1.Assessment of agnique MMF killing effect on A. sundaicus in the shrimp farms of Ca Mau province

Insecticidal effects of Agnique MMF were investigated in the coastal brackish water shrimp farms in the Tan Thuan commune, Dam Doi district of Ca Mau province in 2000. The investigations were made in terracotta jars and shrimp ponds with the surface area 30m2 and 1000m2 each. Agnique MMF was found to have a high and fast killing effect on larvae of An.sundaicus at all three testing doses of 0.3ml/m2, 0.4ml/m2 and 0.5ml/m2. Especially larvae at instars of III, IV and pupae. However, the insecticide produced a low effect on Culex sitiens killing larvae of IV ins tar and only retarding larvae of I, II, III instar. The residual effect of Agnique MMF was found to be 14 days in the terracotta jars and 6 days in the ponds. In the direct observations, Agnique MMF was found to have no negative effects on rearing shrimps

2.SSRIs and SNRIs for Management of Hot Flushing.

For postmenopausal women who fear hormone therapy, women 60 years of age with continuous, severe hot flushing or women with a history of breast cancer, we should consider selective serotonin reuptake inhibitors (SSRIs), serotonin norepinephrine reuptake inhibitors (SNRIs) as therapeutic agents. Base on the results from a meta-analysis and clinical trials regarding hot flushing, paroxetine and the conetrolled-release formultation of paroxetine have been shown to effectively reduce hot flushing by 30~40% and 60~70%, respectively, and 13~41% more reductions as compared to placebo. Venlafaxine reduced hot flushes by 30~60% (133% reductions compared to placebo), and desvenlafaxine reduced hot flushes by 30~70%. Fluoxetine and citalopram were shown to be less effective than paroxetine and venlafaxine, by 20% (113% reductions compared to placebo) and 40~50%, respectively. Sertraline reduced hot flushes 3~18% compared to the placebo group, but was considered ineffective. Citalopram (20 mg), paroxetine (10 mg), venlafaxine (37.5~150 mg), and desvenlafaxine (100~200 mg) not only reduced vasomotor symptoms, but demonstrated additional beneficial outcomes with respect to sleep disturbances, mood, the vigor index, and improved quality of life. Citalopram (20 mg), fluoxetine (20 mg), paroxetine (10 mg), venlafaxine (75~150 mg), and desvenlafaxine (150 mg) are recommended at the corresponding doses after weighing the risks and benefits of these medications. SSRIs and SNRIs were shown to interrupt the conversion of tamoxifen into the active metabolite, endoxifen, and thus SSRIs and SNRIs must not be used in breast cancer patients who are taking tamoxifen. Paroxetine suppressed vasomotor symptoms most potently, followed by fluoxetine, sertraline, citalopram, and venlafaxine.

3.Venlafaxine-Induced Acute Toxic Hepatitis.

Venlafaxine is among the most widely prescribed antidepressants. It is extensively metabolized to O-desmethylvenlafaxine via cytochrome P450 (CYP) 2D6. We report a case of acute toxic hepatitis resulting from venlafaxine in a 54-year-old woman with pain disorder. During venlafaxine treatment, laboratory tests revealed elevated liver enzymes with a maximum of 169 IU/L for aspartate transaminase (AST) and 166 IU/L for alanine transaminase (ALT). AST and ALT levels returned to normal after 6 days of discontinuation of venlafaxine. The patient was finally diagnosed with acute toxic hepatitis through liver biopsy. This case indicates the importance that clinicians should be aware of the hepatotoxicity of venlafaxine in practice.

4.Butanol production from corn stover hydrolysate with in-situ liquid-liquid extraction.

Butanol production from corn stover hydrolysates (CSH) with in-situ liquid-liquid extraction was studied to enhance the production and reduce the fermentation cost. Oleyl alcohol was selected as the suitable solvent and added at the initial fermentation time with the ratio of 1:1 (oleyl alcohol: fermentation broth, V/V). Under this condition, butanol and ABE from CSH with 32.1 g/L total sugars were 3.28 and 4.72 g/L, which were 958.1% and 742.9% higher than those of the controls, respectively. Butanol and ABE production from CSH of 49.7 g/L total sugars after detoxification by ion exchange resin D301 coupled with extraction fermentation were 10.34 g/L and 14.72 g/L with an ABE yield of 0.31 g/g (g ABE/g utilized sugar), which were equal to those of glucose and xylose mixture fermentation. The detoxification and extraction fermentation technology of cellulosic butanol production would provide a crucial technical support to the industrialized production of cellulosic butanol.

5.Distribution behavior of lipophilic drugs in the oil-in-water microemulsions.

Distribution behavior of lipophilic drugs in the oil-in-water (O/W) microemulsions was studied. Fluorescence spectra analysis was performed to investigate the effect of the compositions of microemulsions on the fluorescence spectra of armillarisin and ofloxacin which were used as the model drugs. The fluorescence spectra of the model drugs in the microemulsions with various amount of the compositions were compared. The results showed that the armillarisin were both localized in the interfacial film of microemulsion systems with both phenylmethanol and PEG 400 as the co-surfactants, separately. Ofloxacin was localized in the interfacial film of microemulsion systems with Gradamol GTCC as the oil phase, but in the oil pool of microemulsion systems with oleic acid/olive oil (OA/OO) (1:1) as the oil phase. Besides, it was found that the drug would have the tendency to locate in the microenvironment where the composition with the largest solubility to model drug is located, and its actual localized position would be dependent on the amount of this composition. The results indicate that the localized region of lipophilic drug in the O/W microemulsion systems is related with the solubility of the model drug in various compositions.

