1.Effect of TGP capsules combined with urokinase for the patients with acute cerebral ischemic stroke

Objective To evaluated the effect of Total Glucosides of Paeony (TGP)combined with urokinase for the patients with acute cerebral ischemic stroke.MethodsA total of 152 patients with ACIS were divided into the control group (77 patients) and the observation group (75 patients) by completely randomized grouping method. Patients in the control group were treated with Urokinase by intravenous, and the observation group were treated with TGP combined with Urokinase. All patients were treated with thrombolysis treatment with in the 24 h after the exclusion of cerebral hemorrhage by brain CT and coagulation mechanism disorder. In the following 14 days, they were given the treatment of anti platelet aggregation, lowering blood lipid, trophic nerve and improving circulation. The National Institutes of Health Stroke Scale(NIHSS), the modified Rankin scale(mRS) and the daily life ability index(Barthel) were measured.ResultsThere were 28 (37.3%) patients with the NIHSS scores decreasing 4 points in observation group and 12 (15.6%) in control group after treatment of 7 days,and there was significant difference between two groups (χ2=8.181,P=0.004). There were 68 (90.7%) patients with mRS score less than 4 points in observation group and 51 (15.6%) in control group after treatment of 14 days,and there was significant difference between two groups (χ2=11.945,P=0.001). The NIHSS scores in the observation group was lower than control group (8.3 ± 2.2vs. 9.9 ± 2.5;t=4.192,P<0.05). The mRS score in the observation group was lower than control group (2.3 ± 0.9vs. 2.9 ± 1.1;t=3.684,P<0.01), and the Barthel index in the observation group was higher than the control group (88.7 ± 16.2vs. 77.5 ± 15.2;t=4.401,P<0.01). ConclusionsThe TGP capsules combined with Urokinase intravenous thrombolysis could improve the neurological deficit, and the ability of daily life of ACIS patients.

Studies confirm that gefitinib treatment of epidermal growth factor receptor(EGFR)muta-tions for patients with non-small cell lung cancer(NSCLC)has a clear effect,and with radiotherapy sensitiza-tion effect. Many studies both at home and abroad show that gefitinib combined with radiotherapy can signifi-cantly improve the survival times of patients,especially for the elderly patients or the patients with brain metas-tases,which has fewer adverse reactions and with higher life qualities. Therefore,gefitinib combined with radiotherapy will be an effective treatment for the patients with advanced NSCLC.

Objective To investigate the relationship of B-type natriuretic peptide(BNP),Creactive protein(CRP)and left ventricular diastolic function with cardio-and cerebrovascular disease (CVD)in the elderly patients with hemodialysis.Methods 96 elderly patients with hemodialysis were divided into CVD group(n =35)and non CVD control group(n =61).The patients were followed up once a month for 24 to 30 months.The baseline anthropometric and laboratory parameters were measured.The ratio of mitral peak flow velocity(E)and peak velocities at mitral annuluses (Em)during early diastole of left ventricular was recorded by doppler echocardiography.The time and cause of hospitalization and mortality by CVD were documented.Results The levels of CRP and BNP were significantly higher in CVD group than in control group[(3.1 ± 6.7)mg/L vs.(1.8 ± 1.2)mg/L,(1345.2±1427.8)pmol/L vs.(719.8±1073.8)pmol/L](t=2.14 and-2.82,P<0.05 or P<0.01).The ratio of E/ Em was increased in CVD group as compared with control group(t=5.23,P<0.01).Kaplan-Meier survival curves showed that the morbidity of CVD events was enhanced(P<0.05 or P<0.01)at BNP≥500 pmol/L,CRP≥1.5 mg/L and E/Em≥17 which were the independent risk factors of CVD events for the elderly patients with hemodialysis by COX regression model.There was positive correlation between LogBNP and E /Em(r =0.23,P < 0.05).Conclusions Higher levels of BNP,CRP and E/Em may predict the increasing risks of CVD in the elderly patients with hemodialysis.
5.Evaluation by examining the infection of Helicobacter pylori between precancerous lesions and gastric cancer patients

