1.A Case of Aortic Replacement for a Patient with Bilateral Internal Carotid Stenoses

A 71-year-old man was given a diagnosis of saccular aneurysm of the aortic arch (maximum 48mm in diameter) at the age of 68. When he was 69 years old, he began to take steroids for autoimmune hepatitis (AIH). The following year, the aneurysm was enlarged to 52mm. Further examinations showed the aneurysm to extend to the ostium of the left subclavian artery. Since he had transient ischemic attacks, ultrasonography of the carotid arteries was performed. Bilateral internal carotid stenoses were detected, however, cold Xe CT showed an almost normal pattern of cerebral blood flow. We decided that operation was feasible using retrograde cerebral perfusion (RCP). Liver dysfunction due to AIH improved, and his steroid dosage was tapered. Using RCP, the no-touch technique and the elephant trunk procedure, he underwent the replacement of ascending aorta and aortic arch and was discharged without major complications. RCP and the no-touch technique might enable safer operations on patients with carotid stenoses.
2.A Case of Papillary Fibroelastoma of the Left Ventricular Septum Complicated with a Rheumatic Valve.

A 50-year-old man was referred to our hospital with a tumor in the left ventricle. He had suffered from rheumatic fever when 14 years old. He had shown signs of chronic heart failure due to atrial fibrillation and rheumatic valves (ASr, MSr) for 10 years. There was a history of unaccountable fever and rash, so infective endocarditis was suspected and echocardiography was performed. It showed a homogeneous mass with a diameter of approximately 10mm, fixed directly to the left ventricular septum 20mm below the aortic valvular ring. At operation, the tumor was excised together with endocardium and a part of the muscular coat. The rheumatic aortic and mitral valves were replaced with a 21mm SJM AHP and a 27mm SJM MTK mitral valve, respectively. Tricuspid annuloplasty (TAP) (De Vega 29mm) was also performed. Histopathological examination of the tumor revealed benign papillary fibroelastoma. It suggested that the tumors were secondary to mechanical wear and tear, and represent a degenerative process due to rheumatic valve disease.
3.Efficacy of a Novel Recording Method in Facilitating Rapid Documentation of Disaster Rehabilitation Support Activities

Objective:An efficient disaster relief service necessitates the documentation and dissemination of information acquired from disaster response-related activities. However, recording and summarizing information pertaining to rehabilitation support activities is time-consuming and a pressing concern;therefore, mark-sheet forms have been introduced to reduce the time required for this process. Therefore, this study aimed to examine whether mark-sheet forms can facilitate a more accurate and rapid recording of information than conventional descriptive recording forms.Methods:Fifty physical therapists affiliated with the Osaka Physical Therapists Association with no history of performing disaster rehabilitation support activities were recruited. Participants were randomized into descriptive recording form and mark-sheet form groups. Individuals in both groups were instructed to document the information gathered by watching simulated videos of the rehabilitation support activities on their designated recording forms. A statistical analysis compared the accuracy and time required to record the data obtained from the 10 simulation videos between groups.Results:In all 10 scenarios, the recording time was significantly shorter (p<0.05) for the mark-sheet form than descriptive recording form group. However, no discernible intergroup difference was observed in recorded content accuracy.Conclusion:The use of mark-sheet versus descriptive recording forms facilitated more rapid documentation of disaster rehabilitation support activities.
4.Probiotic Yeast from Miso Ameliorates Stress-Induced Visceral Hypersensitivity by Modulating the Gut Microbiota in a Rat Model of Irritable Bowel Syndrome

Background/Aims:
Recent studies indicate that probiotics, which have attracted attention as a treatment for irritable bowel syndrome, affect intestinal homeostasis. In this study, we investigated whether Zygosaccharomyces sapae (strain I-6), a probiotic yeast isolated from miso (a traditional Japanese fermented food), could improve irritable bowel syndrome symptoms.
Methods:
Male Wistar rats were exposed to water avoidance stress (WAS). The number of defecations during WAS and the visceral hypersensitivity before and after WAS were evaluated using colorectal distension. Tight junction changes were assessed by Western blotting. Some rats were fed with strain I-6 or β-glucan from strain I-6. Changes in the intestinal microbiota were analyzed.The effect of fecal microbiota transplantation after WAS was evaluated similarly. Caco-2 cells were stimulated with interleukin-1β and tight junction changes were investigated after coculture with strain I-6.
Results:
The increased number of stool pellets and visceral hypersensitivity induced by WAS were suppressed by administering strain I-6. The decrease in tight junction protein occludin by WAS was reversed by the administration of strain I-6. β-Glucan from strain I-6 also suppressed those changes induced by WAS. In the rat intestinal microbiota, treatment with strain I-6 altered the β-diversity and induced changes in bacterial occupancy. Upon fecal microbiota transplantation, some symptoms caused by WAS were ameliorated.
Conclusions
These results suggest that traditional fermented foods such as miso in Japan are valuable sources of probiotic yeast candidates, which may be useful for preventing and treating stress-induced visceral hypersensitivity.
