BACKGROUND: In Korea, since 1990, in an effort to reduce the transmission of non-A, non-B hepatitis, all blood donations with alanine aminotransferase (ALT) levels above 65 IU/L are discarded. In 2012, 64.8% of the disposed blood units at the Korean Red Cross blood centers were due to high ALT levels. Pre-donation ALT testing might prevent unnecessary blood donation and save related expenses. We evaluated performance of point-of-care test (POCT) devices for pre-donation ALT screening. METHODS: ALT levels by four ALT POCT devices (Mission C100, Acon; Reflotron Plus, Roche; Labgeo PT10, Samsung; and FDC NX500, Fujifilm) were compared with venous blood results using laboratory chemistry analyzers (AU series, Beckman Coulter Inc.). Intraclass correlation coefficients (ICCs), sensitivity (ability to detect ALT > or =65 IU/L), and specificity (ability to detect <65 IU/L) for each method were calculated. RESULTS: Compared with the laboratory analyzers, the ICCs of ALT measurements by Mission C-100, Reflotron Plus, Labgeo PT10, and FDC NX500 were 0.96 (95% confidence interval (CI): 0.95~0.97), 0.99 (95% CI: 0.99~0.99), 0.98 (95% CI: 0.98~0.98), and 0.94 (95% CI: 0.91~0.96), respectively. The sensitivity was 80.95% for Mission C-100, 83.33% for Reflotron Plus, 78.57% for Labgeo PT10, and 97.62% for FDC NX500. The specificity was 99.13% for Mission C-100, 100.00% for Reflotron Plus, 99.78% for Labgeo PT10, and 98.26% for FDC NX500. CONCLUSION: The ALT POCT devices showed almost perfect agreement with the laboratory analyzers and could be useful for pre-donation ALT screening. However, before implementing ALT POCT devices, cost-effectiveness analyses should be performed.

OBJECTIVETo access the capability of 1H nuclear magnetic resonance (NMR) -based metabonomics in the evaluation of graft function in the perioperation period of liver transplantation.

METHODSPlasma samples of 15 male primary hepatic carcinoma patients were collected for clinical biochemical analysis and 1H NMR spectroscopy 1 day before operation, 1 day and 1 week after the operation. The NMR data were analyzed using principal component analysis.

RESULTSMetabonomic analysis indicated that, compared with those before operation, blood concentrations of valine, alanine, acetone, succinic acid, glutamine, choline, lactate, and glucose increased significantly 1 day after transplantation. One week later, the levels of lipids and choline increased notably, while those of glucose and amino acids decreased. Principal component analysis showed significant difference between metabolic profiles of plasma samples of variant periods of liver transplantation, due to the variation of the levels of glucose, lipids, lactate, and choline. A good agreement was observed between clinical chemistry and metabonomic data.

CONCLUSIONSMetabonomic analysis can clearly identify the difference between the plasma samples of primary hepatic carcinoma patients at different time during the perioperation period of liver transplantation. It therefore may be a promising new technology in predicting the outcomes of liver transplantation.

OBJECTIVETo study the toxicity of water extracts from the fruits of Evodia Fructus in different producing areas.

METHODCompare the toxicity of the extracts from different Evodia Fructus on mice by the methods of acute and subacute toxicity test. The mice were given the extracts for 1 d to test the maximal tolerance dose (MTD) or maximal dose and observe the acute toxic symptoms; The mice were given the extracts for 15 d and then detected the level of serum alanine aminotransferase (ALT), aspartate aminotransferase (AST) and triglyceride (TG). The liver index was calculated, and the liver histological changes were investigated.

RESULTThe MTD of water extracts from the fruits of Evodia Fructus is 62, 44.8, 35.84 g x kg(-1); the MTD of Evodia Fructus is 56, 44. 8, 35.84 g x kg(-1); the maximal dose of Evodia Fructus is 60, 54, 45 g x kg(-1). The toxic symptoms of the mice which had been given the nine samples were almost consistent. Compared with the control group in subacute toxicity test, the level of serum ALT and the liver index were all increased. The liver histological were changed.

