1.Determination of Nitroglycerin Ointment by HPLC

Objective:To optimize and improve the content determination method for nitroglycerin ointment. Methods:An HPLC method was used,the column was Hypersil ODS(150 mm × 4. 6 mm,5 μm),the mobile phase was acetonitrile ∶water(50 ∶50),the detection wavelength was set at 220 nm,the flow rate was 1 ml·min-1 ,the column temperature was 30℃,and the injection volume was 20 μl. Results: The results showed a good linear relationship within the concentration range of 0. 020 3-0. 203 3 mg · ml-1 ( r =0. 999 9),and the average recovery was 99. 51%(RSD=1. 06%,n=9). Conclusion: The method is rapid,accurate and reproduci-ble,and can be used to determine the content of nitroglycerin in nitroglycerin ointment.
2.Effects of hemin on neuroglobin expression after cardiopulmonary resuscitation in rats

Objective To study the effect of Hemin on the level of neuroglobin (NGB) in cerebral cortex,neurodeficit score (NDS) and pathological changes in cerebral cortex after cardiopulmonary resuscitation (CPR) in rats. Method A total of 120 male Sprague Dawley(SD) rats were divided randomly into control group(A), CPR group(B) and Hemin group(C). The animal model of cardiac arrest (CA) induced by asphyxia was established and CPR was performed. The NGB level in cerebral cortex, NDS and pathological changes in cerebral cortex were examined 0.5 h,3 h,6 h, 12 h, 24 h after restoration of spontaneous circulation (ROSC) in each group. Experimental data were analyzed by using one-factor analysis of variance and Tukey test. Results In comparison with group A, the levels of NGB were significantly higher 12 h,and 24 h after ROSC (P <0.05 and P <0.01), the values of NDS were significantly lower at each interval after ROSC ( P < 0.01 ) ,and the pathological changes were more severe at each interval after ROSC in group B. In comparison with group A, the levels of NGB were significantly higher 6 h, 12 h and 24 h after ROSC ( P < 0.05 and P < 0.01 ), and the values of NDS were significantly lower 3 h, 6 h, 12 h after ROSC ( P < 0.01) in group C. In comparison with group B, the levels of NGB were significantly higher 12 h and 24 h after ROSC, the values of NDS were significantly higher 12 h and 24 h after ROSC, and the pathological changes were less in group C. Conclusions The NGB level increased in cerebral cortex, the NDS level decreased and severe pathological changes occurred in rats after CPR. The hemin treatment up-regulated the level of NGB, improved the NDS, mitigated pathological changes, alleviating the cerebral injury after CPR.
3.Pelvic floor muscle strength screening and analysis of its related factors among migrant women in the Pearl River Delta.

OBJECTIVETo investigate the prevalence and related factors of abnormal pelvic floor muscle strength and provide theoretical evidence for the prevention and treatment of pelvic floor dysfunction.
METHODSA total of 1008 migrant women in hospitals of Guangzhou and Foshan were screened for pelvic floor strength at 42 days postpartum from Oct, 2013 to July, 2014. EMG biofeedback equipment was employed to assess the strength of type I and II muscle fiber and a questionnaire was designed to record its related factors.
RESULTSThe incidence of abnormal type I and II muscle fiber strength was 49.21% and 49.60%, respectively. No statistical significance was observed in the abnormal rates among women with different ages or occupations. Compared with women who chose vaginal delivery, women with cesarean section had significantly reduced abnormal rates. A negative correlation was found between parity and type I and II muscle fiber strength.
CONCLUSIONThe delivery mode and parity are factors affecting pelvic floor muscle strength among migrant women in the Pearl River Delta, suggesting the necessity of health education and promotion of family planning policy.

4.Clinical characteristics of 275 pediatric cases of acquired immune deficiency syndrome

Objective To study the clinical characteristic of acquired immune deficiency syndrome (AIDS) patients younger than 15 years old and to explore the influence of human immunodeficiency virus (HIV) infection on them. Methods The clinical information, including demographic profile, clinical stages of the disease, laboratory test results and developmental status were gathered from 275 antiretroviral therapy naive patients. Results Seventy eight point nine percent patients were infected by vertical transmission. Sixteen percent were infected by receiving blood products. The average age was (7.6±3. 7) years, with 5 cases younger than 1 year old, 104 cases ranging from 1 - 5 years and 166 cases elder than 6 years. Seventy point one percent patients were classified as stage 3 or 4 according to World Health Organization definitions. The average CD4 count was ( 137 ± 159 )/μL, ( 304 ± 317 ) /μL and ( 1 246 ± 776 )/μL respectively in children elder than 6 years, ranging from 1 to 5 years and younger than 1 year. One hundred and eighty one cases suffered from anemia on different severity grading. The most common HIV related symdromes included persistent fever, skin damage, persistent diarrhea, oral candidiasis and recurrent upper respiratory tract infection. Among these infected children, 49. 6% showed height lower than x - 2s and 19. 9% showed weight lower than x - 2s. Conclusions Most survival pediatric AIDS patients are elder than 6 years. HIV infection can significantly affect the children's immune system function,growth and development.
5. Minimally invasive treatment of pilon fracture with single main plate combined with multiple planar screws for support-ing fixation

