This paper sampled 79 TCM hospitals above county level in Jiangsu Province for research.With hospital ownership,board governance and hospital directors as three dimension of internal governance structure,the multiple regression model was used to investigate the impact of such structure on its social responsibility performance.Education level,number of hospital directors,and their values are found to be significant factors on their social responsibility performance.

By means of literature review,the authors initiated the behavior expression system of hospitals' social responsibilities,which is further refined and revised with questionnaires and Delphi method.13 expressions of such behavior expressions were determined,namely “providing quality of care to patients”,“undertaking rescue assignment for emergencies”,and “fulfilling government-assigned tasks”.All of the 13 expressions were supported by 75% of the experts during the second round of experts consultation.
4.Analysis of lymphocyte subsets and activated T lymphocyte in patients with common malignant tumors

Objective To comparatively study the difference status of lymphocyte subsets and activated T lymphocyte between preoperation and intraoperation,and to investigate the influence of operation on lymphocyte subsets and their activation in the pa-tients with malignant tumors.Methods The lymphocyte subsets and their activated cells were determined in 42 cases of common malignant tumors by the flow cytometry.Results The levels of CD3 + ,CD4 + ,CD8 + cells during intraoperation were obviously de-creased compared with preoperation,the difference in CD3 + and CD4 + had statistical significance(P <0.05),while the difference of CD8 + had no statistical significance(P >0.05).NK,CD19 + ,CD3 + HLA-DR+ were significantly increased,the difference of NK and CD19+ had no statistical significance(P >0.05),while the difference with CD3 + HLA-DR+ had statistical significance(P <0.05). Conclusion The cellular immune function in the patients with malignant tumor is in inhibitory status.Operation makes the cellular immune function to be further injured.The stress responses of operation and trauma increase the expression of NK,CD19 and acti-vated T lymphocytes.It is suggested that the patients with malignant tumor should use the immune response modifier before opera-tion to enhance the cellular immune function for ensuring the effect of postoperative radiochemotherapy sequential treatment.
5.Prognostic value of clinical and pathological characteristics in 48 women with pseudomyxoma peritonei

Objective To investigate the clinic-pathological characteristics and prognosis of 48 female cases with peritoneal pseudomyxoma(PMP).Methods The clinicopathologic features and follow-up data of 48 female patients with PMP diagnosed in the General Hospital of People's Liberation Army from Jan.1982 to Dec.2011 were retrospectively reviewed.The relationship between clinic-pathological characteristics and prognosis were analyzed using log-rank test and Cox proportional hazards model.Results (1) Clinicopathologic features:the mean age of the 48 cases was 58.8 years (range from 24 to 79 years).Symptoms:abdominal distention and abdominal discomfort were the main symptoms.Imaging examinations showed nonspecific abdominal and pelvic lesions in most cases.Treatment:all the 48 patients underwentlaparotomy and cytoreductive surgery (CRS),in which 15 (31%) patients with completeness of the cancer resection (CCR)-1,24(50%) cases with CCR-2,and CCR-3 in 9(19%) cases.Six (12%) cases were treated by intraoperative hyperthermic intraperitoneal chemotherapy (HIPEC) with cisplatin,20 (42%) patients were treated with different options postoperative adjuvant chemotherapy.Pathological types:the cases were histologically classified into 3 subcategories:disseminated peritoneal adenomucinosis (DPAM),peritoneal mucinous carcinomatosis (PMCA),and PMCA with intermediate or discordant features (PMCA-I/D),which were 22 (46%) cases,9 (19%) cases and 17 (35%) cases,respectively.Appendiceal tumors:44(92%) cases underwent appendectomy,in which 38 cases presented appendiceal tumors (including 20 cases of low-grade appendiceal mucinous adenoma and 18 cases of appendiceal mucinous adenocarcinoma),2 cases were diagnosed as appendicitis,4 cases with unknown pathologic diagnosis.And the other 4(8%) cases,who didn't undergo appendectomy at the first operation,presented peritoneal tumor recurrence and appendiceal mucinous tumors 1,11,32 and 85 months after surgery,respectively.Parenchymal organs involved:ovarian involving was happened in 34 (71%)patients including 15 cases with the right ovary involving,13 cases in both sides,and 6 cases involving the left side.The other parenchymal organs in 10(21%) cases.(2) Prognostic factors:11 patients died,31 survived and 6 cases were lost to follow-up.The mean survival time was 99 months(ranged from 1 to 312 months).The 3-year,5-year and 10-year survival rates were 73.3%,68.0% and 46.6%,respectively.Univariate statistical analysis showed that age,pathological type and parenchymal involvement were significantly relationship with the survival time (all P < 0.05).But the operation times,appendiceal tumor type,ovarian involvement,CCR,intraperitoneal HIPEC and post-operative adjuvant chemotherapy were not significantly correlate with survival time (all P > 0.05).Multivariate analysis showed that age and pathologic type were independent prognostic factors (P < 0.05).Conclusions No specific clinical features presented in PMP.CRS with HIPEC should the recommended treatment.Both ovaries exploration and appendectomy should be carried out routinely in CRS.The 10-year overall survival of PMP is low.Age,pathological type and parenchymal organs involvement other than ovarian are correlated with the prognosis.And the pathological type and age are independent prognostic factors of PMP.
6.Imaging diagnosis of prostatic hyperplasia and prostate cancer

