1.Effect of intrathoracic injection sodium hyaluronic acid on prevention of pleural thickening in patients with excess fluid of tuberculous thoracic cavity

AIM: To evaluate the effect of hyaluronate based gel (HAbg) in prevention of pleural thickening and adhesion in patients with tuberculous pleural effusions. METHODS: 52 patients with middle amount of tuberculous thoracic fluid accumulated, fluid bound, and pleura thickened more than 2 mm were divided into two groups at random. The patients of two groups were all given standard treatments with anti tuberculous drugs. HAbg was injected into intra thoracic cavity in treatment group (27 patients), while saline (NS) was introduced into intra thoracic cavity in control group (25 patients). Before and after HAbg injection, thoracic fluid routine and protein quantification were tested respectively. Pulmonary functions were measured after 2 weeks and 3 months of thoracic fluid absorption. RESULTS: Patients treated with HAbg showed significantly thinner pleura, lower proteins and white blood cell count in thoracic fluid, and higher FEV1% and FVC% for their pulmonary function in comparison with the control group. CONCLUSION: Intrathoracic injection of HAbg can prevent pleural thickening and improve the lung function in patients with excess fluid of tuberculous thoracic cavity.

AIM: To investigate the effects and safe of low molecular weight heparins (LMWH) in the treatment of patients with chronic pulmonary heart diseases (CPHD) at hypercoagulability state. METHODS: 65 patients were randomly assigned to treatment group (n=33) and control group (n=32). All patients were given routine treatments. The patients of treatment group were hypodermically injected with 4 100~6 150 U of LMWH per day for 7 days as a course. RESULTS: After 7 days of the treatments, the patients of treatment group showed better curative effect and significantly increase in PaO_2 and AT-III and decrease in PaCO_2 and FIB in comparison with the patients in control group (P
3.National external quality assessment for HLA low-resolution molecular typing: 2005 -2008 activities

Objective National external quality assessment (EQA) results from 2005 to 2008 for HLA class Ⅰ (A, B) and class Ⅱ (DRB1) low-resolution DNA typing were summarized with the goal of exploring strategies to assure and improve HLA DNA typing performance in clinical testing. Methods HLA allele results from the four consecutive years EQA events were analysed. Different kinds of errors were described and classified, and the possible causes were discussed. Results Participant laboratories were increasing in the four consecutive years with the number of 22, 28, 47 and 61 in 2005, 2006, 2007 and 2008,respectively. 2 844 HLA DNA typings were returned from the participants during the 4 years EQA surveys, and overall 30 errors (1.05%) were identified. These 30 errors were classified into two major types of errors including 25 technical genotyping errors and 5 human errors. The proportion of laboratory participants which made mistakes was 13. 6%, 10. 7%, 10. 6% and 16.4% in 2005, 2006, 2007 and 2008, respectively. Conclusions Above 10% of participant laboratories exhibited errors in the four consecutive years HLA molecular typing EQA surveys. Relevant important attentions should be greatly paid to clinical HLA molecular typing test.
4.Application of in situ Y chromosomal hybridization in the detection of mesenchymal stem cells

BACKGROUND:Transplantation of mesenchymal stem cells (MSCs) to multiple sclerosis (MS) is a new developed treatment to be given more and more attention.However,whether the MSCs function by cell replacement after going cross the blood-brain barricr or immune suppression needs further confirmation.OBJECTIVE:To establish a steady and effective method of Y chromosome in situ hybridization (ISH) and to detect the distribution of MSCs in a mouse model with MS.DESIGN,TIME AND SETTING:The observational study was performed at the Laboratory of Molecular Immunology,Institute of Healthy Science from September 2007 to February 2009.MATERIALS:A total of 30 C57BL/6 mice,aged 6-8 weeks and weighing 15-20 g,were selected.Female mice were used to establish experimental autoimmune encephalomyelitis (EAE).METHODS:Y chromosme specific DNA probe labled with digoxine (DIG) was designed and ISH was performed to confirm that the designed probe was Y chromosome specific and biologically sensitive.We transplanted the MSCs from male mice into the female EAE mice,and tracked the MSCs by Y chromosome ISH.MAIN OUTCOME MEASRUES:Distribution of MSCs in mice with EAE was observed under the optical and fluorescence microscope.RESULTS:The probe was confirmed to be Y chromosome specific and biologically sensitive.What's more,a steady and effective method of ISH was established.Some MSCs were detected in the periphery organs-spleen and lymph node of mice with EAE;seldom in spinal cord which indicated that MSCs might play its roles by immune suppression.CONCLUSION:Some MSCs were detected in the periphery organs-spleen and lymph node of mice with EAE,but seldom in spinal cord in the central nervous system.

