1.Early intervention of neonates with critical congenital heart diseases

Objective Reviewing the surgical outcome of neonates with critical congenital heart diseases(CHD) in the past five years,to summarize the experiences of the treatment for neonates with critical CHD .Methods From Jan.2009 to Dec.2013,521 neonates with critical CHD were admited,all of them were received surgical treatment.Age ranged from 1day to 28 days, mean(21 ±6) days;weights ranged from 2.1 kg to 3.6 kg, mean(2.8 ±0.5)kg.All the children were received op-eration,and cared in cardiac intensive care unit(CCU) postoperatively.Results 22 cases died after surgery,the hospital mor-tality was 4.2%.68 children were left sternal open after the primary repair;39 children inhaled nitric oxide(NO) for the post-operative pulmonary hypertension;22 children needed peritoneal dialysis for acute renal failure;3 children used extracorporeal membrane oxygenation(ECMO) as circulatory support.Follow-up found 18 cases(12.1%) of TGA occurred supravalvular pul-monary stenosis after the arterial switch operation;12 cases(11.0%) of CoA/VSD&IAA/VSD had recoarctation after the aortic arch reconstruction;7 cases(5.6%) of TAPVD occurred postoperative pulmonary venous obstruction.Conclusion Early in-tervention of neonates with critical CHD has achieved excellent results.The time of operation, surgical techniques, cardiopul-monary bypass techniques and perioperative management are key factors to improve the outcome of critical CHD .
2.Preliminary study of the accuracy of hepatic volume measurement by 3D + medical imaging visualization processing system

Objective To investigate the accuracy of liver volume measurement in patients with primary hepatocellular carcinoma by using the 3D + medical imaging visualization processing system. Methods Thirty patients who were clinically diagnosed as primary hepatocellular carcinoma and underwent surgical resection before preoperative assessment were selected from January 2016 to November 2016. Preoperative thin-slice CT images were transferred into the 3D + medical imaging visualization processing system for hepatic volume measurement, the predictive resection volume were measured and statistically analyzed with that of intraoperative specimen according to water measurement method. Results All patients had complete resection of tumor, the average volume of predictive resection calculated by the 3D software was (408.84 ± 177.07) ml, the average volume of intraoperative specimen measured by the water test was (399.95 ± 176.15) ml, the difference was not statistically significant (t=1.502, P=0.144), and the average percentage error was (6.21 ± 4.73) %. Pearson correlation analysis showed that there was significant correlation between them (r=0.983, P<0.05). Conclusion 3D+medical image visualization processing system is a simple, intuitive and accurate method for liver volume measurement, which can provide the basis for the accurate measurement of liver volume and the implementation of complex hepatectomy, and guide the precise hepatectomy.
3.Biological mechanism of periprosthetic osteolysis after total hip arthroplasty

Periprosthetic osteolysis and aseptic loosening after total hip arthroplasty are the common causes for revision surgery,which are initiated by wear particles released from the articular surfaces of prosthetic components.These components include ultra-high molecular weight polyethylene,titanium alloy,Al2O3,ZrO2 and polymethylmethacrylate.Wear particles activate macrophage via phagocytosis or pattern recognition receptor,resulting in release of inflammatory mediators such as macrophage colony-stimulating factor (M-CSF),macrophage chemotactic protein-1 (MCP-1),macrophage inhibitory protein-1 (MIP-1),IL-1β,IL-6 and TNF-α.Furthermore,wear particles stimulate M0 macrophage to polarize to M1 macrophage,promoting the release of inflammatory mediators.The periprosthetic osteolysis process is a cell/cytokine-mediated biological cascade induced by macrophage activation,involving the monocyte/macrophage cell line,such as macrophages,osteoclasts,and dendritic cells.The mesenchymal cells,including osteoblasts,osteocytes,fibroblasts and lymphocytes,lead to a deviation in the balance of osteoblast-osteoclast interactions within the basic multicellular unit,resulting in periprosthetic osteolysis.Fibrobalst,dendritic,and lymphocyte cells can enhance the recruitment and activation of macrophages by secreting MCP-1,MIP-1,and IL-8.Furthermore,the upregulated expression of RANKL,TNF-α,and IL-1 β facilitates the osteoclast differentiation from osteoclast precursor.TNF-α,IL-1 β together with wear particles can upregulate the expression of IL-6,MCP-1 and M-CSF of osteoblast and promote the recruitment and activation of macrophages.Nuclear transcriptional factor NF-κB plays a vital role in mediating periprosthetic inflammatory response and gene expression of bone metabolism.Different inflammatory factors can induce different expression of its downstream gene,ultimately,promoting or hindering the osteolysis process.The individual difference of periprosthetic osteolysis may also relate to single nucleotide polymorphism of IL-1RA,IL-6 and MMP-1 gene.
4.Correlation between Serum Levels of Melatonin TNF-A and EDSS Scores in Multiple Sclerosis Patients