Objective To explore the methods of improving diagnosis correctness between the patients with prostate cancer and benign prostatic hyperplasia.Methods Totally 87 patients with benign prostatic hyperplasia or prostate cancer which confirmed by MRI and prostate biopsy for the PSA significantly increased in our hospital from July 2013 to March 2016 were collected.By using the three methods of the PI-RADS V2 score,the T2WI+DWI/ADC+DCE-MRI+MRS and PI-RADS V2 score+MRS to diagnose,and comparing with the pathology results,the diagnostic consistency of the two physicians were analyzed.The sensitivity,accuracy and specificity of three ways were compared,and the correlation between PI-RADS V2 score and Gleason score were calculated.Results The diagnostic consistency of the two physicians:PI-RADS V2 score,K=0.951;T2WI+DWI/ADC+DCE-MRI+MRS score,K=0.838;PI-RADS V2+MRSI score,K=0.937.The correlation between PI-RADS V2 score and Gleason score,r=0.871,P=0.001 4;diagnostic sensitivity,specificity and accuracy of PI-RADS V2 score were 77.3%,74.4%,75.9%;diagnostic sensitivity,specificity and accuracy of T2WI+DWI/ADC+DCE-MRI+MRS were 88.6%,76.7%,82.8%;diagnostic sensitivity,specificity and accuracy of PI-RADS V2+MRSI score were 86.4%,81.4%,83.9%,respectively.Conclusion Compared with the traditional diagnostic methods,the combination of new prostate report and data system and MRSI can improve the diagnostic accuracy of prostate cancer and benign prostatic hyperplasia.The PI-RADS V2 score is more objective and accurate in the description of the lesion,but the low signal of benign hyperplastic nodules in transitional zone should be dialogued carefully through a variety of image parameters.

8.Modalities in managing postherpetic neuralgia

Postherpetic neuralgia (PHN) is the most troublesome side effect of Herpes Zoster (HZ), which mainly affects the elderly and immunocompromised populations. Despite the current advancement of treatments, PHN persists in many individuals influencing their daily activities and reducing their quality of life. Anticonvulsants, antidepressants, topical therapies including lidocaine and capsaicin, and opioids, are the most widely used therapies for the treatment of PHN. These medications come with their adverse effects, so they should be used carefully with the elderly or with patients with significant comorbidities. Other measures like botulinum toxin, nerve blocks, spinal cord stimulation, and radiofrequency have also contributed significantly to the management of PHN. However, the efficacy, safety, and tolerability of these invasive methods need to be carefully monitored when administering them. Early diagnosis and early initiation of treatment can reduce the burden associated with PHN. The zoster vaccine has effectively reduced the incidence of HZ and PHN. In this article, we discuss the treatment options available for the management of PHN, mainly focusing on the efficacy and safety of different therapeutic modalities.

