1.Application of argon laser photo-coagulation (APC) in the treatment of digestive diseases

Objective To probe into the application of argon laser photo-coagulation (APC) in interventional endoscopic treatment of digestive diseases. Method 62 patients suffering from digestive diseases were treated with interventional endoscopic treatment with the aid of APC, including 393 cases of gastrointestinal polyps, 122 cases of gastritis verrucosa, 5 cases of Barrett's esophageal ulcer and 42 cases of hemorrhage of digestive tract. The effectiveness of the treatment was observed. Result For 350 cases of gastrointestinal polyp, in which the diameter of the polyp was less than 5 mm, the lesions were eliminated when checked by endoscopy 3 months after APC treatment, the curative ratio was 100%. For 43 cases of gastrointestinal polyps, in which the diameter of the polyp was larger than 5 mm, no residual lesions were found in 38 cases, and remarkable improvement was found in another 5 cases of Peutz-Jeghers syndrome. For 122 case of gastritis verrucosa, 116 cases recovered thoroughly, the curative ratio reached 95.1%. In 42 cases of hemorrhage of digestive tract, no hemorrhage was found in 41 cases after APC treatment. In 5 cases of Barrett′s esophageal ulcer treated with APC accompanied by PPIs, squamous cell epithelium was found to have recovered as shown by endoscopy and biopsy 3 months after the treatment, and no recurrence 12 months later. Conclusion APC can be widely used in digestive diseases including gastrointestinal polyps, hemorrhage of digestive tract, Barrett's syndrome, gastritis verrucosa, etc. APC has a special value for the treatment of gastrointestinal polyp when the diameter of the polyp was less than 5 mm, and also gastritis verrucosa.
2.Significance and safety of systematic lymphadenectomy in the treatment of patients with endometrial cancer

Obj ective To explore the effect and safety of systematic lymphadenectomy on the prognosis of patients with endometrial cancer.Methods 54 patients with endometrial cancer were divided into two groups according to the type of surgery.Group A (n= 30)was treated with modified radical hysterectomy plus bilateral salpingo-oophorectomy and pelvic lymphadenectomy, while group B (n= 24 )was treated with modified radical hysterectomy plus bilateral salpingo-oophorectomy and systematic pelvic as well as para-aortic lymphadenectomy.Prognosis and post-operative complications were compared between two groups.Results The two-year recurrence and metastasis rate in group A was 40%,which was significantly higher than 12.5%in group B (P<0.05).The two-year overall survival rates in group A and group B were 70.0%and 91.7%re-spectively,and there was significant difference between two groups (P<0 .05 ).Incidence rate of incomplete intestinal obstruction in group B was significantly higher than that in group A (P <0.05).There were no significant differences in the incidence rates of lymphedema,lymphocyst, deep vein thrombosis and urethral fistula between two groups (P>0 .05 ).Conclusion Systematic pelvic and para-aortic lymphadenectomy can improve the survival rate and prognosis of patients with endometrial carcinoma,and the incidence rate of postoperative complications can be restrained in controllable scope.

4.Prognostic value of combined detection of three indexes in patients with advanced cervical squamous cell carcinoma

Objective:To evaluate the prognostic value of combined detection of platelet-to-lymphocyte ratio (PLR), neutrophil-to-lymphocyte ratio (NLR) and squamous cell carcinoma antigen (SCC) for patients with advanced cervical squamous cell carcinoma undergoing radical radiotherapy.Methods:Clinical data of 127 patients with advanced cervical squamous cell carcinoma who received radical radiotherapy in the Affiliated Tumor Hospital of Nantong University from January 2016 to February 2019 were analyzed retrospectively. The enrolled cases were divided into the survival group and death group according to the survival at the end of 3 years after treatment. The laboratory indexes of peripheral blood were collected before treatment, PLR and NLR were calculated, and the differences of clinical parameters were compared between two groups. The prediction model was established, and the prediction efficiency of PLR, NLR and SCC alone and combined prediction models for 3-year overall survival (OS) in patients with advanced cervical squamous cell carcinoma was compared through the ROC curve. Univariate and multivariate analyses of prognosis were carried out by binary logistic regression model.Results:A total of 127 patients with advanced cervical squamous cell carcinoma were included in the study. There were 96 cases in the survival group and 31 cases in the death group. There were significant differences between two groups in FIGO stage, longest diameter of tumor, lymph node metastasis, PLR, NLR and SCC (all P<0.05). The area under ROC curve (AUC) of PLR, NLR and SCC was 0.660, 0.712 and 0.700, respectively. The AUC of PLR+NLR+SCC combined prediction model was increased to 0.784. Logistic multivariate analysis showed that FIGO Ⅲ, FIGO Ⅳ, lymph node metastasis, PLR≥205.555, NLR≥3.060 and SCC≥6.950 ng/ml were the independent risk factors for 3-year OS in patients with advanced cervical squamous cell carcinoma (all P<0.05). Conclusions:PLR, NLR and SCC have good value in predicting the 3-year OS of patients with advanced cervical squamous cell carcinoma, and the combined prediction model of PLR+NLR+SCC has higher prediction value.
5.Expression of yeast acyl-delta9 desaturase for fatty acid biosynthesis in tobacco.

