1.Analysis of adverse drug reaction induced by Chinese patent medicine in Shouguang city from 2012 to 2014

Objective To recognize the characteristics and causes of the adverse drug reaction (ADR)of traditional Chinese patent medicine,and promote the clinical safe drug use.Methods By retrospective study,ADR reports about Chinese patent medicine collected by Shouguang adverse drug reaction monitoring center from January 1,2012 to December 31,2014 were analyzed statistically.Results A total of 479 reports in accordance with the inclusion criteria were collected,which comprised 479 patients.Of them,224 were male and 255 were female,often occurs in patients over 50 years old[53.0%(254 /479)].Chinese patent medicine involved in these reports were mainly blood -regulating formula,heat agent and formula for resuscitation,respectively 278 cases(58.0%),64 cases (13.4%),51 cases (10.6%).Intravenous administration was an important way to trigger ADR [78.5%(376 /479)],78.5% injection had adverse effects and suitability of medicine were 55.9%.Main clinical manifesta-tions were lesion of skin and its appendages[35.1%(183 /522)]and digestive system damage[18.4%(96 /522)]. Conclusion The application of traditional Chinese medicine syndrome differentiation as the primary principle,atten-tion to individual differences of patients,improve the level of rational use drugs,strengthen the proprietary Chinese medicine preparation process and improve the drug instructions can reduce the adverse reaction occurred to some extent of Chinese patent medicine.
2.Isolation and identification of serum exosomes in the patients with polymyositis/dermatomyositis

Objective To isolate and identify exosomes from serum samples of the patients with polymyositis / dermatomyositis (PM/ DM),and analyze their protein composition preliminarily.Methods Exosomes from serum samples of the patients with PM/DM were isolated and purified by the ExoQuickTM kit.The morphological characteristics and particle size of exosomes were determined by transmission electron microscope (TEM) and NanoSight analyzer,respectively.The surface markers of exosomes such as CD9,CD81 and Flotillin-2 were identified by western blot.The concentration and composition of exosome protein were determined by the BCA method and SDS-PAGE,respectively.Results The exosomes from serum samples of PM/DM patients displayed round or oval vesicles with membrane structure under TEM,and their diameter range was about (92 ± 67) nm.western blot showed that these exosomes expressed CD9,CD81 and Flotillin-2.The total protein concentrations of exosomes in the patients with PM/DM and healthy controls were 14.68 (6.00,32.55) μg/μL and 14.09 (8.00,23.28) μg/μL,respectively.SDS-PAGE showed that high-abundance proteins enriched in 55-70 kD in both PM/DM patients and healthy controls,and that there were different bands in 40-55 kD between them.Conclusion Exosomes are isolated from serum samples of the patients with PM/DM successfully,and their protein concentration and composition are analyzed preliminarily,which provides the experimental evidences for further finding differential proteins.
3.Impact of Valsartan on Sarcoplasmic Reticulum Ryanodine Receptor2 in Myocardiocyte of Heart Failure Rabbits

Objective: To explore sarcoplasmic reticulum ryanodine receptor2 (RyR 2) expression and calcium releasing function in chronic heart failure (CHF) rabbits and to study the impact of long term valsartan treatment in relevant animals. Methods: HF model was established by volume overloading with pressure overloading in experimental rabbits. 27 rabbits were divided into 3 groups: Sham group, HF group and HF+valsartan group. n=9 in each group and the animals were treated for 7 weeks. Left ventricular structure, hemodynamic parameters, expression and functional changes of myocardiocyte sarcoplasmic reticulum RyR 2 were observed and compared among different groups. Results: Compared with Sham group, HF group had increased left ventricular mess index (LVMI), left ventricular end diastolic pressure (LVEDP) and decreased left ventricular shortening fraction, LVEF, all P<0.05. Compared with HF group, HF+valsartan group showed decreased LVMI, LVEDP and increased left ventricular shortening fraction, LVEF, all P<0.05. Sarcoplasmic reticulum RyR 2 expression and calcium releasing function were lower in HF group than Sham group, P<0.05; while they were both higher in HF+valsartan group than HF group, P<0.05. Conclusion: Long term application of valsartan could improve the cardiac function which might be related to increased myocardial sarcoplasmic reticulum RyR 2 expression and calcium releasing function in experimental CHF rabbits.
4.Changes of Calcium/Calmodulin-dependent Protein Kinase-II in Myocardium Nucleus and Sarcoplasmic Reticulum in Heart Failure Rabbits

