1.Analeptic Action of Xingnaojing Injection in Patients Undergoing Total Intravenous Anesthesia

OBJECTIVE: To investigate the analeptic action of Xingnaojing injection(XNJI) in patients undergoing total intravenous anesthesia and the possible mechanism.METHODS: 60 patients undergoing selective abdominal operation with tracheal intubation(scored grade Ⅰ or grade Ⅱ using ASA score) were transferred to post-anesthctic ICU(PACU) after surgery and randomly divided into two groups of 30 each: X group(XNJI 0.5 mL?kg-1) and C group(control group).The X group received XNJI(0.5 mL?kg-1) by drip infusion within 10 minutes,while C group received same amount of normal saline.The vocal reaction recovery time,autonomous breath recovery time,extubation time and duration of PACU stay were recorded.The mean arterial pressure(MAP),heart rate(HR),the concentrations of Epinephrine(E),and Norepinephrine(NE) and ?-endorphin(?-EP) in arterial blood were detected at 0,5,15,30 and 45 minutes before and after medication.RESULTS: No significant differences were noted for X group in hemodynamics parameters after medication as compared with before medication,but significant differences were noted for C group at 30 min and 45 min when compared with before medication or compared with X group at the same different time points.The vocal reaction recovery time,autonomous breath recovery time,extubation time and the duration of PACU stay were all significantly shorter in X group than in C group(P
2.Mechanism of drug resistance of NDM-1 producing Enterobacteriaceae and the clinical feature of its infection

Objeetive To explore the mechanism of drug resistance of New Delhi metallo-β-lactamase-1 (NDM-1) producing Enterobacteriaceae,and to investigate the characteristics of blaNDM-1 carrying plasmid and its gene environment.Methods A total of 48 strains of carbapenem-resistant Enterobacteriaceae were successively collected from six general hospitals in south China during August 2011 and January 2013.Escherichia coli J53 was used for plasmid conjugation.Modified Hodge test was performed,and PCR method was used for the detection of carbarpenase-related genes.The relative molecular mass of the blaNDM-1 carrying plasmid was determined using pulsed field gel electrophoresis (PFGE)assay,and enzyme digestion was performed to investigate the homology and incompatibility group of the plasmid.Clinical feature of blaNDM-1 producing Enterobacteriaceae infection was also investigated.Results Among 48 strains of carbapenem-resistant Enterobacteriaceae,43 were positive in modified Hodge tests.blaVIM,blaGIM and blaSPM genes were negative in all strains,while blaNDM-1 was positive in 19 strains including 3 strains of Escherichia coli,5 strains of Klebsiella pneumoniae,6 strains of Enterobacter cloacae,3 strains of Citrobacterfreundii,1 strain of Klebsiella oxytoca and 1 strain of Providencia rettgeri.All the 19 strains were resistant to imipenem,cefotaxime,ceftazidime,cefepime,aztreonam and piperacillin/tazobactam,47.3% strains were resistant to ciprofloxacin and levofloxacin,but 68.4% strains were sensitive to amikacin.Conjugation experiment showed that,blaNDM-1 carrying plasmids in 13 strains were transmitted to the Escherichia coli J53.The conjugants were resistant to imipenem,ceftazidime,cefotaxime and piperacillin/tazobactam,but were sensitive to amikacin,ciprofloxacin and levofloxacin.All genes in conjugant J-FR90 (Providencia rettgeri) were negative,while the remaining 12 conjugants carried blaNDM-1,blaSHV and aac-(6')-Ib genes.PFGE showed that,the sizes of all blaNDM-1 carrying plasmids were about 50 kb,and more than 80% of their macrorestriction maps were similar.The plasmid belonged to incompatibility group IncX3,and exhibited 100% passage stability after 500 generations of propagation.Among 19 patients infected with NDM-1 producing Enterobacteriaceae,6 died and 13 survived.Conclusions NDM-1 producing Enterobacteriaceae is emerging in south China,and blaNDM-1 is transmitted to Enterobacteriaceae through IncX3.Patients infected with NDM-1 producing Enterobacteriaceae usually have good prognosis.

