Objective To evaluate the role of early cleavage embryo in combination with embryo growth rate and morphology scoring in embryo selection in in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI) cycles. Methods Six hundred and ten IVF/ICSI cycles were randomly assigned to group A(269 cycles) and group B(341 cycles). In group A, transferred embryos were chosen according to embryo growth rate and morphology scoring by 72 h(D3) after fertilization, while early cleavage embryo was added to the selecting system in group B. The pregnancy rate and implantation rate were compared between two groups, and the clinic outcomes were compared between transfers with early cleavage embryos and without early cleavage embryos in group B. Results The pregnancy rate and implantation rate in group B were significantly higher than those in group A (P < 0.05). Transfers with early cleavage embryos also achieved much higher pregnancy rate and implantation rate in group B (P < 0.01). Conclusion Compared with embryo growth rate and morphology scoring, early cleavage embryo in combination with embryo growth rate and morphology scoring can improve the clinical outcomes in IVF/ICSI cycles.

Objective To evaluate the performance of energy discriminating dosimeter and compare the impact of the assessment results. Methods CTLD-J4000 energy discriminating dosimeter was irradiated with different energy photons, and the differences covered by the dosimeters were used. Density filtering materials have different filtering effects on photons, and the ratio curve of the readings to the photon energy was fitted to identify the photon energy. Results The coincidence between the non-filter/light filter of the CTLD-J4000 energy discrimination dosimeter and the manufacturer's reference value was better than that of the non-filter/re-filter. The latter had higher deviation values at 48 keV(12.8%)and 83 keV(15.5%), respectively. Using the average scale factor, the maximum energy response deviation was 23.5%. Conclusion Using a discriminating dosimeter can better identify the ray energy, thereby reducing the energy response error caused by using the average scale factor.

Objective To evaluate the protective effect of aminophylline on myocardium in the patients undergoing prothetic valve replacement operation of heart.Mothods Thirty patients undergoing prothetic valve replacement operation of heart were randomized to be treated either with aminophylline(n=15)or without aminophylline treatment(n=15). Aminophylline(5mg/kg)was injected intravenously at 15 minutes after induction of anesthesia.Cardiac troponin I(cTnI), cyclic adenosine monophosphate(cAMP),myeloperoxidase(MPO),ratio of aortic blood neutrophil count to coronary vein sinus blood neutrophil count,hemodynamics,time of aortic cress-clamping and other clinical data were recorded during the operation.Results There were no differences between the two groups in the major perioperative variables.Plasm cTnI concentration in both groups increased after off-clamping than that before CPB,however,it was lower in aminophylline group than that in control group.Concentration of cAMP in both groups after off-clamping was lower than that before CPB, however cAMP concentration in aminophylline group after off-clamping was higher than that in control group.Myocardial MPO activity and neutrophil count ratio after aortic off-clamping in aminophylline group was significantly lower than that in control group.Conclusion These results suggest that aminophylline is helpful to unprotection of myocardial and decreases the sequestration of neutrophil in myocardium.The mechanism of the protection may be related to the cAMP increased in myocardium.

Objective To observe the influence of nandrolone phenylpropionate(NP) on the ultrastructure of aorta in rats with or without movement training,and investigate the side effects of NP on the cardiovascular system. Methods Forty male SD rats were randomly divided into sedentary control group,sedentary+medicine group,exercise control group and exercise+medicine group.For the groups with medical treatment,NP of 10 mg/kg one time every three days was injected into the rats via gluteus for eight weeks.For the exercise groups,rats were trained to run on treadmill five days per week for eight weeks.The aortae were sampled and specimens were obtained for transmission electron microscopy. Results The ultrastructure of aorta was normal in sedentary control group.For sedentary+medicine group,mitochondrial swelling,vacuolated cytoplasm and lysis of endothelial cells were observed,disruption of intercellular conjunctions,widening of subendothelial spaces and furcation and breakage of internal elastic lamina were found,and smooth muscle cells changed into synthesis type.For exercise control group,no obvious morphologic change was observed,except that part of the internal elastic lamina disrupted.In exercise+medicine group,breakage and lysis of endothelial cells were observed,widening of subendothelial spaces and lysis of internal elastic lamina were found,and autophagosome and myelinoid body were seen in smooth muscle cells. Conclusion NP may lead to the impairment of endothelial cells and the change of smooth muscle cells into synthesis type.Exercise with NP administration may result in more severe impairment in vessel wall.

