1.Emergent treatment of severe traumatic intracranial hematoma with minimally invasive technique

Objective To evaluate the value of minimally invasive technique in treatment of severe traumatic intracranial hematoma at emergency department.Method From January 1995 to December 2005,126 patients were treated by using the minimally invasive technique in Affiliated Chaoyang hospital once patients were diagnosedas severe traumatic cerebral hernia resulted from intracranial hematoma and also those whose clinical data were retrospectively analyzed.In the emergency department,the per-operative plan was done according to the CT imaging,including the sute of burr hole on the skull,the direction and depth of carmulation,and the drainage cannula was put into the hematoma cavity for external drainage under local anesthesia.Results The successful rate of puncture was 100%.The patients's dukated oyouk contracted immediately after drainage.There are 79/ 95 patients(83.1%)had single pupil dilated and 17/31(54.8%)patients had bilateral pupils dilated.The immediately clinical effective was 76.2%.According to Glasgow outcome scale,43 of 126 patients had good recovery,26 had moderate deficits and 18 had severe sequelae,16 patients were in vegetative state,18 died. Conclusions Clinical prospective study proves that minimally invasive technique can ameliorate the cerebral hernia,prolong the operative therapie window time.
2.Absorption of extractive Polygonum orientale in rat everted gut sacs.

Using in vitro everted gut to investigate the intestinal absorption of the extracts from Polygonum orientale at different concentration. UPLC-MS/MS was used to detect the content of protocatechuic acid, isoorientin, orientin, vitexin, cynaroside, quercitrin, kaempferol-rhamnoside in different intestinal segments, then compared the results with the absorption of chemical components of extractive P. orientale in each intestinal segments, and calculated the absorption parameter. We took the statistic analysis with SPSS statistic software. The influence significance of each factors were analyzed to describe the character of absorption. The absorption of each component is linearity in different intestinal segments and different dose, and the square of coeficient correlation exceed 0.95, which consistent with zero order rate process. The K(a) increase along with the raised dosage of the extractive P. orientale (R2 > 0.95), indicated it is the passive absorption; different intestinal segments have different absorption. And the absorption trend in intestinal is duodenum, jejunum, ileum are greater than the colon. As ingredients are selectively absorbed in intestinal sac, the everted intestinal sac method is selected to assess the intestinal absorption charcteristics of ingredients of extractive P. orientale.

3.Researching on fingerprint of Inulacappa by HPLC.

OBJECTIVEThis study is to establish the fingerprint and find out the common chromatographic peaks of Inula cappa by HPLC.
METHODThe HPLC analysis was performed on an Agilent Eclipse Plus C18 column (2.1 mm x 150 mm, 1.8 μm) with 0.1% fomic acid aqueous solution-0.1% fomic acid acetonitrile solution as mobile phase at a flow rate of 0.3 · mL(-1) · min(-1); The detective wavelength is 325 nm; The column temperature is 45 °C.
RESULTThe results indicated that 5 of 17 common peaks were identified . The similarity about 10 groups of Inulacappais is over 0.95.
CONCLUSIONThis method is able to be a scientific basis of quality assessment according to its convenient and reliable.

4.Mediating effect of coping efficacy on relationship between psychological stress and adjustment in college freshmen

Objective:To explore the mediating effect of coping efficacy on the relationship between psychological stress and adjustment.Methods:This study was a longitudinal study.Totally 267 college freshmen (137 males and 130 females,aged 19 to 21) were selected to fill in the China College Student Psychological Stress Scale (CCSPSS),China College Student Adjustment Scale (CCSAS) and Coping Efficacy Questionnaire (CEQ),each measurement interval two months,three times totally.Results:The CCSPSS scores were negatively correlated with scores of CCSAS and CEQ cognitive dimension (r =-0.29--0.08,Ps < 0.05),and positively correlated with measurement period (represented by a continuous and dummy variable,r =0.08,P < 0.05).The CCSAS scores were positively correlated with scores of CEQ cognitive dimension (r =0.25,P < 0.01),and negatively correlated with measurement period (r =-0.13,P < 0.01).The CEQ cognitive dimension scores was negatively correlated with measurement period (r =-0.14,P < 0.01).The mediating effect analysis showed that coping efficacy played a partly mediating role between adjustment and psychological stress,measurement time,and the mediating effect were accounted for the total effect 45.7%,33.6%.Conclusion:It suggests that psychological stress and measurement time may not only directly affect the adjustment,but also indirectly affect adjustment through mediating effect of coping efficacy.
5.Clinical prospective study on maternal-fetal transmission of human bocavirus.

