Objective To evaluate the correlation between the level of CD4+CD25+regulatory T cells in periphend blood and disease severity in patients with sepsis.Methods Thirty-six septic patients and 5healthy controls were enrolled.Septic patients were divided into sepsis group(n=10),severe sepsis group (n=15)and septic shock group(n=11).The lymphocyte was seperated from peripheral blood and marked by PE-CD4 and FTTC-CD25 monoclonal antibody,the level of CD4+CD25+ regulatory T cells was detected by flow cytometry,and the clinical data of APACHE Ⅱ scores of septic patients was considered in 24 hours.The correlation between level of CD4+CD25+ regulatory T cells in peripheral blood and APACHE Ⅱ scores in septic patients was analyzed.Results Compared with the healthy controls [(5.48±0.98)%],the level of CD4+ CD25+ regulatory T cells in sepsis group(10.31±2.32)%,severe sepsis group(14.27 43.33)%,septic shock group(15.32±3.98)% had a significant increase(P＜0.05 or＜0.01).The log value of regulatory T cells in each group correlated positively with the APACHE Ⅱ scores(r=0.829,P=0.032;r=0.868,P=0.021;r=0.913,P=0.009),and the total coefficient of correlation was 0.903(P=0.013).Conclusion The level of CD4+CD25+ regulatory T cells in peripheral blood in septic patients has an abnormal increase,and their levels are related with the severity of disease.

AIM:To investigate the safety and effect of femtosecond laser-assisted cataract surgery with Cionni modified capsular tension ring (MCTR) implantation in the management of traumatic lens subluxation.METHODS: Totally 11 patients (11 eyes) with traumatic lens subluxation were divided into three groups according to the severity of lens dislocation, ranging from 90° to 120° (4 eyes), 120° to 180° (5 eyes) and 180° to 270° (2 eyes).The contact LenSx femtosecond laser cataract surgery platform was applied to create the capsulotomy, prepare nuclear fragmentation and make corneal wound creation.Anterior vitrectomy was performed in some patients during the surgery.After capsular retractors insertion and phacoemulsification, the MCTR was inserted to the capsular bag and fixed to the sclera.Finally, the IOL was implanted into the capsular bag.Postoperative visual acuity, intra-and post-operative complications, anterior capsular opening, IOL and MCTR position and intraocular pressure (IOP) were assessed.RESULTS:The duration of follow-up was 2mo.All the operations were completed successfully.Five eyes underwent cataract surgery combined with anterior vitrectomy.Four eyes had been inserted with 2-eyelet MCTR and seven eyes with 1-eyelet MCTR.The best corrected visual acuity (BCVA) after operation was better than 0.5 in 4 eyes, between 0.3 and 0.5 in 3 eyes, between 0.1 and 0.3 in 3 eyes, and less than 0.1 in 1 eye.Compared with preoperative BCVA, the difference was statistically significant (P<0.05).All the IOLs were stably centered and the eyelet of MCTR was fixated steadily between the iris and the anterior capsule.The common intra-and post-operative complications were subconjunctival hemorrhage, incomplete capsulotomy, residual cortex, secondary glaucoma and posterior capsular opacification.CONCLUSION:Femtosecond laser-assisted cataract surgery can improve the success rate of capsulorhexis, and reduce the difficulty of nuclear fragmentation.Femtosecond laser-assisted cataract surgery combined with MCTR implantation is an ideal surgical method for traumatic lens subluxation.

OBJECTIVE: To explore the efficacy of Y-type anatomical plate in patients with distal humeral fracture. METHODS: Thirty three patients suffered from comminuted distal humeral fracture undertook the operation. Among them, 15 were supracondyle fracture, and the other 18 were intercondyle fracture. All patients were treated with middle post elbow incision, reposition, and fixed with Y-type anatomical steel plate. RESULTS: All patients were followed up for 16 months after the operation, and all fractures healed. The average healing time was 3. 5 months. The elbow joint function according to Aitken and Rombeck's elbow function rating system was evaluated, and the results were as follows: the function recovery was excellent in 29 patients, good in 2 and fair in the other 2 patients. The excellence rate was 93. 9%. CONCLUSION Y-type anatomical steel plate is excellent in the treatment of distal humeral fracture.

OBJECTIVETo establish the rat model of acute pulmonary embolism, and study the changes of vascular active substances in pulmonary embolism rats, and investigate the interventive effect of anticoagulant drugs on vascular active substances.

METHODSOne hundred and twenty-eight rats were randomly divided into four groups: control group, model group, low-molecular-weight heparin and warfarin treated group and rivaroxaban-treated group (n = 32 in each group). The method of autologous thrombosis was used to establish the animal model of acute pulmonary embolism. The animals were treated with saline or different anticoagulant drugs. The physiological and biochemical parameters were detected at different time points after embolization. The rats were killed after embolism of 24 h, 3 d, 5 d or 1 week respectively and the pathologic samples of lung tissues were collected to analyze the pulmonary pathological changes in different groups.

