1.Expression of TGF-? in Villus and Deciduas after Medical Abortion

Objective To investigate the expression of transforming growth factor-?(TGF-?) in the villus and deciduas of different outcomes after administration with mifepristone and misoprostol in the termination of early pregnancy. Methods From May 2004 to March 2005,50 women who were comfirmed early pregnancy and requested termination of pregnancy in the outpatient department were recruited.Patients with medical abortion were divided into two groups according to the final outcomes: group of complete abortion(n=15) and group of incomplete abortion((n=)15).Another 20 patients with surgical abortion were served as control.The villus and deciduas were collected at the third day of medical abortion,whereas the same was done at the day of surgical abortion.The expression of TGF-? in villus and deciduas of the three groups were measured by immunohistochemical method. Results The age,gestational age,diameter of gestational sac and levels of serum E_(2),P and ?-HCG before abortion were similar in these groups.Compared with the other two groups,the expressions of TGF-? in the villus and deciduas in the group of complete abortion were significantly decreased(P

4.Approach to CDISC SDTM Implementation for Clinical Trials Data Submission

This study was aimed to discuss the ways of converting clinical data entered in the Case Report Form (CRF) to Study Data Tabulation Model (SDTM) database format in clinical research of traditional Chinese medicine (TCM). Currently, there were three approaches for implementing SDTM, which were the pure SDTM approach, submission-only approach and database-only approach. This article compared and analyzed advantages and disadvantages of three approaches and introduced experiences of the Clinical Evaluation Center of China Academy of Chinese Medical Sciences using database-only approach for SDTM implementation. The results showed that pure SDTM approach can maximally embrace SDTM standards. However, the current process and software system should be modified. Therefore, it was time-consuming and expensive. The submission-only approach was the most economic way in the application of SDTM standards. However, the data quality and traceability may not be guaranteed. The database-only approach built the study database based on the SDTM standard by writing transformed program before data entry while the data collection system was not SDTM-compatible. It was concluded that database-only approach for implementation SDTM was a suitable and practical way to TCM clinical research.
5.Advances in research on VEGF-related signaling pathways in angiogenesis

Tissues and organs generate angiogenesis under the stimulation of angiogenic factors in physiological or pathological conditions.Multiple signal pathways including VEGF, Notch, Wnt/β-catenin, Ang1(2)/tie2 and PIK-Akt etc.have effects on various stages of angiogenesis.VEGF exerts irreplaceable effects on the whole process of angiogenesis through multiple signal pathways.Over the past few years, new progress has been made in the researches of mechanisms regulating angiogenesis through VEGF-related signal pathways both at home and abroad.These findings provide us new theoretical basis for clarification of the pathogenesis of many diseases and clinical drug development.In this article we will summarize the recent research progress in this field, hoping to provide new possibilities for the treatment of angiogenesis-related diseases.
6.Expression of HOXB6 Gene on Differentiation of Hematopoietic Stem Cells and Intervention Research

Objective To observe the expression level of HOXB6 gene on the differentiation and proliferation of hematopoietic stem cells(HSC) to colony forming unit-granulocyte-monocyte(CFU-GM),erythroid progenitor(CFU-E) and colony forming unite-T-lymphocyt(CFU-TL).And the proliferation procress was affected by all-transretinoic acid(ATRA).Methods 1.By the colony culture in vitro,the impact of ATRA on the CFU-GM,CFU-E,CFU-TL colony formation were surveyed;the expressions of HOXB6 gene were observed on the differentiation procress of HSC to CFU-GM,CFU-E and CFU-TL affected by ATRA on the 3rd,7th,12th.2.Real-time fluorogenic quantitative reserve transcription-polymerize chain reaction(FQ-RT-PCR) method was used to explore the possible mechanism of ATRA up-regulated to human cord blood CFU-GM,CFU-E and CFU-TL in gentic level.Results 1.HOXB6 gene had a regulatory function in the differentiation procress of hematopoiesis.During the differentiation and proliferation of HSC to colony forming CFU-GM or CFU-E,CFU-TL in vitro,the expressions of HOXB6 gene were significant positive.2.Compared with the expressions of CFU-GM and CFU-TL HOXB6 mRNA on day 3,the quantity of HOXB6 mRNA was obviously higher on day 7 and lower on day 12,respectively in each group.Compared with the expression of CFU-E HOXB6 mRNA on day 3,the quantity of HOXB6 mRNA was obviously higher on day 12 in each group.3.Compared with the HOXB6 gene of control group,the expressions of HOXB6 gene of group ATRA were up-regulated remarkable.Conclusions 1.HOXB6 gene has a regulatory function in the differentiation procress of hematopoiesis.HOXB6 gene plays an important role in the progress which can be associated with the regulating effect of HOXB6 gene on the differentiation and proliferation of CFU-GM,CFU-E and CFU-TL.2.During the differentiation and proliferation of HSC,the expression of HOXB6 gene is regular in time.3.Low concentration of ATRA(60 ?mol?L-1) can up-regulate the expression of HOXB6 gene,which confirms the theory that the normal hematopoie-tic lineage determination and maturation rely on the stable and consistent expression of HOXB6 gene.
7.Quality Survey of Data Management of TCM Chronic Clinical Researches

