1.Clinical and pathological features of colon polyps and risk factors for colon polyps

Objective To investigate the clinical and pathological features of colonic polyp canceration and related risk factors of colon polyps,and explore clinicopathologic features and major risk factors of the canceration of polyps of colon polyps.Methods Totally 80 patients with colon polyps including 6 cases of cancerations and 20 patients with colon cancers were selected from Xuzhou Tumor Hospital Admissions.Those patients were divided into 74 cases of non cancer group and 26 cases of cancer group.The clinical data,the detection index and the statistical analysis were performed.Results The clinical features of patients included 51 cases of multiple polyps,30 cases of fatty liver,13 cases of fecal occult blood positive,and 30 patients with colonoscopy.There were 218 polyps,191 data intact,and 6 cases showed cancer (3.14%).Inflammatory polyps mainly occurred in rectum,cecum,and significant risk other than the colon.Tubular adenoma occurred mainly in the transverse colon.Sigmoid colon cancer colon polyps of the highest risk showed OR =2.30 (P < 0.05).The cancer risk from high to low was from tubular villous adenoma to villous adenoma hair (P <0.01),and no difference in tubular adenoma cancer risk (P >0.05).There was no difference between the indexes of the canceration group and the non cancerous group (P > 0.05).The single factor analysis showed pathological type,and the lesion were the main risk factors of malignant colonic polyps (P < 0.05).Multi-factor regression analysis showed WBC,pathological type,and the lesion were the main risk factors of malignant colonic polyps (P < 0.05).Conclusions The colonic polyps in male have higher incidence with fatty liver,hypertension and other pathological features,and are mainly tubular adenomas,inflammatory and hyperplastic polyps,with different lesions,pathological types occur and different cancer risk.
2.The value of HP combined with serum CEA, CA72-4 and CA24-2 in early gastric cancer screening

Objective To investigate the value of Helicobacter pylori (HP),blood carcinoembryonic antigen (CEA),carbohydrate antigens 72-4 (CA72-4) and CA24-2 in the screening of gastric cancer.Methods The contents of HP,blood CEA,CA72-4 and CA24-2 in 50 healthy controls,45 patients with benign gastric lesions mnd 70 patients with gastric cancer were detected.According to the number of positive cases,the positive rate of single detection and the positive rate of combined detection were calculated.Sensitivity,specificity,and accuracy of serum tumor markers were calculated according to HP single and combined detection.Results The positive rate of each individual detection and 4 joint detection in gastric cancer group was significantly different from that in the benign lesion group and the normal control group (P <0.01).The positive rate of 4 joint detection in gastric cancer group was 89.25%,and the difference was statistically significant (P <0.01).There was no significant difference in the positive rate of the single test and the 4 joint detection in benign stomach group and normal control group (P > 0.05).The sensitivity,specificity and accuracy of HP,blood CEA,CA72-4,CA24-2 joint detection were significantly higher than single detection (P < 0.05).Conclusions Joint detection was significandy better than single detection,and can significantly improve the detection rate of gastric cancer.
3.Non-surgical transcatheter arterial implant and subcutaneous infusion port for chemotherapy of hepato-gastrointestinal neoplasms

The authors reported their experience in treatment of 17 patients with hepato-gastrointestinal neoplasms with lipiodol embolization chemotherapy via subcutaneous infusion port by a non-surgical method since March, 1994. According to the seldinger method implanted catherter was placed in the suitable target artery, and the Subcutaneous infusion port was linked to the implanted catheter through a subcutaneous tunnel. Antitumor chemotherapeutic drugs, biological response modifiers and lipiodol were injected through the port. These results suggest that the non-surgical implant method is simple, safe and effective for regional chemotherapy.

5.Protective effects of glutamine pretreatment on occludin protein in rats with intestinal ischemia-reperfusion injury

AIM:To determine the effects of glutamine ( Gln) pretreatment on occludin protein in the rats with intestinal ischemia-reperfusion ( I/R ) injury.METHODS: Male Wistar rats ( n =30 ) were randomly divided into 3 groups (n=10):sham group, I/R group and Gln pretreatment group.The rats in Gln pretreatment group were pretreated with Gln at dose of 1 g? kg-1? d-1 by orogastric route for 7 d, and those in the other 2 groups were pretreated with the same volume of normal saline .Intestinal I/R was induced by 30-min occlusion of the superior mesenteric artery followed by 24 h of reperfusion.After the operation, the levels of IL-10, IL-2, TNF-α, SOD and MDA were measured.The occludin protein was determined by the methods of immunohistochemistry and Western blotting .RESULTS: The occludin protein level in I/R group was significantly lower than that in sham group and Gln group (P<0.05).The levels of MDA and TNF-αin I/R group were significantly higher than those in sham group and Gln group ( P<0.05 ) .The levels of SOD , IL-10 and IL-2 in I/R group were significantly lower than those in sham group and Gln group ( P<0.05 ) .CONCLUSION:Glutamine has a protective effect on occludin protein in intestinal ischemia-reperfusion injury .The mechanism may be rela-ted to oxidative stress response and inflammatory inhibition .
6.Dynamic changes of NT-3 and FGF in hippocampus and thalamus after cerebral ischemia reperfusion in vascular dementia rats

