1.Experimental study on influence of storage time in vitro on bone autograft healing

Objective:To discover the influence within four hours of different storage time in vitro on the osteogenic capability in bone autograft.Methods:The ilium blocks in 0.9% saline were kept at room temperature for 0、1、2、 3 or 4 hours before replacement to extra-skeletal muscle gaps in the same rabbit.The grafts transplanted to muscle gaps were divided into two types:irrigating bone marrow with saline and non-irrigating bone marrow.The histological specimens of the autograft in extra-skeletal muscle gaps were taken at the 3rd day and 1、2、4、6 weeks after operation, stained by ALP.PAS and HE,and analysed by histomorphometry.Results:There were survival osteocytes and osteoblasts in all ectopic graft groups three days after operation,but no new bone formation.New osteoid tissues were formed one week after operation, and they increased at the second week and were gradually transformed into woven bone and lamellar bone tissues in all ectopic graft groups. The results of histomorphometry analysis showed there was no remarkable difference among all ectopic graft groups.Conclusion:Storage of the bone blocks in 0.9% saline for four hours before implant has no remarkable influence on the capacity of bone formation in bone autograft.Irrigating bone marrow with saline would not influence the survival of autograft and the capacity of new bone formation.Therefore,it is a reasonable method to keep bone blocks in saline before implant.
2.Metal on metal hip replacement for the traumatic arthritis developed from fracture of acetabulum

Objective:to explore the clinical curative effect of treatment for the traumatic arthritis developed from fracture of acetabulum with metal on metal hip replacement.Methods:a total of 11 cases(aged from 31~55,averaged 43.2 and the mean body mass index was 24?2.3,with a range between 20.3 and 29.2)of traumatic arthritis developed from open reduction and internal fixation for the fracture of acetabulum were admitted from September 2003~April 2007,among which 9 cases were male,2 female,and the aetiology of the acetabular fractures could be classified as follows:traffic accident trauma in 8 cases were,high falling injury in 2 cases,heavy pound in 1 case.According to the Letournel classification,there are 3 cases of posterior column accompanied with posterior wall fracture,2 cases of posterior wall accompanied with posterior dislocation,2 cases of transverse accompanied with posterior wall,2 cases of both column,one cases of posterior wall and one cases of transverse fracture.All the cases were examined before the former and this operation respectively by X-ray and 3D CT to evaluate the fracture configuration and the bone defect.The involved hip function was evaluated according to the Harris score pre-and 2 years postoperatively.Results:All the incisions healed by first intension,and no complications like dislocation, nerve and blood vessel injury were developed.All the 11 cases were followed up,time ranged from 24 to 67 months,and averaged 38.5? 9.5 months.According to the Harris score system,the involved hip function showed excellent result in 2 cases,satisfactory in 8 cases, poor in one case,and the excellent and the satisfactory cases accounted for 90.1%.Conclusion:Metal on metal hip replacement for the traumatic arthritis developed from fracture of acetabulum could preserve the hip function with less complications,which makes the patients satisfied and deserves clinical expansion.
3.A comparative assessment of short-term outcomes of Corail entirely hydroxyapatite-coated versus cemented femoral stem prosthesis in total hip arthroplasty for elderly femoral neck fracture

4.The performance evaluation of reticulocytes detected by XN-3000 automatic blood analyzer

Objective:To evaluate the performance of reticulocyte (Ret) detected by Sysmex XN-3000 automatic blood analyzer.Methods: 96 blood samples were collected randomly in clinical detection of reticulocytes, and respectively were detected by XN-3000 automatic blood analyzer and microscopy. The precision, stability, linearity and carryover of Ret in blood samples detected by XN-3000 automatic blood analyzer were evaluated by the correlation analysis of the results from the two methods and the correlation analysis would be completed by microscopy.Results: precision, linear range and sensitivity of Ret measured by XN-3000 were within the allowable range. The stability did not change significantly below 4℃ within 48h (CV<1%). The carryover rate was 0.14%. The results measured by XN-3000 were well compared with microscopy (r=0.979).Conclusion: Sysmex XN-3000 automatic blood analyzer is applied to the clinical detection of large quantities of specimens and it is ideal for measuring Ret.
5.The acute inhibitory effect of adrenomedullin on hypoxia-induced pulmonary hypertension in rats

AIM: The aim of this study is to investigate the acute inhibitory effects of adrenomedullin(ADM 1-52 ) on hypoxia pulmonary hypertension and its influence on systemic blood pressure. METHODS: Thirty-six male Wistar rats were divided into two groups. Eighteen were exposed to hypoxia for 21 days as hypoxic pulmonary hypertension group,and another eighteen were kept in ambient as control group. Each group were divided into three subgroups which were injected intravenously with ADM 0 1 nmol/kg, 0 3 nmol/kg, 1 nmol/kg, respectively,then hemodynamic parameters were recorded. Plasma cyclic adenosine 3,5,-monophosphate (cAMP) was measured by radioimmunoassay before,during and after injection of 0.3nmol/kg adrenomedullin(ADM 1-52 ). 6.Treatment of humeral proximal fracture with locking proximal humerus plate