OBJECTIVETo investigate maternal-fetal transmission at human bocavirus (HBoV).
METHODSIgG antibody to HBoV in serum samples of 316 mothers were determined with ELISA and HBoV DNA was determined with real time PCR in the sera of the mothers and their infants.
RESULTSHBoV-IgG was positive in 40.20 percent (127/316) of the mothers, while it was positive in 29.43 percent (93/316) of the cord blood specimens of the infants. The difference between the two groups was significant (X2=8.12, P less than 0.005); 93 samples of both the mothers and the infants were positive for HBoV-IgG.
CONCLUSIONHBoV-IgG can cross the placenta to the fetuses through placenta. Further study is needed to answer the question whether vertical maternal-fetal transmission occurs.

6.Effect of enalapril folic acid tablet on peripheral blood inflammatory factors in type H hypertensive of rats

Objective To study the effect of enalapril folic acid on peripheral blood inflammatory factors in type H hypertensive of rats.Methods Sixty Wistar Spontaneous hypertension male rats were randomly divided into control group,model group,enalapril group and enalapril folic acid group,15 rats in each group.Type H hypertensive rat model were established by feeding with 3% methionine in model group,enalapril group and enalapril folic acid group.And enalapril folic acid group were given 10 mL · kg-1 enalapril maleate folic acid solution,enalapril group were given 10 mL · kg-1 enalapril solution,and the model group were given 10 mL · kg-1 double distilled water,and 4 groups were intervened for 8 weeks everyday.The serum levels of homocysteine (Hcy),serum tumor necrosis factor alpha (TNF-α),hypersensitivity C reactive protein (hs-CRP)and soluble OX40 ligand(OX40L) were measured by enzyme-linked immunosorbent assay.The nitric oxide(NO)level were measured by End-point assay.The structure of carotid artery was observed under microscope.Results Compared with Hcy(64.22 ±6.82) μ mol · L-1 and NO(41.45 ±5.25) μmol · L-1 in the enalapril group,levels of Hcy(19.75 ±2.14) μmol · L-1 were lower,and NO(57.68 ±6.43) μmol · L-1were higher in the enalapril folic acid group with significantly (P < 0.05).Compared with TNF-α (72.72 ± 6.89) pg · mL-1,hs-CRP (1.15 ±0.12) pg · mL-1,OX40L(5.24 ±0.47) ng · mL-1in the enalapril group,levels of TNF-α(58.18 ±5.76)pg · mL-1,hs-CRP(0.86-±0.11) pg · mL-1,OX40L(3.03 ±0.39) ng · mL-1 were lower in the enalapril folic acid group with significantly(P <0.05).Compared with the carotid artery IMT (61.14±7.73) μm,and IMCSA levels(0.12 ± 0.02) mm2 in the enalapril group,the carotid artery IMT (52.74±6.29)μm,and IMCSA (0.10 ±0.01) mm2 in the enalapril folic acid group were lower with significantly(P < 0.05).Compared with the levels of carotid artery LD(951.38 ± 6.08) μm in the enalapril group,the levels of carotid LD (969.79 ± 6.17) μm in the enalapril folic acid group was higher with significantly(P <0.05).Conclusion enalapril folic acid can effectively reduce levels of peripheral blood Hey,TNF-oα,hs-CRP and OX40L in type H hypertensive rats,improve levels of NO,reduce inflammation,and improve the structure of carotid artery.
7.Clinical observations of the effects on the lower limb function after lumbar or sacral nerve root transferring to reconstruct urination function.

OBJECTIVETo observe the effects on the lower limbs function after lumbar or sacral nerve root transferring to reconstruct urination function.
METHODSNine patients with bladder dysfunction and normal lower limb function after spinal cord injury were treated with anastomosis the S2 or S3 nerve root with the normal lumbar or sacral nerve root to reconstruct a new bladder artificial reflex arc. Then the alterations on the sensation and motor function of the lower limb after the surgery were observed.
RESULTSMyodynamia of the legs decreased slightly, and the decreasing about half grade of the myodynamia in the plantar flexion of the ankles were detected in 4 of 9 patients with S1 transferring. And the myodynamia recovered 3 months postoperatively. No obvious decreasing of the myodynamia appeared in the other cases.
CONCLUSIONNo obvious effects on the motor function can be found after the single lumbar or sacral nerve root transferring to reconstruct urination function.

