1.Clinical analysis for systemic complications of patients in the dental emergency room

Objective:To analyze the clinical features of oral emergency patients in general health, and to guide the oral emergency patients in the treatment, and prevention of accidents.Methods:The retro-spective study was performed on the day visits to the emergency department in Peking University School and Hospital of Stomatology from February to November 2013.The basic information of the newly diag-nosed adult, their overall health status, American society of anesthesiologists(ASA) classification and oral diagnosis information were analyzed and summarized.Results:The cardiovascular and cerebrovascular diseases(6.4%), hypertension(13.4%), endocrine system diseases(4.2%)were most common in the patients.With the increase of age, the associated systemic diseases had increased in number and in ASA classification.Periapical disease ( 24.6%) , dental pulp disease ( 24.4%) , and periodontal disease (18.0%) accounted for about 2/3.Conclusion:The general health status of patients with oral emergen-cy worsened with the increase of age.Oral surgeons should make risk assessment before treatment.ASA classification system can help doctors predict the patient risk, take corresponding measures in advance, and help to reduce the occurrence of accidents.
2.Analysis of medical disputes in the emergency department of the hospital in a decade

Emergency departments of hospitals are exposed to risks of patient complaints and disputes, and the emergency department of a stomatology hospital is no exception. Medical disputes at the emergency department of Peking University Stomatology Hospital, as registered at the department of medical affairs of the hospital in the past ten years, are analyzed with statistical processing. The results are used in studying characteristics of medical disputes frequently found in stomatology emergency cases, for references and recommendations on prevention and avoidance of such disputes in future.
3.Design and application of Managerial and Maintenance System of Army Medical Equipments

In order to meet the requirements of logistic supply in peacetime and wartime, General Health Department developed Managerial and Maintenance System of Army Medical Equipments, which is the important subject of 10th 5-year Army Medical Research. This system is based upon Basic Medical Equipments Distribution Standard, Army Medical Equipments Management System, joint support system and three-level maintenance network. An efficient platform for whole-survival-circle medical equipments monitoring is developed using computer information technology. This system can greatly advance the informatized, normative management of medical equipments, and thus is of avail in military and economic aspects.
4.Clinical analysis of dental trauma patients in the emergency room

0.05).Maxillary incisors were easily to be affected,especially the maxillary central incisor.Crown fracture was the most common traumatic type.The combination of dental trauma and oral and maxillofacial soft tissue injury was experienced by 692 patients(71.0%).Conclusion: Dental trauma was one of the common diseases in oral emergencies.Knowledge of the etiological and clinical characteristics of emergency dental trauma could help us to treat them and make specific preventive measures.Clinical examination should be comprehensive and meticulous to avoid misdiagnosis,for emergency dental trauma often involving multiple teeth,and associated with different degrees of maxillofacial soft tissue injury.
5.Effects of Extracts of Ginkgo Leaves on the Experiment Myocardial Ischemia and Blood Hemorheology

Objective To observe the effects of extracts of ginkgo leaves (EGL) on acute myocardial infarction, acute myocardial ischemia and blood hemorheology.Methods The effects of EGL on acute myocardial infarction were observed in dogs model induced by the ligation of anterior descending branch of left coronary artery; effects on acute myocardial ischemia were observed on rats model caused by intravenous injection of pituitrin and effects on blood hemorheology were observed in rabbits. Results EGL significantly reduced the myocardial infarction area, decreased the activities of lactate dehydrogenase and creatine phosphokinase in dogs, reduced the elevation of ST segment of electrocardiogram in rats with myocardial ischemia and decreased whole blood viscosity and plasma viscosity in rabbits. Conclusion EGL has protective effect against myocardial ischemic injury and can improve the parameters of blood hemorheology in rabbits. Its mechanism may be concerned with the reduction of the myocardial infarction area and infarction degree and the relief of acute myocardial ischemia.
6.Clinical Efficacy of Benzoyl Peroxide Gel with Different Concentrations in Acne Vulgaris

Objective To assess the clinical efficacy and safety of benzoyl peroxide gel(BPG)with different concentrations in the treatment of acne vulgaris,and to compare the quality between the domestic products with imported products.Methods The study was an open-controlled clinical trial.The patients were divided into4groups:imported2.5%,5%,10%gel and domestic5%gel.All preparations were ap-plied twice daily for6weeks.Study visits took place at baseline and week2,4and6.Results Different concentrations of benzoyl peroxide gel were effective for inflammatory lesions.The longer the course of treat-ment and the higher the drug concentration were,the better global clinical efficacy was,and the optimum concentration was5%or10%.In addition,the higher the drug concentration was,the higher adverse reac-tion rate was.Transient and mild local skin irritation occurred but was well tolerated by the patients.The imported benzoyl peroxide5%gel was as effective as domestic benzoyl peroxide5%gel,but the adverse re-action rate was less than the latter.Conclusion Different concentrations of benzoyl peroxide gel are all ef-fective and safe in the treatment of acne vulgaris,with the optimum concertration is5%or10%.
7.Risk assessment of different grades of hypertension during the treatment of patients with acute pulpitis