Objective To study the scientificity and rationality in data management of the project of Chinese Chronic Disease Clinical Research System and its Achievements Application Mode;To provide evidence and reference for the improvement of follow-up work. Methods According to the relevant management regulations, guidelines, and other relevant documents of national and international clinical research data,“Questionnaire for Research Data Management Assessment of Chinese Chronic Disease Prevention” was designed. The questionnaires were used in 10 clinical topics during the on-site inspection survey, and 20 valid questionnaires were retrieved and analyzed statistically. Results 50-60 percentage of subjects in data management and statistics were performed by the third party;70 percentage of subjects were performed by their own central monitoring;50 percentage of the subjects adopted a cooperative manner with others to develop appropriate data management systems;7 centers had system and plan for data management;Data management was carried out;implementation degree of training, database, CRF tracing, and others 7 aspects was low. Conclusion Improving the quality of data management of TCM clinical research still needs to be perfected from many aspects, and quality mode of large-scale clinical trials should be improved.
8.The tolerance to 188Re-HEDP treatment in patients with bone pain from osseous metastases

Objective To study the tolerance to 188Re-1-hydroxy-1 ,1-ethylidene disodium phosphonate(HEDP) in patients with bone pain caused by osseous metastases. Methods Thirty-one patients(10with prostate cancer, 9 with breast cancer, 3 with lung cancer, 5 with liver cancer, 2 with rectal cancer, 1with esophageal cancer and 1 with renal cancer) received a single injection dose of 188Re-HEDP. The patients were divided into four groups according to the injection dose: 20 MBq/kg (6 patients), 30 MBq/kg(6 patients), 40 MBq/kg (9 patients), and 50 MBq/kg (10 patients). Haematological toxicity (WHO grading) of grade Ⅲ- Ⅳ was considered unacceptable. Vital signs and adverse effects after injection were recorded for 8 weeks. Blood counts were measured weekly during a period of 8 weeks. Biochemical parameters and electrocardiogram were assayed at week 4 and 8. Statistical analysis was performed for per-protocol (pp) population (t-test). Results Twenty-seven patients belonged to PP population with 5 in the group of 20 MBq/kg, 5 in the group of 30 MBq/kg, 8 in the group of 40 MBq/kg and 9 in the group of 50 MBq/kg.No obvious adverse effects and no significant change of vital signs, electrocardiogram, liver and renal function were found after injection. Alkaline phosphatase was slightly higher than baseline at week 4 and 8 after therapy, but the difference was not statistically significant. In the 20 MBq/kg group, reversible grade Ⅰ leucopenia was noted in 1 patient. In the 30 MBq/kg group, 2 patients showed reversible grade Ⅰ leucopenia including 1 alone with reversible grade Ⅲ thrombopenia. In the 40 MBq/kg group, reversible grade Ⅰ leucopenia and thrombopenia was observed in 1 patient and reversible grade Ⅱ leucopenia and thrombopenia in another patient. In the .50 MBq/kg group, 3 patients showed reversible grade Ⅱ leucopenia. The lowest level of thrombopenia was at week 4(143.5 × 109/L), leucopenia at week 6 (5.4 × 109/L) and anaemia at week 8(t = 3.1325, 3.3156, 3.4917, all P < 0. 05 compared with baseline). At week 8, the mean level of platelet and leucocyte recovered to baseline. "Bounce pain" was found in 2 of 27 patients (7.41%).Conclusions The dose of 20 MBq/kg, 30 MBq/kg, 40 MBq/kg or 50 MBq/kg of 188Re-HEDP do not cause significant side effects on cancer patients with bone metastases, though there is a tendency that the haematological toxicity may increase as the dose of 188Re-HEDP increases.
9.Introduction to an individual-based standardization method-propensity score weighting

In this article,we presented the rationale and calculation procedures of a propensity score weighting method,with its application in epidemiological studies.The rationale for propensity score weighting method is similar to those for traditional standardization methods.Propensity score is used to estimate the weight for each individual.As the propensity score serves the function of observed covariates,the propensity score weighting can balance the distribution of the observed covariates between the comparison groups.There are two weighting methods according to the target standard populations:the Inverse probability of treatment weighting(IPTW)and the Standardized mortality ratio weighting(SMRW).Results of the example show that the distribution of the covariates tended to be consistent after weighting,and the IPTW and SMRW methods showed similar effect estimates.Propensity score weighting method can effectively balance the distribution of the confounding factors between the compared groups in non-randomized controlled trials.
10.Effect of Morroniside on Cerebral Infarction Volume in Focal Cerebral Ischemia-reperfusion Rats

Objective To observe the effect of morroniside on cerebral infarction volume in focal cerebral ischemia-reperfusion rat model.Methods The animal model was induced by occlusion of middle cerebral artery with suture embolus, ischemia for 30 minutes, and reperfusion for 7 days. The infarction volume was measured by 2,3,5-triphenyltetrazolium chloride(TTC) staining technique.Results Compared with sham operation group, the cerebral infarction volume ratios increased obviously in model group and in the drug-treated(90 mg/kg,270 mg/kg)groups. Compared with model group, the cerebral infarction volume ratios decreased obviously in the morroniside-treated(90 mg/kg,270 mg/kg)group,while the cerebral infarction volume ratios in vitamin E-treated(35 mg/kg)group didn't change.Conclusion Morroniside may decrease the cerebral infarction volume after cerebral ischemia-reperfusion. It possesses protective effect against cerebral ischemia-reperfusion injury.