Intravenous glucose tolerance test(IVGTT)and oral glucose-insulin releasing test(OGIRT) in 12 subjects with normal glucose tolerance were performed.The results showed that the peak of insulin secretion Was at 40 minutes during()GIRT.The ratio of the change of insulin-to-glucose at 40 minutes(△I40/△G40)is an index in reflecting insulin sensitivity and β-cell function.
6.Screening of core genes in pediatric hepatoblastoma based on omics data mining and co-expression network model

Objective:Based on the microarray data mining method, the function and pathway of differential genes were analyzed after the differential genes were screened. At the same time, the core genes that determine the prognosis of pediatric hepatoblastoma were screened by coexpression network, and their predictive ability was evaluated.Methods:The microarray expression profile of pediatric hepatoblastoma used in this study was from the European Institute of bioinformatics (http: //www.ebi.ac.uk/embl/). The deadline for data collection was December 31, 2018. Firstly, the differentially expressed genes (gene expression level increased to 2 times or decreased to 1/2 of the original) were screened by SAM method, then the core genes were screened by coexpression network model based on dimension reduction principle, and the gene regulation evaluation score was calculated by MCODE algorithm to evaluate its regulation ability in the whole network model.Results:According to the enrichment results of 213 differentially expressed genes, the highest enrichment degree of signal pathway was metabolic pathways (2 122.529). The misjudgment rate of signal pathway enrichment results was less than 0.001, and the misjudgment rate was statistically significant by SAM method ( P<0.001). A total of 213 differentially expressed genes in different prognosis groups were used as the basis for the construction of the coexpression network. A total of 12 differentially expressed genes were included in the coexpression network. Using the poor prognosis group as the experimental group, and the better prognosis group as the control group, the MCODE algorithm was used to calculate the gene regulatory ability score. The results showed that the highest gene for determining the prognosis control ability of children hepatblastoma was ADH1A gene with a score of 19. In addition, the regulatory ability scores of HAO1, ADH1B, ALDOB and DPYS genes were higher than or close to 5, so they could be considered as the core genes in the coexpression network module. Conclusions:According to the results of coexpression network model, ADH1A gene is closely related to the occurrence and development of hepatoblastoma in children, and its molecular biological evidence needs to be further explored to guide the clinical development of tumor targeted intervention therapy.
7.Application of Canadian Occupational Performance Measure on Survivors from Sichuan Earthquake

Objective To apply Canadian Occupational Performance Measure (COPM) on the patients injured in Sichuan Earthquake. Methods COPM was applied to evaluate 51 patients in hospital before and 1 months after treatment. The first evaluation confirmed the problems on occupational activities for them. Then the plan was made to solve the problems. The second evaluation was to assess the effect of the treatment.Results The problem of self-care activity is more than that of productive activities and leisure activities(P<0.01). Total scores of performance of occupational activity and satisfaction improved(P<0.01, P<0.01)Conclusion COPM is helpful to confirm the problems of occupational activities and contribute to develope the primary goals for rehabilitation and treatment programs.
8.Biological characteristics of mesenchymal stem cells from bone marrow of patients with aplastic anemia.

To investigate the differences of proliferation capacity and phenotype properties of mesenchymal stem cells (MSCs) derived from bone marrow (BM) of aplastic anemia patients, fetuses and children, MSCs were isolated from BM of patients with aplastic anemia and expanded in vitro; MSCs derived from BM of fetuses and children were used as normal control groups, three sources of MSCs were compared by morphology, in vitro proliferation capacity, phenotype and immunocytochemistry. The results showed that MSCs could be isolated and expanded from aplastic anemia patient BM. MSCs derived from BM of aplastic anemia patients shared a similar morphology and phenotype with derived MSCs from BM of fetuses and children. However, in vitro proliferation capacity of MSCs derived from BM of aplastic anemia patients after 20 population doublings (PD) was significantly lower, compared with MSCs from BM of fetuses and children. BM MSCs derived from children and fetuses proliferated for more than 30 PD. It is concluded that BM MSCs from aplastic anemia patients appears to be normal in phenotype but their proliferation capacity is lower in comparison with control groups.

9.Development and systematical evaluation of an up-converting phosphor technology based lateral flow assay for quantitative detection of Burkholderia pseudomallei.

