1.Masturbation and orgasm as migraine headache treatment: Report of a case

This is a report of an unusual case, where the patient used clitoral and/or vaginal masturbation and orgasm
for treatment of migraine attack. While the non drug treatment was effective, she subsequently developed
depression. The orgasm from masturbation, resulting in the rush of endorphins, probably relieved the
migraine. However, as demonstrated in our patient, the patient also had sexual aversion, tiredness,
feeling of shame and guilt, followed by depression from the alternative non-drug treatment.

2.Cluster-tic syndrome and bilateral internuclear ophthalmoplegia as the manifestation of multiple sclerosis

This report describes a 35-year-old female suffering from a cluster-tic syndrome and bilateral internuclear
ophthalmoplegia as the initial manifestation of multiple sclerosis. Magnetic resonance imaging of the
brain revealed multiple pontine hyperintense lesions. To our knowledge, there is no previous report
of multiple sclerosis presenting as cluster-tic syndrome and bilateral internuclear ophthalmoplegia in
the literature. The cluster headache attacks and peri-ocular neuralgiform pain resolved after treatment
with intravenous methyl-prednisolone and oral carbamazepine.

3.Protective effect of Genistein against N-nitrosodiethylamine (NDEA)-induced hepatotoxicity in Swiss albino rats

In the present study, we studied the effect of Genistein against the hepatotoxicity induced by N-nitrosodiethylamine (NDEA). NDEA is present in almost all kinds of food stuff and has been reported to be a hepatocarcinogen. The male rats were exposed to NDEA (0.1 mg/mL) dissolved in drinking water separately and along with 25, 50, 100 mg/mL of Genistein for 21 days. The activities of serum glutamic oxaloacetic transaminase (SGOT), serum glutamic pyruvic transaminase (SGPT), alkaline phosphatase (ALP) and lactate dehydrogenase (LDH) were measured in blood serum. Lipid peroxidation, protein carbonyl content, micronucleus frequency and DNA damage (Comet assay) were performed on rat hepatocytes. The results of the study reveal that the treatment of NDEA along with Genistein showed a significant dose-dependent decrease in the levels of blood serum enzymes i.e., SGOT, SGPT, ALP and LDH (Po0.05). The HE staining of histological sections of the liver also revealed a protective effect of Genistein. A significant dose-dependent reduction in the lipid peroxidation and protein carbonyl content was observed in rats exposed to NDEA (0.1 mg/mL) along with Genistein (Po0.05). The results obtained for the comet assay in rat hepatocytes showed a significant dose-dependent decrease in the mean tail length (Po0.05). Thus the present study supports the hepatoprotective role of Genistein.
4.Some ecological attributes of malarial vector Anopheles superpictus Grassi in endemic foci in southeastern Iran

Objective:To determine the bionomics and susceptibility status of the malarial vector Anopheles superpictus (An. superpictus) to different insecticides in the Sistan-Baluchestan province which has the highest malarial prevalence in Iran.
Methods:Different sampling methods, in addition to scoring abdominal conditions, were used to define the seasonal activity and endo/exophilic behavior of this species. In addition, the standard WHO susceptibility tests were applied on adult field strains.
Results: Most adult mosquitoes were collected from outdoor shelters. The peak of seasonal activity of An. superpictus occurred at the end of autumn. Most larvae were collected from natural and permanent breeding places with full sunlight and no vegetation. Blood feeding activities occurred around midnight. Compared with the abdominal conditions of adult mosquitoes collected indoors, the abdominal conditions of adult mosquitoes collected outdoors were gravid and semigravid. This species was suspected to be resistant to DDT, but was susceptible to other insecticides.
Conclusions:An. superpictus was present in almost all outdoor shelters, and the ratios of gravid, semigravid/unfed, and freshly fed confirmed that this species had a higher tendency to rest outdoors than indoors. This behavior can protect An. superpictus from indoor residual spraying in this malarious area. To the best of our knowledge, this is the first report on the susceptibility status of An. superpictus in Southeastern Iran. We do not suggest the use of DDT for indoor residual spraying in southeast Iran.
5.Carbamazepine and the QTc interval: any association?

Objective: To determine whether carbamazepine monotherapy in epilepsy patients is or is not associated
with prolongation of the QTc interval. Methods: This case-control study enrolled 100 consecutive
patients with generalized tonic-clonic seizures. Fifty patients were already taking carbamazepine for a
variable time, and the rest (n=50) were not on any antiepileptic drug. The QTc interval was calculated
after doing a resting 12-lead ECG examination on a single occasion. Results: Of the 50 patients who
had received carbamazepine, 11 patients displayed prolongation of their QTc interval, while 8 patients
out of the 50 in the control group had QTc interval prolongation after correction for gender; p value
=0.49, OR 1.36, 95% CI 0.54-3.29.
Conclusion: This study demonstrated no statistically signifi cant association between carbamazepine
monotherapy and prolongation of the QTc interval. Carbamazepine does not seem to prolong the QT
interval when used as monotherapy for epilepsy. The presence of prolonged QTc interval in such
patients should prompt a search for co-factors that prolong this interval, such as multiple medications,
electrolytes disturbances, structural heart disease, and congenital long QT interval syndromes.

