1.Study on ELISAs with rTpN17, rTpN47 and its fusion protein as antigens in the detection of serum samples from syphilis patients.

OBJECTIVETo construct the prokaryotic expression systems of tpnl7 and tpn47 genes and tpn17-tpn47 fusion of Treponema pallidum, and to establish ELISAs based on rTpN17, rTpN47 and rTpN17-TpN47 as antigens to evaluate the sensitivity and specificity of the ELISAs for detection of serological diagnosed syphilis.
METHODStpn17 and tpn47 genes were amplified and cloned by routine molecular biological methods. PCR with linking primers was used to construct artificial fusion gene tpn17-tpn47. The prokaryotic expression systems of the genes were then constructed. SDS-PAGE was used to measure the expression of the target recombinant proteins rTpN17, rTpN47 and rTpN17-TpN47. Ni-NTA affinity chromatography was applied to extract the three recombinant proteins, while Western blot was performed to determine their immunity. Using rTpN17, rTpN47 and rTpN17-TpN47 as the coated antigens, ELISAs (rTpN17-ELISA, rTpN47-ELISA and rTpN17-TpN47- ELISA) were established to detect serum samples from 200 healthy individuals, 25 RA patients, 17 SLE patients and 419 syphilis patients. Results of the ELISAs were compared to those with TRUST and TPHA.
RESULTSThe sequence similarities of the cloned tpnl7 and tpn47 genes and the constructed tpn17-tpn47 fusion gene were 100%, compared with the corresponding sequences in GenBank. The expression outputs of rTpN17, rTpN47 and rTpN17-TpN47 were 37.2%, 23.3% and 29.8% of the total bacterial proteins, respectively. Each of the three purified recombinant proteins showed a single fragment in gel after electrophoresis, and could take place remarkable conjugation reactions to the positive sera from syphilis patients. The detection results of rTpN17-ELISA, rTpN47-ELISA and rTpN17-TpN47-ELISA were negative for the serum samples from healthy individuals, RA and SLE patients, while presented 84.4%, 82.3% and 98.1% positive detection rates for the serum samples from syphilis patients. The positive detection rates of rTpN17-ELISA and rTpN47-ELISA were lower than that of TPHA (P<0.01), while the positive detection rate of rTpN17-TpN47-ELISA was similar to that of TPHA (P>0.05). All the positive detection rates from ELISA tests were higher than that of TRUST (71.4%).
CONCLUSIONrTpN17-ELISA, rTpN47-ELISA and especially rTpN17-TpN47-ELISA established in this study were of great hope as it was rapid, simple, convenient, safe, with high sensitivity and specificity for serological screening and detection of syphilis.

2.Treatment of complex acetabular fractures with combined ilio-inguinal and Kocher-Langenbeck approach

ObjectiveTo investigate the clinical effect and treatment experience of complicated acetabular fractures treated with open reduction with combined ilio-inguinal and Kocher-Langenbeck (K-L) approaches.MethodsSixty-six patients with complicated acetabular fractures were treated surgically with combined ilio-inguinal and K-L approaches from February 2004 to December 2009.Among them,45 cases were followed up for 8-45 months,with the average 26 months.ResultsOperation time was 1.4-5.7 h,with the average of 2.8 h.Intraoperative blood loss was 530-2300 ml,with the average of 1250 ml.According to Matta's score system,the excellent and good rate was 86.7%.According to modified Merle d' Aubigne and Postel score system,the excellent and good rate of function of hip was 86.7%.According to Brooker heterotopic ossification standard assessment system:Ⅰ degrees in 6,Ⅱ degrees in 3.According to Letournel and Judet clinical classification system,postoperative traumatic arthritis were 6 patients.Among them,3 patients with Ⅲ- Ⅴ period with symptoms of serious hip pain and joint activities obstacles were treated with total hip replacement secondly.Six cases of sciatic nerve injury and 9 cases of lateral femoral cutaneous nerve damage were recovered gradually after six months to a year.All incisions healed primarily.All got bony union,the healing time was 8 to 16 weeks,with the average of 10.4 weeks.None was complicated with femoral head necrosis.ConclusionFor the patients with complicated acetabular fractures,open reduction with combined ilio-inguinal and K-L approaches can get clinical advantages of revealed fully,fracture restoration convenient,fixed tightly,fewer complications and satisfactory result.
3.Survey and analysis of the pain status in the patients of emergent hand injury