9.Biocompatibility of domestic porous tantalum carrying bone morphogenetic protein 7 in the erector spinae muscle of rabbits

BACKGROUND:Bone morphogenetic protein 7 (BMP-7) can induce bone and cartilage formationin vivo, and induce chondrogenic and osteogenic differentiation of mesenchymal cels in muscles and around the vessels.
OBJECTIVE:To observe the structure of domestic tantalum-muscle interface fibrous capsule, growth of muscle and smal blood vessels into the porous tantalum and the ability of ectopic osteogenesis after implantation of porous tantalum loaded with BMP-7 into the erector spinae of rabbits.
METHODS: Porous tantalum slices loaded with BMP-7 (experimental group) and porous tantalum slices (control group) were implanted into the erector spinae muscle of New Zealand white rabbits. And the porous tantalum slices with surrounding muscle tissues about 0.5 cm thick were removed at 2, 4, 8 weeks after implantation, and observed under scanning electron microscope for hematoxylin eosin staining, Masson staining and hard tissue slice observation.
RESULTS AND CONCLUSION:(1) Hematoxylin-eosin staining: Fibrous capsule formation was observed around the materials in the two groups, and with the extension of time, the fibrous capsules were slightly dense, and thinned. There was no obvious inflammatory reaction in the interface between the material and the muscle. There was no significant difference between the two groups in the fibrous capsules thickness. (2) Scanning electron microscope: 2 weeks after the surgery, a smal amount of colagen and muscle fibers were formed in the porous tantalum pores in the two groups, and some of colagen fibers attached to the pore wals. At 8 weeks after the surgery, al the pores of porous tantalum were ful of muscle fibers that were combined with the pore wal closely. There was no significant difference between the two groups. (3) Hard tissue slices: 2 weeks after the surgery, a smal amount of fibroblast cels and muscle fibers grew into the pores of porous tantalum in the two groups and new capilaries grew into the pores of porous tantalum in the experimental group. At 8 weeks after the surgery, the porous tantalum and al the pores were ful of muscle fibers that were combined with the pore wal closely, the number of smal blood vessels and cels decreased, and the tantalum and the muscle were fused closely. (4) Masson staining: 8 weeks after the surgery, a large number of mesenchymal cels, ossein and cartilage matrix formed in the muscle gaps and a few cartilage bone tissues were formed in the experimental group, but no cartilage was found in the control group. 10.Relationship among cardioversion,cerebral infarction and NT-proBNP level in patients with heart fail-ure complicated atrial fibrillation

Objective:To explore the relationship among cardioversion ,cerebral infarction (CI) and N terminal pro brain natriuretic peptide (NT‐proBNP) level in patients with heart failure (HF) complicated atrial fibrillation (AF) . Methods :A total of 150 HF + AF patients received intravenous drip of amiodarone for cardioversion therapy .Ac‐cording to cardioversion results ,they were divided into cardioversion group (n=100 ) and non‐cardioversion group (n=50) ,NT‐proBNP level change was observed in two groups before and after cardioversion .According to CI on‐set or not ,patients were divided into CI group (n=20) and non‐CI group (n= 130) ,NT‐proBNP level was com‐pared between two groups before and after onset .Results :Within 48h after administration ,a total of 100 patients (66.67% ) recovered to sinus rhythm .Compared with before cardioversion ,NT‐proBNP level significantly reduced [(967.04 ± 366.16) pg/ml vs .(496.21 ± 142.54) pg/ml] after cardioversion in cardioversion group ,and was signifi‐cantly lower than that of non‐cardioversion group (996.76 ± 351.28) pg/ml , P<0.01 all . In CI group ,compared with small size CI group ,there were significant rise in NT‐proBNP level [ (784.21 ± 231.26) pg/ml vs .(1983.24 ± 32.96) pg/ml ,(3562.19 ± 1468.32) pg/ml] in medium and large size CI group , P< 0.05 or <0.01 .Conclusion:NT‐proBNP level at hospitalization possesses predictive value for drug cardioversion effect in HF + AF patients . NT‐proBNP level is related with CI onset .After acute CI ,the higher NT‐proBNP level is ,the larger infarct size is , the poorer prognosis is .
