1.The magnetic resonance imaging features and clinical manifestations of citrullinemia

Objective To investigate the brain magnetic resonance imaging (MRI) features and clinical manifesta-tions of the patients with citrullinemia, and to promote awareness of, early diagnosis of and better treatment for the disease.Methods One case with citrullinemia was reported, and other eight cases reported in the literature in nearly 14 years were reviewed.Results The case was a 15-month-old girl with type I citrullinemia diagnosed by the mutation analysis of the ASS1 gene performed in local hospital after birth. The patient was admitted to our hospital for recurrent lethargy for a year and the high level of blood ammonia (311 μmol/L, normal range 9-33 μmol/L). The blood ammonia reduced to normal on the 11th day after arginine treatment. On MRI scans, the diffusion weighted imaging (DWI) showed diffuse hyperintensity on bilateral frontal, parietal and temporal cortex, which indicated restricted diffusion due to cytotoxic edema. On the follow-up MRI after 10 day's treatment, the affected regions was similar but the intensity decreased compared to the previous scan,which accompanied by cere-bral atrophy. Eight cases in the literature were reviewed, and the clinical manifestations in these patients were lack of speciifcity, the most common features included feeding dififculties, lethargy, and vomiting. Brain MRI was performed on 7 cases, computed tomography (CT) was performed on 1 case, with the result of cytotoxic edema in 3 cases and atrophy in 2 cases.Conclusions Citrullinemia often lacks of speciifc symptoms in the early phase. Brain MRI could provide the clinician a valuable help for early diagnosis and treatment of this disease.
2.Establishment of a new screening method for anti-HIV-1 drugs

Objective To establish and assess a new screening method for anti-HIV-1 drugs.Methods JLTRG cells were co-cultured with different proportions of H9/HTLV-Ⅲ B cells for 24,48,72 and 96 h.Intensity and density of green fluorescent protein were observed under fluorescence microscope,and were tested using flow cytometry.The optimal proportion of cells in co-culture system and the culture time were determined.The effectiveness of Enfuvirtide (T20) and Efavirenz (EFV),and their half maximal inhibitory concentrations (IC50) were determined by using cell co-culture system and half life of drugs.HIV load was detected using RT-PCR for HIV-1 p24 antigen,and its correlations with drug concentration and mean fluorescent intensity were analyzed by Spearman rank correlation analysis.Results Experiments demonstrated that JLTRG cells co-cultured with H9/HTLV-ⅢB cells at the proportion of 10 ∶ 1 for 72 hours was the best.Along with the concentrations of T20 and EFV changed,JLTRG cells were infected with HIV-1 in different degrees,and the IC50s of T20 and EFV were 10 nmol/L and 5 nmol/L,respectively.The concentrations of T20 and EFV were negatively correlated with mean fluorescent intensity and viral load (r =-1,-0.986 and-1,-1,P < 0.01); and mean fluorescent intensity was positively correlated with viral load (r =0.986 and 1,P < 0.01).Conclusion The drug screening method established in this study is efficient and easy to operate,which provides a new option for anti-HIV-1 drug screening.
3.Distribution and Drug Resisance of Pathogens Isolated from Ventilator-associated Pneumonia Patients in ICU

OBJECTIVE To know the distribution and drug resistance of the ventilator-associated pneumonia(VAP) bacteria in the intensive care unit(ICU),and to provide a reasonable basis for the clinical use of antibiotics. METHODS We used the Tiek Biagnostic systems to identifly microorganisms and antibiotic susceptibility.The result of the drug sensitivity test was analyzed with SPSS13.0. RESULTS A total of 538 strains were isolated from the sputum samples of the ventilator-associated pneumonia patients in the ICU over the last 4 years,in which Gram-negative bacteria were 361(the isolation rate 69.3%).Pseudomonas aeruginosa was the most common.Gram-positive bacteria were 143(the isolation rate 26.6%),in which Staphylococcus aureus was the most common.Twenty-two strain were fungi(the isolation rate 4.1%).Most of the pathogenic bacteria maintained high sensitivity rate to imipenem,vancomycin and quinupristin/daefopristin;but the drug resistance rate to other antibiotics was high and on the rise year by year. CONCLUSIONS The pathogens types of VAP was,complex,and multi-drug resistant.We should emphasize the reasonable application of the antibiotics and strengthen the monitoring of drug resistantce,and rational use of antibiotics to improve the cure rate.
4.Avian influenza A(H7N9) and the closure of live bird markets