7.Personality disorders in pateints with multiple sclerosis: Prevalence and association with depressive and anxiety disorders and clinical features

Objective: This study examines the current prevalence of mood, anxiety and personality disorders
in patients with multiple sclerosis (MS), the impact of personality disorders on clinical features of
MS and the existence of depressive and anxiety disorders. Methods: The study sample comprised of
55 patients with relapsing-remitting MS and 56 control subjects. Axis I diagnoses including mood
and anxiety disorders and personality disorders were ascertained by means of the Structured Clinical
Interview for the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition / Clinical
Version and the Structured Clinical Interview for DSM, Revised Third Edition Personality Disorders,
respectively. The Expanded Disability Status Scale (EDSS) was used to determine degree of disability
due to MS. Results: The prevalence of any mood, any anxiety and any personality disorders in patients
with MS were 40.0%, 38.2% and 45.5%, respectively, which are significantly higher than the control
subjects. Major depression, generalized anxiety disorder, avoidant personality disorder and obsessivecompulsive
personality disorder were more frequent in patients with MS compared to the controls. In
comparison to MS patients without personality disorders, MS patients with personality disorders had a
more frequent occurence of any mood disorder, any anxiety disorder, major depression, posttraumatic
stress disorders and MS attacks, and higher scoresof EDSS.
Conclusions: Mood, anxiety and personality disorders are frequently seen in patients with MS. The
results also suggest that personality disorders may negatively affect the existence of mood and anxiety
disorders as well as the clinical course of MS in the patients.

8.Biomechanical Properties of the Cranial Dura Mater with Puncture Defects: An In Vitro Study

OBJECTIVE: The primary aim of this investigation was to explore the nature of dura mater biomechanics following the introduction of puncture defect(s).METHODS: Twenty-eight dura mater specimens were collected during autopsy from the department of forensic medicine of the authors' institution. Specimens were divided randomly into one of four groups : group I (cranial dura mater; n=7), group II (cranial dura mater with one puncture defect; n=7); group III (cranial dura mater with two puncture defects; n=7), and group IV (cranial dura mater with three puncture defects; n=7).RESULTS: The mean±standard deviation tensile strengths of the dura mater were 8.35±3.16, 8.22±3.32, 7.13±1.77, and 6.94±1.93 MPa for groups I, II, III, and IV, respectively. There was no statistical difference between all groups. A single, two or more punctures of the dura mater using a 20-gauge Quincke needle did not affect cranial dura tensile strength.CONCLUSION: This biomechanical study may contribute to the future development of artificial dura mater substitutes and medical needles that have a lower negative impact on the biomechanical properties of dura mater.

9.Late development of a mandibular second premolar.

In this report, we present the case of a girl with delayed odontogenesis of a lower second premolar for which she was followed up for 8.5 years. Congenital absence of permanent mandibular second premolars was observed at the initial radiographic examination at 8 years and 1 month. One year later, during the treatment period, an unexpected odontogenesis of a right second premolar was diagnosed on follow-up radiography. The original treatment plan was revised and a new plan was successfully implemented. Th is unusual case showed that the orthodontist's clinical philosophy must be flexible because unexpected situations can arise, especially when treating growing patients.

10.Effect of different provisional cement remnant cleaning procedures including Er:YAG laser on shear bond strength of ceramics.

PURPOSE: The purpose of this study was to evaluate the effect of provisional cement removal by different dentin cleaning protocols (dental explorer, pumice, cleaning bur, Er:YAG laser) on the shear bond strength between ceramic and dentin. MATERIALS AND METHODS: In total, 36 caries-free unrestored human third molars were selected as tooth specimens. Provisional restorations were fabricated and cemented with eugenol-free provisional cement. Then, disc-shaped ceramic specimens were fabricated and randomly assigned to four groups of dentin cleaning protocols (n = 9). Group 1 (control): Provisional cements were mechanically removed with a dental explorer. Group 2: The dentin surfaces were treated with a cleaning brush with pumice Group 3: The dentin surfaces were treated with a cleaning bur. Group 4: The provisional cements were removed by an Er:YAG laser. Self-adhesive luting cement was used to bond ceramic discs to dentin surfaces. Shear bond strength (SBS) was measured using a universal testing machine at a 0.05 mm/min crosshead speed. The data were analyzed using a Kolmogorov Smirnov, One-way ANOVA and Tukey HSD tests to perform multiple comparisons (alpha=0.05). RESULTS: The dentin cleaning methods did not significantly affect the SBS of ceramic discs to dentin as follows: dental explorer, pumice, cleaning bur, and Er:YAG laser. CONCLUSION: The use of different cleaning protocols did not affect the SBS between dentin and ceramic surfaces.
