1.Study on Nosocomial Infection and Susceptibility of Related Pathogens to Antibiotics in Patients With the Systemic Lupus Erythematosus

Objective To study the percentage and the distribution of nosocomial infection, and susceptibility of related pathogens to antibiotics in patients with systemic lupus erythematosus (SLE). Methods The clinical data of in patients with SLE in our hospital from June, 1990 to June, 1998 were analyzed. The susceptibility of the pathogens to antimicrobial agents was tested by K-B paper dilution method. Results The percentage of nosocomial infection in these cases was 16.1% . The major pathogens were staphylococcas aureus, Escherichia coli and Candida. The most highly resistant microbes were E.coli, S.arueus and Pseudomonas aeruginosa. Conclusions The nosocomial infection is one of the most common complication of SLE in the in patients. It is necessary to examine the pathogens regularly and use antibacterial agents according to the susceptibility.
2.The application of syphilis detection in the pre-pregnancy eugenics inspection

Objective To investigate the application of syphilis detection in the pre-pregnancy eugenics inspection.Methods 12 900 women who attended the national free pre-pregnancy eugenics inspection from March 201 1 to October 2013 were selected as subjects.The treponema pallidum particle agglutination assay(TPPA)and rapid plasma regain test(RPR)were conducted among them before pregnancy,and the results of syphilis detections were recorded and compared.Results In 12 900 women,63 cases (0.49%)were syphilis positive in RPR tests,129 cases(1.00%)were syphilis positive in TPPA test,and 45 cases(0.35%)were syphilis positive both in RPR and TPPA tests.There were significant differences in positive rates of syphilis between that in the RPR test and that in the TPPA test(P <0.05),while no significant differences were found in positive rates of syphilis between that in the RPR test and that in the combined detection of RPR test and TPPA test.There were significant differences in positive rate of syphilis between that in the TPPA test and that in the combined detection of RPR test and TPPA test(P <0.05).The sensitivity of RPR test and TPPA test was 71.4% and 34.9%,respectively.Conclusion Both the RPR test and TPPA test could be the fast and ef-fective method in diagnosis of syphilis.However,RPR test might be more sensitive,which could be widely used in the pre-pregnancy eugen-ics healthy screening.Moreover,the diagnosis results could be confirmed by combination of RPR test and TPPA test.

Four qi theory is an important part of property theory of traditional Chinese medicine. Current researches mainly focus on two aspects of cold and heat properties, which are not only important elements of treating diseases, but also are factors of causing adverse reaction. In this article, through the study on the toxicity mechanism of cold and hot properties, authors put forward to the control measures of toxic and side effects of traditional Chinese medicine with cold and hot properties, and pointed out that the importance of controlling toxicity of traditional Chinese medicine with cold and hot properties lies in the modernization and internationalization of traditional Chinese medicine.

5.Principal factor analysis of symptoms of rheumatoid arthritis and their correlations with efficacy of traditional Chinese medicine and Western medicine

OBJECTIVE: To explore the correlations between symptom-based principal factors of rheumatoid arthritis (RA) and the effect of traditional Chinese medicine or Western medicine on RA after factor analysis of RA symptoms. METHODS: Four hundred and thirteen RA patients from 9 clinical centers were included in the clinical trial. They were randomly divided into Western medicine (WM) treated group with 204 cases and traditional Chinese medicine (CM) treated group with 209 cases. A complete physical examination and 18 common clinical manifestations were recorded before the randomization and after the treatment. The WM therapy included voltaren extended action tablet, methotrexate and sulfasalazine. The CM therapy included glucosidorum Tripterygll totorum tablet and Yishen Juanbi Tablet. The American College of Rheumatology 20 (ACR20) was used for efficacy evaluation. All data were analyzed on SAS 8.2 statistical package. Eighteen symptoms in the RA patients were analyzed by factor analysis and the relationships between the factors and effects were analyzed with Chi-Square test. RESULTS: Four principal factors were obtained from the analysis results of the 18 symptoms. The factors could represent the symptoms related to joints, cold-syndrome, deficiency syndrome and heat-syndrome in traditional Chinese medicine (TCM), respectively. The effect of WM therapy was better than CM therapy. After 12 weeks of treatment, the effect of CM on patients without deficiency-syndrome was better than the patients with deficiency-syndrome. After 24 weeks of treatment, WM therapy showed better effect on patients with cold syndrome than patients without cold syndrome. CONCLUSION: The results based on the factor analysis of RA symptoms are similar to the results of syndrome differentiation of TCM, and the factor-related different categories of symptoms are associated with the curative effect, thus further research on the symptoms is necessary.
6.Study on human UDP-glucuronosyltransferase (UGT) isoforms involved in in vitro metabolism of trans-resveratrol.