Objective:
To explore the clinical effect of minimally invasive treatment of C type pilon fracture with single main plate combined with multiple planar screws for supporting fixation.
Methods:
From January 2013 to March 2017, data of 22 patients treated by minimally invasive treatment with single main plate combined with multiple planar screws for supporting fixa-tion were retrospectively analyzed. There were 17 males and 5 females, aged from 23 to 69 years (average, 51.2 years). All cases were closed fractures involving fibula including weight hitting (5 cases), traffic accident (7 cases) and fall injury (10 cases). There were 3 cases of 43-C1 type, 11 cases of 43-C2 type and 8 cases of 43-C3 type according to AO/OTA classification. According to the Rüedi-Allgöwer classification, there were 5 cases of type Ⅱ and 17 cases of type Ⅲ. Complications were recorded postopera-tively and the articular surface reduction was evaluated using the Burwell-Charnley score. At the last follow-up, Tornetta’s pilon fracture clinical treatment outcome criteria was used to evaluate ankle joint function.
Results:
All the patients were followed up for 13 to 25 months (average, 17.3 months). There were 20 cases of anatomical reduction, 1 case of fair reduction and 1 case of poor reduction according to the Burwell-Charnley score. All the 22 patients were healed with healing time of 3 to 6 months (aver-age, 4.8 months). The efficacy was evaluated according to the evaluation criteria of the clinical treatment results of pilon fracture by Tornetta: excellent in 8 cases, good in 10 cases, fair in 3 cases, and poor in 1 case. The excellent and good rate was 81.8% (18/22) . All patients had no complications such as wound edge blistering, skin necrosis and infection. Among them, 1 case of internal fixa-tion rejection occurred, and the incision was well healed after removal of the internal fixation after 3 months.
Conclusion
Mini-mally invasive treatment of C type pilon fracture with single main plate combined with multiple planar screws for supporting fixa-tion not only provides a better anatomical reduction of the articular surface, but also effectively reduces or even avoids incision complications. The long-term clinical results are satisfactory.
6.Adjuvant capecitabine and oxaliplatin for gastric cancer after D2 gastrectomy (CLASSIC): Chinese subgroup analysis.

OBJECTIVETo evaluate the efficacy and safety profile of XELOX (capecitabine/oxaliplatin) in patients with locally advanced gastric cancer who underwent curative D2 resection in China.
METHODSThis is a subgroup analysis of Chinese patients in the capecitabine and oxaliplatin adjuvant study in stomach cancer (CLASSIC study), which was a randomised, open-label, multicentre, parallel-group, phase III( study in the Asia-Pacific region. A total of 100 gastric cancer patients who received curative D2 gastrectomy were enrolled in this study and were randomly assigned to either XELOX group (oral capecitabine combined with intravenous oxaliplatin chemotherapy) or the control group (surgery alone). This study aims to compare the 3-year disease-free between the two groups.
RESULTSSubgroup analysis showed that 3-year DFS rate were 78% and 56% in XELOX and control group, respectively. The risk of relapse in XELOX group was reduced by 59% (HR=0.41, 95%CI:0.20-0.85, P=0.013), compared with the control group. The 3-year overall survival rate were 78% and 66% in XELOX and control group, with no statistically significant difference (HR=0.55, 95%CI:0.26-1.16, P=0.110).
CONCLUSIONAdjuvant XELOX chemotherapy following D2 gastrectomy may improve the survival in patients with advanced gastric cancer in China.

7.Effect of pharmacogenetic polymorphism on the antiplatelet aggregation effect of ticagrelor