The disease incidence of aortic dissecting aneurysm showed an increasing tendency.Recently,the clinical application of endovascular stent-graft implantation received good results in treating aortic dissecting aneurysm,which is characterized by minimally invasive,safe,with high success rates.However,the chosen of anesthesia is still in dispute.Local anesthesia,intraspinal anesthesia and general anesthesia can be used in this operation.The investigation demonstrated that intubation general anesthesia is more security in this operation with less complication,which is conductive to controlling blood pressure and braking conditions.
4.Effect of alprostadil deliberate hypotension on heart rate variability in elderly patients

bjective To clarify the heart rate variability (HRV) changes in patients subjected to deliberate hypotension with alprostadil.Methods Eighty-six elderly patients scheduled for nasal endoscopic surgery in general anesthesia were divided into alprostadil group (group A) and nitroglycerin group (group N) with 43 cases each by random digits table.Hypotension was induced with alprostadil and nitroglycerin through continuous infusion respectively,and the mean arterial pressure (MAP) was reduced to about 25%-30% of baseline MAP.The MAP,heart rate (HR),total power ( TP ),low frequency (LF),high frequency (HF),and LF/HF were continuously monitored and blood was taken for analysis of lactate (LAC) before deliberate hypotension (T0),at the time of dropping to target blood pressure (T1),at 5 and 30 min after deliberate hypotension (T2 and T3),stopping deliberate hypotersion (T4) and 30 min after recovery from hypotension (T5).Results HR at T1-T4 was faster than that at T0in group N(P < 0.05),and faster than that in group A( P < 0.05 ),HR at T5 was faster than that at T0 in group N( P < 0.05 ),but HR was stable at T1-T5 in group A (P > 0.05 ).There was no significant difference in LAC of group A and group N whether in interior group or between two groups (P> 0.05).TP,LF and HF at T1-T4 were lower than those at T0 in two groups ( P < 0.05 ),HF at T1-T4 was higher in group A than that in group N(P < 0.05 ) ; LF/HF was no change at T1-T4 compared with that at T0 in group A (P > 0.05),but increased in group N (P < 0.05 ) which was higher than that in group A at the same time ( P < 0.05 ) ;TP,LF and HF was recovered at Ts in two groups,and HF at T5 in group A was higher than that in group N( P < 0.05 ) ; LF/HF at T5 in group A maintained the level at T0,but decreased in group N and lower than that at T5 in group N(P < 0.05).Conclusion Alprostadil for deliberate hypotension is more likely to maintain cardiac autonomic nerve balance that is helpful for perioperative security of elderly patients.
5.Construction of decision-making aid for rehabilitation training in patients with ischemic stroke

Objective To construct a decision-making aid for rehabilitation training in patients with ischemic stroke,providing structured guidance and support for patients'decision-making.Meth-ods Based on literature review and semi-structured interviews,a decision-making aid for rehabilita-tion training in patients with ischemic stroke was initially developed.The Delphi expert consultation method was used to conduct a two-round consultation for 15 experts.Results The response rate and effective rate of the questionnaires in the two rounds of expert consultation were both 100%,indica-ting a high enthusiasm among the experts;the authority coefficients of the experts in the two rounds of consultation were 0.88 and 0.89,respectively,indicating a high level of expert authority;the Ken-dall's W coefficients of concordance in the two rounds of expert consultation were 0.298 and 0.398,respectively(P<0.001),indicating a high degree of consistency among expert opinions.The final decision-making aid for rehabilitation training in patients with ischemic stroke included 4 primary in-dicators,9 secondary indicators,and 25 tertiary indicators.Conclusion The decision-making aid for rehabilitation training in patients with ischemic stroke constructed in this study is both scientific and practical,and can help improve the quality of decision-making of patients.

7. Application of bronchoalveolar lavage fluid in patients with pulmonary complications after allogeneic hematopoietic stem cell transplantation