OBJECTIVETo study major human UGT isoforms involved in trans-resveratrol (TR) phase II metabolism.
METHODtrans-resveratrol and 12 major human UGT isoforms were incubated in vitro and then glucuronic acid metabolites were determined by HPLC-MS, in order to preliminarily analyze the structure and observe the effect of different UGT isoforms on the generation rate of glucuronic acid metabolites.
RESULTIn in vitro metabolic system, two metabolites-4'-O-monoglucuronide resveratrol (M-1) and 3-0-monoglucuronide resveratrol (M-2)-were generated from trans-resveratrol after being catalyzed by UGT. During the cause, generation of M-1 and M-2 were catalyzed by UGT1A1, UGT1A3, 1A8, 1A9 andlA10, whereas only UGT1A6 and 1A7 contributed to the forma-tion of M-2. Both the formation rate of M-1 and M-2 catalyzed by UGT1A1, 1A10 and the formation of M-2 catalyzed by UGT1A8 slowed down with the increasing concentration of substrates, causing the phenomenon of "substrate inhibition".
CONCLUSIONUGT1A1, 1A8, 1A9, 1A10 get involved in the formation of M-1, and of them UGTIA9 is the most important contributor. UGT1A3 also makes small contribution to the formation of M-1 and M-2, while other UGT isoforms show hardly any reaction with the trans-resveratrol phase II metabolites.

7.Species analysis of fungi isolated in a laboratory from patients with mycoses during 1960-2006

Objective To analyze fungal isolates from patients with superficial fungal infections during 1960-2006.Methods Fungal strains isolated from patients with superficial (mucocutaneous and cutaneous)fungal infections and identified in the Medical Mycology Clinical Laboratory,Department of Dermatology and Venereology,Union Hospital,from 1960 to 2006 (data from September 1991 to July 1992 were unavailable),were subjected to a classification and statistical analysis.Clinical samples for mycological examination were taken from outpatients or inpatients of different departments in hospitals of Hubei province and surrounding areas.Morphological,physiological and biochemical methods were applied for species identification.Results A total of 11 989 Candida strains were isolated,which belonged to 23 species and 16 genera.They fell into 3 groups,i.e.,dermatophytes,Candida and yeasts (including Malassezia),and non-dermatophyte moulds.Since 287 strains of moulds were suspected to be contaminating fungi,11 702 residual isolates were analyzed.Of the analyzed isolates,Candida species (5642/11 702,48.2% )and dermatophytes (5279/11 702,45.1% )predominated,followed by yeasts (449/11 702,3.8%) and Malassezia species (332/11 702,2.8%).The most frequently isolated species was Trichophyton rubrum (3865/11 702,33.0%),Candida albicans (3110/11 702,26.6% ) and non-albicans Candida species (2532/11 702,21.6% ).Dermatophyte strains were mostly isolated from lesions of smooth skin with an exception of palmoplantar and interdigit regions (1787/5279,37.7%).The most common dermatophyte species was Trichophyton rubrum,followed by Trichophyton violanceum.Candida was mainly isolated from mucous membrane lesions (4099/5642,72.7%),with Candida albicans being the predominant species.Conclusions Candida species and dermatophytes predominate in patients with superficial fungal infections during 1960-2006,with Trichophyton rubrum being the most common species.

