1.Natural History of Intracranial Aneurysms

Intracranial aneurysms are a severe cerebrovascular disease.Its incidences ranged from 2% to 4% in general population.With the population aging and the development of imaging technique,the prevalence of intracranial aneurysm is increasing year by year.The research of natural course of aneurysm has important significance for neurosurgeons to evaluate the validity of therapeutic measures.There are significant differences between the patients with aneurysm who have had subarachnoid hemorrhage and those who have not.The rates of ruptured intracranial aneurysms per year ranged from 0.066% to 2%.The risk factors that affect ruptured intracranial aneurysms including the size and place of aneurysm,whether it is multiple aneurysms or not,growth rate,and self-factors of patients,such as age,sex,and history of hypertension and smoking,etc.
2.Henoch-Sch(o)nlein purpura nephritis in children: an update on the diagnosis and treatment

Henoch-Sch(o)nlein purpura (HSP) is the common form of systemic vasculitis in children,which characterized by the deposition of immunoglobulin E within the small vessels.The long-term prognosis of HSP depends on the severity of renal involvement.Renal involvement in HSP affects 10%-100% patients and the clinical manifestations vary from microscopic hematuria to nephritic/nephritic syndrome even renal dysfunction.Early diagnosis,early treatment and long-term follow-up affect the prognosis of HSP patients.As for the treatment of HSP nephritis (HSPN),there have been many reports dealing with the immunosuppressive agents.Now glucocorticoid is the first-line treatment for HSPN,and other immunosuppressive agents offer more choices to treat HSPN especially for severe HSPN.But,the use of immunosuppressive agents still needs large sample multicenter randomized controlled trials and evidence-based study.
3.Clinical efficacy of improved vaginal hysterectomy and its influences on blood glucose and serum corticosteroid

Objective To compare clinical efficacy between improved vaginal hysterectomy (IVH)and traditional vaginal hysterectomy (TVH) and its influences on blood glucose and serum corticosteroid levels.Methods One hundred patients undergoing hysterectomy were divided into using IVH (treatment group,50 patients) and TVH (control group,50 patients).Clinical efficacy,blood glucose and serum corticosteroid levels were compared between two groups.Results There was no significant difference in operation time between two groups (P> 0.05).The amount of intraoperative bleeding,postoperative exhaust time and postoperative vaginal bleeding time in treatment group were significantly less than those in control group [(86.7 ± 5.7) ml vs.(131.2 ± 14.5) ml,(24.8 ± 12.9) h vs.(42.8 ± 17.7) h,(16.5 ± 2.9) d vs.(24.3 ±6.7) d,P < 0.05],and the time of blood glucose and serum corticosteroid levels returned to normal levels in treatment group were much earlier than those in control group.Conclusion The clinical efficacy of IVH is preonunced,and postoperative stress reaction is mild.
4.Electrochemical Properties of Sodium Selenite on a Bionic Membrane of Gold Electrode Modified by L-Cysteine Self-assembled Monolayer

The cyclic voltammetric behaviors of sodium selenite on the gold electrode modified by L-cysteine self-assembled monolayer (L-Syc,Au/SAMs) has been investigated in 0.05 mol/L H_2SO_4 solutions with sodium selenite. Sensitive anodic stripping current of selenium (Se) with the peak potential at 0.99 V (vs. SCE) has been observed. In comparison with the electrochemical properties of sodium selenite on the modified electrode with those on the naked gold electrode, it was found that selenite was reduced to selenium by the redox interaction with sulfur on the monolayer and the monolayer catalytically oxidizes Se as the electrode was positively scanned. According to the electrochemical properties of sodium selenite both on the naked and modified gold electrode, the Se deposition mechanism and catalytic mechanism as well as bionic membrane model were proposed for the bio-utilization of selenite through nano scale selenium.
5.The influence of exercise intensity on cardiorespiratory function in the sedentary elderly with coronary heart disease

Objective To observe the influence of exercise intensity on cardiorespiratory function in the elderly with coronary heart disease using simplified 24-action Taijiquan and jogging. Methods A total of 115 sedentary elderly patients with heart disease were randomly divided into a control group ( no exercise), a low-intensity exercise group, a medium-intensity exercise group (whose exercise was simplified 24-action Taijiquan at 40-60% of their individual maximum oxygen consumption (VO2max) for 90 to 200 minutes per week) and a high-intensity exercise group ( whose exercise was jogging on a treadmill at > 70% of their maximum oxygen consumption for 200 minutes per week). The regime was maintained for 6 months. All patients accepted a treadmill exercise test before and after the 6 months of exercise to evaluate the changes in VO2max, maximum heart rate ( HRmax), VO2max/HRmax and exercise time (t). Results ( 1 ) After 6 months of exercise, VO2max, HRmax, VO2/HRmax and t had increased sisnificantly in all three exercise groups. Work capacity had increased 18.6% , 33% and 34.4% and HRmax had increased 9.3% , 13.8%, 19.1%. There was no difference between before and after in the control group. (2) Compared with the control group the average VO2max, HRmax and t of all three exercise groups had increased significantly. VO2 max was 9.4% , 26% and 23. 1% better; HRmax was 6.7% , 8.8% and 12% better; t was 3.5,4 and 4.9 minutes longer. (3) Compared with the low-intensity exercise group, VO2max, HRmax and t in the medium- and high-intensity groups increased significantly more. VO2max increased 16.6% and 13.7% more; HRmax increased 2.1% and 3.2% more and average t was 0.5 and 1.4 minutes longer. (4) There was no significant difference between the medium- and high-intensity exercise groups in terms of VO2max. (5) The incidence of self-reported adverse responses to exercise such as dizziness, chest stuffiness and fatigue in the high-intensity exercise group increased significantly compared with the other 3 groups. Conclusions Exercise can gradually improve cardiorespiratory function in the sedentary elderly with heart disease. The effects of medium-intensity exercise are similar to those of high-intensity exercise, but medium intensity is safer.
6.Effect of Chinese herbal immersion combined with Beecher's exercise in prevention and treatment of grade-0 diabetic foot