OBJECTIVETo examine the expression of calreticulin (CRT) and the changes of intracellular free calcium and neuronal apoptosis in the cerebral cortex of neonatal rats with hypoxic-ischemic brain damage (HIBD), and to investigate the intervention effects of Shenfu injection.

METHODSSeven-day-old rats were randomly assigned to three groups: control, hypoxic-ischemia (HI) and Shenfu-treated. Each group (n=50) was subdivided into 5 groups sacrificed at 3, 6, 12, 24 and 72 hours. Rat models of HIBD were prepared according to the Rice's method. Rats in the control group only underwent the separation of right common carotidartery. Shenfu injection was administered by intraperitoneal injections right after HI insults and then once daily at a dosage of 10 mL/kg for 3 days in the Shenfu-treated group. The expression of CRT in the cerebral cortex was detected by RT-PCR and Western blot. The free calcium concentrations were determined under a fluorescent microscope. The apoptosis rate was measured by the flow cytometry.

RESULTSCompared with the control group, the expression levels of CRT in the HI and the Shenfu-treated groups were obviously up-regulated (P<0.05), and the expression levels of CRT in the Shenfu-treated group were notably higher than those in the HI group (P<0.05) at all time points. The concentrations of intracellular free calcium and the apoptosis rate of neurons in the cerebral cortex in the Shenfu-treated group were significantly reduced compared with those in the HI group (P<0.05), but increased significantly compared with those in the control group at all time points (P<0.05).

CONCLUSIONSShenfu injection may have neuroprotective effects against HIBD by up-regulation of CRT expression and relief of calcium overload.

ObjectiveTo study the early diagnosis and prevention of fungal infection in severe acute pancreatitis(SAP). Method 1.SAP patients from July 1998 to June 2002 were prospectively randomized into 3 groups: garlicin prevention group, fluconazole (low dosage) prevention group and control group, the incidence of fungal infection in SAP was compared between the groups. Objective To detect the feasibility of magnetically labeled swine bone marrow mesenehymal stem cells(MSCs)with SHU 555A combined with poly-L-arginine(PLL),under MR imaging in vitro and in vivo.Methods Swine mesenehymal stem cells were isolated and culture-expanded 3 passages in vitro,then magnetically labeled by incubation with SHU 555A(25?g Fe/ml,Resovist,Schering)for 24 hours with 750 ng/mL poly-L-lysine(PLL;average MW_275 kDa)added 1 hour before incubation.Cellular iron incorporation and detention at 0 d,4 d,8 d,12 d,16 d,20 d after labeling was qualitatively assessed using Prussian blue and quantified at atomic absorption spectrometry.Cell viability was assessed by trypan-blue exclusion test.Cell suspensions underwent MR imaging with T_1-and T_2-weighted spin-echo and fast field-echo sequences on a clinical 1.5 T MR system.At last,1?10~6 SHU 555A labeled and unlabeled MSCs were transextracardially implanted into the infracted and normal myocardium approximately 2 week following the ligation of left anterior descending coronary artery in 1 swine respectively,and finally performed 1.5-T MRI within 1 week after infarction.Results①Intracytoplasmic particles stained with Prussian blue stain were detected for all cells with mean cellular iron content of(13.13?2.30)pg per cell.With division of stem cells, the stained particles decreased gradually with iron content(0.68?0.20)pg per cell.at 16 days after labeling, approximately to the prelabeled baseline values.(0.21?0.06)pg per cell(P＞0.05).The viability of the labeled cells at various time points were not significantly different with that of nonlabeled cells(P＞0.05).②MR images showed signal intensity changed most obviouly in T2*WI in vitro.The percentage change of signal intensity increased with increasing cell numbers,and decreased with the time.As few as 5?10~4-1?10~5 cells could be detected by using this approach.③Two injected sites containing MR-MSCs were detected in vivo,presentingas low signal intensity areas with the T_2*WI scanning sequence.Conclusion Swine bone marrow MSCs can be labeled with SHU555A-PLL and depicted with a standard 1.5-T MR imager in vitro and in vivo.(J lntervent Radiol,2007,16:115-121)

