1.Application of propofol combined with midazolam in the bronchoscope of children

Objective To observe and compare the anesthesia effect of propofol combined with midazolam used in the bronchoscopes and the removal of children's intratraeheal foreign body.Methods 60 patients in hospital were choser to do the operation of bronchoscopes and the removal of children's intratracheal foreign body,the patients of selective operation were randomized into K group(n=30)related to midazolam-ketamine and P group(n=30)related to midazolam-propofol.The patients were respectively injected midazolam slowly given in a dose of 0.1~0.15mg/kg,and then offer oxygen 5~10 minutes with a face mask.After that the patients in group K were injected ketamine slowly in the dose of 2mg/kg,and 1~2mg/kg after laying the bronchoscope according as ease.The patients in group P were injected propofol slowly in the dose of 2mg/kg also,add 1~2mg/kg according as case.Observing the patients' BP,HR,SpO2,RR and if or not holding breath,bucking,postoperation laryngeal edema,glossocoma at the different periods:preoperation,laying the bronehoscope,during the operation and after the operation.Results To compare the two groups,the MAP,HR of group P are lower obviously than group K before and after laying the bmnchoscope(P<0.05).The HR,MAP of groug P descended significantly after laying bronchoscope(P<0.05),while the HR fluctuate little but MAP ascend significantly in the group K(P<0.05).After laying bronchoscope the RR descend significantly than that when patients entered operation room(P<0.05),and also lower than that ofgroup K(P<0.05).The SpO2 in the two groups have no difference.The complications as holding breath,postoperative glossocema,laryngeal edema,holding breath during reviving time,in the group K happened at a high rate than the group P(P<0.05),the awaking time is also longer in the former(P<0.05).Conclusion The anesthesia effect of midazolam-propofol is superior to that of midazdam-ketamine in the removal of children's intratraeheal foreign body.
2.An inquiry to experimental conditions and cell-counting assays in reconstituted basement membrane invasion experiment in vitro

Objective To explore the most suitable chemostatic condition and a method of cell-counting in reconstituted basement membrane invasion experiment in vitro. Method Reconstituted basement membrane invasion experiment was employed, and conditioned medium, fibronectin (FN), and a combination of conditioned medium and FN were respectively used as chemoattractants was placed in the lower compartment of the chamber. After the filters were stained with hematoxylin and eosin (HE), two kinds of cell-counting methods were employed. One was random cell-counting to count the cells in 5 fields infiltrated in the different areas of the filter membrane under 40?magnification, and the other was whole cell-counting of the infiltrating cells on the whole filter membrane with 4? magnification and image analysis system. Results The number of infiltrating cells in the presence of the chemoattractants was much larger than that of the control group. The number of infiltrating cells of the conditioned medium group was larger than that of FN group. The numbers of infiltrating cells in the combination of conditioned medium and FN groups were significantly larger than those of conditioned medium and FN alone groups. The accuracy of method of random cell-counting under 40? magnification was poorer than that of whole cell-counting with image analysis system. Conclusion Chemoattractants exert strong effect on the tumor cells invasion, and different chemoattractants show different chemotactic abilities. By using image analysis system to count all the infiltrative cells of the whole filter membrane is an objective and accurate cell-counting method.
3.Effect of Cyclosporin Based Triple Immunosuppressive Therapy on Glucose Tolerance in Renal Transplant Recipients

Objective:To study the effect of cyclosporin A based triple immunosuppressive therapy on the plasma glucose in renal transplant recipients.Method:680 renal transplant recipients treated with cyclosporine A combined with prednisone and mycophenolate mofefil from Jan.1996 to May 2007 were analysed.Result:The morbidities of impaired fast- ing glucose,impaired glucose tolerance and post-transplantation diabetes mellitus (PTDM) were 3.97%,5.15% and 8.09%,respectively.The daily doses and concentrations of CsA and the daily doses of prednisone in the impaired glucose tolerance group and PTDM group were significantly higher than those in the normal plasma glucose group.Conclusion:The daily doses and concentrations of CsA and the daily doses of prednisone were closely related to the impaired glucose toler- ance and PTDM of renal transplant recipients.
4.Analysis of pre-pregnancy laboratory examination results of 16 626 cases

