Observing a 16 years old male of classical Friedreich's ataxia with progressive ataxia, dysarthria, areflexia, loss of vibration and proprioception, Babinski sign, pes cavus, scoliosis, hyperglycemia, optic atrophy and hearing deficits, the nerve conduction studies and several evoked potential studies are performed. The sensory nerve potentials are markedly decreased in amplitude and the average technique has to be used to get the potentials, worse distally. The motor nerve functions, however, are normal. There is no consistent waves recorded on BAEP even with maximal stimulation bilaterally. VEP reveals prolonged latencies bilaterally but with normal amplitudes. On median nerve SEP stimulated at wrist, the amplitudes of potentials at Erb's point look decreased and no definite evoked potentials are recored at contralateral scalp. In contrast, the cervical evoked potentials are normal in amplitudes and latencies.

Down syndrome is the most common autosomal chromosomal abnormality characterized by mental and growth retardation, and by various typical features including prominent epicanthal fold, oblique palpebral fissure, flat nasal bridge, short and broad hand, wide toe interspace, etc. The overall incidence has been shown to be 1:800 deliveries, increasing with advancing maternal age. However, twin cases are extremely rare, and thus far only 500 cases were reported worldwide. We have recently observed 10-day-old male monozygotic twins with Down syndrome, born to a mother of 30 years of age with one normal child. Both have VSD confirmed by 2D-echocardiography, in addition to various typical features. Cytogenetic examination revealed that both have karyotypes of 47, XY, +21. This is the first report in Korea as the authors are aware of.

The purpose of this study was to identify the effect of foot reflexology on premenstrual syndrome and dysmenorrhea in female college students. The research design of this study was a quasi-experimental design. Of the forty female college students, twenty were assigned to the experimental group and, twenty to the control group. The data were obtained over 2 months(November 26, 2001 to January 31, 2002) from a nursing of C college located in S city. The instrument used to assess premenstrual syndrome and dysmenorrhea was Keele?s VAS(Visual Analogue Scale) and opening records. Subjects in the experimental group received foot reflexology for 6 times with 1 hours during 60 days, and subjects assigned to the control group did not receive foot reflexology. Data were analysed with percentage, mean, standard deviation, X(2)-test, unpaired t-test, and repeated measure ANOVA, using SAS Program. The results of the study are as follow, 1.The symptoms which the group of experimental and the group of control discomforts the most are sensitiveness(35%), abdominal pain (30%),lower abdominal pain (30%) and lumbago (20%). The method of relieve premenstrual syndrome and dysmenorrhea by which the subjects employ the most to solve their premenstrual syndrome and dysmenorrhea is the getting along by enduring(67.5%) and bed rest (32.5%). 2.The mean score of the premenstrual syndromes and dysmenorrhea before foot reflexology was 8.35, it was 4.16 at the first menstruation after foot reflexology and 3.25 at the second menstruation for the experimental group. 3.The relieved symptoms after foot reflexology was fatigue(50%), insomnia(40%), abdominal pain(35%), lower abdominal pain (30%) and constipation(30%). Foot reflexology was effective in improve the symptoms of the female college students who have the premenstrual syndrome and dysmenorrhea.

This study investigated the degree of knowledge achievement and self-confidence in aseptic technique as a part of the fundamental nursing practice classes. The subjects were 220 nursing students in a junior college in Chonnam and an applied Quasi -experimental research methodology was used thirty of them comprised an experimental group to examine learning according to the teaching method. Wata were collected over 80 days, from April to June 1988, analyzed using descriptive statistics, percentages, means, t-test, one way ANOVA Pearson's correlation coefficient and stepwise multiple regression with the SAS program. The conclusions are as follows. 1. The mean score of the degree of knowledge achievement of aseptic technique of the 220 students was 0.68(possible score 1.0) before the lecture but was 0.88 after lecture(p=.000). 2. The mean score for aseptic technique of the 220 students was 3.41(possible score 5.0) before practice but was 4.27 after practice(p=.000). 3. The correlation of self-confidence before and after practice was relatively low(r=.25, p=.000). 4. The mean score of the degree of knows edge achievement of aseptic technique in the experimental group(selected 30 students) was 0.72 before the lecture but was 0.90 after the lecture. 5. The mean score of self-confidence on aseptic technique in the experimental group(selected 30 students) was 3.18 before practice but was 4.32 after practice (p=.000). 6. The mean score of learning according to the type of teaching to aseptic technique for the experimental group(selected 30 students) was 23.2(possible score 34) after lecture and presentation, was 27.3 after learning through video tape, was 31.7 after presentation through practice, was 33.7 after first practical training, and was 34 after secondary practical training. As a result of this study, learning methods in the education of fundamental nursing skill effective for knowledge achievement and competence in practicing skill.

One month following carbon monoxide poisoning, a 39 year-old man developed incontinence, memory impairment, disorientation and emotional instability. He was hospitalized 7weeks later, and during hospitalization he exhibited myoclonic movements of the neck and lower limbs. He was given piracetam intravenously for 11 days. The myoclonus was significantly reduced by the third day of treatment and had disappeared by the seventh day. There was no recurrence following cessation of treatment.

There are widely recognized unusual remote neurological manifestations of internal malignancy. However, as far as the author's knowledge, the generalized myokymia associated with myokymic discharge has not been reported as paraneoplastic syndrome earlier. The patient was a 63-year-old female with generalized myokymia and paresthesia for 4months. The nerve conduction study revealed mild sensorimotor polyneuropathy and EMG study showed the generalized myokymic discharges, which were not changed by sleeping or brachial plexus block. The carbamazepine reduced the amount of myokymia and myokymic discharges but not the benzodiazepine. The diagnosis of lung cancer was confirmed by needle aspiration biopsy, and the cell types of lung cancer were mixed undifferentiated and small cell type. By the administration of anti-cancer drugs including cisplatin, the myokymia and myokymic discharges were completely disappeared. The patient died at 8 months after the diagnosis.

Sulpiride, a selective antagonist for adenylate cyclase-independent dopamine receptors, was administrated to 25 patients with blepharospasm and oromandibular dystonia(Meige's syndrome). Of the 25, 7 patients (28%) exhibited marked and lasting improvement with sulpiride and 12 patients (48%) showed mild or transient improvement. This favorable therapeutic response to sulpiride suggests that striatal glutamate underactivity may play a role in the pathophysiology of Meige's syndrome as a primary or secondary defect.

There has been considerable controversy concerning divergence paralysis, an entity described as early as 1883 by Parinaud. We recently observed a patient with divergence paralysis, who on CT scan presented a small hematoma in the tegmentum of the brainstem. This case may support the theory that the center for divergence exists in the upper brainstem.

It is generally considered that enteral feeding is superior to parenteral nutritional support. Thus enteral meal should be given whenever patients have proper gastrointestinal function to take enteral feeding. Because the morbidity and mortality for surgical jejunostomy have been reported as high as 50% and 10% respectively, direct percutaneous endoscopic jejunostomy has been developed to reduce the morbidity and mortality. A 55-year-old male patient, who was suffering from dysphagia and oropharyngeal aspiration, was transferred to the division of gastroenterology to be done permanent enteral feeding. His stomach was resected (subtotal gastrectomy with billroth II anastomosis) due to peptic ulcer hemorrhage 10 years before. We performed direct percutaneous endoscopic jejunostomy without any complication. Herein, we report a successful case.