Objective To investigate the correlations between vascular dementia and neurotrophin-3 (NT-3), fibroblast growth factor (FGF). Methods Female and male Wistar senile rats were divided into the control group and the operation groups at random. The operation groups consisted of the cerebral ischemia 15 min group and ischemia 15 min reperfusion groups in which the rats were reperfused at 0, 1st, 6th hour and on 2nd, 4th, 9th day with five rats per group. The expressions of NT-3 and FGF in hippocampus and thalamus were examined in different periods of ischemia-reperfusion with ABC method of immunohistochemistry. Results There was a little expressions of NT-3 and FGF in hippocampus and thalamus of normal senile rats; the expression of NT-3 in hippocampus increased quickly and steadily from 1 h to 4 d after ischemia-reperfusion! there were significant differences compared with control group (P0. 05). The expression of FGF in hippocampus increased from 6 h to 2 d after ischemia-reperfusion (P 0. 05). FGF in thalamus began to increase on 2nd day after ischemia-reperfusion and there were significant differences compared with control group (P0. 05). Conclusion Hippocampus has the protective mechanism of NT-3 and FGF against ischemia injury which is produced quickly and has endurable effects, but thalamus lacks the good protective mechanism of NT-3 and FGF.

8.Assessment of quality of life in patients with chronic urticaria and reliability of the Dermatology Life Quality Index (DLQI)

Objective To evaluate the influence of chronic urticaria on the quality of life in patients by the DLQI, and to assess the factor structure and reliability of the DLQI. Methods The DLQI (Chinese version) was used to evaluate the quality of life of outpatients with CU. The reliability and factor structure of the DLQI were evaluated by reliability analysis and factor analysis. Results Eighty-five patients were asked to complete the DLQI and 13 patients were excluded for other complicated disorders or unanswered items in the DLQI. The mean age was 33.43 ± 0.90 years in the 72 patients with a median clinical course of 6 months. The DLQI score, which varied from 1 to 23 with an average of 9.46 ± 0.61, was uncorrelated to the patients' age,sex, or clinical course of CU (all P > 0.05). The impact of CU was observed mainly on the "synptoms and feeling" (50%), followed by "work and study" (40.67%). Factor analysis indicated that there were two dimensions with eigenvalues greater than 1.0, accounting for 53.7% of the variances. Reliability analysis showed that the Cronbach's α value was 0.836, and reached 0.849 if the item 1 in the DLQI was deleted. An analysis of variance showed that the F value was 31.88 (P < 0.01 ), and Hotelling T2 test yielded an F value of 29.87 (P <0.01 ). Conclusions CU has a moderate impact on the quality of life in patients. DLQI can be applied to the evaluation of quality of life in patients with CU with a high reliability, but more attention should be paid to the interference from item 1 to the reliability of the assessment.
9.The protective effect of glutamine pretreatment on intestinal ischemia-reperfusion injury and eNOS/NO levels in rats

Objective To investigate the protective effect of glutamine(Gln) pretreatment on intestinal ischemia-reperfusion (I/R) injury and endothelial nitric oxide synthase (eNOS)/nitric oxide (NO) signaling pathway in rat model. Methods Thirty male Wistar rats were randomly divided into three groups(n=10 for each group):sham group, I/R group and Gln group. Animals were pretreated with 1 g/(kg·d)Gln by orogastric route for 7 days in Gln group, and normal saline was given to the other two groups in the same dose. Intestinal I/R was induced by 30 min occlusion of the superior mesenteric artery followed by 24 h of reperfusion. After the operation, the intestinal histopathological changes, the plasma endotoxin level, serum D-lactic acid, eNOS, inducible NOS(iNOS)activity and NO levels were detected by ultraviolet spectrophotometer. The mRNA expressions of myocardial eNOS and iNOS were detected by real-time fluorescence quantitative PCR (RT-PCR). Results After reperfusion, in IR group, extensive epithelial sloughing and mucosal ulceration of villous tips were observed, whereas these findings did not occur in Gln group and sham group. Compared with IR group, the serum NO, eNOS levels and eNOS mRNA expression of intestinal tissue were elevated in Gln group (P<0.01), but the plasma endotoxin level, serum D-lactic acid, serum iNOS and intestinal iNOS mRNA expression decreased in IR group(P<0.05). Conclusion Glutamine pretreatment has protective effects on intestinal ischemia-reperfusion injury in vivo. The mechanism may be related to the inhibition of iNOS expression and the increased expression of eNOS, thereby increasing NO activity.
10.Effect of network peer education on the 131I treatment adherence in patients with thyroid cancer

Objective To study the effect of network peer education on the 131I treatment adherence in patients with thyroid cancer.Methods A total of 120 patients with thyroid cancer who performing 131 I treatment from April 2012 to April 2014.They were divided into intervention group 61 cases and control group 59 cases according to the hospital ward number.Control group was given routine health education and intervention group was given network peer education.The data from the habits,drug therapy,grasp the situation,the equivalent monitoring of the relevant knowledge of 4 different dimensions of treatment adherence for patients were investigated and compared before discharge by using questionnaire and examination method.Results The incidence of the good habits,drug therapy,grasp the situation,and the equivalent monitoring reaching the standard were 96.7%(59/61),98.4%(60/61),96.7%(59/61),100.0%(61/61) in intervention group,and 54.2%(32/59),76.3%(45/59),79.7%(47/59),89.8%(53/59) in control group,there were significant differences,x2=29.54,13.38,8.47,4.56,P<0.01 or <0.05.Conclusion Network peer education for patients with thyroid cancer during the 131I treatment can effectively improve the patient's treatment adherence,has positive significance for the treatment.