Objective To explore clinical effect of Chinese herbal immersion combined with Beecher's exercise for prevention and treatment of grade-0 diabetic foot.Methods A total of 107 patients who were diagnosed with grade-0 diabetic foot were selected in Department of Endocrinology of our hospital from July 2009 to October 2013.In addition to regular treatments,their feet were immersed in Chinese herbal liquid,and then were taken care by doing Beecher's exercise.The symptoms of diabetic foot were self-rated by each individual patient and mastering of knowledge about diabetic foot was evaluated both before the intervention and after three courses of treatment.Results After three courses of treatment,the self-rating symptom scored (4.37±1.45) totally,significandy lower than that before the intervention,(5.62±1.55),t=7.91,P < 0.05.The mastering of six testing indexes about diabetic foot obviously improved compared between those before and after three courses of treatment [62.62% (67/107)],23.36% (25/107),30.84% (33/107),46.73% (50/107),57.94% (62/107),50.47% (54/107) vs.96.26% (103/107)],86.92% (93/107),89.72% (96/107),85.98% (92/107),94.39% (101/107),87.85% (94/107),x2=37.78,87.35,77.46,36.92,39.16,35.05,all P < 0.05.Conclusions For grade-0 diabetic foot,Chinese herbal immersion treatment combined with Beecher's exercise can be applied straight forwardly on the disease focus;it's clinically safe and effective,and easy to be operated.It improves clinical symptoms of grade-0 diabetic foot by taking advantages of traditional Chinese medicine,prevent and control further development of diabetic foot and is valuable to be applied in clinical work.

Objective To study the relationship between the levels of serum IL-6, TNF-? and L-carnitine and atherosclerosis in hemodialysis patients. 8.An Investigation of Psychological Health Status of Middle-aged Intellectuals and Its Influencing Factors

[Objective] To probe into the psychological health status of middle-aged intellectuals and its influencing factors. [Methods]A cluster sampling of 1 474 middle-aged intellectuals was measured by the SCL-90 table and was analyzed by the Reliability Analysis, Principal Component Analysis and Analysis of Variance. [ Results ] The measurement table is highly reliable (Cronbach a = 0.949) and the Principle Component Analysis has main factors (A -1.97) and cumulative contribution rate is 80% . [Conclusion]Marital status, income and gender are the main influencing factors of psychological healthy status in middle-aged intellectuals.
9.The effect of intravenous thrombolysis on TIMI flow of patients with acute myocardial infarction

Objective To observe the effect of intravenous thrombolysis on TIMI flow of acute myocardial infarction(AMI).Methods A total of 229 patients with AMI analysed retrospectively were divided into two groups:intravenous thrombolysis(IVT)group(n=131)and primary percutaneous coronary intervention(PCI)group(n=98).The treatment time and acute-phase outcomes were compared between the IVT group and the PCI group;104 patients in the IVT group received rescued or delayed PCI(IVT+PCI);TIMI flow was analysed between the IVT+PCI group and the PCI group.Results The time from the emergency room door to initiation of treatment was shorter in the IVT group than in the PCI group(67.79 min vs 134.54 min,P=0.000).At the initial coronary angiography,TIMI 3 flow and TIMI 2+3 flow were higher in the IVT+PCI group than in the PCI group(40.4% vs 24.5%,P=0.016;63.5% vs 36.7%,P=0.000 respectively).There was no significant difference between the IVT group and the PCI group in terms of major adverse cardiac events(MACE).Conclusion IVT as an initial treatment for AMI might achieve earlier reperfusion at TIMI≥2 flow,and it should be popularized in the primary hospitals.
10.The Effects of Hemodialysis on the Oxidizing Stress and NO Level in Hemodialysis Patients

Objectives To explore the effects of hemodialysis on oxidizing stress and plasma nitrogen monoxide (NO) levels in maintenance hemodialysis patients. Methods 67 maintenance hemodialysis patients (male 39, female 28, and mean age 58?13y) were enrolled in this study. Serum malondialdehyde (MDA) level was measured by colorimetry, and plasma NO level was determined by enzymatic method. The MDA and NO levels before and after hemodialysis were measured in 48 cases of the patients. Results After hemodialysis, the serum MDA levels significantly increased, and the plasma NO levels decreased. The patients with cardiovascular diseases had lower NO levels and higher MDA levels than the patients without ones before hemodialysis. The serum MDA levels of the patients using biological incompatible dialysis membrane increased after hemodialysis. Conclusion Hemodialysis treatment, especially using biological incompatible dialysis membrane increased oxidizing stress and decreased plasma NO levels in maintenance hemodialysis patients, and was significantly correlated with the pathogenesis of cardiovascular diseases.