Objective:To evaluate the vital signs changes,influence factors in different grades of hy-pertension patients during the treatment of acute pulpitis,in order to obtain the risk prevention measures. Methods:In this study,90 different grades of hypertension patients with acute pulpitis were recruited from February 201 4 to February 201 5 in the Department of Oral Emergency,Peking University School and Hospital of Stomatology.The information about the patients’general health,oral treatment,life signs of change information was collected.Patients were divided into high risk group,middle risk group, and low risk group (30 patients for each group).Results:(1 )Compared with the preoperative,systolic blood pressure (90%),diastolic blood pressure (80%),heart rate increase (1 00%)were increased in the high risk group.The increase rates of the middle risk group and the low risk group were significantly lower than those of the high risk group (P<0.01 ).At the same time,the systolic blood pressure of 1 /4 (26.7%)patients in high risk group increased more than 20 mmHg (1 mmHg=0.1 33 kPa),and the diastolic blood pressure of 2/5 patients in high risk group increased more than 1 0 mmHg,the difference was statistically significant compared with the other two groups (P<0.05).(2)Compared with the pre-operative,the average increase of the maximum peak were increased [systolic blood pressure (1 8.0 ± 1 .5)mmHg,diastolic blood pressure (8.0 ±1 .7)mmHg],the mean of heart rate changes [(7.0 ± 0.3)beats per minute]was also increased in the high risk group,while these two indicators were de-creased in the low risk group and the middle risk group.The electrocardiogram (ECG)was changed in 6 cases during the treatment in the high risk group.No significantly changed were observed in the low risk group and the middle risk group.(3 ) Compared the risk assessment in preoperative with that in postoperative,in the middle risk group,23 cases were evaluated as medium risk in final evaluation,6 as low risk,and 1 as high risk (risk assessment increased);in the high risk group,20 cases were evaluated as high risk,7 as very high risk,and 3 as medium risk (risk assessment decreased).Conclusion:Oral treatment is very safe for patients with hypertension,but the risk factor,target organ damage,and com-plications will also increase the risk of cardiovascular events in elderly patients during the acute pulpitis treatment.Dentist should take some measures to avoid the risks.
8.Clinical investigation of the positioning accuracy of acute pulpitis pain.

OBJECTIVEThis study aims to investigate the positioning accuracy of acute pulpitis pain and its possible factors.
METHODSThe clinical symptoms and physical signs of 3 432 cases of acute pulpitis were recorded and analyzed by using questionnaire forms, which included age, gender, tooth position, infection origin, pain history, time of acute attack, duration and nature of pain, pain frequency, referred pain areas, percussion examination, temperature pulp test, pulp bleeding, and positioning accuracy. Univariate analysis and multivariate stepwise regression analysis were used for data processing.
RESULTSPain location was accurately identified by 39.1% of the patients with acute pulpitis. Referred pain could reduce the positioning accuracy of pain (P < 0.05), whereas infection originating from the periodontium could raise it (P < 0.05). Other factors might not be directly related to positioning accuracy (P > 0.05).
CONCLUSIONSome cases of acute pulpitis pain can be located accurately. Referred pain and periodontium infection origin are related to the positioning accuracy of acute pulpitis pain. The exact cause of this correlation needs further study.

Epidemiology is a discipline characterized by complicated theory and practice.How to make the practice course function better is a topic worthy of exploring in educational reform for clinical students.The article explored the‘Student-Dominated’ Model based on ‘Problem-Based Learning ’ and ‘Team Based Learning ’ in teaching process and compared the model with the traditional one ( Teacher-Dominated Model) .Suggestions were given to further improve effectiveness of epidemiology practice courses.

Objective To obtain a better understaning of the clinical features of Castleman tumor associated paraneoplastic pemphigus. Methods The clinical features and therapy of 10 cases of this disease, diagnosed in the Department of Dermatology of Peking University First Hospital were analyzed. Results Castleman tumor was shown to be the most common neoplasm associated with paraneoplastic pemphigus in China. The clinical presentations, histopathologic characteristics, CT scan findings, and immunologic features were all unique. The early diagnosis and removal of the Castleman tumor are crucial for the treatment of this tumor-associated autoimmune disease. Conclusions Because Castleman tumor is directly related to the induction of autoimmunity, early diagnosis and prompt removal of the tumor are essential to the management of this disease.
