV (n=25): Non Smoking Non Drinking Vegetarians, 2) NSNDO (n=10): Non Smoking Non Drinking Omnivores, 3) SDO (n=37): Smoking, Drinking Omnivores, 4) SDM (n=11): Smoking Drinking Meat-eaters. Total serum cholesterol, HDL-cholesterol and hematocrit were analyzed. The results were as follows: Weight (p<0.05) and BMI (p<0.01) were significantly higher in SDM than NSNDV. Dietary protein (p<0.01), iron (p<0.05) and dietary lipids (p<0.01) from animals in NSNDV were significantly lower than the results in the SDM. Interestingly enough, we noted no significant differences between the four groups in terms of average total dietary protein, dietary lipids and dietary iron, total serum cholesterol, or HDL-cholesterol. Dietary fiber intake was significantly higher in NSNDV than in SDM (p<0.05). Weight, BMI, and total cholesterol were significantly positively correlated with animal protein (p<0.05) and animal lipid (p<0.01) intake. These results showed that vegetarian collegians who haven't smoked tobacco nor drunk alcohol evidenced lower body weight, BMI, and dietary animal lipid intake, but higher dietary fiber intake, than the other three groups. Additionally, intake of animal protein (p<0.001) and intake of animal lipids (p<0.001) were significantly positively correlated with body weight, BMI, and total serum cholesterol in male collegians.

3.Effects of Some Drugs and Toxins on Positive Rate of Y-Body in Leukocytes of Rabbits.

This study was carried out to investigate the response of interphase Y chromosome to some drugs and toxins by observing the rate of leukocytes showing Y-body in the peripheral blood. The interphase Y chromosomes of blood leukocytes were stained with quinacrine mustard and the animals used were rabbits, rats and guinea pies. Y-bodies of leukocytes were studied in the animals as well as in man. Changes in positive rate of Y-body in leukocytes and total leukocyte count were observed in the rabbits administered with drug or toxin. The results concerning the rate of Y-body were as follows: 1. Y-bodies were present in the blood leukocytes of the animals. However positive rates in mononuclear and polymorphonuclear leukocytes were lower in animals that in man. The positive rate of Y-body was higher in mononuclear leukocytes than in polymorphonuclear leukocytes in the animals as in man. 2. Bacterial toxins such as typhoid, D.P.T. and cholera vaccines and anticancer drugs such as busulfan and endoxan reduced the positive rates of Y-body both in mononuclear leukocytes and in polymorphonuclear leukocytes. 3. Benzene known as bone marrow toxin reduced the positive rate of Y-body in mononuclear leukocytes, but not that in polymorphonuclear leukocytes. Quinine known as general protoplasmic poison reduced the positive rate of Y-body not only in mononuclear leukocytes but also in polymorphonuclear leukocytes. 4. Antibiotics such as tetracycline and chloramphenicol and steroid hormones such as estrogen, testosterone and prednisolone had no effects on the positive rate of Y-body both in mononuclear leukocytes and in polymorphonuclear leukocytes.

4.Studies on Vasectomy: X. Immediate Sterilization Technique Following Vasectomy.

A new technique of immediate sperm clearance has been developed by killing and irrigating residual sperm in the distal ductal system following vasectomy with a spermicidal solution at the time of the operation. For this, biochemical study, histologic study, and clinical study were conducted ,and by this, the need for postoperative contraception, and the need for postoperative semen testing were eliminated. Among the various spermicidal solutions such as tap water, distilled water,KMnO4,acetic acid, lactic acid, rivanol, alcohol, the KMnO4 solution has been proved to be the most suitable for the spermicidal solution, through biochemical study and histologic study. About 10ml of the KMnO4 solution,0.5% were injected into the distal ductal system under pressure, and killed redisual sperm immediately after the application. Number of redisual sperm in the distal ductal systems was significantly reduced by the KMnO4 application. No complication nor harmful effects could be observed after the procedure.

