1.Value of the combining detection of p53, p27 and bcl-2 in early diagnosis and the implementation of the intervention for non-small cell lung cancer

BACKGROUND: The quality of life(QOL) of patients with lung cancer is ffected due to depression, reduced lung function, subjective reduced body force, fatigue, and poor stamina, etc., and the survival of the patientswould be affected by complications or advanced stage cachexia.OBJECTIVE: To investigate the relationship between three gene proteins including p53, p27 and bcl-2 and the pathological characters of non-small cell lung cancer (NSCLC).DESIGN: An experimental trial by employing pathological specimens as subjects.SETTING: Department of respiration of a university affiliated hospital and the Central laboratory of a university.PARTICIPANTS: Totally 76 specimens of NSCLC after surgical resection between June 1997 and December 2002, which were all primary lung cancer without any other therapy.METHODS: The expression of three gene proteins in 76 NSCLC specimens was detected by SP immunohistochemical analysis.MAIN OUTCOME MEASURES: Positive expression of p53, p27 and bcl-2.RESULTS: Among 76 specimens, 28 cases(37%, 28/76) with excessive expression of p53, 34 cases(45%, 34/76) with excessive expression of p27, 37 cases(49%, 37/76) with excessive expression of bcl-2, and 7 cases with excessive expression in all three proteins. The positive expression of p53 elevated with the reduced gradation in differentiation; bcl-2 and p27positive expressions reduced with the reduced gradation in differentiation and there was significant difference between high-differentiation group and low-differentiation group( P < 0. 05) . However, there was no significant relationship between the positive expressions of three proteins and the histological classification, lymph node metastasis, and pathological aging of lung cancer( P > 0.05).CONCLUSION: The excessive expression of p53, p27 and bcl-2 genes might be related with the occurrence and development of NSCLC.
2.Aorto-right atrial fistula after ascending aortic replacement or aortic valve replacement

Objective To evaluate the CT features of aorto-right atrial fistula after aortic valve replacement(AVR) or ascending aortic replacement.Methods Eighty-seven patients with aortic-right atrial fistula underwent CT after operation.The CT features were retrospectively analyzed.Fistula was measured according to maximum width of the shunt.Results Aorto-right atrial fistula was detected in 87 patients after aortic valve replacement or ascending aortic replacement by CT scan. Among them,25 patients were diagnosed as mild aorto-right atrial fistula,47 patients as moderate,and 15 patients as severe.Thirty-seven patients underwent follow-up CT.Among them,10 patients with mild to moderate aorto-right atrial fistula were considered to have complete regression,8 patients with mild aorto-right atrial fistula considered to have incomplete regression,14 patients with mild to moderate aorto-right atrial fistula considered to have stable condition,and 5 patients with moderate aorto-right atrial fistula considered to have progression at the 3-month follow-up.Conclusion CT is a useful tool for defining aorto-right atrial fistula after AVR or ascending aortic replacement and for evaluating it in follow-up.
3.Computed tomography diagnosis of cardiovascular involvement in Behcet syndrome

Objective To evaluate the computed tomography (CT) characteristics of cardiovascular involvement in Beheet syndrome. Methods Eleven patients with clinically diagnosed Behcet syndrome were studied retrospectively from July 1995 to December 2007. Electron beam CT or 64-slice helical CT scanner was used and CT characteristics were reviewed. Results Eleven patients were diagnosed according to the criteria reported by the international study group for Behcet syndrome. Of them, 4 patients presented with aortic valve prolapse (2 patients with mitral valve prolapse), false aneurysm of right coronary artery was demonstrated in 2 patients, false aneurysm of left subclavian artery, aortic aneurysm and penetrating ulcers, aortic arch false aneurysm, aortic dissection, pulmonary embolism and interatrial scptum aneurysm in 1 case, respectively. Conclusion CT is a very useful method for the diagnosis and foUow-up of Behcet syndrome.
4.The Imaging and Clinical Feature Analysis for Behcet's Disease With Coronary Artery Involvement in 6 Patients

Objective: To explore the imaging and clinical features of Behcet's disease with coronary artery involvement in order to improve the diagnosis and treatment in clinical practice. Methods: A total of 6 Behcet's disease patients with coronary involvement diagnosed and treated in our hospital from 1999-08 to 2016-11 were analyzed. Coronary angiography (CAG) and coronary CTA were performed for diagnosis. There were 5 cases received CAG and 5 cases received CTA examinations. Results: All 6 patients had the first clinical visit by sudden pericardial tapenade, myocardial infarction or cardiac shock. For diagnosis: the imaging presentation included coronary stenosis, occlusion and pseudo aneurysm formation. 3 patients had anterior descending artery involvement, 1 had the far-end of left circumflex involvement and 2 had the middle segment of right coronary involvement; all patients were combining large amount of pericardial effusion. For treatment: there were 3 patients with aneurysm received conservative treatment (1 case had open thoracic exploration), 1/3 lost contact and 2 having long-term normal life; 3 patients received coronary stent implantation without optimal effect. Conclusion: The patients of Behcet's disease with coronary involvement may easily form pseudo aneurysm; CAG/CTA has been helpful for diagnosis. Clear pre-operative diagnosis of etiology is important for treatment.
5.Application effect of standardized nursing program in pneumonia of immunocompromised host infected with non-human acquired immunodeficiency virus

Objective:To explore the application value of standardized nursing program in pneumonia of immunocompromised host infected with non-human acquired immunodeficiency virus (HIV) .Methods:Using the convenient sampling method, a total of 158 non-HIV immunocompromised patients who were hospitalized in China-Japan Friendship Hospital from August 2016 and August 2018 were selected and they were randomly divided into two groups. 75 patients in the observation group received standardized nursing, and 83 patients in the control group received conventional nursing. Clinical indexes, activity of daily living and satisfaction were compared between the two groups.Results:The antipyretic time, cough recovery time, phlegm recovery time and length of hospital stay in the observation group were shorter than those in the control group, and the differences were statistically significant ( P<0.01) . The activity of daily living and satisfaction degree of the observation group were better than those of the control group, and the differences were statistically significant ( P<0.01) . Conclusions:In the pneumonia of non-HIV immunocompromised host, the strict implementation of standardized nursing can promote the early recovery of patients, shorten the length of hospital stay, improve activity of daily living and improve their satisfaction.
6.A study on expression of p53, p27 and bcl-2 protein in non-small cell lung cancer.