Objective To provide the basis for early detection and treatment of precancerous lesions of gastric carcinoma, and reference for early discovery, diagnoses and treatment of gastric cancer, by examining the differences of expression of Helicobaeter pylori (Hp), both in the gastric mueosa and in blood serum, between precancerous lesions of gastric carcinoma patients and gastric cancer patients. Methods Used immunechromatographic method and protein chip system to determine the expression of Hp in blood serum and used immunohistochemistry to detect the expression of Hp in gastric mucosa. The patients had been diagnosed by pathological methods, 170 patients with precancerous lesions of gastric carcinoma, 111 patients with gastric carcinoma and 31 patients with superficial gastritis. Results The expression of Hp was detected on every stage of precancerous lesions and gastric carcinoma. Compared with the detection rates of Hp, precancerous lesions of gastric carcinoma was higher than gastric carcinoma and superficial gastritis, superficial gastritis was the lowest. The differences of the three were significant (P<0.01). The detection rate of Hp in the gastric mueosa (86.2%, 66.7%, 38.7%, respectively)was obviously higher than in blood serum (80.6%, 52.3%, 32.3%, respectively). Conclusions Hp participates in the process of the precancerous lesions and gastric carcinoma. The expression of Hp both in the gastric mueosa and in the blood serum could be one of the indexes, which might report the process and curative effect of precancerous lesions. With such advantages as simple and applicability, the detection of Hp in the blood serum could be used widely. The endoscopy and the detection of Hp in the gastric mucosa are suggested for patients with Hp positive in the blood serum.
6.Uric acid stimulates rat glomerular mesangial cells proliferation via activation of extracellular signal-regulated kinases

Objective To explore the effect of uric acid (UA) on the rat glomerular mesangial cells (GMCs) and its possible mechanism in vitro. Methods Cultured rat GMCs were treated with various concentrations of UA (50 μmol/L, 100 μmol/L, 300 μmol/L) in the presence or absence of U0126, apocynin, rotenone. The incorporation of 3H-thymidine (3H-TdR) and cell counting were used to measure GMCs proliferation. GMCs cell-cycle was analyzed by flow cytometry. CyclinD1 and cyclin A2 expression were determined by real-time PCR and Western blotting analysis. ERK1/2 phosphorylation was detected by Western blotting. Reactive oxygen species (ROS) was measured by 2, 7-dichlorofluorescein diacetate (DCFDA) fluorescence. Results (1) Uric acid increased GMCs proliferation in dose-dependent manner compared with the control groups, as assessed by 3H-TdR incorporation and cell counting. GMCs proliferation induced by 300 μmol/L uric acid was increased by more than 1.5-fold assessed by both of the methods. (2) Uric acid decreased cell number in G1/G0 phase and increased cell number in S phase in dosedependent manner, as assessed by flow cytometry. (3) Uric acid iuduced cyclin D1 and cyclin A2 expression in dose-dependent manner. (4)Uric acid increased pospho-ERK1/2 in dose-dependent manner and ERK1/2 specific inhibitor U0126 could suppress uric acid-induced cell proliferation.The inhibition percentage of U0126 was 22% and 31% assassed by cell counting and 3H-TdR incorporation, respectively. (5) Uric acid increased ROS production in dose-dependent manner.NADPH oxidase inhibitor apocynin could also significantly inhibit uric acid-induced ROS production, ERK phosphorylation and cell proliferation. In contrast, rotenone had no effect on them.Conclusion Uric acid can stimulate rat GMCs proliferation, partially by the activation of ERK pathway via NADPH oxidase-derived ROS generation.
7.Clinical values of superoxide dismutase and malondialdehyde detection in cord blood of newborns with fetal distress

Objective To investigate the relations between intrauterine asphyxia and peroxidation and newborn hypoxic-ischemic encephalopathy(HIE). Methods The levels of superoxide dismutase (SOD) and malondialdehyde (MDA) in cord blood of 60 newborns with intrauterine asphyxia during labor(which was divided into two groups,39 cases with asphyxia in groupⅠ, and 21 cases with asphyxia in groupⅡ),and in 30 newborns without intrauterine asphyxia(control group) were determined. The levels of SOD and MDA in cord blood of newborns with HIE were compared with those in newborns without HIE. The incidence of HIE was estimated simultaneously. 9.Evaluation of the relationship between donor graft position and recipient position after endothelium keratoplasty by AS-OCT

