Thunderclap headache is a sudden severe headache. It is an uncommon type of headache. Identifying it in time and accurate diagnosis are very important because this type of headache is often complicating serious cerebral diseases. This article reviews its causes and diagnostic assessments.
2.HOXB6-mRNA and its gene expression in the differentiation process of human cytomegalovirus-infected hematopoietic stem progenitor cells into granulocyte and erythrocyte progenitor cells

BACKGROUND: Is the inhibition of the hematopoietic stem progenitor cell (HSPC) proliferation and differentiation after human cytomegalovirus (HCMV) infection associated with abnormal expression of infected cell proliferated gene?OBJECTIVE: To observe the HOXB6-mRNA expression in the process of proliferation and differentiation of HCMV-infected HSPC into colony-forming unit granulocyte-macrophage (CFU-GM) and colony-forming unit erythroid (CFU-E).DESIGN: A controlled observation.SETTING: Laboratory for Molecular Biology, Affiliated Hospital of Luzhou Medical College, Lanzhou, Gansu Province, China.MATERIALS: All cord blood (CB) specimens were provided by the Obstetrics Department of Affiliated Hospital of Luzhon Medical College. They were collected from the umbilical vein of normal term neonates delivered spontaneously. All neonate mothers were healthy and HBS-Ag-negative. HCMV-IgM antibody revealed by routine ELUSA and HCMV-DNA checked by PCR were undetectable. Written informed consent for the laboratory measurements was obtained from each neonate mother, and the protocol was approved by the hospital's Ethics Committee. HCMV-AD169 strains were obtained from the Institute of Virology, Chinese Academy of Preventive Medicine. All-trans retinoic acid (ATRA, lot No. 20010126) was provided by Chongqing Huapont Pharm. Co., Ltd., China.METHODS: This study was performed at the Laboratory of Molecular Biology (state-level), Affiliated Hospital of Luzhou Medical College of Luzhou Medical College from April 2006 to April 2007. Cord blood mononuclear cells were separated for HSPC culture. According to different interventions, the study consisted of 4 groups. Control group: no HCMV virus solution was added and equal volume of culture medium was added instead. HCMV group: 105 PFU/mL HCMV-AD169 virus solution was added to the culture system. ATRA group: ATRA was added into the cultivation system at the final concentration of 60 μ mol/L. HCMV+ATRA group: ATRA was added into the HCMV group, and its final concentration was also 60 μ mol/L.MAIN OUTCOME MEASURES: In each group, cells were harvested on days 3,7 and 12. HOXB6 mRNA expression levels in CFU-GM and CFU-E were detected by real-time fluorescent-based quantification PCR.RESULTS: In the control group, both CFU-E and CFU-GM expressed HOXB6-mRNA. The HOXB6 mRNA expression was increased as a function of time. The HOXB6-mRNA expressed by CFU-E reached its peak level on day 12, while that expressed by CFU-GM reached its peak level on day 7. Compared to control group, the expression levels of CFU-E and CFU-GM HOXB6-mRNA genes in normal cord blood were significantly lower in the HCMV group (P<0.05)and significantly higher in the ATRA group (P<0.05) at each time point after HCMV infection. Furthermore, the expression levels were significantly higher in the ATRA+HCMV group than in the HCMV group at each time point(P<0.05-0.01).CONCLUSION: HOXB6-mRNA expression is stable and lasting in the proliferation and differentiation of HSPC into CFU-GM and CFU-E. HCMV could down regulate HOXB6 gene expression, and ATRA could up regulate HOXB6 gene expression.
4.Synergistic effect of TRPV1 activation on promoting mild therapeutic hy-pothermia

