1.Analysis on correlation between multiple biochemical indicators and T2DM complicating gout

Objective To explore the correlation between the levels of uric acid(UA),plasma total cholesterol(TC),triglyceride (TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol (LDL-C),lipoprotein A(LPL)and hyper-sensitive C-reactive protein(hs-CRP)with type 2 diabetes mellitus(T2DM)complicating gout.Methods The levels of UA,TC, TG,HDL-C,LDL-C,LPL and hs-CRP in the T2DM complicating gout group,the T2DM without gout group and the gout group were measured and statistically analyzed to evaluate whether the differences of these indicators among 3 groups having statistical significance.Results Compared with the T2DM without gout group,the TG,LDL-C,hs-CRP and UA levels in the T2DM compli-cating gout were increased (P <0.05),while the HDL-C level was decreased(P <0.05).Ccompared with the gout group,the GLU, TG,LDL-C,LPL,hs-CRP and UA levels in the T2DM complicating gout group were significantly increased (P <0.05 )and the HDL-C level was decreased(P <0.05).The UA level in the T2DM complicating gout group was positively correlated with GLU, TC,TG,LDL-C,LPL and hs-CRP levels(P <0.05),the correlation coefficient were 0.313,0.283,0.422,0.297 and 0.436 respec-tively,which was negatively correlated with HDL-C(P <0.05),the correlation coefficient was -0.523.Conclusion The UA,TC, TG,HDL-C,LDL-C,LPL and hs-CRP levels have close correlation with T2DM complicating gout,detecting these biochemical indi-cators is helpful to monitor and control the occurrence and the development of T2DM complicating gout.

3.Literature review of global economic burden of diseases due to needlestick inj uries among health care workers

Objective To understand the global economic burden of diseases due to needle stick injuries(NSIs), obtain relevant evidence,and prompt the relevant departments to pay attention to the precaution of NSIs.Methods Literatures about NSIs published from 1990 to May 2016 were searched from PubMed,ScienceDirect,EBSCO-host,Cochrane,CNKI,and Wanfang database.According to world bank inflation rate and currency rate in 2015, cost of needle stick injury in each study was adjusted to US dollars in December 2015,merge comparison analysis was performed.Results A total of 7 literatures were included,3 American studies and 4 studies from Sweden,Ko-rea,Belgium,and Taiwan Region of China respectively.Studies in mainland China only focused on the incidence of NSIs,studies about cost were not found.Two studies did not identify studied population,the remaining 5 studies were about all staff in the medical institutions.Cost analysis method:Of 7 literatures,3 were first-hand data analy-sis,4 were derived from the model.Scope of cost research:4 studies estimated the direct cost,2 calculated direct and indirect cost respectively,only 1 study estimated the summation of direct and indirect cost.The total cost per inj ury (direct cost + indirect cost)was $747-$2173,direct and indirect cost were $167-$617 and $322-$455 respectively.Conclusion Global economic burden of NSIs is heavy and still undervalued;NSIs occur frequently in China,but attention is inadequate,research on economic burden is lacking,relevant departments should pay atten-tion to the prevention and follow-up treatment process of NSIs.

OBJECTIVE:To raise the competition power of China's pharmaceutical enterprises,make them adapt to the principle of WTO better,and finally to build a safe,effective,and convenient pharmaceutical circulation channel.METHODS:To interview the pharmacy-related departments and enterprises in22provinces and cities to collect the firsthand materials,and consult a lot of literature.RESULTS&CONCLUSION:The government should enhance the macro-management for the purpose to build a fair,unified and ordered national market;enterprise should promote and enhance themselves to improve the core competition capacity;the trade association should play a role in micro-harmonization.

7.Bladder symptoms and its impact on quality of life in type 2 diabetes mellitus

Objective To estimate the bladder symptoms, and explore its impact on patients′ quality of life in type 2 diabetes mellitus. Methods Overactive bladder symptom score (OABSS) was used to evaluate patients′ bladder symptoms, and Medical Outcome Study 12-Item Short Form Health Survey Scale (SF-12), Overactive Bladder Questionnaire Short Form(OABq-SF)were conducted to test the effect of bladder symptoms on general and disease-specific quality of life. Results Among the four bladder symptoms, nocturia was the most prevalent symptom (46.7%,479/1025 ), followed by urinary frequency (14.9%,153/1025 ),urinary urgency (14.2%,146/1025 ), urge urinary incontinence (7.9%, 81/1025). Nocturia and urge urinary incontinence significantly increased symptom bother (nocturia B=0.337,P<0.01;urge urinary incontinence B=0.437,P<0.01) and decreased disease-specific quality of life (nocturia B=-0.364,P<0.01;urge urinary incontinence B=-0.346, P<0.01) in these patients. Meanwhile, urge urinary incontinence was negatively associated with general mental health (B=-0.255, P=0.011). Conclusions The prevalence of bladder symptoms in type 2 diabetes patients was high, and was found to be associated with patients′general and disease-specific quality of life. Health care providers should be aware of the high prevalence of bladder symptoms and its impact on QOL to facilitate early diagnosis and treatment of bladder symptoms during diabetic care.
8.Efifcacy of Shenqifuzheng injection combined PG therapy in treatment of non-small cell lung cancer