Background The advantages of endothelium keratoplasty(EK)include without corneal surface incision and sutures,remaining normal corneal topography,minimizing astigmatism and providing a healthy donor endothelial cell count and function.Using anterior segment optical coherence tomography(AS-OCT) to examine the morphology of the corneal graft after EK is helpful for the evaluation of the outcome of EK.ObjectiveThis study applies AS-OCT to observe the relationship of recipient cornea and donor graft and offer a basis for the evaluation of operative outcome.MethodsThe clinical data from 44 eyes of 42 patient who received DSAEK in Peking University Eye Center from September 2007 through April 2009 was reviewed retrospectively.These patients included 30 male and 12 female with the age from 15 to 83 years.Structure of cornea and relationship between recipient cornea and graft disc after surgery were clinically examined and recorded by slit-lamp digital camera and AS-OCT.This study was approved by the Ethics Committee of Peking University.Written informed consent was obtained from all of the patients before the operation.ResultsThe donor graft attached to posterior central surface of recipient in 28 eyes under the slit-lamp microscopy and AS-OCT.The thickness of donor graft was decreased,and graft sticked more closely to recipient cornea and was more transparent with the lapse of time.Complete dislocation of the donor graft in early stage was found in 5 eyes(11.36%) and partial dislocation in 2 eyes(4.55%).Curliness of graft edge occurred in 1 eye(2.27%) and eccentricity of donor graft in 8 eyes(18.18%).ConclusionAS-OCT can display the morphology and structure of cornea as well as the relationship between donor graft and recipient after DSAEK.It is helpful for evaluating the clinical effectiveness of DSAEK.
10.Clinical features and drug resistance of Listeria monocytogenes infection in neonates

Objective To study the clinical distribution of Listeria monocytogenes infection and the changes in drug resistance of Listeria monocytogenes isolated from inpatients during recent 3 years,and to increase the awareness of the situation and provide data for clinical antibiotics application.Methods The clinical distribution of 22 cases of neonatal Listeria infection and drug resistance changes of Listeria were retrospectively analyzed in Bayi Children's Hospital from Jan.2011 to Dec.2013.Results Neonates began to be attacked by Listeria monocytogenes of 0.5 hours to 5 days (an average of 17.45 hours) after birth.The average birth weight was (2 331.82 ± 677.64) g.There were 7 full term cases and 15 premature infants,13 cases with low birth wcight.The average hospitalization was (21.91 ± 17.64)days.The cure rate was 45.45% (10/22 cases).All the mothers of 15 cases had fever in the third trimester of pregnancy and the temperature was 37.5-39.5 ℃.Infection rate with Listeria monocytogenes in neonatal was 0.03% (2/7 137 cases),0.11% (8/7 281 cases) and 0.19% (12/6 394 cases) in 3 years,respectively.From 2011 to 2013,the sensitive rate of antimicrobial drugs with Listeria monocytogencs to commonly used antimicrobial was 82.72%,75.40% and 50.66%,and the rate of drug resistance was 17.28%,17.50% and 11.01%,respectively.During 3 years,the rates of drug resistance had no significant difference (x2 =3.65,P > 0.05),and the sensitive rates had a trend of declination year by year(x2 =36.87,P < 0.01).The sensitive rates and the drugs resistant rates of penicillin were 33.93% (19/56 cases)and 51.79% (29/56 cases),respectively.In 3 years,the drugs resistant rates of penicillin was 100.00%,40.00%,and 46.43%,and the sensitive rate was 0,60.00%,25.00%,respectively.There was a high sensitivity of Listeria monocytogenes to ampicillin,aminoglycoside,sugar peptide,tetracycline,macrolides,lincosamides,quinolone,sulfa and other classes (such as rifampicin).It showed the different drug resistance rates with 33.33%-100.00% to oxacillin,penicillin G and nitrofurans.Conclusions These children has the characteristics of early-onset infection.The pregnant women and newborns are susceptible to high-risk groups.Infection rates with Listeria of neonatal and Listeria monocytogenes isolated from inpatients showed a trend of increase year by year.The cases were very sensitive to commonly used antimicrobial for killing Listeria monocytogenes.There was a trend of the declination for drug resistance to penicillin,but it was still at a higher level.The drugs resistance rate to oxacillin,penicillin G and nitrofurans were high.