Objective To develop a pharmacogenomics study of ticagrelor in patients with acute coronary syndrome (ACS), identify the genetic factors that can predict individual differences in antiplatelet aggregation effects of ticagrelor, and provide a reference for the development of individualized regimens for ticagrelor. Methods 75 ACS patients of Chinese Han in a hospital in Fujian province in 2018 who met the entry criteria were recruited. The patient was given the tests for platelet function test, platelet aggregation rate and DNA detection. The whole exon sequencing method (WES) was used to detect the single nucleotide polymorphisms of SLO1B1, UGT2B7, P2Y12, PEAR1, ITGA2B and ITGB3. At the same time, the general clinical data of the patients were collected and recorded. The correlation between antiplatelet aggregation effects of ticagrelor and pharmacogenetic polymorphism was analyzed by one-way analysis of variance, multiple linear regression analysis and binary logistic regression analysis. Results One-way analysis of variance showed that SLCO1B1 rs2306283 mutant allele G could affect the antiplatelet aggregation effect of ticagrelor, the average platelet aggregation rate of patients carrying at least one allele G (AG+GG type) was significantly lower than that of wild homozygotes AA patients (8.07%±6.17% vs 13.88%±6.39%, P≤0.05). However, multivariate regression analysis after adjusting for confounding factors showed that SLCO1B1 rs2306283 mutant allele G was not an independent variable affecting the antiplatelet effects of ticagrelor (P>0.05). Conclusion Single nucleotide polymorphisms of genes related to ticagrelor transport receptors, targets, and platelet membrane receptors (including SLO1B1, UGT2B7, P2Y12, PEAR1, ITGA2B, ITGB3) in ACS patients of Han Chinese in Fujian province will not significantly affect the antiplatelet aggregation effect of ticagrelor, which provides a new treatment option for patients with genetic defects who are not suitable for clopidogrel.
8.Inhibitory Effect of Water Extract of Broussonetiae Fructus on Hepatocellular Carcinoma Induced by Diethylnitrosamine in Mice and Effect of PTEN/PI3K/Akt Signaling Pathway

ObjectiveTo explore the inhibitory effect of water extract of Broussonetiae Fructus on hepatocellular carcinoma (HCC) induced by diethyl nitrosamine (DEN) in mice based on homologous phosphatase and tensin homolog/phosphatidylinositol 3-kinase/protein kinase B (PTEN/PI3K/Akt) signaling pathway. MethodThe primary HCC mouse model was constructed by intraperitoneal injection of DEN solution, and the HCC mice were randomly divided into model group, sorafenib group (0.01 g·kg-1·d-1), low-dose Broussonetiae Fructus water extract group (0.9 g·kg-1·d-1), medium-dose Broussonetiae Fructus water extract group (1.8 g·kg-1·d-1), and high-dose Broussonetiae Fructus water extract group (3.6 g·kg-1·d-1), with 10 mice in each group. Another 10 C57BL/6 mice were selected as a control group and intraperitoneally injected with an equal volume of normal saline. Mice were treated with different concentrations of Broussonetiae Fructus water extract when liver cancer-like white nodules appeared. sorafenib group was treated with sorafenib. The control group and model group were intraperitoneally injected with normal saline. The activities of alkaline phosphatase (ALP), aspartate aminotransferase (AST), alanine aminotransferase (ALT), and γ-glutamyl transferase (γ-GT) in the serum of mice were detected by the biochemical analyzer. The expression levels of alpha-fetoprotein (AFP) and carcinoembryonic antigen (CEA) were detected by enzyme-linked immunosorbent assay (ELISA). The degree of hepatocyte canceration and hepatocyte injury were observed by Hematoxylin-eosin (HE) and Masson staining. The proliferation of HCC cells was observed by immunohistochemical staining. The apoptosis of HCC cells in mice was observed by erminal-deoxynucleotidyl transferase mediated nick end labelling (TUNEL) staining. The expression levels of PTEN, PI3K, Akt, and p-Akt proteins related to the PTEN/PI3K/Akt signaling pathway were detected by Western blot. ResultCompared with the control group, the activities of ALP, AST, ALT, and γ-GT, as well as the expression levels of AFP and CEA in the model group were significantly increased (P<0.01). Carcinogenesis and inflammatory cell infiltration were obvious in liver tissue of mice, and a large number of blue collagen fiber hyperplasia was found. The number of Ki67 positive cells was significantly increased (P<0.01), and the expression level of PTEN protein was significantly decreased, while PI3K and p-Akt protein expression was increased (P<0.01). Compared with the model group, the activities of ALP, AST, ALT, and γ-GT, as well as the expression levels of AFP and CEA in the medium-dose and high-dose Broussonetiae Fructus water extract groups were significantly decreased (P<0.05, P<0.01). The degree of carcinogenesis and inflammatory cell infiltration in liver tissue were reduced, and the collagen fiber hyperplasia was significantly reduced. The number of Ki67 positive cells was significantly decreased, and the number of TUNEL positive apoptotic cells was significantly increased (P<0.05, P<0.01). PTEN protein expression was increased, while p-Akt protein expression was significantly decreased (P<0.05, P<0.01). ConclusionThe water extract of Broussonetiae Fructus has a significant inhibitory effect on DEN-induced primary HCC in mice, and its mechanism may be related to the regulation of key protein expressions in the PTEN/PI3K/Akt signaling pathway.