OBJECTIVETo develop an up-converting phosphor technology-based lateral-flow (UPT-LF) assay for rapid quantitative detection of Burkholderia pseudomallei on site.
METHODSThe strip Bps-UPT-LF strip was prepared with up-converting phosphor (UCP) particles as the bio-label using double-antibody sandwich method. Detection performance, including sensitivity, quantitative accuracy, precision, and specificity, were first evaluated using bacterial suspensions of Burkholderia pseudomallei, the related species and the strains which had similar routes of transmission with serial standard concentrations diluted by phosphate buffer, then biological and chemical reagents and simulated samples with series concentrations were employed for sample tolerance evaluation, while the operation error during on site detection was also evaluated through adjusting liquid measure.
RESULTSThe whole detection was accomplished within 20 minutes, and the sensitivity was 10(4) CFU/ml with linear quantitative range from 10(4) CFU/ml to 10(7) CFU/ml, which covered four orders of magnitude. Bps-UPT-LF strip demonstrated high specificity with the absence of any false-positive result even at 10(7) and 10(8) CFU/ml of non-specific bacterial contamination. Not only Bps-UPT-LF strip could tolerate to high concentration of the extreme acid and basic matter (pH 1-12), saline matter (≤ 2 mol/L mixture of NaCl and KCl), viscous materials (≤ 50 g/L of PEG 20000 and ≤ 20% of glycerol) and bio-macromolecule (≥ 400 g/L of bovine serum albumin or ≥ 80 g/L of casein), but also it can directly detect animal, environmental and powder specimen, such as ≥ 400 g/L of milk powder, flour powder, fruit juice, fresh and decomposed viscera, and ≤ 200 g/L of putty powder, sucrose, gourmet powder, and soil. Operation errors of liquid measure had few effects on sensitivity and specificity, including -50%-200% of sample, -22%-44% of sample-treating buffer and -30%-30% of loading mixture.
CONCLUSIONThe good detection performance and tolerance performance bring the bright future for Bps-UPT-LF strip to detect Burkholderia pseudomallei on site rapidly and quantitatively for nature foci surveillance and anti-bioterrorism.

10.Study on the relationship between urinary polycyclic aromatic hydrocarbons metabolites and pulmonary function in community population

Objective:To investigate the relationship between urinary polycyclic aromatic hydrocarbons(PAHs) metabolites and pulmonary function in community population.Methods:A total of 4 812 participants were recruited from two communities in Wuhan city from April to May 2011 and two communities in Zhuhai city in May 2012. Information of demographic characteristics and life style was collected by semi-structural questionnaire. Physical examination was performed and pulmonary function was measured. Morning urine was also collected. The concentration of 12 urinary PAHs metabolites was tested and classified into four types by chemical structure, including hydroxynaphthalene, hydroxyfluorene, hydroxyphenanthrene and hydroxypyrene. The level of pulmonary function in different group of urinary PAHs metabolites was compared by using covariance analysis. The association of the urinary PAHs metabolites and pulmonary function was analyzed by using generalized linear model, and the interaction of different population characteristics and life style on the association was analyzed.Results:The age of participants was(51.99±13.64) years old, and 67.66% ( n=2 565) of the population were women. The M ( P25, P75) of concentration of total urinary PAHs metabolites was 5.72 (3.91,8.72) μg/mmol Cr. After controlling for variables including age, gender, city, Waist-to-Hip Ratio, smoking status, drinking situation, physical activity, cooking meals at home or not, kitchen ventilation and exposure to dust as confounding factors, generalized linear model showed that each 1-unit increase in log-transformed levels of hydroxynaphthalene, hydroxyfluorene, hydroxyphenanthrene, hydroxypyrene and total PAHs was associated with 26.83 (95% CI: -48.18, -5.48) , 21.86 (95% CI: -40.49, -3.23), 26.18(95% CI: -48.27, -4.09), 34.95 (95% CI: -55.95, -13.94), and 35.23 (95% CI: -58.93, -11.54) ml reduction of FVC and 29.36 (95% CI: -47.23, -11.48), 20.79 (95% CI: -36.39, -5.19), 22.65 (95% CI: -41.15, -4.15), 31.44(95% CI: -49.03, -13.85), and 33.20 (95% CI: -53.04, -13.36) ml reduction of FEV 1 respectively (all P values<0.05). Compared to non-alcohol users and participants with home cooking, the association was more evident in alcohol users and participants without home cooking( P for interaction<0.05). Conclusion:The exposure to PAHs was associated with decreased pulmonary function, and the association was more evident in alcohol users and participants without home cooking.