Ceramic-on-polyethylene components have better wear characteristics than metal-on-polyethylene components in total hip arthroplasty (THA), and thus, extensive wear resulting in penetration of the femoral head through the acetabular cup is rare after ceramic-on-polyethylene THA. However, several reports have been issued regarding catastrophic polyethylene failure in ceramic-on-polyethylene systems. Here, the authors report the first case of bilateral complete polyethylene wear failure with acetabular cup perforation after ceramic-on-polyethylene THA.

7.History of the Artificial Hip Joint.

For several centuries, orthopaedic surgeons have been given the task of managing the pain, limp, and gait disturbance that develop in association with various trauma and diseases of the hip joint. Progress has been made in development of surgical methods for the hip joint, and a recent concept was determined by introduction of the 'Clean Air Operating Enclosure' system and 'low frictional arthroplasty' in total hip arthroplasty (THA) by John Charnley, in 1960. THA has been performed as an exclusive method for hip joint reconstruction. We report on understanding of the current status from the past, present, and future trends with an introduction of the prior method of THA and a discussion of the history.

8.Eleven-Year Follow Up of Bilateral Revision Total Hip Arthroplasty: A Case Report

In the future, we will have more aged patients, more cases of bilateral total hip arthroplasty (THA) and more cases of bilateral revision THA. There are not, however, many reports of long-term follow-up of bilateral revision THA. We report a case of bilateral revision THA followed up for 11 years. A 74-year-old woman presented with bilateral painful hip in 1996. She had undergone left THA in 1984 and right THA in 1986 at another hospital. Her JOA(Japan Orthopaedic Association) scores on her first visit (right/left) were 32 points/31 points. Plain radiographs revealed a clear zone in her bilateral proximal femur. We diagnosed her as having looseness of bilateral THA. She required bilateral revision THA and underwent left revision THA (Charnley) in January 1997 (14 years after the original operation) and right revision THA (Charnley) in July 1997 (12 years after the original operation). After bilateral revision THA, we were able to follow up this patient for 11 years. She had no complaints about her bilateral hip joints, and X-ray examination showed no looseness. The JOA scores and range of motion illustrate the good result achieved for this case.

9.Fixing Leg Length Discrepancies after Total Hip Arthroplasty.

Leg length discrepancies are a common cause of patient dissatisfaction after total hip arthroplasty (THA). The equalization of limb lengths and restoration of the anatomic geometry of the hip to restore normal gait and function are the primary goals during THA. Patients recognize a leg length discrepancy when one leg is shorter than the other by 6 mm or longer than the other by 10 mm after THA. Outside of this range, several problems would occur. Therefore, we should try to maintain leg length during THA via preoperative and intra-operative planning.

10.Medially Directed TRUS Biopsy of the Prostate: Clinical Utility and Optimal Protocol.

PURPOSE: The objective of this study is to determine whether medially directed transrectal ultrasound (TRUS)-guided biopsy is necessary for detection of prostate cancer and for establishment of an optimal biopsy regimen tha is equivalent to a systematic 12 core biopsy. MATERIALS AND METHODS: A total of 302 patients underwent a TRUS-guided systematic 12 core biopsy consisting of both medial sextant biopsy obtained between the parasagittal line and midline and lateral sextant biopsy obtained between the parasagittal line and lateral border. We obtained cancer detection rates of various biopsy regimens that were produced from a systematic 12 core biopsy. RESULTS: Using a systematic 12 core biopsy, cancer was detected in 116 (38.4%) of 302 patients. No significant difference was observed between cancer detection rates of medial sextant biopsy and lateral sextant biopsy (33.8% versus 31.5%, p >.05). Biopsy regimens that were equivalent to the systematic 12 core regarding cancer detection rate included medially directed cores that were obtained from both medial portions of the apex. CONCLUSION: Both medially directed biopsy and laterally directed biopsy are necessary for detection of prostate cancer and for establishment of an optimal biopsy regimen.