Objective To summarize the clinical effect of open reduction and internal fixation for the proximal humeral fracture with locking proximal humerus plate (LPHP). Methods A total of 45 cases with complex humeral fractures were operated via deltoid-pectoral approach. According to the Neer classification, there were 33 cases with three-part fractures and 12 with four-part fractures. Neer nu- merical rating system was employed to evaluate postoperative function of the involved shoulder. Results Of all, 38 cases were followed up for a mean 14.7 months, which showed fracture union within 3 months in 35 cases and fracture union within 4 months in 3. According to the Neer numerical rating system, re- suh was excellent in 20 eases and satisfactory in 15, with total excellence rate of 92%. Conclusion LPHP is characterized by stable fixation, minor complication and high satisfaction, and hence is an effec- tive method for proximal humeral fractures.
7.Expression and significance of CD44V6 in thyroid neoplasms

Objective To investigate the expression of CD44V6 in thyroid neoplasms and its significance. Methods The expression of CD44V6 in paraffin-embeded thyoid carcinoma tissue from 100 patients (50 with lymph node metastasis,50 without) and 20 patients with thyroid adenoma was measured by immunohistochemistry staining. Results Positive expression of CD44V6 in the group of thyoid carcinoma was 72%, significantly higher than the group of thyroid adenoma.Positive expression of CD44V6 in the group of thyoid carcinoma with lymph node involvement was 90%,significantly higher than the group of thyoid carcinoma without lymph node involvement.In patients with different pathological grade,different clinical stage and different survival time, the positive expression of CD44V6 was significantly different. Conclusions The results suggest that CD44V6 can be regarded as an valuable molecular markers to distinguish benign thyroid tumor from malignant tumor, to predict lymph node metastasis and prognosis.

Objective A method was developed for the simultaneous determination of tryptophan(Trp) and its metabolites kynurenine(Kyn) and kynurenic acid(Kyna) by high performance liquid chromatography with fluorescence detection(HPLC-FD),and testing serum levels of Trp metabolites in systemic lupus erythematosus(SLE) patients.Methods Serum samples were deproteinized by equal volume of 0.624 mol/L perchloric acid.The analytical column was Hypersil C18 column,and the mobile phase was 0.20 mol/L zinc acetate,8.3 mmol/L acetic acid,and 2.5% acetonitrile;flow rate was 1.5 ml/min.The excitation and emission wave length of fluorescence detector were 365 nm and 480 nm in 0~11 min,344 nm and 404 nm in 11~15.5 min,254 nm and 404 nm in 15.5~20 min,respectively.Results The linear range of Trp was 0.610~196 ?mol/L,the detection limit was 0.005 ?mol/L,and the average recovery was 103.71%.The linear range of Kyn was 0.049~98 ?mol/L,the detection limit 0.025 ?mol/L,and the average recovery was 97.45%.The linear range of Kyna was 1.050~1047 ?mol/L,the detection limit was 0.050 nmol/L,and the average recovery was 100.60%.Inter-and intra-day precisions were both less than 5%.Phenylalanine,tyrosine,and 5-hydroxytryptamine had no interference.The assay was employed to analyze serum samples of SLE patients.The result showed significant difference in Trp,Kyn,and Kyna content,Kyn/Trp ratio between SLE patients and control group.Conclusions A new method was established for simultaneous determination of Trp,Kyn,and Kyna in serum.The method is simple,fast,sensitive,specific,and suitable for applicability to clinical measurement.
10.Electrocochleography in the diagnosis of Meniere's disease.

OBJECTIVE:
To evaluate clinical value of electrocochleography in diagnosing the Meniere's disease.
METHOD:
Patients were saperated as having definite, probable and possible Meniere's disease according to the 1995 AAOHNS guidelines. Then set the probable and possible Meniere's disease as a suspected group. We used four different stimulus (clicks, 1000 Hz tone burst, 2000 Hz and 4000 Hz tone burst) to record electrocochleo-graphy. Those patients in each group received electrocochleography examination, and summating potential (SP)/action potential (AP) ratios were calculated. A prolonged action potential latency shift (AP shift) evoked by rarefaction and condensation clicks was also measured.
RESULT:
The click SP/AP positive rate in definite Meniere's disease group was 41.2%, tone burst at 1,000 Hz. 2,000 Hz,4,000 Hz were respectively 80.4%, 72.5%, 37.3%, AP shift positive rate was 45.1%. Using McNemar test, the positive rate of tone burst 1000 Hz (chi2 = 15.04, P < 0.01) and that of tone burst 2,000 Hz (chi2 = 8.65, P < 0.01) were statistically significant different when compared with that of click SP/AP, besides the tone burst at 1,000 Hz yielded has the highest sensitivity. The click SP/AP positive rate in suspected group and definite group were respectively 12% and 45.1% (chi2 = 6.610, P < 0.05) and AP shift positive rate of them were respectively 8.0% and 41.2% (chi2 = 10.460, P < 0.01). By chis-quare test, the positive rates of click SP/AP and AP shift between these two groups were statistically significant different.
CONCLUSION
Electrocochleography is a reliable test in the diagnosis of Meniere's disease. Compared with clicks, tone burst significantly has a higher sensitivity, especially at 1000Hz and 2000Hz frequencies. A prolonged action potential latency shift evoked by rarefaction and condensation clicks is also a useful addition to diagnosis Meniere' disease.