BACKGROUNDTo study the relationship between the expression of p53, p27, bcl-2 protein and the clinical pathological characteristics of non-small cell lung cancer (NSCLC).
METHODSExpression of p53, p27 and bcl-2 protein was detected in 76 NSCLC samples by immunohistochemistry.
RESULTSThe positive rate of p53, p27 and bcl-2 protein was 36.84% (28/76), 44.74% (34/76) and 48.68% (37/76) respectively, and 7 cases were positive for p53, p27 and bcl-2 protein. Positive rate of p53, p27 and bcl-2 protein was not related to the pathological type, lymph node metastasis, and TNM stage. The positive rate of p53 in higher differentiation group was significantly lower than that in lower differentiation group (P < 0.05). The positive rate of p27 and bcl-2 in higher differentiation group was significantly higher than that in lower differentiation group (P < 0.05).
CONCLUSIONSOverexpression of p53, p27 and bcl-2 genes may play an important role in the oncogenesis and development of NSCLC.
7.The status quo of knowledge,attitude and practice of influenza in inpatients in the Department of Respiratory and Critical Care Medicine and its influencing factors

Objective? To explore the status quo of knowledge, attitude and practice (KAP) of influenza in inpatients in the Department of Respiratory and Critical Care Medicine and analyze its influencing factors. Methods? Totally 470 inpatients treated in the Department of Respiratory and Critical Care Medicine in a Class ⅢGrade A hospital in Beijing from November 2018 to March 2019 were selected using convenient sampling and investigated with the self-designed questionnaire. Results? A total of 470 questionnaires were distributed and 451 valid questionnaires were returned.Among the 451 patients, the score of knowledge about influenza was (25.49±5.51); the score of attitude was (16.53±2.74); and the score of practice was (28.78±3.51). Univariate analysis revealed that there was statistically significant difference in knowledge about influenza between patients with different gender, age, marital status, educational background, profession, time between sustaining influenza and the present (P<0.05); there was statistically significant difference in attitude toward influenza between patients with different gender, age, marital status and profession (P<0.05). And there was statistically significant difference in practice about influenza between patients with different gender, age and educational background (P<0.05). According to Logistic regression analysis taking patients' KAP questionaire score about influenza as dependent variables and statistically significant variables found in univariate analysis as independent variables, gender, age, marital status, profession, time between sustaining influenza and the present were the independent influencing factors to the KAP about influenza. Conclusions? The KAP levels about influenza in inpatients are uneven. Targeted health education about influenza should be strengthened in inpatients.
8.Construction andeffect evaluation of precise oral nutrition guidance program for postoperative patients with colorectal cancer undergoing accelerated rehabilitation

Objective:To construct and evaluate the effect of precise oral nutrition guidance program for colorectal cancer patients with accelerated rehabilitation after operation.Methods:This study was a quasi experimental study. A total of 116 patients with colorectal cancer from department of Colorectal Surgery, Jiangsu Cancer Hospital from March to Deamber, 2021 were selected by convenience sampling method. They were divided into two groups according to their admission time, the experimental group (58 cases), was given precise oral nutrition guidance program after colorectal cancer surgery based. In the control group, 58 cases were treated with the conventional dietary guidance for rapid rehabilitation. The gastrointestinal tolerance rate, the compliance rate of oral nutritional supplements, the first defecation time after operation, albumin, total albumin, prealbumin and other nutritional indexes were compared between the two groups before and after intervention.Results:Finally, 111 cases were included, induding 56 in the experimental group and 55 in the control group. The compliance rate of oral nutritional supplements after intervention in the experimental group was 73.2% (41/56), higher than that in the control group 40.0%(22/55), and the difference between the two groups was statistically significant ( χ2 = 12.47, P<0.05). The first defecation time after operation in the experimental group was (29.51 ± 5.52) h, while in the control group was (61.48 ± 8.39) h, the difference between the two groups was significant ( t = -12.06, P<0.05). The postoperative enteral nutrition tolerance rate of the experimental group was 91.1% (51/56), which was higher than the 69.1%(38/55) in the control group, the difference between the two groups was statistically significant ( χ2 = 8.44, P<0.05). The serum total protein, albumin and prealbumin levels of the experimental group at discharge were (66.56 ± 5.15), (46.78 ± 7.62) g/L and (276.07 ± 53.57) mg/L, compared with the (60.27 ± 4.37), (39.12 ± 6.41) g/L and (230.14 ± 44.58) mg/L in the control group, the differences were statistically significant ( t = 6.66, 5.02, 4.91, all P<0.05). Conclusions:The implementation of precise oral nutrition guidance program is conducive to guiding the clinical practice of oral nutritional supplements in patients with colorectal cancer after surgery, improving patient compliance, reducing gastrointestinal intolerance, promoting intestinal function recovery, and improving nutritional status of patients.