Objective To learn the pain status in the patients of emergent hand injury and analyze the relative factors which influenced the pain. Methods A total of 210 patients of emergent hand injury were investigated with short McGill pain questionnaire and anxious status scale. Results The VAS at the injury happened and at the visit in 210 emergent hand injury patients were (4.87±2.42 ) and (4.70±2.31 ). During the visit, the PRI was ( 3.34±2.35 ), feeling PRI was ( 3.17±2.26 ), emotional PRI was (0. 17±0.67 ),PPI was (2.80±0.26) ,The variance analysis found the VAS of patients had different degree injury in the past had different VAS of this visit ( F=2. 29, P < 0. 05 ). Relative analysis found the severity of this hand injury ( r = 0. 262, P < 0. 01 ) and the S-AI of this visit ( r = 0.390, P < 0.01 ) had positive correlation with the VAS of this visit. The education ( r = - 0. 172, P < 0. 05 ) had negative correlation with the VAS. Conclusions The pain in the patients of emergent hand injury was medium, releasing the anxiety of the patients at the visit could reduce the pain at that time.

OBJECTIVETo study the chemical constituents of Bauhinia championii.
METHODCompounds were isolated from the ethanolic extract of B. championii by silica gel column Chromatography, and their structures were elucidated by spectral analyses.
RESULTFive compounds were isolated and elucidated as 2,4,6-trimethoxyphenol 1-O-beta-D-(6'-O-galloyl)-glucopyranoside (1), (+/-)-lyoniresinol (2), daucosterol (3), beta-sitosterol (4) and gallic acid (5).
CONCLUSIONCompounds 1-4 were isolated from B. championii for the first time.

5.Effect of Contralateral Acupuncture on Motor Function in Acute Stroke Patients

Objective To investigate the clinical efficacy of contralateral acupuncture in treating motor dysfunction in patients with acute ischemic stroke.Methods Sixty patients with acute ischemic stroke were randomized to treatment and control groups, 30 cases each. Both group received electroacupuncture in addition to conventional rehabilitation training. Besides, the treatment group received electroacupuncture at points on the healthy side in the morning and on the affected side in the afternoon and the control group, electroacupuncture at points on the affected side in both the morning and afternoon. Motor function was assessed using the Fugl-Meyer Assessment (FMA), the modified Barthel Index (MBI), integrated electromyography (iEMG) and median frequency (MF) in the two groups of patients before and after treatment.Results There were statistically significant pre-/post-treatment differences in the FMA score, the MBI score, iEMG and MF in the two groups (P<0.01). There were statistically significant post-treatment differences in the FMA score, the MBI score and iEMG between the treatment and control groups (P<0.01,P<0.05).ConclusionContralateral and healthy side acupuncture can markedly improve motor function in acute stroke patients.
7.Combination of improved sex therapy and sildenafil for erectile dysfunction in Uigur men: retrospective analysis of 2505 cases.

OBJECTIVEThe sex therapy is not yet popularized at present. This study aimed to evaluate the effect of the combination of the improved sex therapy and oral sildenafil on erectile dysfunction (ED).
METHODSA total of 3130 Uigur cases of ED received in Xinjiang Bogda Hospital were divided into a control group (n=625) and a trial group (n=2505), the former treated with oral sildenafil alone, and the latter by the combination of the improved genital therapy and sildenafil, both for 3 months and followed up at 6 and 12 months after the treatment. The therapeutic effects were evaluated and compared using IIEF-5.
RESULTSThe IIEF-5 scores of the control group were 12.80 +/- 3.76 and 18.10 +/- 2.61 before and after the treatment, and 17.35 +/- 2.73 and 16.64 +/- 2.63 at 6 and 12 months, respectively, while those of the trial group were 12.73 +/- 3.52 and 19.06 +/- 4.07 before and af- ter the treatment, and 19.86 +/- 2.42 and 20.47 +/- 2.38 at 6 and 12 months, respectively, with statistically significant differences either between pre- and post-treatment (P < 0.05) or between the control and trial groups at 6 and 12 months (P < 0.05).
CONCLUSIONThe combination of the improved sex therapy and oral sildenafil is superior to sildenafil alone in the treatment of ED, and its efficacy is relatively stable at 12 months.

8.Atherosclerotic aortic ulcers monitoring by electron beam CT or multi-slice CT.