Objective To investigate healthy status and disease distribution of couples through analysing the results of pre‐preg‐nancy laboratory examination and to explore high risk factors ,in order to provide scientific references for the realization of eugenics . Methods The routine blood test ,serum markers of hepatitis B ,five items(TORCH) examination ,detection of female vaginal secre‐tions were carried out in 8 313 couples planning pregnancy from 2013 to 2014 ,and the results of pre‐pregnancy health examination were analysed and evaluated .Results The thyroid dysfunction accounted for 17 .43% of all women undergoing examination ,which was the most common disease .Among items carried out in all couples ,thalassemia and positive of hepatitis B surface antigen were the most common ,accounted for 10 .20% and 9 .94% respectively .Conclusion The pre‐pregnancy health examination could screen high risk factors ,which is benefit for guiding eugenics and effectively reducing the birth defects and adverse pregnancy and is a sig‐nificant measure for eugenics .

Objective To investigate the efficacy of dexmedetomidine as an adjunct to combined intravenous-inhalational anesthesia for removal of foreign body in the trachea in children.Methods Sixty ASA Ⅰ or Ⅱ children,aged 1-4 yr,weighing 8-23 kg,were randomly divided into 2 groups(n =30 each):combined intravenous-inhalational anesthesia group(group Ⅰ)and dexmedetomidine as an adjunct to combined intravenous-inhalational anesthesia group(group Ⅱ).In group Ⅱ,8% sevoflurane was inhaled by mask to induce sleep after entering the operating room,and the concentration was reduced to 4% after the children sere asleep.Anesthesia was maintained with iv infusion of dexmedetomidine 0.5 μg/kg,and then iv injection of propofo1 2 mg/kg,followed by iv infusion of propofol at 6 rag· kg-1· h-1 and remifentanil at 0-15 μg· kg-1 ·min-t.In group Ⅰ,dexmedetomidine was not used and the other procedures were the same as those in group Ⅱ.Sevoflurane inhalation was stopped 2 min later and the rigid bronchoscope was inserted.HR and SpO2 were monitored and recorded before insertion and at 1 and 5 min after insertion.Complications such as respiratory depression,laryngeal edema,and bradycardia were reconded.The amount of propofol and remifentanil consumed,satisfactory level of bronchoscopy,and emergence time were recorded after operation.Results Compared with group Ⅰ,HR was significantly decreased at 1 and 5 min after insertion,SpO2 was significantly increased at 1 min after insertion,the amount of propofol and remifentanil consumed was significantly reduced,the operation time was significantly shortened,the emergence time was significanlly prolonged,the satisfactory level of bronchoscopy was significandy increased,and the incidence of respiratory depression and laryngeal edema was significantly decreased in grmp Ⅱ(P < 0.05).Conclnslon The efficacy of dexmedetomidine as an adjunct to combined intravenous-inhalational anesthesia is better than that of combined intravenous-inhalational anesthesia for removal of foreign body in the trachea in children,with fewer complications.
7.Expression and clinical significance of Bmi-1,Mdm2 and Ki-67 in extrahepatic biliary duct carcinoma

Objective To investigate the expression of Bmi-1,Mdm2 and Ki-67 in extrahepatic biliary duct carcinoma and to evaluate their clinical significance. Methods The expression of Bmi-1.Mdm2 and Ki-67 was detected in 10 normal extrahepatic biliary duct tissue and 30 cases of extrahepatic billary duct carcinoma with immunohistochemic0al staining and the their clinical significance were analyzed. Results No POsitlVe Bmi-1 expression was found in 10 normal extrahepatie biliary duct cages, while 18 of 30 extrahepatic biliary duct carcinoma cases showed positive Bim-1 expression.The expression of Bmi-I in extrahepatic biliary duct carcinoma(60.0%)was statistically higher than that in normal extrahepatie biliary duct(P<0.05).The positive expression rate of Mdm2 in extrahepatic biliary duct carcinoma was significantly higher than that in normal extrahepatic biliary duct (56.7%vs 20.0%,P<0.05).The cases of extrahepatic biliary duct carcinoma with Ki-67 proliferation index higher than 5%reached 60.0%,while the Ki-67 index was all lower than 1%in 10 normal extrahepatic biliary duct tissue, suggesting the higher proliferating activity of extrahepatic biliary duct carcinoma.No differences were found in the expressions of Bmi-1,Mdm2 and Ki-67 among extrahepatic biliary duct carcinoma cases with different clinical pathological features. Conclusion The results indicate that the expression of Bmi-1 and Mdm2 up-regulation may play an important role in carcinogenesis of extrahepatic biliary duct caroinoma. No clinical significances of the expressions of Bmi-1,Mdm2 and Ki-67 are found in extrahepatic biliary duct carcinomas.
8.Efficacy and Safety of Fluvastatin vs. Xuezhikang for Hyperlipidemia after Renal Transplantation

