1.Closed-loop control for chest compression based on coronary perfusion pressure: a computer simulation study.

In this study, a closed-loop controller for chest compression which adjusts chest compression depth according to the coronary perfusion pressure (CPP) was proposed. An effective and personalized chest compression method for automatic mechanical compression devices was provided, and the traditional and uniform chest compression standard neglecting individual difference was improved. This study rebuilds Charles F. Babbs human circulation model with CPP simulation module and proposes a closed-loop controller based on a fuzzy control algorithm. The performance of the fuzzy controller was evaluated and compared to that of a traditional PID controller in computer simulation studies. The simulation results demonstrated that the fuzzy closed-loop controller produced shorter regulation time, fewer oscillations and smaller overshoot than those of the traditional PID controller and outperforms the traditional PID controller in CPP regulation and maintenance.

2.Sense of coherence among elderly patients with Parkinson's disease

Objective To study the level and influencing factors of sense of coherence (SOC) among elderly patients with Parkinson's disease (PD).Method Totally 118 elderly patients with PD were investigated by a self-designed demographic questionnaire,sense of coherence-13 (SOC-13) and social support rating scale (SSRS),and to explore the level and influencing factors of SOC among the patients.Results The total score of SOC-13 was (52.27±9.30),and it was at a low level.Multiple regression analysis showed that educational level,severity of the disease,family economic status and social support were the influencing factors of SOC.Conclusion SOC of elderly patients with PD is at a low level,which is influenced by individual factors,nurses should take targeted measures to improve the patients' SOC status by the influencing factors.

4.Study of the gene expression profiles in human allergic rhinitis

OBJECTIVE To explore the changes in gene expression in patients with allergic rhinitis(AR). METHODS 14500 gene DNA microarray(Affymetrix) was used to examine the gene expression in 6 AR nasal mucosal samples and 6 normal nasal mucosal samples.The differentially expressed genes were identified and subjected to real time RT-PCR analysis. RESULTS Compared with normal sinus tissues,161 genes were found to be differentially expressed in AR samples.Forty-seven genes were upregulated,114 genes were downregulated.The differentially expressed genes mostly involved in immune transcription regulatory molecules,signal transduction.Real-time RT-PCR results of CCL20,GPK4 were consistent with that of gene chip analysis.CONCLUSION Microarray expression profile of AR samples is differential.GPK4 and CCL20 may play an important role in cell signal transduction in mechanisms of AR.

6.Off-line analysis of gross tumor volume changes in nonoperatively treated lung cancer patients during radiotherapy by kilovoltage cone-beam computed tomography

Objective To investigate the gross tumor volume (GTV) changes in nonoperatively treated lung cancer patients during radiotherapy by off-line analysis using kilovoltage cone-beam computed tomography (KVCBCT).Methods Eighteen nonoperatively treated lung cancer patients were divided into group A (n =13) to receive conventional radiotherapy (1.8-2.2 Gy/fraction) and group B (n =5) to receive accelerated radiotherapy (5-8 Gy/fraction).Group A was further divided into subgroup A1 (n =10) and subgroup A2 (n =3) according to GTV changes.Each patient in group A underwent KVCBCT scan before treatment once a week,and each patient in group B underwent KVCBCT scan before each treatment.KVCBCT and CT images were registered in the treatment planning system to analyze GTV changes.Results Of all patients in group A,77% showed > 20% GTV reduction.Subgroup A1 had the maximum GTV reduction in the 4th week of treatment (the 20th treatment) ;Subgroup A1 had a mean reduction of (0.94 ± 9.94)%,with a maximum value of-56.76%.Subgroup A2 showed no correlation between GTV changes and treatment time.Group B had a mean GTV reduction of (-7.41 ± 1.76)%,with a maximum value of -15.91%.Of all patients in group B,71% showed ≤ 10% GTV reduction.Small GTV changes were observed in group B.Conclusions There are no regular GTV changes in nonoperatively treated lung cancer patients during radiotherapy.Adaptive radiotherapy is recommended in the cases where GTV is reduced over 20% in the 20th treatment.
7.Application and clinical significance of 24-hour multichannel intraluminal impedance-pH monitoring in gastroesophageal reflux disease

