1.Effect evaluation of programmed management in nursing management of emergency department

ObjectiveTo investigate the effect of programmed management of nursing quality in nursing management of emergency department.Methods861 cases of emergency department patients were selected as research subjects and programmed management of nursing quality was implemented.Then 850 cuses of emergency department patients who did not received nursing quality programmed management were chosen as the control The nursing quality before and after the implementation of programmed management of nursing quality were compared.ResultsAfter the implementation of the nursing quality programmed management,the admisson time,turnout time,the rate of transfusion errors and the rate of nursepatient disputes were significantly reduced,while the specilist assessment score,rescue success rate and patient satisfaction degree with nursing were significantly increased compared with those before the implementation.ConclusionsApplication of nursing quality programmed management in nursing management of emergency department can effectively improve the nursing quality and working efficiency.

The developmental speed,scale,structure,and efficiency of postgraduate education in recent 20 years in military medical universities were described and compared with those in the other universities in China.We also discussed the advantages and insufficiency of postgraduate education in military medical universities and investigated primarily the mutual relationship between the speed,structure,and the scale,so as to lay basis for formulating the further developing strategies.
3.Chemical constituents of Rheum emodi

Objective To isolate and determine the structures of chemical constituents from the roots and rootstalks of Rheum emodi.Methods The chemical constituents were isolated and purified by silica gel and polyamide column chromatography.Chemical methods and spectroscopic methods,such as 1H-NMR,13C-NMR,and MS spectra were used for the structure identification.Results Eighteen compounds were obtained.Sixteen of them were identified as:chrysophanol(1),physcion(2),?-sitosterol(3),emodin(4),aloe-emodin(5),rheumin(6),daucosterol(7),d-catechin(8),piceatannol(9),piceatannol-4'-O-?-D-glucopyranoside(10),piceatannol-4'-O-?-D-(6″-O-p-coumaroyl)-glucopyranoside(11),chrysophanol-8-O-?-D-glucopyranoside(12),physcion-1 and 8-O-?-D-glucopyranoside(13a and 13b),emodin-8-O-?-D-glucopyranoside(14),and sucrose(15).Meanwhile,one compound structure simi-lar to rhein was detected.Conclusion Compound 11 is a new compound named rheoside,compounds 13a and 13b are obtained from R.emodi for the first time.
4.A Novel Fluorescence Sensor for Highly Sensitive Detection of Glucose

A fluorescence nanosensor based on an easily prepared fluorescent molecule, 1-oxo-1H-phenalene-2,3-dicarbonitrile (OPD), was developed for highly sensitive detection of glucose.Under the catalysis of horseradish peroxidase (HRP), 3,3′,5,5′-tetramethylbenzidine (TMB) was oxidized into oxidized TMB (oxTMB) by H2O2.And the fluorescence of OPD was quenched by the intense absorption of the formed oxTMB, thus realizing effective quantitative detection of H2O2.The linear range was 0.05-0.8 μmol/L and 1-10 μmol/L respectively, with limit of detection of 0.02 μmol/L.Besides, on the basis of transformation of glucose into H2O2 through the catalysis of glucose oxidase, this nanosensor could be further exploited for highly sensitive detection of glucose.The TMB-HRP-OPD sensor exhibited linear range of 0.1-3.0 μmol/L and 4.0-30 μmol/L respectively for detection of glucose, with limit of detection of 0.02 μmol/L.Furthermore, it was successfully applied to the determination of glucose in real human serum and the results were in good agreement with the clinical data.

Objective: To study the relationship between immune function and psychosocial factors of patients with malignant tumors Method:37 inpatients with malignant tumors were collected They had no chemotherapy or radiotherapy in the past month and without history of immune disease or medication influencing immune system 37 inpatients of internal department without tumor were collected as control All subjects completed EPQ, SCL-90, SAS, SDS, LES, SSRS and DSQ Researchers assessed their mood with HAMA and HAMD, and their immune function by measuring serum IgG, IgA, IgM, C 3, C 4, CH50 and sub-groups of lymphocyte Results:Study group had lower IgM and less CD 16 + than control The level of IgM was associated with course of disease, educational level, gender, personality, defense style, negative life events and social support The level of CD 3 + was associated with depression in addition to other factors mentioned above Conclusion: Patients with malignant tumors have immune dysfunction, which associated with psychosocial factors such as negative life events, social support and depression