Objective: To explore the changes of protein expression and activity of calcium/calmodulin-dependent protein kinase-II (CaMK-II) in myocardium nucleus and sarcoplasmic reticulum in experimental rabbits with heart failure (HF).
Methods: A total of 16 rabbits were divided into 2 groups: Sham group and HF group, the HF model was established by volume overload plus pressure overload.n=8 in each group and all animals were treated for 7 weeks. Left ventricular structure, hemodynamic parameters and protein expression and activity of CaMK-II in myocardium nucleus and sarcoplasmic reticulum were examined and compared between 2 groups.
Results: Compared with Sham group, HF group presented increased left ventricular mass index (LVMI) (1.32 ± 0.06) g/kg vs (3.61 ± 0.09) g/kg, LVEDP (-1.50 ± 0.50) mmHg vs (23.00 ± 2.37) mmHg, allP<0.05; while decreased left ventricular shorten fraction (37.83 ± 3.58) % vs (17.38 ± 3.13) % and LVEF (71.92 ± 4.56) % vs (38.50 ± 6.07) %, allP<0.05. The protein expression and activity of CaMK-II were both higher in HF group than Sham group, allP<0.05.
Conclusion: Increased protein expression and activity of CaMK-II in myocardium nucleus and sarcoplasmic reticulum might be one of the mechanisms for HF occurrence in experimental rabbits.
5.Sinicization and reliability and validity of the Questionnaire evaluating the Disability of Upper Extremities in daily activities among patients undergoing maintenance Hemo-Dialysis

Objective To translate English version of the Questionnaire evaluating the Disability of Upper Extremities in daily activities among patients undergoing maintenance Hemo-Dialysis (QDUE-HD) and test the reliability and validity of QDUE-HD. Methods After translation, back translation, 2 rounds of Delphi expert advice and pre-investigation, the initial questionnaire was formed. Two hundred and eleven patients undergoing hemo-dialysis from 4 hospitals located in Suzhou, Wuxi, Shanghai and Zhejiang were investigated using the initial questionnaire. And reliability and validity of QDUE-HD were evaluated. Results Two common factors containing 10 items and 2 dimensions(i.e. gripping and upper and lower arm movements) were extracted by exploratory factor analysis. The rate of cumulative contribution was 69.473%. In addition, the content validity index, Cronbach α coefficients, half-fold coefficient and test-retest reliability coefficient of whole scale were 0.850, 0.906, 0.929 and 0.983, respectively. Conclusions The reliability and validity of QDUE-HD (Chinese version) are reliable and effective. This questionnaire is simple and convenient to use in clinic.

Post-stroke depression (PSD) refers to a series of affective disorder syndrome characterized by low mood and lack of interest after stroke, often accompanied by physical symptoms, which is a common complication of stroke. Non-invasive brain stimulation techniques, such as repetitive transcranial magnetic stimulation and transcranial direct current stimulation have been widely used in the treatment of PSD in recent years. This article summarizes the research progress of repetitive transcranial magnetic stimulation and transcranial direct current stimulation in the treatment of PSD.
7.Effects of an enriched environment on cognition and sonic hedgehog signaling after cerebral ischemia

Objective:To observe any effect of an enriched environment (EET) on cognitive functioning and sonic hedgehog (SHH) signaling in rats modeling cerebral ischemia.Methods:Sixty adult male Sprague-Dawley rats were randomly divided into a blank control group, a model group and a training group. A model of cerebral ischemia was established in the model and training groups by thread thrombus. The training group was given an EET. After 7 and 14 days, the rats′ cognition was tested using the Morris water maze and the platform jumping test. Apoptosis of brain cells in the hippocampus was detected by using TUNEL staining, and the expression of SHH, Gli2 and PTCH1 proteins in the hippocampus were measured using qRT-PCRs, western blotting and immunohistochemistry.Results:After 14 days the average escape latency in the Morris water maze test had shortened more in the training group than in the model group, while the average swimming speed, the number of platform crossings and the time spent in the quadrant had increased significantly more. They also received fewer electric shocks and spent significantly less time on the platform in the platform jumping test on average. Apoptosis in the hippocampus after 14 days was significantly less in the training group with significantly greater expression of SHH and Gli2 protein and significantly less PTCH1 protein expression.Conclusion:An EET can significantly improve cognition after cerebral ischemia, at least in rats. Its mechanism may be related to enhanced activation of the SHH signaling pathway.
8.Effects of Digital Occupational Therapy System training under suspension on upper limb motor function for stroke patients with hemiplegia

ObjectiveTo observe the effect of Digital Occupational Therapy (OT) System training under suspension on upper limb motor function after stroke. MethodsFrom February, 2021, to February, 2022, 90 stroke hemiplegic patients with upper limb motor dysfunction from the First Affiliated Hospital of Bengbu Medical College were randomly divided into routine group (n = 30), digital OT group (n = 30) and suspension group (n = 30). All the groups received routine upper limb rehabilitation, medication, and routine OT, while the digital OT group received the upper limb training of Digital OT System and the suspension group received the upper limb training of Digital OT System with the upper limb suspended. They were assessed with Fugl-Meyer Assessement-Upper Extremities (FMA-UE) and modified Barthel Index (MBI) before and four weeks after treatment, while the mean electromyography (AEMG), root mean square (RMS), median frequency (MF) and mean power frequency (MPF) of the radial extensor carpi dorsi muscle during contraction were measured with surface electromyography (sEMG). ResultsThe FMA-UE score, MBI score, AEMG, RMS, MF and MPF improved after treatment in all the groups (|t| > 9.202, P < 0.001), and all the indexes were the best in the suspension group (F > 8.791, P < 0.001). ConclusionDigital OT System training under suspension can promote the recovery of upper limb motor function for stroke patients, as well as the activities of daily living and the upper limb muscle function.