Objective To observe the shenqifuzheng injection (SFI) combined with PG therapy (gemcitabine and cisplatin) in patients with non-small cell lung cancer (NSCLC). Method 76 patients with NSCLC were selected from August 2011 to August 2013 and randomly divided into observation group and control group. Each group had 38 cases. The control group received gemcitabine and cisplatin, observation group were gave SFI on the basis of control group. The total efficiency, disease control rate, quality of life and incidence of adverse reactions were observed after treatment in each group. Results The total effective rate was 47.37%in observation group , 42.11%in control group, there was no significant difference between two groups. The disease control rate in observation group was 81.58%higher than 57.89%in control group, the difference was statistically significant (χ2=3.990, P<0.05). The improvement rate in quality of life was 60.53%in observation group better than 31.58%in the control group , the difference was statistically significant (χ2=5.296, P<0.05).The decrease incidence of white blood cell (χ2=4.491, P<0.05) and platelets (χ2=4.491, P<0.05) in observation group were significantly lower compared with the control group, while, there were no difference of liver damage and gastrointestinal reactions between two groups. Conclusion SFI is helpful to improve the efficacy of PG therapy in patients with NSCLC and relieve the side effects of chemotherapy.
9.Clinical study on procalcitonin in septicemia of infected

Objective To study the value of procalcitonin in septicemia. Methods A total of 60 patients with sepsis, 60 local intection patients from our hospital, the level of white blood cell(WBC), neutrophil ratio(Neu%), C -reactive pro-tein(CRP) and procalcitonin (PCT) were observed on the first day of enrollment. Results Patients with sepsis were main infection with Gram-negative bacilli (47/60); local intection patients were main infection with gram positive coccus(43/60). Patients with sepsis had a higher level of Neu%、CRP and PCT than local intection patients(all P<0.01 ). The area under the curve of ROC of PCT was significantly larger than others. The sensitivity and specificity of PCT was 83.3%and 86.7% respecitively on diagnosis of septicemia. Conclusion PCT has higher sensitivity and specificity than WBC、Neu%and CRP. PCT has high clinical value in septicemia.
10.Microembolic signals and outcome in patients with acute ischemic stroke: a prospective case series study

Objective To investigate the correlation of microembolic signals (MES) and outcome in patients with acute ischemic stroke.Methods The patients with acute ischemic stroke were enrolled in the study.The MES of middle cerebral artery was monitored dynamically using transcranial color Doppler ultrasound.The early lesions of ischemic stroke were evaluated by MRI.The National Institutes of Health Stroke Scale (NIHSS) was used to evaluate neurological deficits.The modified Rankin scale was used to evaluate the outcome,and the stroke recurrence was recorded.Results A total of 135 patients with acute ischemic stroke were enrolled,in which,33 were cardiogenic cerebral embolism,49 were large artery atherosclerotic stroke,24 were small arterial occlusive stroke,and 29 were other clear causes or cryptogenic stroke.Multivariate logistic regression analysis showed that coronary heart disease (odds ratio [OR],5.862,95% confidence interval [CI] 2.008-17.114; P =0.000) was the independent risk factor for positive MES within 48 hours after stroke onset,while the history of antithrombotic treatment (OR 0.376,95% CI 0.141-0.998; P =0.045) was its independent protective factor.In addition,coronary heart disease (OR 4.879,95% CI 1.257-18.939; P =0.033),hypertension (OR 4.958,95% CI 1.029-23.882; P =0.030),and diabetes (OR 3.659,95% CI 1.027-13.034; P =0.050) were the independent risk factors for positive MES within 1 week after stroke onset.The NIHSS scores of the patients of the positive MES at baseline and 1 week and the clinical outcome at 3 months had no significant differences with the patients of negative MES,however,stroke recurrence and deaths increased significantly (P =0.019).Conclusions MES within 48 hours of onset was not associated with the outcome in patients with acute ischemic stroke at 3 months,however,the incidence of endpoint events such as recurrence and death was significantly higher in patients of positive MES within 3 months.