OBJECTIVETo evaluate the evolution of medically treated atherosclerotic aortic ulcers by computed tomography (CT).
METHODSThirty-five patients (31 men and 4 women, aged from 40 to 79 years, mean 56.2 +/- 10.8 years) with known aortic ulcers were monitored by CT (follow up time 7 - 730 days, mean 135 days), 80 - 100 ml contrast media (Ultravist 300 or 320, or Omnipaque 300 or 320 mg/ml) was injected with a rate of 3.5 - 4.5 ml/s. The scan delayed time was 18 - 30 s. Ulcers dimensions were measured according to maximum depth, maximum length and maximum width.
RESULTSThirty-one patients with intramural hematomas and 1 patient with atherosclerotic aortic arch aneurysm without intramural hematoma were medically treated and another 3 patients were surgically treated. Intramural hematoma regression was monitored in 31 medically treated patients with intramural hematomas. CT was repeated at 2 weeks, 3 and 6 months. Intramural hematoma resolved gradually during follow up [thickness: (7.69 +/- 4.24) mm at 3 months, (3.06 +/- 1.67) mm at 6 months, P < 0.05 vs. 1st CT: (11.96 +/- 4.16) mm while ulcer maximum depth (11.17 +/- 6.03) mm at 3 months, (11.35 +/- 5.59) mm at 6 months, P < 0.05 vs. 1st CT: (7.36 +/- 6.61) mm, maximum width (14.40 +/- 6.35) mm at 3 months, (18.55 +/- 10.94) mm at 6 months, P < 0.05 vs. 1st CT: (7.15 +/- 6.39) mm, maximum length (17.12 +/- 7.15) mm at 3 months, (18.13 +/- 10.89) mm at 6 months, P < 0.05 vs. 1st CT: (11.64 +/- 10.06) mm increased progressively during follow-up].
CONCLUSIONCT was a useful tool for deflecting atherosclerotic aortic ulcers and monitoring therapeutic effects.

9.Prognostic factors of stage IB and IIA carcinoma of the cervix treated by surgery.

OBJECTIVETo evaluate prognostic factors in patients with stage IB-IIA of cervical carcinoma treated by surgery.
METHODSBetween December 1992 and December 2001, 111 patients with stage IB-IIA cervical cancer surgically treated were analyzed. Median age 40 years. According to 1994 FIGO Staging System: IB 80 (IB1 40, IB2 40) and IIA 31. There were 93 cases of squamous cell carcinoma (83.5%), 17 cases of adenocarcinoma (15.3%) and one case of small cell carcinoma. All patients were treated by radical hysterectomy and pelvic lymphadenectomy, 74 patients had preoperative adjuvant radiotherapy, 24 patients had postoperative adjuvant treatment. Kaplan-Meier method was used to evaluate the survival, the related prognostic factors were assessed by Cox regression and chi(2) test.
RESULTSThe overall 5-year survival rate was 85.9%, being 89.1%, 90.7% and 78.4% for stage IB1, IB2 and IIA, respectively. Univariate analysis showed that tumor size (hazards ratio [HR] = 1.479, P = 0.152), tumor type (HR = 1.440, P = 0.264), clinical stage (HR = 1.380, P = 0.354), adjuvant treatment (HR = 1.210, P = 0.450), lymph node metastasis (HR = 1.432, P = 0.540), endocervical involvement (HR = 2.244, P = 0.036), depth of myometrial invasion (HR = 3.295, P = 0.06) and multiple sexual partners during pregnancy (HR = 10.172, P = 0.000) were of prognostic significance. The latter two were the most important factors indicative of poor prognosis.
CONCLUSIONThe depth of myometrial invasion and multi-partners combined with pregnancy are closely related to the prognosis while the pre- and/or postoperative adjuvant therapy should be considered for stage IB-IIA cervical cancer with deep myometrial invasion and in pregnant patients with multiple sexual partners.

10.Correlation of cyclooxygenase 2 expression with microlymphatic density and its clinical significance.

OBJECTIVETo study the expression of cyclooxygenase-2 (COX-2) in esophageal carcinoma and its correlation with microlymphatic density (MLD), and to investigate the clinicopathological and prognostic significance of COX-2 and MLD in patients with esophageal cancer.
METHODSCOX-2 expression and MLD were detected by immunohistochemistry in 100 cases of esophageal squamous cell carcinoma. Follow up was available in 76 patients. Multivariable Cox regression was used to analyze the association between the laboratory indices and overall survival of patients with esophageal carcinoma.
RESULTSCOX-2 expression was present in 73% of patients. MLD in patients with high COX-2 expression (99.71±39.62) was significantly higher than that in those with low or no COX-2 expression (80.22±30.36) (P<0.05). No correlations were observed between the over expression of COX-2 and clinicopathologic parameters including tumor size and lymphatic metastasis (P<0.05). However, MLD was associated with lymphatic metastasis and the depth of invasion (P<0.05, P<0.01). In the 76 patients with followed up, the median survival was 25.5 months. Cox regression showed that the COX-2 expression, histological grade of the tumor and MLD were risk factors of overall survival of esophageal carcinoma.
CONCLUSIONSCOX-2 may contribute to the lymphangiogenesis in the tumor. COX-2 may be a new target point for the treatment of esophageal carcinoma. COX-2 expression and microlymphatic vessel density are of significant prognostic value for esophageal carcinoma.
