1.Chemical constituents of Trichosanthes kirilowii Maxim.

To study the chemical constituents of Trichosanthes kirilowii Maxim., chromatographic methods such as D101 macroporous resin, silica gel column chromatographic technology, Sephadex LH-20, octadecylsilyl (ODS) column chromatographic technique and preparative HPLC were used and nine compounds were isolated from a 95% (v/v) ethanol extract of the plant. By using spectroscopic techniques including 1H NMR, 13C NMR, 1H-1H COSY, HSQC and HMBC, these compounds were identified as 5-ethoxymethyl-1-carboxyl propyl-1H-pyrrole-2-carbaldehyde (1), 5-hydroxymethyl-2-furfural (2), chrysoeriol (3), 4'-hydroxyscutellarin (4), vanillic acid (5), alpha-spinasterol (6), beta-D-glucopyranosyl-a-spinasterol (7), stigmast-7-en-3beta-ol (8), and adenosine (9), separately. Among them, compound 1 is a new compound, and compounds 3, 4 and 5 are isolated from the genus Trichosanthes kirilowii Maxim. for the first time.
2.Effect of CO2 pneumoperitoneum on the expression levels of CX-CL12 and CXCR4 in nude mice xenografts from human ovarian cancer SKOV3 cells

Objective:To investigate the effect of the expression levels of CXCL12 and CXCR4 of carbon dioxide (CO2) pneu-moperitoneum on nude mice xenografts from human ovarian cancer SKOV3 cells in the same pressure at different times. Methods:A total of 40 animal models of human ovarian cancer SKOV3 cell-bearing nude mice xenografts were established. The animals were ran-domly divided into four groups (n=10), namely, the simple anesthesia group, 0.5 h laparotomy group, and 0.5 and 1 h CO2 pneumoperi-toneum groups. After the aforementioned treatments in the groups, the expression levels of CXCL12 and CXCR4 mRNAs of tumor tis-sues were detected by reverse transcription-polymerase chain reaction (RT-PCR), whereas those of the CXCL12 and CXCR4 proteins were detected by Western blot. Results:The 40 animal models of human ovarian cancer SKOV3 cell-bearing nude mice were success-fully established. After the treatment, RT-PCR results reveal that the CXCL12 mRNA expression was significantly higher in the 1 h CO2 pneumoperitoneum group than that in the other three groups. However, no differences were observed in the CXCR4 mRNA expression of each group. Western blot analysis also obtained the same results. Conclusion:The expression level of CXCL12 in SKOV3 cells of the 1 h CO2 pneumoperitoneum group increased, which possibly promoted the growth of peritoneal tumors, and had a slight effect on tu-mor metastasis.
3.Suppressive effect of nickel-titanium stent on macrophage cells reactivity to lipopolysaccha-ride(LPS) from gram-negative bacterium

Objective To study the effect of nickel-titanium stent(NTS) and consequent anti-allergy dexamethasone therapy on macrophage cells reactivity to lipopolysaccharide (LPS) from gram-negative bacterium .Methods The macrophage cell line Raw 264 .7 and dexamethasone-pretreated Raw264 .7 were co-cultured with NTS for 4 days ,and stimulated with LPS for 24 hours .The surface marker CD molecules of CD80 ,CD86 and FasL were detected with flowcytometr method ,the supernant cytokine production of proinflammatory cytokines IL-6 and TNF-αwas valued with ELISA method ,and intracellular inflammatory signal pathway acti-vation of NF-κB ,GAS ,ISRE and STAT3 was checked with signal molecule specific promoter lunciferase analysis .Results The stent pre-treatment improved LPS-mediated CD80 expression ,suppressed FasL production ,decreased IL-6 secretion and NF-κB ac-tivation ,the results have statistical significance (P<0 .05) .The dexamethasone treatment improved stent-mediated up-regulated ex-pression of CD80 ,FasL and TNF-α,and suppressed the activation of intracellular inflammatory signal pathway of NF-κB ,ISRE and STAT3 ,the results have statistical significance(P<0 .05) .Conclusion NTS inhibit macrophage cells Raw264 .7 react to TLR4 ag-onist LPS ,and dexamethasone treatment improved the function .
4.Significance of T-cell dot test and combined detection of adenosine deaminase in tuberculous pleurisy at different ages

