2.A Rare Case of Cerebral Sinovenous Thrombosis Associated with MTHFR A1298C and C677T Mutations.

Neonatal cerebral sinovenous thrombosis (CSVT) is a rare disease with severe neurological sequelae. Methylenetetrahydrofolate reductase (MTHFR) is a key enzyme in the folate cycle, and mutations in MTHFR are associated with vascular diseases. Here, we report the case of a newborn with MTHFR mutation-associated CSVT. Analysis of MTHFR in the patient detected heterozygous C677T (677CT) and A1298C (1298AC) mutations. Analysis of MTHFR in the patient's mother did not detect a C677T (677CC) mutation but detected a homozygous A1298C (1298CC) mutation. Our results suggest that the presence of heterozygous MTHFR C677T and A1298C mutations affect thrombophilic activity in the neonate, resulting in the development of refractory seizure and CSVT. Moreover, presence of the homozygous MTHFR A1298C mutation in the patient's mother, who did not show any symptoms associated with thrombophilic activity, and conditions during gestation may have affected the patient's condition.

3.A Rare Case of Rapidly Progressive Severe Encephalitis Associated with Epstein-Barr Virus Infection.

Epstein-Barr virus rarely causes encephalitis which has a benign outcome. About 90% of children have a benign clinical course without neurologic sequelae. However, 10% have residual persistent deficits and a mortality rate of up to 10% has also been reported. An 11-month-old boy was admitted after general weakness and poor oral intake. On day 7 of hospitalization, three vomiting episodes occurred and followed by a seizure. Brain T1-weighted magnetic resonance imaging (MRI) showed a hyperintensity with mild diffusion restriction in the cortex and subcortical white matter of the bilateral frontal, parietal, and occipital lobes. Analysis of a cerebrospinal fluid (CSF) sample revealed WBC count of 10 /microL (neutrophils 21%, lymphocytes 78%), red blood cell count of 19,000 /uL. CSF EBV polymerase chain reaction (PCR) was positive. Positive results were also obtained for serum EBV viral capsid antigen (VCA) IgM (>4 U/mL), IgG (>8 U/mL), EBV Ebstein Barr nuclear antigen (EBNA) IgG (>8 U/mL). Despite therapy with acyclovir, phenobarbital and steroids, a brain MRI conducted on day 34 showed extensive parenchymal volume atrophy and secondary ventricular dilatation, diffuse progressive signal change in the entire cerebrum and diffuse gyral enhancement in the entire cerebrum. The patient was discharged on day 129 and was transferred to other hospital. After 3month of discharge, the patient's mental status was still drowsy, both arms and legs showed rigidity, and deep tendon reflex were hyperactive. We report an 11-month-old child with rapidly progressive severe encephalitis associated with Epstein-Barr virus.

4.Evaluation of R Wave Amplitude Changes on Treadmill Exercise Testing.

For the evaluation of exercise induced R wave amplitude changes in lead V5 (big up tri, openR) and multiple leads(summation operatorR), we studied 20 normal subjects and 21 ischemic heart disease patients with positive exercise test. All underwent submaximal, multiple leads multistage treadmill exercise testing. 14 to 20 normal subjects(70%) there were decreased R wave amplitude in multiple leads on treadmill exercise testing. 16 of 21 patients(76%) there were increased R wave amplitude on treadmill exercise testing. In normal group, R wave amplitude between rest and exercise was significantly decreased in lead V5 and multiple leads. In patients grous, R wave amplitude between rest and exercise was significantly increased in lead V5 and multiple leads. It is suggested exercise induced R wave amplitude changes probably enhance the diagnostic reliability by ST segment changes on treadmill exercise testing.

Pilomatricoma is a benign adnexal tumor, differentiating towards elements of the hair matrix and shaft. The tumor typically presents as a solitary, deep-seated, firm nodule with overlying normal epidermis. Herein, we report three cases of pilomatricomas with the dermal bullae overlying tumors. Histopathological examination of the lesions showed marked lymphatic dilatations, edema, and attenuated collagen and elastic fibers above the pilomatricomas in the upper dermis. We diagnosed them as lymphangiectatic pilomatricomas.