Palmitoleic acid (16:1delta9), an unusual monounsaturated fatty acid, is highly valued for human nutrition, medication and industry. Plant oils containing large amounts of palmitoleic acid are the ideal resource for biodiesel production. To increase accumulation of palmitoleic acid in plant tissues, we used a yeast (Saccharomyees cerevisiae) acyl-CoA-delta9 desaturase (Scdelta9D) for cytosol- and plastid-targeting expression in tobacco (Nicotiana tabacum L.). By doing this, we also studied the effects of the subcellular-targeted expression of this enzyme on lipid synthesis and metabolism in plant system. Compared to the wild type and vector control plants, the contents of monounsaturated palmitoleic (16:1delta9) and cis-vaccenic (18:1delta11) were significantly enhanced in the Scdelta9D-transgenic leaves whereas the levels of saturated palmitic acid (16:0) and polyunsaturated linoleic (18:2) and linolenic (18:3) acids were reduced in the transgenics. Notably, the contents of 16:1delta9 and 18:1delta11 in the Scdelta9D plastidal-expressed leaves were 2.7 and 1.9 folds of that in the cytosolic-expressed tissues. Statistical analysis appeared a negative correlation coefficient between 16:0 and 16:1delta9 levels. Our data indicate that yeast cytosolic acyl-CoA-delta9 desaturase can convert palmitic (16:0) into palmitoleic acid (16:1delta9) in high plant cells. Moreover, this effect of the enzyme is stronger with the plastid-targeted expression than the cytosol-target expression. The present study developed a new strategy for high accumulation of omega-7 fatty acids (16:1delta9 andl8:1delta11) in plant tissues by protein engineering of acyl-CoA-delta9 desaturase. The findings would particularly benefit the metabolic assembly of the lipid biosynthesis pathway in the large-biomass vegetative organs such as tobacco leaves for the production of high-quality biodiesel.

6.Current status of hospital infection management in psychiatric hospitals in the Ningxia Hui Autonomous Region

ObjectiveTo investigate current status of hospital infection management in psychiatric hospitals in the Ningxia Hui Autonomous Region, so as to provide references for improving the level of hospital infection management of psychiatric hospitals. MethodsIn December 2020, on-site supervision was conducted on hospital infection management in all 9 psychiatric hospitals in the Ningxia Hui Autonomous Region, meantime, the self-compiled questionnaire on hospital infection management status was used for investigation. ResultsAmong the selected hospitals, nine (100.00%) psychiatric hospitals had the main hospital leaders in charge of hospital infection management, five (55.56%) hospitals had established a hospital infection management committee, six (66.67%) hospitals had established an independent hospital infection management department, and one (11.11%) hospital had developed all 13 systems mentioned in the questionnaire related to hospital infection management and job responsibilities. In terms of hospital infection management staff, there were 23 staff members in the nine psychiatric hospitals, including 3 in the specialty (13.04%) and 20 in the part-time setting (86.96%). The score of the implementation of the basic system of hospital infection management in nine hospitals was (3.28±2.22). ConclusionThe system specification related to hospital infection management in the Ningxia Hui Autonomous Region psychiatric hospitals needs to be improved and further strengthened, the professionalism of hospital infection management personnel needs to be improved.