Objective To investigate late stent malapposition or vessel remodeling post chitosan/heparin layer-by-layer self assembly coating stent (LBL) implantation in porcine.Methods A total of 32 stents [ bare metal stent ( BMS,n =9),sirolimus-eluting stent ( SES,n =11 ) and LBL ( n =12 ) ] were implanted into coronary arteries of 16 porcine.Intravascular ultrasound (IVUS) was performed immediately after stenting and at 1 month after stenting to measure vessel area (VA),stent area (SA) and lumen area (LA).Neointima area (NA) was measured at 1 month post stenting by IVUS to detect signs of stent malapposition and to determine remodeling index (RI). Histopathology was performed at 1 month post stenting to observe vessel wall structure and stent malapposition status.Results No sign of stent malapposition was detected,VA and SA/LA were similar among groups immediately after stent implantation.At 1 month follow-up,none of three groups showed stent malapposition.VA,SA,NA and LA were ( 7.30 ±0.77),(6.83 ±0.76),(1.40 ±0.96) and (5.43 ±0.88)mm2 in LBL group,(7.13 ±0.69),(6.63 ±0.71 ),(0.28 ±0.35) and (6.34 ±0.89)mm2 in SES group,(7.48 ±0.70),(7.00 ±0.52),(2.69 ±1.58) and (4.31 ± 1.28)mm2 in BMS group.VA and SA were similar among groups ( all P ＞0.05 ).LA in LBL group was smaller than SES group ( P ＜ 0.01 ) and significantly larger than in BMS group ( P ＜0.05 ).NA in LBL group was larger than SES group (P ＜0.01 ) and significantly smaller than in BMS group (P ＜ 0.05 ).RI in LBL,SES and BMS groups was 0.95 ± 0.07,1.02 ± 0.04 and 0.98 ± 0.04 ( P ＞0.05).Conclusions There is no late stent malapposition or abnormal remodeling post LBL,SES and BMS implantation up to 1 month in this porcine model.LA in LBL group is smaller than SES group and larger than BMS group at 1 month after implantation in this porcine model.

Objective To elucidate the experience in hemioplasty in patients undergoing peritoneal dialysis. Metbods From January 2002 to July 2008, 45 patients having 48 hemias were studied. Inguinal (50.0%) and umbilical (39.6%) were the most common sites. Multiple hemias were detected in 3 patients. All patients were operated upon with tension-free hemioplasty technique using a polypropylene mesh. Automated peritoneal dialysis was reinstituted in 97.8% of the cases in 24 h postoperatively. Results Forty-eight hemias were detected in 45 patients. Mean operation time and post-operative hospital-stay were (63±38) minutes and 4 (1-26) days. Complications occurred in 7(14.6%) hemioplasties (2 wound infections, 2 wound bleeding, 2 seroma and 1 peritonitis). The average follow-up was (29±21) months. None of the patients had leakage or early hemia recurrence due to early resumption of automated peritoneal dialysis (APD); there were no long-term complications (leakage or recurrence). Conclusions Problems accompanying hernia in continuous ambulatory peritoneal dialysis patients can be safely solved by using a careful technique of tension-free hemioplasty. Postoperative dialysis with APD can be applied after operation.