Objective:
To evaluate the diagnostic value of bronchoalveolar lavage (BAL) for pulmonary complications in patients after allogeneic hematopoietic stem cell transplantation (allo-HSCT) and its safety.
Methods:
Patients with pulmonary complications after allo-HSCT underwent BAL. Microbiological smears, culture, PCR of CMV-DNA, EBV-DNA and TB-DNA, macro genomes new generation sequencing (mNGS) techniques were performed to detect pathogens in BAL fluid (BALF) .
Results:
A total of 73 allo-HSCT patients with 86 times of pulmonary complications enrolled this prospective study. They underwent 132 times of BAL procedures. The clinical diagnoses of 88.4% cases were made based on BALF analysis. Of them, 67 cases (77.9%) had infectious pulmonary complications, including 29 cases (33.7%) of fungal infection, 18 cases (20.9%) of mixed infection, 11 cases (12.8%) of viral infection and 9 cases (10.5%) of bacterial infection. The other 9 cases (10.5%) of non-infectious pulmonary complications included 8 cases (9.3%) of idiopathic pneumonia syndrome (IPS) and 1 case (1.2%) of pulmonary infiltration of lymphoma. The diagnoses of the remaining 10 cases (11.6%) were not determined. The platelet counts of 33 patients were less than 50×109/L before BAL. None of them developed severe bleeding complications during or after BAL. Transient fever occurred in 10 patients after BAL. Blood cultures showed staphylococcal bacteremia in them and anti-infection therapies were effective. No life-threatening complications occurred in all of the patients during or after BAL.
Conclusion
BALF analysis was informative for the diagnosis of pulmonary complication and safe for patients with pulmonary complications after allo-HSCT.
8.Effect of remazolam on sleep rhythm and postoperative delirium in elderly patients undergoing spinal surgery

Objective To investigate the effects of remazolam on melatonin secretion,sleep rhythm and postoperative delirium(POD)in elderly patients undergoing spinal surgery.Methods A total of 160 elderly patients,76 males and 84 females,aged 65-80 years,BMI 18.5-24.0 kg/m2,ASA physical status Ⅱ or Ⅲ,from November 2023 to January 2024 undergoing spinal surgery(lumbar interbody fusion,posterior approach)under general anesthesia were selected.The patients were divided into two groups by random number table method:the remazolam group(group R)and the propofol group(group P),80 pa-tients in each group.Group R was given remazolam 0.3 mg/kg for anesthesia induction,and then continued to pump remazolam 0.3-0.8 mg·kg-1·h-1 for anesthesia maintenance.Group P was given propofol 2.0 mg/kg for anesthesia induction,and then continued to pump propofol 4-6 mg·kg-1·h-1 for anesthesia maintenance.The levels of melatonin and cortisol were recorded at 04:00 a.m.on the day of surgery,1 day,2 and 3 days after surgery.Pittsburgh sleep quality index(PSQI)and VAS pain scores at rest were recorded 1 day before surgery,1 day and 2 days after surgery and before discharge.The occurrence of POD and post-operative sleep disorder(POSD)were recorded.The duration of extubation,PACU stay time,total analgesia pump compressions,effective analgesia pump compressions,number of remedial analgesia cases,intraoperative hypotension,postoperative nausea and vomiting,dizziness,respiratory depression and other adverse events were recorded.Results Compared with group P,the extubation time and PACU residence time in group R were significantly shortened(P<0.05),the level of melatonin was significantly increased while the level of cortisol was significantly decreased at 4:00 a.m.,and the PSQI was significantly decreased 1 day after surgery(P<0.05),the incidence of POD,POSD,and intraoperative hypotension were signifi-cantly decreased(P<0.05).There were no significant differences in extubation time,total and effective times of analgesic pump,rate of remedial analgesia,intraoperative hypotension,postoperative nausea,vom-iting,dizziness and respiratory depression between the two groups.Conclusion Remazolam has little effect on the secretion rhythm of melatonin and cortisol and the sleep rhythm after spinal surgery in elderly pa-tients,and the recovery is faster,which can reduce the incidence of POD and alleviate the sleep disorder af-ter spinal surgery.
9.Effect of opioid-free anesthesia combined with quadratus lumborum block in laparoscopic radical colorectal cancer resection