OBJECTIVE:To evaluate the efficacy and safety of Fluvastatin vs. Xuezhikang for hyperlipidemia in patients after renal transplantation. METHODS: 56 hyperlipidemia patients after renal transplantation were enrolled: 32 were assigned to receive Fluvastatin (40~80 mg) po qd for 8 weeks and 24 to receive Xuezhikang (0.6 g) po bid for 8 weeks. Total cholesterol (TC),triglyceride (TG),low-density lipoprotein-cholesterol (LDL),high-density lipoprotein-cholesterol (HDL),liver function and renal function in two groups were measured before and after treatment. RESULTS: In Fluvastatin-treated group,the TC decreased from(7.39?1.98)mmol?L-1 to(5.62?0.93)mmol?L-1,LDL reduced from(3.68?1.13)mmol?L-1 to (2.86?0.83)mmol?L-1;in Xuezhikang-treated group,TC decreased from(6.82?1.29)mmol?L-1 to (5.56?1.19) mmol?L-1 and LDL decreased from (3.26?0.73) mmol?L-1 to (2.78?0.80) mmol?L-1,all showing significant differences as compared with before treatment(P 0.05). No obvious adverse effect was noted in either group during treatment. CONCLUSION: Both Fluvastatin and Xuezhikang are safe and effective for hyperlipidemia in patients after renal transplantation.
9.Effects of berberine on the transport of P-gp substrates across Caco-2 and L-MDR1 cell monolayers

Aim To investigate effects of berberine (Ber) on the transport of P-gp substrates including cyclosporine A (CsA) and digoxin across Caco-2 and L-MDR cell monolayers. Methods Permeability coefficients and transport rates of digoxin and CsA across Caco-2 and L-MDR1 monolayers were measured in the presence of Ber(50 ?mol?L-1~5 mmol?L-1). Results The inhibition studies for digoxin transport showed a dose-dependent decrease in basal-to-apical direction across Caco-2 and L-MDR1 monolayers in the presence of Ber (50 ?mol?L-1~5 mmol?L-1). Both a dose-dependent decrease in basal-to-apical direction and an increase in apical-to-basal direction were observed for CsA transport across Caco-2 monolayers with different concentrations of Ber. The IC50 values calculated for Ber-induced inhibition of digoxin transport are 1.44 mmol?L-1 in Caco-2 cells and 1.24 mmol?L-1 in L-MDR1 cells, respectively. The IC50 value for Ber-induced inhibition of CsA transport is 607 ?mol?L-1 in Caco-2 cells. Conclusion It was suggested that the inhibition and saturation of P-gp transport activity might be involved in interactions between Ber and CsA.
10.Research on Prevention Measure Against Vagal Reflex Occurrence after PTCA

Objective To investigate the preventive measurement of vagal reflex occurrence on patients with percutaneous transluminal coronary angioplasty (PTCA).Methods On the reason of vagal reflex occurrence,to 75 patients with PTCA adopt the measurement to prevent hypovolemia,control the excessive rebound of cavity viscera,adjust the patients mental state,and so on.Then to observe the level of patients with vagal reflex occurrence,and compare with the control group of 75 patients under common care.Results The prevent group level of patients with vagal reflex occurrence is 2.61%;the control group is 28%.Conclusion To patients with PTCA adopt the measurement to prevent hypovolemia,control the excessive rebound of cavity viscera,adjust the patients mental state,and so on.The effect to decrease the level of vagal reflex occurrence is great.