8.Different levels of nitric oxide in seminal plasma of fertile and abnormospermic men.

OBJECTIVESTo understand the relation between nitric oxide (NO) level in seminal plasma and male fertility.
METHODSThe levels of NO in seminal plasma of 174 fertile males and 217 abnormospermia patients were measured by using the reductase of nitric acid, Greiss reagent and spectrophotometry.
RESULTS1. NO was found in all 174 samples (20-49 years) of fertile males which was (27.78 +/- 5.81) mumol/L. The NO level in seminal plasma in fertile males was became higher after age 40, and there was no significant difference between 20-29-year-old [(26.25 +/- 5.52) mumol/L] and 30-39-year-old[(28.11 +/- 5.87) mumol/L]. But the group of 40-49-year-old[(30.17 +/- 6.14) mumol/L] had a higher level of NO in seminal plasma than 20-29-year-old (P < 0.05). 2. The seminal plasma samples from nine types of abnormospermia were measured, which all had a higher level of NO than fertile males. In abnormospermia, the level of NO in seminal plasma of the patients with single abnormality increased slightly, two abnormality obviously increased, and the highest level of NO in seminal plasma appeared in three abnormality.
CONCLUSIONSThis results confirmed that proper level of NO in seminal plasma may regulate the spermatogenesis.

9.Biomechanical characteristics analysis on discs with coflex fixation on the different segments of lower lumbar spine.

OBJECTIVETo investigate the biomechanical differences between the surgery and adjacent segments of intervertebral discs in the lower lumbar spine, which were implanted with Coflex into the segments of L4, and L5S1, respectively.
METHODSThree finite-element models (the model of the intact lower lumbar sacrum,the L4.5 and L5S1 segments implanted by Coflex) were developed, respectively. According to the spinal three-column loading theory, three models were forced by the physiological loads of upright standing, flexion and extension. The stress of the different areas of the disc annulus, the changes of intervertebral dorsal height and the degree of nucleus pulposus pressure were compared and analyzed.
RESULTSCoflex implanted into the L4.5 and L5S1 segments in compression and extension could both decrease the stress of the posterior area of intervertebral disc in the surgery segment, resist the changes of the intervertebral disc dorsal height and reduce the perssure of nucleus pulposus. Furthermore, the stress of the L5S1 segment decreased when Coflex fixed the L4.5 segment in extension. However, when Coflex fixed the L5S1 segment, the stress of L4.5 segment had no significant changes.
CONCLUSIONCoflex fixing the L4,5 and L5S1 segments can effectively decrease the stress of the surgery segmental discs, respectively. Furthermore, Coflex fixing L4,5 segment may play a biomechanical role in reducing the stress of L5S1 segment.

10.Changes of peripheral plasmacytoid dendritic cells and lymphocyte subsets in chronic hepatitis B patients treated with consensus interferon alpha.

OBJECTIVETo study the changes of peripheral plasmacytoid dendritic cells and lymphocyte subsets in patients with chronic hepatitis B (CHB) during consensus interferon (CIFN) treatment.
METHODSTwenty-three patients with CHB were treated with CIFN for 24 weeks and followed up for another 24 weeks. Peripheral plasmacytoid dendritic cells and lymphocyte subsets were measured throughout the treatment and follow-up periods.
RESULTSAfter CIFN treatment, 43.5% of the patients had virological and biochemical responses. The percentage and absolute number of peripheral plasmacytoid dendritic cells decreased significantly (P less than 0.05), the number of CD3+ cells, CD4+ T cells, B cells and the ratio of CD4+/CD8+ cells decreased also (P less than 0.05), but the number of CD8+ T cells and NK cells increased (P less than 0.05).
CONCLUSIONSConsistent virological and biochemical responses can be seen in some patients with CHB virus infection after CIFN treatment, and the percentage and number of their peripheral plasmacytoid dendritic cells greatly decreased, but the number of CD8+ T cells and NK cells increased.