Objective To explore the effect of opioid-free anesthesia(OFA)combined with quad-ratus lumborum block(QLB)in laparoscopic radical colorectal cancer resection.Methods Sventy patients were selected for undergoing laparoscopic radical colorectal cancer resection from March to December 2023,49 males and 21 females,aged 18-75 years,BM1 18.5-28.0 kg/m2,ASA physical status Ⅱ or Ⅲ.The patients were divided into two groups using random number table method:the OFA group(group OFA)and the conventional opioid anesthesia group(group OA),35 patients in each group.Group OFA underwent bi-lateral posterior QLB under ultrasound guidance before anesthesia induction(0.25%ropivacaine 30 ml on each side),and anesthesia induction and maintenance were performed using opioid-free anesthesia regimen.And group OA cannot undergo QLB,and anesthesia induction and maintenance were carried out using opioid containing regimen.The patient's HR and MAP were recorded before anesthesia induction(T0),1 minute after endotracheal intubation(T1),before pneumoperitoneum establishment(T2),1 minute after pneumo-peritoneum establishment(T3),1 hour after surgery(T4),the end of surgery(T5),and leaving the oper-ating room(T6).The time from the patient's anesthetic discontinuation to extubation,the length of stay in the PACU,and the Steward and VAS pain scores when the patient leaves the operating room,which were recorded.NRS scores at rest and in the motor state 6,12,24,and 48 hours after surgery,time to first ex-haust,time to first ambulation,and length of postoperative hospital stay,effective PCIA pressing times and use of additional analgesic drugs within 48 hours after the operation,and postoperative adverse reactions(nausea,vomiting,hallucinations)were also recorded.Results Compared with T0,the MAP of the two groups decreased significantly at T1,T2,T4,and T5(P<0.05),the HR in group OFA increased signifi-cantly at T1 and slowed down at T4 and T5(P<0.05),and the HR in group OA decreased significantly at T1-T5(P<0.05).Compared with group OA,the HR in group OFA increased significantly at T1-T3,and the MAP increased significantly at T1(P<0.05).Compared with group OA,the extubation time and PACU residence time were significantly delayed(P<0.05),and the Steward score when leaving the oper-ating room was significantly lower in group OFA(P<0.05).Compared with group OA,NRS score was sig-nificantly deceased 6,12,24,and 48 hours after the surgery(P<0.05),time to first exhaust,time to first ambulation,and length of postoperative hospital stay were significantly shartened(P<0.05),and ef-fective PCIA pressing times and additional analgesia times were significantly decreased(P<0.05),inci-dence of intraoperative hypotensionand postoperative nausea and vomiting(P<0.05).Conclusion OFA combined with QLB can safely and effectively complete laparoscopic radical colorectal cancer surgery.Pa-tients with such anesthetic methods are hemodynamically more stable during anesthesia induction and intrao-peratively.These patients have better postoperative analgesia and less need for additional analgesics,and less incidence of postoperative nausea and vomiting.This approach is more beneficial for the recovery of the patient's gastrointestinal function.
10.Compound oleum lithospermi in improving clinical symptoms of mild to moderate diaper dermatitis: a multicenter, open-labeled, randomized controlled clinical study

Objective:To evaluate clinical efficacy and safety of topical compound oleum lithospermi in the treatment of mild to moderate diaper dermatitis.Methods:A multicenter, randomized, positive-drug parallel-controlled clinical trial was conducted in 19 hospitals from July 2019 to August 2020. Children aged 0 - 12 months with mild to moderate diaper dermatitis were enrolled and randomly divided into 2 groups using a random number table: test group topically treated with compound oleum lithospermi, and control group topically treated with zinc oxide cream. The treatment was carried out 6 - 8 times a day for 7 days. Visits were scheduled on days 0 and 7, and total response rate and clinical healing time were evaluated. Changes in the dermatitis family impact (DFI) score were compared between the test group and control group, and adverse events were recorded. Statistical analysis was carried out by using independent-sample t test for normally distributed continuous data, Wilcoxon rank sum test for non-normally distributed continuous data, and chi-square test or Fisher′s exact test for unordered categorical data; survival curves were drawn, and log-rank test was used for comparisons between two groups. Results:A total of 343 children with diaper dermatitis were enrolled in this study. Among them, 31 children violated the protocol, so 312 were included in the per protocol set, including 157 in the test group and 155 in the control group, and all completed the visits on days 0 and 7. The total response rate was significantly higher in the test group (87.26%, 137/157) than in the control group (78.71%, 122/155; χ2 = 4.04, P = 0.044) . The clinical healing time was significantly shorter in the test group (5.33 days) than in the control group (6.13 days; χ2 = 4.67, P = 0.025) . After 7-day treatment, the DFI score significantly decreased in both the 2 groups compared with that before the treatment, but there was no significant difference in the DFI score between the 2 groups (test group: 4.02 ± 6.96, control group: 3.58 ± 5.90, Z = -0.39, P = 0.686) . The incidence of adverse events was 2.92% (5/171) and 5.45% (9/165) in the test group and control group respectively, and there was no significant difference between the 2 groups ( χ2 = 0.03, P = 0.865) . Conclusion:Compound oleum lithospermi can markedly reduce the clinical severity of diaper dermatitis, improve the total response rate, shorten the clinical treatment period, and improve the quality of life of children′s families with a favorable safety profile.