On 31 March 2013, the National Health and Family Planning Commission, China notified the World Health Organization of three cases of human infection with avian influenza A(H7N9) from Shanghai and Anhui.1 By 8 May, 131 cases, including 26 deaths, had been notified from 11 provinces/municipalities.1,2 The majority (81%) of reported cases were from Shanghai municipality and Zhejiang and Jiangsu provinces. Available data indicate that more than three quarters of cases (59/77, 76%) had recent exposure to animals. Among these, 58% (34/59) had direct contact with chickens and 64% (38/59) visited a live bird market (LBM).3 Provincial and national authorities in China have collected more than 80 000 samples from LBMs, poultry slaughter houses, poultry farms, wild bird habitats, pig slaughter houses and their environments. As of 7 May, 50 samples were positive for avian influenza A(H7N9): 39 samples from poultry from LBMs in Anhui, Jiangsu, Jiangxi, Guangdong, Shanghai and Zhejiang provinces/municipalities (26 chickens, three ducks, four pigeons, six unknown) and 11 environmental samples from LBMs in Shanghai, Henan and Shandong provinces.4 None of the samples from poultry farms or pigs were positive
5.Comparative Proteomics Analysis of LCM Purified Nasopharyngeal Carcinoma and Normal Nasopharyngeal Epithelial Tissue

To search for nasopharyngeal carcinoma (NPC) biomarkers,laser capture microdissection (LCM) was used to purify the target cells from NPC and normal nasopharyngeal epithelial tissues (NNET). Two-dimensional gel electrophoresis (2-DE) was performed to separate the total proteins of microdissected NPC and NNEC,PDQuest software was applied to analyze 2-DE images,and the differential protein spots between the two types of tissues were identified by both MALDI-TOF-MS and ESI-Q-TOF-MS. The expression of cytokeratin 8(CK8),one of the differential proteins,in the microdissected NPC and NNEC as well as 4 NPC cell lines with different differentiated degrees and/or metastatic potentials was detected by Western blot. Immunohistochemistry was also used to detect the expression of CK8 in paraffin-embedded tissues including 63 cases of primary NPC,28 cases of NNET and 20 cases of cervical lymphonode metastasis. In the present study,2-DE patterns of microdissected NPC and NNEC were established,and 29 differential proteins in the above two tissues were identified,of which 15 only expressed or up-regulated in NPC and 14 only expressed or up-regulated in NNET. The expression level of differential protein CK8 between the NPC and NNET was selectively confirmed,and was found to be related to the differentiation and/or metastasis of NPC cell lines. Significant down-regulation of CK8 was observed in NPC compared with NNET,and significant up-regulation of CK8 was also observed in lymphonode metastasis compared with primary NPC. The data suggest that CK8 may be related to the differentiation and lymphonode metastasis of NPC,and may serve as molecular biomarkers for metastasis and differentiation of NPC.
6.Application effect of comprehensive nursing intervention in elderly pa-tients with intestinal polyps enucleation

Objective To study the application effect of comprehensive nursing intervention in elderly intestinal polyps enucleation. Methods A total of 78 elderly patients with intestinal polyps enucleation were randomly divided into con-trol group(n=39) and observation group (n=39), two groups were given routine nursing,observation group was given com-prehensive nursing intervention based on routine nursing. Results SAS and SDS scores of the observation group after nursing intervention were (37.43±5.63)scores and (48.82±7.46)scores, those were lower than (52.16±8.37)scores and (52.16±8.31)scores of the control group (P<0.05). Intestinal polyps knowledge awareness and nursing satisfaction of ob-servation group were 94.87%and 97.44%, higher than 79.49% and 82.05% of the control group (P<0.05). Conclusion Comprehensive nursing intervention in elderly patients with intestinal polyps enucleation during perioperative can effec-tively relieve patients' anxiety and depression, and improve the recognition of intestinal polyps and nursing quality.
7.Establishment of protein expression profile of laser capture microdissection-purified nasopharyngeal carcinoma tissue.

OBJECTIVE:
To establish the protein expression map of nasopharyngeal carcinoma (NPC), and provide a basis for proteomic study of NPC.
METHODS:
Laser capture microdissection (LCM) was used to isolate cancer cells from NPC tissues. Two-dimensional gel electrophoresis(2-DE) was used to separate the total proteins of LCM purified NPC cells. Matrix-assisted laser desorption/ionization-time of flight-mass spectrometry (MALDI-TOF-MS) was performed to identify the proteins, and bioinformatics was used to construct the 2-DE database of NPC proteome.
RESULTS:
A 2-DE reference map of NPC was established. On the 2-DE map, a total of (1 312+/-30) protein spots were detected, and 427 protein spots representing 241 non-redundant proteins were identified. The 2-DE database of NPC proteome was constructed. These data could be accessed at our website (http://www.xyproteomics.org).
CONCLUSION
A protein expression profile of LCM purified NPC tissues has been established for the first time, which provides useful information and source for proteomic study of NPC.