7.Spermatic Cord Liposarcoma in a Man with Multiple Lipomas: A Case Report.

Lipomas are the most frequent soft tissue tumors. They tend to occur singly or in limited numbers, but about 5% of patients have multiple tumors. Liposarcoma of the spermatic cord is a rare tumor. Most of them are low-grade malignancies and treatment is radical orchiectomy with wide local excision. We report a case of spermatic cord liposarcoma with multiple lipomas elsewhere occurring in a 66-year-old man who had a history of lipomas. Radical inguinal orchiectomy and excision of lipoma in the thigh were performed as a treatment. The patient has survived 12 months without evidence of recurrence.

8.A Clinical Study of Dermatologic Diseases in Elderly Patients.

BACKGROUND: According to developments in socioeconomic status and medical science, the population of aged individuals has significantly increased. Therefore, this group needs a fundamental epidemiologic survey of dermatologic diseases. OBJECTIVE: The purpose of this study was to evaluate the prevalence of dermatologic diseases in the elderly and to compare with those of previous studies. METHODS: 729 out-patients over 65 years of age who had visited the Department of Dermatology, Yonsei University Wonju Christian Hospital from January, 2004 to December, 2010 were analyzed retrospectively. RESULTS: The elderly group over 65 years of age accounted for 9.7% of all new patients (729/7,532 cases). The ratio of males to females was 1 to 0.99 (366 cases : 363 cases). Patients between 65 and 69 years of age were the majority (36.2%). They visited the hospital most frequently in the summer (31.8%), especially August (11.0%). The most common disease was herpes zoster (10.5%), followed by fungal infection (10.3%), and allergic contact dermatitis (8.4%). Fungal infection was the most frequent disease in patients between 65 and 69 years old and herpes zoster was the most frequent disease between 70 and 79 years old. Scabies was the most frequent disease in patients over 80 years old. Pruritis was found in 9.7% and xerosis in 7.0%. Eczema was the most common disease group (in 22.0%) and the second mmost common was viral infection (12.3%). CONCLUSION: This study provides useful data on the prevalence and characteristics of dermatologic diseases in elderly Korean patients.

9.Urethral Hemangioma as a Cause of Postcoital Hematuria: A Case Report.

Urethral Hemangioma are rare benign lesion that may cause hematuria or bloody urethral discharge. These lesions tend to recur if the surgical excision is inadequate. A urethral hemangioma was found in a 42-year-old female complaining of hematuria at first voiding after sexual intercourse. Transurethral resection of the tumor was performed. She remains asymptomatic for 3 months after surgery.

10.Measurement of the Steering Torque in Cervical Cord Injured Persons Using BTE Work Simulator.

OBJECTIVE: To evaluate the standards of handicapped driver's ability test in Korea and available driving aids in cervical cord injured persons. METHOD: Forty-two cervical cord injured persons (C5~C8) were evaluated with BTE (Baltimore Therapeutic Equipment) work simulator. The isotonic turning torque was measured during clockwise and counter-clockwise turning. And isometric turning torque was measured at 12 directions. RESULTS: By the isotonic measurement, the turning torque increased in the lower level of injury. In the C6 tetraplegics, the counter-clockwise turning torque was significantly higher than clockwise. Among 25 persons with C5 or C6 tetraplegia, only 2 with C6 tetraplegia could operate the power steering with the lowest wheel turning torque (3.0 Nm). Only three persons (1 person with C7 tetraplegia, 2 persons with C8 tetraplegia) could pass the handicapped driver's ability test of Korea (9.4 Nm). By the isometric measurement, turning torque was lowest at left upper quadrant (10 to 12 o'clock direction) in clockwise rotation. CONCLUSION: Among the 42 cervical cord injured persons, only 3 persons with C7 and C8 tetraplegia could pass the handicapped driver's ability test of Korea. And among 25 C5 and C6 tetraplegia, only 2 persons with C6 tetraplegia could operate the power steering. It seems to be essential to modify the standards of handicapped drivers ability test and to evolve the driving aids for the C5 and C6 tetraplegics in Korea.