CONCLUSIONWhen water extracts from the fruits of Evodia Fructus are given to mice one or more times. It may be toxic and induce liver damage. There is no significant correlation between the toxicity and Evodia orgins, while the toxicity seems to be more closely related to the producing area.

In this article, canonical correlation analysis was used to explore the relationship between the toxicity-attenuating effect and the variation of chemical contents in rhubarb caused by processing. With quasi-acute toxicity test, the difference of hepatic and renal toxicity to mice with the processed materials of rhubarb was researched. The chemical contents of anthraquinones and tannins in rhubarb were measured by UV-vis spectrophotometry and high performance liquid chromatography. The results showed that there were toxic effects to liver and kidney in mice after repeated intragastric administration of rhubarb and its processed materials for 14 days at a dosage of 76 g x kg(-1). The toxic effect of processed materials was much lower than crude drug. With canonical correlation analysis, the sequence of the hepatic and renal toxicity of chemical contents in rhubarb were found as follows: total anthraquinone glycosides (AQGs) > tannins (Tns) > total anthraquinones (AQs); aloe-emodin (AE) > physcione (Ph) > rhein (Rn) > emodin (Ed) > chrysophanol (Ch) and AEG > PhG > ChG > EdG > RnG of glycosyl-anthraquinone. It could be concluded that processing would attenuate the toxicity of crude drug of rhubarb. The toxicity-attenuating effect might be correlated to the decline of the contents of both anthraquinone glycosides and tannins, especially the aloe-emodin glycoside and physcione glycoside. The results also suggested that the serum alanine aminotransferase (ALT) and creatine (CREA) would be useful to monitor the hepatic and renal toxicity of rhubarb.

OBJECTIVEA study was carried out to investigate the effects of seed oil and sarcocarp oil of Hippophae rhamnoides on rats with experimental hepatocirrhosis, and comparison between the two.

METHODA rat model of experimental hepatocirrhosis was set up by feeding CCl4. Different concentration of seed oil and sarcocarp oil of H. rhamnoides were feed to those rats for 45 d, then the changes of activity of ALT in serum and SOD in liver were measured.

RESULTBoth of seed oil and sarcocarp oil can control the increase of ALT in serum and the decrease of SOD evidently, and the effect of seed oil was turn out to be a little better than sarcocarp oil.

CONCLUSIONSeed oil was more effective than sarcocarp oil of H. rhamnoides in alleviating liver injury caused by CCl4.

BACKGROUND: Nonalcoholic fatty liver disease (NAFLD) has recently been recognized to be one of the leading causes of chronic liver disease. Obesity and type 2 diabetes are associated strongly with NAFLD. Although often used to detect liver disease, sometimes the cause of elevated Alanine Transaminase (ALT) activity was unclear. In the absence of other causes, the author examined whether BMI and metabolic markers of NAFLD can explain the cause of abnormal ALT in Korean. METHODS: From April 2002 to November 2003, 9997 persons visited health promotion center of Kyungpook National University Hospital and took health screening. After excluding persons with excessive alcohol consumption, abnormal gamma-GTP, hepatitis B, hepatitis C, 8379 persons were included in this study. All patients underwent anthropometric measures, blood chemistry analysis and abdominal ultrasonographic exam. RESULTS: Elevated ALT levels were found in 10% (835 persons) of the all subjects. In univariate analysis, factors associated with elevated ALT levels (p<0.05) included male sex, younger age (20-40 years), higher Body Mass Index (BMI), higher blood pressure, higher fasting blood glucose, higher triglyceride, lower HDL and more fatty change in ultrasonography. The proportion of elevated ALT with fatty change in ultrasonography was 57% and with higher BMI (>or=23 kg/m2) was 79% in abnormal ALT group. In multivariate logistic regression study, total cholesterol and blood pressure were eleminated but sex, BMI, triglyceride level, ultrasonographic fatty change were strongly associated with elevated ALT level (p<0.01). CONCLUSION: In this study, unexplained ALT elevation was strongly associated with obesity, adiposity and other features of metabolic syndrome, and thus may represent nonalcoholic fatty liver disease. However, more research is needed for the definite clinical determinants of nonalcoholic fatty liver disease in Korean.