RESULTSRats in embolization group after blood clots injection showed shortness of breath, oral cyanosis; quicken heart rates and other symptoms. All embolization groups had pulmonary hypertension, the levels of type B natriuretic peptide (BNP) were increased significantly. The ratio of endothelin-1 (ET-1)/NO and thromboxane (TXB2) and prostacyclin (6-k-PGFla) were abnormal. After treated with effective anticoagulant drugs, the levels of BNP, ET-1, NO, TXB2 and 6-k-PGF1a were tended to the normal levels in the control group. The pulmonary hypertensions were gradually decreased. The efficacy of rivaroxaban on pulmonary embolism was the same as that of the low molecular weight heparin or warfarin.

CONCLUSIONAnticoagulation therapy can effectively improve endothelial function after pulmonary embolism, reduce pulmonary hypertension, and revise the increased BNP levels to normal levels. The efficacy of rivaroxaban is not inferior to that of low molecular weight heparin and warfarin.

OBJECTIVETo study the technical conditions of the extraction and purification of the active composition from Polygonum orientale and Crataegus pinnatifida Bge. in Hongye Xingtong soft capsules with the macroporous resin.

METHODThe orientm, isorientm and hyperoside were used as index to screen the five kinds of resins. And the technical conditions of the enrichment and purification of D101 resin selected out of above were all-round studied.

RESULTThe D101 was fit for adsorbing orientm, isorientm and hyperoside. Under the optimal conditions, the transfer rate of orientm, isorientm and hyperoside was above 91%, and the total solid was cut down by more than 60%.

CONCLUSIONThe D101 is greatly effective for the enrichment and purification of the active composition of P. orientale and C. pinnatifida Bge.

OBJECTIVETo establish a method of quality control on qianliebeixi capsules.

METHODTo clarify semen vaccarine by TLC, to determinate the contents of quertin and kaempferol in prepations, by HPLC. Chromatographic conditions: C18 column with METH-0.2%H3PO4(47:53) as mobile phase; the temperature of column was 40 degrees C, and quercetin was detected at 370 nm with flowing rate 1 ml/min.

RESULTThe spots of semen vaccarine were clear in TLC, no interference in negative contrast. The average recovery rate of quertin was 98.9% and the RSD was 2.2%; the average recovery rate in kaempferol was 100.4% and the RSD was 1.7%.

CONCLUSIONThe resolution of spots were perfect in TLC, the spots were easy to distinguish, The method is simple, rapid as well as reproducible and can control the quality of qianliebeixi capsules effectively.

OBJECTIVETo establish a RP-HPLC method for the determination of gallic acid in Chinese herb Melastoma dodecandrum, and to evaluate the quality of the herb.

METHODThe RP-HPLC analysis was achieved by using a Polaris C18 column and tetrahydrofuran-methanol-0.2% phosphoric acid (0.5:0.5:99) as the mobile phase, with a flow rate of 1.0 mL x min(-1), and detected by UV at 274 nm.

RESULTThe content of gallic acid was from 0.020% to 0.081%, in ten groups of M. dodecandrum collected from different locations.

CONCLUSIONThe method is a simple, convenient and rapid. It can be used for quality evaluation of M. dodecandrum.

OBJECTIVETo establish the fingerprint detecting standard of xinshao Injection by reversed phase high performance liquid chromatography.

METHODThe chromatographic conditions were as follows: an Diamonsil C18 column was used, the mobile phase was composed of 80% acetontrile and 0.1% phosphoric acid with gradient elution, the flow rate 1.0 mL min(-1), column temperature was 40 degrees and the UV absorbance detection was set at 230 nm.

RESULTA standard HPLC fingerprint procedure was developed for xinshao Injection, with 8 common peaks. Under the same selected chromatographic conditions, the HPLC fingerprints of raw drugs and their intermediate, finished products were obtained with good separation and correlation (no less than 0.97) according to the technical requirements of fingerprint on Injection of Chinese traditional medicine.

CONCLUSIONThis method was accurate, repeatable and can be used as the important parameters of the quality control for xinshao injection.

CONCLUSIONSix compounds were isolated from P. oriental for the first time.

OBJECTIVETo study use of nasal lining flap in correction of the wide complete unilateral cleft lip.

METHODS78 patients with wide complete cleft lip were. operated, firstly to be sure. That the lip length on both sides was equal. Then the nasal lining flap was used to enhance the height of the lip on cleft side for keeping symmetry with the non-cleft side. The rest of procedures was the same as "Millard" method.

RESULTS84.6 percent of patients had good appearance at one and half years after surgery.

CONCLUSIONSIt is an effective way to take advantage of the nasal lining flap to correct the wide complete unilateral cleft lip.
