1.The Kashima Scan Study 2: a protocol for a prospective observational cohort study of cerebral small vessel disease in neurologically healthy adults.

BACKGROUND:
Our previous observational cohort study, the Kashima Scan Study (KSS), identified associations between lifestyle, cerebral small vessel disease (SVD) as detected by magnetic resonance imaging of the brain, and disease outcomes including cognitive impairment and vascular diseases. However, established modifiers of the outcomes such as genetic background, drinking and exercise habits, and socioeconomic status were not considered. Regarding genetic factors in particular, the ALDH2 rs671 variant, East Asian-specific diversity, and APOE status are expected to have strong effects. The aim of KSS-2 is to examine the interactions of genetic background, lifestyle factors including drinking habit, socioeconomic status, and/or SVD markers for cognitive impairment, vascular disease, and death.
METHOD:
The KSS-2 is a prospective regional observational study of a healthy Japanese cohort that will clarify lifestyle habits to better maintain brain health from midlife by genotype. Japanese adults who underwent brain health checkups at their own expense are enrolled and will be followed-up for 10 years. We will extend the protocol of the KSS to include genetic background and potential confounding factors, including lifestyle (including drinking and exercise habit) and socioeconomic status, and perform survival analyses. The study outcomes are cognitive impairment, vascular events, and death.
RESULTS:
We enrolled 908 healthy adults (mean age 64.2 years; range 35 to 84 years; 41% male) from September 1, 2018 until December 31, 2024.
CONCLUSION
This study will provide important insights into the development of individualized health intervention strategies.

2.Intervention by PCT and Problems Awaiting Solution at Anjo Kosei Hospital: Usefulness of Rounds of Patients Receiving Opioids

In order to intervene in the management of pain of cancer from an early stage. Our palliative care team (PCT), including pharmacists, makes the ward rounds (screening rounds) of the patients receiving opioids at our hospital. The purpose of this study was to analyze the effects of screening rounds activity by the PCT and its current problems, and to explore how to resolve the problems. We retrospectively studied the records of 196 patients who had receivede interventions by the PCT, with regard to intervention status and prescription proposal (228 subjects) about drug therapy by us. Study groups were as follows: 103 patients to whom interventions were deliveed at the request of medical doctors (intervention request group) and 93 patients who had interventions by the PCT after PCT-screening rounds (screening group). PCT-screening rounds caused to increase the number of interventions by the PCT. After PCT-screening rounds, the cases of intervention started by the request of medical doctors, who had given no heed to PCT intervention, also increased in numher. In this study, some problems with palliative intervention were also brought to light. Even in the screening group where the PCT largely intervened, 33% of prescription proposal by the PCT was ignored. This problem may be, at least in part, due to inadequate communication between PCT and ward staff through an electronic medical recording card, leading to poor relationship between PCT and ward staff. In the future, the PCT needs to work cooperatively with ward staff through direct communication such as medical conference to perform better intervention.
3.Effect of self-weight exercise on changes in the bone mass in young women

In this study, the influence of self-weight exercise on changes in the bone mass was evaluated in young Japanese women. The subjects were 69 female university students(age: 21.20±0.30). They were assigned to an exercise intervention (Ex, n=26) or control(C, n=43) group according to their requests. Group Ex performed two sets of 10 sit-up, 10 back-extensions, and 10 squats every day for six months. The height, weight, body fat percentage (TANITA, TBF102), bone mass (quantitative ultrasound, Lunar, A-1000EXPRESS), and urinary deoxypyridinoline (DPD) were measured, and their previous and present exercise habits were examined. Group Ex showed no differences in the bone mass or DPD after the training. Group C, on the other hand, showed a significant decrease in the bone mass (p<0.01) and an increase in DPD (p<0.01) after the training. It was suggested that the bone mass can be possibly maintained through self-weight exercise.
