1.Comparison of Emotional and Psychological Characteristics between Suicide Attempters and Non-Attempters in Depressed Patients : Using MMPI-2 Profiles.

OBJECTIVES: To examine emotional and psychological characteristics associated with suicide attempts in depressed patients. METHODS: A sample of 37 inpatients diagnosed with major depressive disorder or depressive disorder NOS was divided into two groups : lifetime suicide attempters(N=15 ; 40.54%), non-attempters(N=22 ; 59.46%). Beck Depression Scale(BDI), Beck Anxiety Scale(BAI), Hamilton Depression Rating Scale(HDRS), Hamilton Anxiety Rating Scale(HARS), and MMPI-2 were used to evaluate symptoms severity and psychological characteristics. RESULTS: Suicide attempters scored higher on the BDI though there were no group differences on the HDRS and on the both anxiety scales. Also they showed higher scores on the F, Fb, Pa, RC1, DEP, HEA, PK, AAS among MMPI-2 subscales. Our findings suggest that suicide attempters among depressed patients undergo more severe subjective distress and difficulties in adjustment than non-attempters. Also they were more hostile to others and showed lower trust. Lastly, they showed more somatic complaints and substance related problems. CONCLUSION: The present study showed that suicide attempters among depressed patients have distinct emotional and psychological characteristics. MMPI-2 would be helpful to assess suicidal risk of depressed patients.

2.Signaling Role of NADPH Oxidases in ROS-Dependent Host Cell Death Induced by Pathogenic Entamoeba histolytica

All living organisms are destined to die. Cells, the core of those living creatures, move toward the irresistible direction of death. The question of how to die is critical and is very interesting. There are various types of death in life, including natural death, accidental death, questionable death, suicide, and homicide. The mechanisms and molecules involved in cell death also differ depending on the type of death. The dysenteric amoeba, E. histolytica, designated by the German zoologist Fritz Schaudinn in 1903, has the meaning of tissue lysis; i.e., tissue destroying, in its name. It was initially thought that the amoebae lyse tissue very quickly leading to cell death called necrosis. However, advances in measuring cell death have allowed us to more clearly investigate the various forms of cell death induced by amoeba. Increasing evidence has shown that E. histolytica can cause host cell death through induction of various intracellular signaling pathways. Understanding of the mechanisms and signaling molecules involved in host cell death induced by amoeba can provide new insights on the tissue pathology and parasitism in human amoebiasis. In this review, we emphasized on the signaling role of NADPH oxidases in reactive oxygen species (ROS)-dependent cell death by pathogenic E. histolytica.
3.Paneth Cell-rich Carcinoma of the Stomach: A case report .

Paneth cell-rich carcinoma is essentially an adenocarcinoma with a predominance of Paneth cells. A 60-year-old male patient was admitted with a history of abdominal distension for several months. Endoscopic examination revealed a large ulceroinfiltrative tumor involving most of the areas of the stomach. The biopsy of the lesion confirmed poorly differentiated adenocarcinoma and total gastrectomy was followed. The submitted total stomach contained a diffuse infiltrative Borrmann type IV mass with ulceration, 8.0 3.5 cm, at the body along the lesser curvature. Microscopically, it was composed of Paneth cell differentiated cancer cells and poorly differentiated tubular adenocarcinoma cells. The Paneth cell differentiation was characterized by cytoplasmic coarse eosinophilic granules, which were PAS-positive and positive reaction for lysozyme. Electron microscopic examination showed numerous, spherical, electron-dense, homogeneous granules corresponding to those in Paneth cells as well as mucin granules in the signet-ring cells, and various intermediate forms in some cancer cells, which might be immature in the Paneth cell lineage.

6.Percutaneous catheter drainage of lung abscess.

From March 1987 to July 1989, six patients (five dadults and one child) with lung abscess (size, 5-13cm in diameter) were treated with percutaneous aspiration and drainage. In each case, the puncture was made where the wall of the abscess was in contact with the pleural surface. An 8 to 10 Fr catheter was inserted for drainage. Five of 6 had a dramatic clinical response within 24 hours of the drainage. Percutaneous drainage was successful with complete abscess resolution in four and partial resolution in one patient. No response was seen in the rest one. The duration of drainage ranged from 7 to 18 days (average, 15.5days) in successful cases. One case of the failure in drainage was due to persistent aspiration of the neurologically impaired patient. In one patient, the abscess resolved after drainage but recurred after inadvertent removal of the catheter 7 days after insertion. In two patients, concurrent pleural empyema was resolved completely by the drainage. Computed tomography provided anatomic details necessary for choosing the puncture site and avoiding a puncture of the lung parenchyma. Percutaneous catheter drainage is a safe and effective method for treating patients with lung abscess.

7.Couvade Syndrome of Pregnant Women's Spouses.

PURPOSE: This study was intended to investigate the types and seriousness of the couvade syndrome, pregnancy-related physical and psychological symptoms among expectant fathers whose spouses were pregnant. METHOD: The subject was consists of 100 expectant fathers at one hospital in Seoul, Korea. The pregnant women had not been diagnosed any medical complication. Data were analyzed by SPSS/PC program. RESULT: 1) The total mean score was 1.85: the mean score of perceived physical symptoms (1.87) revealed higher than the mean score of psychological symptoms (1.81). 2) With the respect to the general characteristics of subjects, there were statistically significant correlations between subject's level of education and couvade symptoms (r=-.209, p=.037), gestational age and couvade symptoms (r=-.227, p=.023), family total income and couvade symptoms (r=-.198, p=.048), perceived self health status and couvade symptoms (r=-.254, p=.011). 3) With the respect to the general characteristics of subjects, there were statistically significant differences in pregnant woman's age (t=1.363, p=.044),occupation of subject (F=3.594, p= .009), educational level of subject (t=3.506, p=.002), family total income (F=16.822, p= .000), perceived self health status (F=3.151, p=.047). CONCLUSION: Couvade syndrome is an issue for nurses who perform an important role in the care of pregnant women and their spouses.

8.A Case of Severe Air Leak Syndrome Treated with High - Frequency Oscillatory Ventilation ( HFOV ).

In air leak syndrome, a significant portion of the volume delivered during a positive pressure breath can be lost through the leak. HFOV can achieve adequate ventilation at lower peak and/or mean intrapulmonary pressure than conventional mechanical ventilation (CMV) and has been an effective treatment of already established air leak syndrome. We report a 1-day-old male infant with severe respiratory failure from pneumothorax and pneumomediastinum, who was refractory to CMV with chest tube drainage. HFOV was applied to this patient for 114 hours, and improvement of oxygenation and ventilation as well as significant reduction of pneumothorax followed.

Mid-arm circumference and mid-arm circumference/head circumference ratio(MAC/HC) were measured in 207 AGA(appropriate for gestational age) infants delivered at 26 to 42 weeks of gestation from January 1990 to December 1993 in Yeungnam University Hospital, Taegu, Korea. There were linear relationships between MACs and MAC/HC ratios and gestational age(MAC : y=03181x -2.2069, r=0.81, p<0.001 ; MAC/HC ratio : y=0.049x+0.1128, r=0.62, ; < 0.001). Using standard curves of MAC and MAC/HC ratio according to the gestational age, measurement of MAC or MAC/HC ratio can be a noninvasive, simple method to evaluate the intrauterine growth of newborn infants and the nutritional status of growing premature infants.