Objective To study the serum levels of melatonin,tumor necrosis factor alpha (TNF-α) and interferon gamma (IFN-γ) in multiple sclerosis (MS) patients and the correlation with disability.Methods Forty patients with multiple sclerosis were collected as MS group and 30 healthy participants were collected as control group.Serum levels of melatonin and cytokines,including IFN-γ and TNF-,were detected in all participants by the enzyme-linked immunosorbent assay (ELISA) method;disability status of patients with MS was evaluated by EDSS scale.The relevant analysis between serum melatonin,TNF-α,IFN-γ levels and EDSS score in patients with MS was conducted.Results The concentration of serum melatonin in MS group was significantly lower than control group (P<0.01).TNF-α levels were higher than control group (P<0.05) and the difference was statistically significant between MS patients and control group.Among MS group and control group,no significant correlation with the serum levels of IFN-γ was seen.The serum melatonin level was inversely correlated with EDSS score in MS patients (r =-0.76,P<0.01),and positively correlated with TNF-α (r =0.83,P<0.01) and as compared to IFN-γ,no significant correlation was found (r =0.17,P>0.05).Conclusion The decrease of melatonin and increase of TNF-α can be a factor in the inflammatory reaction in patients with MS,and is closely related with dysfunction occurring in multiple sclerosis.Serum melatonin and TNF-α can be used as laboratory indicators to monitor clinical curative effect and evaluate prognosis of MS.
6.Long-term effect on the treatment of early chronic angle-closure glaucoma in two kinds of surgical method

?AlM: To evaluate the long - term effects of laser peripheral iridectomy ( LPl ) and trabeculectomy in treating early chronic angle-closure glaucoma.
?METHODS: Ninety-eight patients (102 eyes) with early chronic primary angle-closure glaucoma were randomly divided into two groups. Group A of 50 patients (54 eyes) was treated with LPl and group B of 48 patients (48 eyes) with trabeculectomy. After 3 - 8y of follow - up observation, comparison would be made from the perspectives of postoperative eyesight, intraocular pressure, anterior chamber angle, visual field and cup/disc ratio ( C/D) .
?RESULTS:ln group A, 24 eyes with eyesight declining, 22 eyes with theintraocular pressure>21mmHg (1mmHg=0. 133kPa), 21 eyes with chamber angle synechia >180o, 21 eyes with visual field narrowed, 21 eyes with C/D ratio enlarged. The results of group B for the same items were 10, 5, 4, 4, 4 eyes respectively. The comparative difference was statistically significant (P<0. 05).
?CONCLUSlON:Good effects will be achieved for early-stage chronic angle - closure glaucoma with surgical method. Trabeculectomy is obviously better than LPl for the long-term effects.
7.Adjuvant effect of flavored Siwu granules for phlegm and blood stasis mutual junction retinal vein occlusion

AIM: To evaluate the effect of flavored Siwu granules in the clinical adjuvant treatment for phlegm and blood stasis mutual junction retinal vein occlusion.
METHODS: Sixty patients (60 eyes) were randomly divided into treatment group and control group, both with retinal laser photocoagulation and intravitreal injection of triamcinolone acetonide, but the treatment group used flavored siwu granules. Vision, fundus change, fundus fluorescein angiography, clinical syndrome of traditional Chinese medicine ( TCM ), hemorheology examination and safety assessment were evaluated before and 8wk after treatment.
RESULTS: At 8wk after treatment, there were differences on vision improvement, retinal circulation time, whole blood low shear viscosity and total efficient rate of clinical syndrome of TCM, compared with control group(P<0. 05).
CONCLUSION: Flavored siwu granules adjuvant for phlegm and blood stasis mutual junction retinal vein occlusion can improve visual acuity and reduce the eye complications.

10.The effect of spironolactone on the expression of TGF-?_1 , PDGF-BB and ?-SMA in hepatic fibrosis tissues in rats

Objective To investigate the preventive effect of spironolactone on hepatic fibrosis. Methods Ninety SD rats were randomly divided into three groups. Control group consisted of 8 rats that , fed by normal food, were injected with peanut oil subcutaneously. Model group consisted of 42 rats whose liver fibrosis was induced by compound factors. Spiro nolactone-prevention group consisted of 40 rats that were given 100 mg?kg -1 spironolactone per day, by the same methods of making models as those of the model group.