AIM:To look for a way of produ cing mild therapeutic hypothermia through regulating transient re-ceptor potential cation channel subfamily V member 1 (TRPV1) pathway by dihydrocapsaicin (DHC).METHODS:Mice were subcutaneously injected with DHC at different doses (2 mg/kg, 3 mg/kg and 4 mg/kg) in order to find the best dose for reaching the target temperature (32~34℃).20%DMSO dissolved in normal saline was used as control group .After a single subcutaneous injection of DHC at an optimal dose was given , awaken CD1 mice were continuously infused with DHC at dose of 1 mg? kg-1? h-1 for providing a more rapid and stable temperature drop and duration of therapeutic mild hypothermia.The adult mice (9~10 weeks) and aged mice (24~27 months) were subcutaneously injected with DHC at the same dose, and the changes of the body temperature were monitored .RESULTS:DHC at 2 mg/kg resulted in a de-crease in the core temperature within the target therapeutic range (32~34 °C).After a bolus dose (2 mg/kg) was deliv-ered at 0 min followed by continuous infusion (1 mg? kg-1? h-1 ) beginning at 30 min, a rapid drop of body temperature to 34 ℃was achieved and the body temperature was maintained within the ranges of 32 to 34℃for the duration of the 6 h continuous infusion .DHC-mediated hypothermia did not lose its effectiveness in the adult and aged models .CONCLU-SION:DHC-induced activation of TRPV1 pathway produces mild therapeutic hypothermia .Besides, this method achieves stronger and longer center hypothermia and is suitable for the animals at different ages .

6.Gene chip screen in mice kidney with acute paraquat poisoning and preliminary analysis of the differentially expressed genes

Objective To study the gene expression profile in mice kidney with acute paraquat (PQ) poisoning and identify key genes related to renal injury.Methods A total of 20 mice (C57BL/6) were randomly (random number) divided into four groups, namely control group (group A, n =5) and poisoned groups (groups B, C, D, n =5/group).In group A, mice were administrated with distilled water (0.01 mL/g weight) while in groups B, C, D were administered with equivalent volume of PQ solution (diluted from 20% to 0.05% with distilled water) dissolved in distilled water via a gastric tube.Mice of group A were sacrificed immediately and mice of groups B and C at 6 h and 24 h after administration of PQ.The gene expression profile changes of kidney tissue were measured by cDNA Arraychip technology.Mice of group D were observed for mortality rate 48 h later.Results The body weights of mice decreased significantly after administration of PQ.The mortality in group D at 48 h after PQ poisoning was 100%.Compared with the control group, totally 1 792 genes with differential expression variations were identified in 6 h group and 24 h group.There were 8 key genes selected through bioinformatics analysis and they were arranged in real-time PCR: Nlrc5 , Serpinb9 , Cd40 , Rnf135 , Dhx58 , Spl 10 , Fcgrl , and Arhgef12.And then, Nlrc5 , Serpinb9 and Rnf135 were under Western blot investigation.The results of PCR and Western blot showed no significant difference to those from bioinformatics genetic analysis.Conclusions The investigation based on genome wide chip in researching related genes of PQ kidney has offered a novel idea in studying pathogenesis of acute PQ intoxication.
7.The dignostic value of serum vitamin B12 in predicting rebleeding in patients with liver cirrhosis of esophagogastric varices

Objective:The aim of this study was to investigate the prognostic value of vitamin B12 as the non-invasive biomarker to predict long-term rebleeding rate in cirrhotic patients with esophagogastric varices.Methods:From Dec 1, 2016 to Dec 31, 2017, cirrhotic patients with esophagogastric varices who had been admitted to Zhongshan Hospital affiliated to Fudan University were enrolled. All these patients received endoscopic treatment to prevent variceal rebleeding. The serum vitamin B12 and folic acid levels were measured in all of them. The receiver operating characteristic (ROC) analysis, Kaplan-Meier analysis, univariate and multivariate cox regression analysis were conducted to explore the value of vitamin B12 in predicting 3-year variceal rebleeding in cirrhotic patients with esophagogastric varices after endoscopic treatment.Results:115 patients were included. The ROC curve analysis indicated that the optimal cutoff value of vitamin B12 for 3-year variceal rebleeding was 567.25 pg/ml. According to the cut-off value, the patients were divided into high-level vitamin B12 group ( n=49) and low-level vitamin B12 group ( n=66). Compared with the low vitamin B12 group, the high vitamin B12 group had lower albumin level, less male (63.3% vs 80.3%), and higher 3-year rebleeding rate ( P<0.05). Cox analysis showed that vitamin B12 and platelet were independent prognostic factors for 3-year rebleeding in patients with variceal bleeding. Conclusions:Elevated peripheral blood vitamin B12 predicts a higher risk of long-term rebleeding in patients with liver cirrhosis and esophagogastric varices.
8.Clinical effects of improved urine specimen collection for female infants