Objective To analyze the clinical value of combined detection of tuberculous T cell enzyme-linked immuno spot assay (T-SPOT.TB) and adenosine deaminase (ADA) in tuberculous pleurisy patients of different ages.Methods From February 2014 to February 2018,three hundred and thirty-six patients with pleural effusion were admitted to Hebei Thoracic Hospital.Among them,two hundred and fifty five cases were diagnosed as tuberculous pleurisy and 81 cases were diagnosed as non-tuberculous pleurisy.The patients were divided into two groups according to their age.The younger group (214 cases) was 16-59 years old and the older group (122 cases) was over 60 years old.The sensitivity and specificity of T-SPOT.TB combined with ADA in the diagnosis of tuberculous pleurisy were compared between the two groups.Results The sensitivity and specificity of T-SPOT.TB were 85.5% (153/179) and 71.4% (25/35) in the young and middle-aged group,73.7% (56/76) and 58.7% (27/46) in the old group,respectively.The sensitivity of the young and middle-aged group was significantly higher than that of the old group (x2 =4.990,P =0.045).The sensitivity and specificity of T-SPOT.TB combined with ADA were 98.9% (177/179) and 94.3% (33/35) in the young and middle-aged group,96.1% (73/76) and 89.1% (41/46) in the elderly group,respectively.There was no significant difference in sensitivity and specificity between the two groups (x2 =0.256,P=0.393、x2=0.655,P=0.218).Conclusion The diagnostic efficacy of T-SPOT.TB combined with ADA in patients with tuberculous pleurisy at different ages has been improved,especially for those who can not tolerate pleural biopsy and elderly patients.
5.Research status of Agrin involvement in the occurrence and development of malignant tumors

Malignant tumor has become the largest disease threatening human health.The global incidence of cancer is in-creasing year by year.At present,the poor quality of life and drug resistance of patients with advanced and recurrent cancer are be-coming increasingly obvious.Reducing the invasion and metastasis of tumor,improving the survival and prognosis of tumor patients,are urgent problems to be solved in tumor treatment.Agrin is a membrane protein associated with synaptic formation,and recent studies have shown that Agrin plays an important role in the occurrence and development of tumors.Agrin is expressed in malignant tumors and immune cells,playing an important regulatory role in tumor angiogenesis,cell proliferation and migration,chemotherapy resist-ance,and neutrophil infiltration.This article summarizes and analyzes the current research status of Agrin,especially its related role and mechanism in the occurrence and development of malignant tumors.
6.Analysis of APC gene mutation in a familial adenomatous polyposis pedigree.

OBJECTIVETo analyze mutation of adenomatous polyposis coli (APC) gene in a family affected with familial adenomatous polyposis.
METHODSThe diagnosis was made based on clinical manifestations, family history, presence of numerous polyps in the colon as well as pathological examination. Peripheral blood samples were collected, and genomic DNA was extracted. Potential mutation of the APC gene was detected by polymerase chain reaction (PCR) and DNA sequencing. After finding the mutation in the proband, the same mutation was screened among other family members. The mutation was also confirmed with PCR-restriction fragment length polymorphism (RFLP), with which 100 unrelated healthy controls were examined.
RESULTSA novel heterozygous nonsense mutation c.2891T>G (L964X) of the APC gene was identified in this pedigree. The mutation has led to premature termination of translation. The same mutation was not detected among the 100 healthy controls.
CONCLUSIONThe c.2891T>G (L964X) of the APC gene probably underlies the familial adenomatous polyposis in this pedigree. The combined DNA sequencing and PCR-RFLP method is efficient and accurate for the diagnosis.

7.Visualized analysis on trend and hot spot of research on multimorbidity using knowledge map

Objective:To analyze the trend and hot spots of research on multimorbidity.Methods:The Chinese and English literature in databases of CNKI, Wanfang data and Web of science from inception to July 2022 were retrieved using "multimorbidity" and "multiple chronic conditions" as the search terms. The scientific knowledge mapping software VOSviewer was used to analyze the co-occurrence network of scientific knowledge from the aspects of countries, Chinese and English keywords,burst terms and existing research results; and the status quo and hot trends of studies on multimorbidity at home and abroad were summarized and compared.Results:A total of 251 Chinese publications and 6 351 English publications were included. From the perspective of research trends, the overall trend of domestic and international publications showed a fluctuating upward trend in general, but the pace of development was different. The cooperation network represented by the United States, England, Canada, China, Japan, Australia and other countries was the largest regional cooperation network. Germany, the Netherlands, the United States, Australia and other countries were the first regions to carry out studies on multimorbidity. Studies on multimorbidity in China mainly focused on three topics: diagnosis, treatment and care of multimorbidity, common disease management of multimorbidity, and research methods of multimorbidity. The English literature mainly focused on the clustering of three topics: multi-disease co-diagnosis and care research, multi-disease co-management model research, and multimorbidity related research. The analysis of burst terms indicated the research focus and trend change in different time periods. The results of the outburst word analysis showed that the recent research hotspots of multimorbidity were public health, machine learning, geriatric medicine and so on.Conclusions:The number of studies on the multimorbidity is rapidly increasing both domestically and internationally, and related research in China is still in the early stages of development. In terms of research hotspots, the research of multimorbidity has gradually shifted from phenomenon description to topics such as medical services and research methods of multimorbidity.