BACKGROUND: We evaluated the analytical performance of Wako assays for albumin, alanine aminotransferase (ALT), aspartate aminotransferase (AST), blood urea nitrogen (BUN), complement C3 and C4, calcium, creatine kinase (CK), C-reactive protein (CRP), direct bilirubin (DBIL), iron, gamma-glutamyl transferase (GGT), HDL cholesterol (HDLC), inorganic phosphorus (IP), LDL cholesterol (LDLC), total bilirubin (TBIL), total protein (TP), and uric acid (UA), as well as the performance of Sekisui assays for albumin, BUN, calcium, CRP, HDLC, IP, LDLC, TP, and UA by using Hitachi LABOSPECT 008 (Hitachi High-Tech Co., Japan). METHODS: Liquid Assayed Multiqual Control (Bio-Rad Laboratories, USA) and pooled patients' sera were analyzed for 20 days. Wako linearity material (Wako Pure Chemical Industries, Ltd., Japan) and Sysmex Interference Check A Plus kit (Sysmex Co., Japan) were used to test linearity and interference, respectively. Concentrations of the target analytes were measured using Hitachi LABOSPECT 008 in 100 residual patient specimens and compared to those in Pureauto S series reagent (Sekisui Medical, Japan), which were measured using Hitachi 7600 (Hitachi High-Tech Co., Japan). RESULTS: Total coefficients of variation (CVs) for the tested analytes were 0.91-9.26% in Wako and 1.04-7.46% in Sekisui assays. Linearity was demonstrated up to the highest concentration within the analytical range in all the assays except for Wako albumin and Sekisui TP. Wako and Sekisui albumin, BUN, CRP, HDLC, and LDLC assays, and in Wako C3, C4, calcium, and UA assays showed no interference with the test concentrations used. All the tested assays, except for Wako AST, LDLC, and TP, and Sekisui calcium and TP, demonstrated comparability with comparative method for at least one medical decision level. CONCLUSIONS: Our study results showed that the analytical performances of Wako and Sekisui chemistry assays evaluated using Hitachi LABOSPECT 008 had appropriate analytical performance for clinical use.

The aims of this study were to prepare Cu(2+)-loaded montmorillonite (Cu-MMT) and investigate its bactericidal activity and mechanism. Cu-MMT was prepared by the method of ion exchange reaction. The structure and surface characteristic of Cu-MMT were determined. Minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of Cu-MMT against the strain of Escherichia coli were determined. The activities of intracellular enzyme in bacterial solution were measured, and the morphology of E. coli was observed during the interaction between Cu-MMT and bacteria. The results showed that treatment with Cu2+ increased cation exchange capacity of montmorillonite, but specific surface area and surface negative charge density were decreased. The MIC and MBC of Cu-MMT against the tested E. coli were 0.16 and 0.64 mg x m(L(-1), respectively. Cu-MMT could destroy bacterial cellular membrane and then resulted in leakage of intracellular enzymes such as asparate aminotransferase, lactate dehydrogenase and alanine aminotransferase. These suggest that Cu-MMT has a strong bactericidal activity. The bactericidal mechanism of Cu-MMT may be that bacteria are adsorbed by Cu-MMT, and then morphology and permeability of cellular membrane are changed. This leads to an efflux of intracellular contents and the death of bacteria.

OBJECTIVETo observe the combination effect of doxorubicin and Citrus hystrix (kaffir lime's) peel ethanolic extract (ChEE) on blood serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) activity and cardio-hepato-histopathology of female Sprague Dawley rats.

METHODSDoxorubicin and ChEE (5 rats per group) were administered in five groups of 3 rats each for 11 d. Group I: doxorubicin (dox) 4.67 mg/kg body weight; Group II: dox+ChEE 500 mg/kg body weight; Group III: dox+ChEE 1 000 mg/kg body weight; Group IV: ChEE 1 000 mg/kg body weight; Group V: untreated (control).

RESULTSChEE repaired cardiohistopathology profile of doxorubicin induced cardiotoxicity and hepatotoxicity rats, but did not repair neither hepatohistopathology profile nor reduce serum activity of ALT and AST.

CONCLUSIONChEE has potency to be developed as cardioprotector agent in chemotherapy.