Objective To explore the clinical effects of improved urine specimen collection for baby girl.Methods Totals of 82 female infant were recruited and used improved method and traditional method to collect urine,Put the asepsis tampon into the the opening of the collector when collected the urine specimen in improved method group,while the collector was directly used to collected the urine specimen.Then,the average number of collector,total times of collectors polluted by ease nature,average time of specimen collection,and the assay in two groups were observed and compared.Results The average number of collectors,total times of collectors polluted by ease nature,average time of specimen collection of improved method group respectively were(2.43 ±0.18),(24.02 ±8.43)and (2.32 ±0.16) hours,and that of the traditional method group were ( 6.78 ± 0.87 ),(78.01 ± 11.32 ) and (6.69 ± 0.35 ) hours,and the differences were statistically significant ( t =44.84,34.65,103.53,respectively;P < 0.01 ),while there was no difference in the lab tests such as pH,blood and epithelial cell counts and the specific gravity of urine(P > 0.05),Conclusions Improved urine sample collection method for female infant can significantly improve the success rate of urine specimen collection,reduce the average number of collectors and the time and have no influence on the assay,which can be popularized in clinic.
9.MK-801 attenuates NOS expression and NO content in the spinal cord of rats with inflammatory pain

Using NADPH-d histochemistry, the effect of NMDA receptor antagonist MK-801 on the expression of nitric oxide synthase (NOS) in the dorsal horn of the spinal cord was investigated during inflammatory pain and hyperalgesia induced by injection of formalin into the right hind paw. The course of the change in the nitric oxide (NO) content of the lumber intumescence was observed by measuring the ratio of nitrate/nitrite (NO3/NO2) and also the end product of NO. The results showed that the NOS expression and NO contents significantly increased 24 h after formalin injection, which were substantially inhibited when MK-801 was intrathecally injected 15 min prior to formalin injection or 12 h after formalin injection. The results suggest that the increases in the expression of NOS and NO contents in the dorsal horn of the spinal cord are mediated by activation of NMDA receptors during pain and hyperalgesia after formalin injection.
10.Survey on health management' needs among hospitalized patients after discharge in one grade 3 and first-class hospital of Xinjiang

Objective To investigate the need of health management of hospitalized patients after discharge in one class A comprehensive hospital so as to explore the patients expect health management mode.Methods In a three level of first-class comprehensive hospital,totals of 700 patients were investigated with questionnaire when they discharge between March and May 2012.Results 92.9% of respondents were most eager to get basic knowledge of disease.Patients with different education degree,personal income,hospitalization costs payment model,age had significantly different need regarding telephone follow-up frequency (x2 =32.97,37.19,31.35,34.51 ; P < 0.05).And patients with different nation,profession,hospitalization costs payment model,place of residence had significantly different needing of text messages reminding frequency (x2 =30.314,39.669,29.146,13.013 ;P < 0.05).Patients with different nation,profession,education level,hospitalization costs payment model,place of residence had significantly different needs regarding follow-up time quantum via telephone (x2 =42.747,69.404,83.346,63.249,37.009,63.122 ; P < 0.05).Patients with different nation,profession,education level,hospitalization costs payment model,place of residence and age had significantly different needs regarding time quantum of remind via telephone message (x2 =45.099,54.699,57.745,31.76;P < 0.05).Conclusions Discharged patients desire to get the health management that way diversification,the time and frequency concretization,time period rationalization,and hospital should increase the corresponding form to meet the specific needs.Sex and marriage are not the important factors for the remind frequency of telephone follow-up and text messages,time limit and time quantum.