6.Forensic science meets clinical pharmacology: pharmacokinetic model based estimation of alcohol concentration of a defendant as requested by a local prosecutor's office.

Drunk driving is a serious social problem. We estimated the blood alcohol concentration of a defendant on the request of local prosecutor's office in Korea. Based on the defendant's history, and a previously constructed pharmacokinetic model for alcohol, we estimated the possible alcohol concentration over time during his driving using a Bayesian method implemented in NONMEM®. To ensure generalizability and to take the parameter uncertainty of the alcohol pharmacokinetic models into account, a non-parametric bootstrap with 1,000 replicates was applied to the Bayesian estimations. The current analysis enabled the prediction of the defendant's possible blood alcohol concentrations over time with a 95% prediction interval. The results showed a high probability that the alcohol concentration was ≥ 0.05% during driving. The current estimation of the alcohol concentration during driving by the Bayesian method could be used as scientific evidence during court trials.

7.Experiences of treatment for Alopecia Areata by smearing HDM alcohol and Chloral salicilic solution

35 patients with Alopecia Areata (18 females, 17 males) were treated at Military hospital N0 108 from March 2002 to March 2005, age from 4 to 58 years old. All patients were treated by HDM alcohol and Chloral salicylic solution 2% in 4 months. The results showed that: after first month of treatment, 77.14% of patients had grew hair root; after 2 months, there was only 8.57% of patients had not yet grew hair root. After 3 months, the rate of grew hair with different levels was 100%, among which 60% had normal grew hair, and after 4 months 82.86% of patients had grew hair as normally, 17.14% had grew hair from 2 cm. Side effects were observed in 10 patients (28.57%) involved erythema 8.57%, itch 14.29% and deciduous skin 5.71%.

PURPOSE: This study analyzed the prognostic factors affecting admission in acute alcohol-intoxicated traumatic brain injury (TBI) patients visiting the emergency room. METHODS: A multicenter, retrospective observational study was conducted on 821 acute alcohol-intoxicated adult trauma patients, who visited 10 university hospital emergency centers from April to November 2016. The primary outcome was hospital admission. The secondary outcome was in-hospital mortality. RESULTS: One hundred sixty-eight patients diagnosed with acute alcohol-intoxicated TBI were analyzed. The increase in blood alcohol concentration was associated significantly with a mild decrease in admission (adjusted odds ratio, 0.993; 95% confidence interval, 0.989 to 0.998; p=0.01). Moderate to severe TBI patients showed a significant increase in admission compared to mild TBI patients (adjusted odds ratio, 12.449; 95% confidence interval, 3.316 to 46.743; p < 0.001). CONCLUSION: This study showed that the admission was inversely correlated with the blood alcohol concentration and is correlated directly with the increase in the severity in TBI. Therefore, emergency physicians may be required to identify the severity of TBI rapidly and accurately in acute alcohol-intoxicated trauma patients visiting the emergency room.

9.Accidental Choking Deaths with Octopus minor and Octopus ocellatus

In Korea, small octopus (Octopus minor) and webfoot octopus (Octopus ocellatus) are food items and fatal laryngeal choking due to ingestion of live octopus is not uncommon. We recently encountered two autopsy cases of accidental choking on small octopus and webfoot octopus. Case 1 involved a 58-year-old fisherman who ingested two live webfoot octopuses in his fishing boat and collapsed. He was immediately taken to the hospital but died. During autopsy, one of the webfoot octopuses was found between his pharynx and esophagus; it was obstructing the epiglottis and upper esophagus. His blood alcohol concentration was 0.140%. Case 2 involved a 55-year-old man who ingested an intact body part of a small octopus and was found dead in his house. He had a history of cerebral infarction and angina pectoris. During autopsy, an intact body part of the small octopus was found to be lodged in the laryngeal inlet.

10.Efficacy of pulmicort suspension plus salbutamol and ipratropium bromide for management of acute asthma exacerbation in children: a comparative study.

OBJECTIVETo evaluate the efficacy of 3 commonly used protocols for management of acute exacerbation of asthma in children.
METHODSTotally 113 asthmatic children were randomized into 3 groups. In group A (53 cases), the children were treated with inhalation of nebulized budesonide suspension plus salbutamol and ipratropium bromide twice daily for 5 days; in group B (41 cases), budesonide plus salbutamol and ipratropium aerosol was administered, and in group C (29 cases), dexathmisone plus aminophylline injection was given once daily for 5 days. All the children received basic treatment with fluid infusion, antibiotics or/and anti-virus medications.
RESULTSThe children in both groups A and C showed effectively controlled asthma attack, with significant differences in the therapeutic effects (P>0.05). In contrast, only a few children showed improvement in group B, suggesting the ineffectiveness of the treatment.
CONCLUSIONNebulized medicine is one of the best means for management of acute asthma exacerbation in children, and inhalation of budesonide suspension plus salbutamol and ipratropium bromide can effectively relieve the asthmatic symptoms in these children with good compliance and convenient administration.