7.Psychosocial Factors of Depression in Late Life

Objective:To explore psychosocial factors of depressed elderly patients. Methods:33 elderly people with the first episode of depression diagnosed according to CCMD-3, and elderly healthy people were investigated with EPQ, SSRS (social support rating scale), LES (life event scale) and MMSE (mini mental state examination). Results:The rates of psychiatric family history, somatic diseases, risk factors for vascular disease and long-term difficulties were significantly higher in the depressive group than in control group. The score of MMSE was significantly lower in depressive group than in control group. The scores of negative life events and problems in social intercourse were significantly higher in depressive group than in control group. There were lower score of social support and its utility in the depressive group than in control group. The score of neuroticism was significantly higher in the depressive group. Most depressive elderly patients had the personality of neuroticism.Conclusion:Chronic stress, high neuroticism and severe cognitive impairment are important risk factors for depression in late life.[
8.A clinical investigation for ultra-early microtrauma drainage in the treatment of hypertensive cerebral hemorrhage

Objective To evaluate the curative effect of ultra-early microtrauma drainage in the treatment of hypertensive cerebral hemorrhage. Methods Sixty-one cases of hypertensive cerebral hemorrhage were divided into two groups: the ultra-early group (31 cases; received surgery within 6 hours after the onset) and the control group (30 cases; received surgery between 24 hours and 1 week after the onset).All these cases underwent microtrauma drainage for cleaning the hematoma. Results The effective rate and the marked effective rate in the ultra-early group were 83 9% (26/31) and 67 7% (21/31),respectively,both of which were higher than those in the control group (60% and 40%; ? 2=4 322,4 725;P =0 038, 0 030).The death rate in the ultra-early group (16 1%,5/31) was lower than that in the control group (40%,12/30; ? 2=4 322,P =0 038).Of the life quality of survivors,the values of ADL Ⅰ~Ⅲ accounted for 84 6% (22/26) in the ultra-early group and 55 6% (10/18) in the control group ( ? 2=3 182;P =0 074). Conclusions Ultra-early microtrauma drainage for hypertensive cerebral hemorrhage provides better curative effect and lower death rate than postponed operation.
9.Outcome and safety analysis of colonoscopy in 1 249 patients aged 80 years and over

Objective To investigate the value and safety of colonoscopy in patients aged 80 years and over.Methods Clinical and endoscopic data of 1 249 patients aged 80 years and over collected from December 2005 to December 2015 at PLA army General Hospital were analyzed retrospectively.Results The average age was 83.03 years.Reasons for receiving colonoscopy included constipation with abdominal distension(19.38 %)and hematochezia(10.57%).The completion rate of colonoscopy for the entire length was 94.50 %.There were no abnormal findings in 492 cases (39.39 %).Colonic polyps (31.62 %) and colorectal cancer (14.25 %) were among the major lesions detected with colonoscopy.Colorectal neoplasms were found in 58.33% of the 132 patients with hematochezia.The total complication rate from colonoscopy and treatment was 0.72%,with the complication rate from treatment at 1.32%.Conclusions Constipation with abdominal distension and hematoehezia are the main reasons for undergoing colonoscopy for very elderly patients.Colon polyps and colorectal cancer are common disorders in elderly patients over 80 years of age and people with hematochezia in this age group are at high risk of having colorectal cancer.Colonoscopy is a safe and effective procedure and an important examination method for very elderly patients with hematochezia.
10.Investigation and analysis of nurses' status of fatigue and perceived social support

Objective Through the investigation of nurses'situation of fatigue and perceived social support, to know about the status of nurses' fatigue and the degree of perceived social support, and influenc-ing factors for a preliminary regression analysis of nurses in order to further the prevention of the occurrence of chronic fatigue for reference. Methods By using fatigue assessment scale (FAI), fatigue scale (FS-14)and perceived social support scale (PSSS) to investigate 200 female nurses in Jining city, 191 valid questionnaires were got, the reclaimed efficiency was 95.5%. The results underwent analysis. Results Nurses who had accounted for 69.11 percent of fatigue, had more severe fatigue than nurses who had less fatigue, and they had tense response to rest or sleep. Nurses' physical fatigue and mental fatigue were significantly higher than the general healthy population. Nurses received higher scores of social support. Age and other factors and perceived social support went through regression analysis: the degree of job tension exacerbated the severity of fatigue, however, social support to some extent, mitigated or alleviated the situation of nurses in the total fatigue. Conclusions Nurses are high-risk population for fatigue. The degree of job stress is the main influencing factor. And social support and care for nurses, especially home care support, adequate rest, in-creasing the number of nurses, good working environment can effectively reduce fatigue.