9.The predictive value of writhing stage general movement assessment for motor development outcomes in infants with severe neonatal jaundice

Objective To study the predictive values of the general movements (GMs) assessment in writhing stage for motor development outcomes in infants with severe neonatal jaundice.Method From December of 2012 to December of 2017,infants with severe neonatal jaundice (serum bilirubin reaching the corresponding level of exchange transfusion according to the reference nomogram) in our hospital were enrolled in the study.Inclusion criteria included corrected gestational age of 37 to 48 weeks,serum bilirubin level below phototherapy intervention value after treatment and general and detailed assessment were carried out in writhing stage when the infant was stable.The patients were regularly followed-up until one-year-old to evaluate the predictive values.Result A total of 241 patients with severe neonatal jaundice were enrolled in the study,including 153 males (63.5%) and 88 females (36.5%),with gestational age between 35 and 42 weeks.The mean gestational age was (37.9± 1.8) weeks,the average birth weight was (3 057±480) g,and the mean serum bilirubin value was (458.9± 119.1) μmol/L.The general evaluation of the GMs was normal in 15 cases (6.2%),and abnormal in 226 cases (93.8%) with 217 cases (90.0%) were poor repertoire (PR) and 9 cases (3.7%) were cramped-synchronized (CS).The predictive values of abnormal GMs for abnormal motor development outcomes were as following:sensitivity 100%,specificity 7.6%,negative predictive value(NPV) 100%.The predictive values of CS for cerebral palsy were as following:sensitivity 22.2%,specificity 97.8%,NPV 94.0%.Detailed evaluation of 241 subjects showed that 13 items had statistically significant differences in the prediction of cerebral palsy (P<0.05),and 18 items in the prediction of abnormal motor development (P<0.05).Conclusion The CS pattern and detailed assessment of GMs in the writhing stage may be correlated with the outcomes of motor development in infants with severe neonatal jaundice until one-year-old.
10.Complete genome sequencing and traceability analysis of SARS-CoV-2 in Binhai New Area, Tianjin

Objective:To analyze the whole genome traceability and variation analysis of SARS-CoV-2 in local COVID-19 outbreaks in Binhai New Area, Tianjin.Methods:The whole-genome high-throughput sequencing was performed on throat swab samples collected from one local asymptomatic infected person and five confirmed cases of COVID-19 in Binhai New Area of Tianjin from November 7 to December 5, 2020. The sequencing data were assembled by De novo. MAFFT v7.0 multiple sequence alignment program and MEGA X software were used to compare the above data and construct phylogenetic tree (Neighbor-joining method).Results:The genetic similarity between the sequences of 6 SARS-CoV-2 strains and Wuhan reference sequence (Wuhan-Hu-1) was greater than 99.9%. Two of six strains were genetically identical, conform to the L-Lineage European Branch Ⅱ.1(America Branch)/B.1; The other four strains had the same genes and were in line with the characteristics of L-Lineage European Branch Ⅰ/B.1.1.These six strains belonged to different evolutionary branches and two different transmission chains. There were 18 nucleotide mutation sites in sequences of six SARS-CoV-2 strains, eight of which were synonymous mutation sites, nine of which were missense mutation sites, resulting in nine amino acid mutation sites, and important mutation sites of RDRP-P323L and S-D614G were found in all of the six samples.Conclusions:In this study, there were two COVID-19 outbreaks in Binhai New Area of Tianjin, and the sequences of six SARS-CoV-2 strains belonged to different evolutionary branches and two different transmission chains. It might come from porters′ contact with imported cold chain items contaminated with SARS-CoV-2 from different sources. All the sequences of six SARS-CoV-2 strains had P323L and D614G mutations, which indicated that the virus mutation and transmission ability were stronger. The surveillance of important employees of the cold chain in Tianjin and local and imported cases should be continuously strengthened.