Objective To investigate the application and clinical significance of 24-hour multichannel intraaluminal impedance-pH (MII-pH) monitoring in gastroesophageal reflux disease (GERD).Methods Fiftythree patients with GERD were enrolled in this study according to the Montreal consensus (consulting for twice reflux a week or above) from July 2011 to June 2012.Patients were divided into erosive esophagitis (EE,n =25)group and non-erosive reflux disease (NERD,n =28) group after endoscopy and MII-pH monitoring.Fifteen healthy volunteers were recruited as the normal controls.The change of pH and MII-pH parameters were compared among the three groups and the significance of the change was investigated.Results Twenty-four-hour pH monitoring showed that all the acid reflux events in the EE and the NERD groups were significantly higher than those in the control group (P < 0.05).MII-pH monitoring showed that the frequencies of total reflux,acid reflux,weakly acidic reflux and percentage of acid reflux in the GERD group were higher than in the control group (total reflux:83 (54,118) vs.62 (44,111) vs.42 (20,70),P =0.003 ; acid reflux:45 (25,79) vs.22 (11,45) vs.3(1,10),P =0.000 ;weakly acidic reflux:36(18,47) vs.43(21,82) vs.23(11,43),P =0.001 ;percentage of acid reflux:53% (37%,81%) vs.32% (13%,48%) vs.11% (1%,23%),P =0.002).The frequency and percentage of acid reflux in the EE group were higher than those in the NERD group (P =0.000)The percentage of weakly acidic reflux in the NERD group was higher than in the EE group (66% (43%,79%) vs.46% (21%,57%),P <0.01).The frequencies of liquid reflux and mixed reflux in the GERD groups were higher than those in the control group (22 (12,40) vs.18 (12,26) vs.9 (4,18) ; 54 (39,79) vs.42 (25,77) vs.29(14,48) ;P <0.01).The frequency of gas reflux in the control group was higher than in the GERD group (86(56,207) vs.31 (14,62) vs.34 (15,119),P < 0.01).The frequency and percentage of proximal reflux in the GERD group were significantly higher than in the control group (28(18,41) vs.16(12,34) vs.3 (2,9) ; 33% (22%,49%) vs.29% (22%,35%) vs.11% (6%,22%),P < 0.001).The percentage of symptom positive index in patients with acid reflux,non-acidic reflux and total reflux in the EE group were 36.0% (9/25),20.0% (5/25) and 56.0% (14/25) respectively which were higher than in the NERD group (21.4% (6/28),14.3% (4/28) and 35.7% (10/28)).Conclusion MII-pH monitoring can detect more reflux events.Acid reflux plays an important role in GERD.The detective rate of GERD will be elevated when combined with MII-pH monitoring in the diagnosis.MII-pH monitoring has a distinct advantage in diagnosing GERD.

9.Prokaryotic expression of human ubiquitous mitochondrial creatine kinase, preparation of its antiserum and clinical application

Objective:To clone the full-length cDNA of human ubiquitious mitochondrial creatine kinase(uMtCK) and express its protien in E. coli. The protein was used to immunize rabbit to prepare anti-uMTCR polyclonal antibody which was then used to detect the expression of uMtck in gastrointestinal carcinoma. Methods: A 1 062 bp segement was amplified from human HaLa cell by RT-PCR,which was then inserted into pMD18-T plasmid and proved to be the coding sequence of uMtCK by endonuclease digestion and sequencing. The coding sequence of uMtCK was inserted into pQE30 plasmid and recombinant plasmid pQE30-uMtCK was transformed into E. coli. The expressed protein was purified with a His. Bind column and was analyzed with SDS-PAGE and Westen blot and the activity of uMtCK was tested. An antiserum against uMtCK was perpetrated by immunizing rabbit with purified uMtCK. The expression of uMtCK in 59 patients with gastric and colonic cancer was detected by immunohistochemistry. Results:The coding sequence of uMtCK was successfully obtained by RT-PCR and the product was successfully inserted into pQE30 to construct a recombinant vector pQE30-uMtCK. The expression of uMtCK was effective and soluble, A good specific antiserum against uMtCK was proved by Western blot. The expression of uMtCK of gastric and colonic cancer was 76. 5%. Conclusion:The coding sequence of uMtCK is coined and expressed in E.coli. uMtCK is a prospective marker for the diagnosis of gastric and colonic cancer.

Objective To ummarized the clinical characteristics of lactic acidosis combined with hypoglycemic coma,diabetic ketoacidosis and hyperkalemia,so to improve the physician's treatment ability by summary the clinical feature of lactic acidosis combined with hypoglycemic coma,diabetic ketosis and hyperkalemia.Methods Analyzed the clinical characteristics and disease development of one case with lactic acidosis combined with hypoglycemic coma,diabetic ketoacidosis and hyperkalemia,and reviewed literature.Results The patient had hypoglycemia and acidosis at the same time due to taking phenformin and glibenclamide.After 2 days,the patient was better through correcting acidosis,intravenous fluid therapy,lowering blood potassium and controlling blood glucose.Conclusion Because acidosis caused the potassium transfer from intracellular to extracellular,the latter leads to hyperpotassemia,lactic acidosis caused by phenformin should be treated with intravenous fluid therapy,correcting acidosis.
