2.Wandering Behavior in Korean Elders with Dementia Residing in Nursing Homes.

PURPOSE: This study investigated various factors relatied to wandering behavior of Korean elders with dementia (KED). METHODS: A sample of 160 ambulatory residents with dementia from 14 long term care facilities was used to examine demographic, individual, cognitive, physical health, and environmental characteristics by comparing wanderers (N=108) to nonwanders (N=52). Subjects were evaluated by Korean versions of the Mini-mental State Exam (K-MMSE), the Physical and Instrumental Activities of Daily Living (K-PIADL), and the Revised Algase Wandering Scale Nursing Home version (KRAWS-NH) along its six dimensions. Demographic and environmental data were also obtained. Independent sample ttests, Chi-square test, Fisher's Exact tests, and ANCOVAs were used to examine differences between wanderers and nonwanders. RESULTS: Wanderers were significantly (p<.05) older and had more limitations in K-PADL and K-IADL. The degree of overall wandering and certain features of wandering were significantly different (p<.05) by total number of residents in the facility, type of bedroom (i.e., "Ondol"), and color of bedroom and living-room walls (i.e., sky blue). CONCLUSIONS: Findings of this study may be useful in understanding wandering behavior of KEDs and thus developing more culturally specific management strategies.

3.Statistical Study on Congenital Heart Disease.

A Statistical observations were done on 587 pediatric patients who were diagnosed as having congenital heart disease by cardiac catheterization angiography and operation at Dong San Hospital Keimyung University, during the period of January 1982 to june 1987. The results were as follows 1) Out of 587 patients, 317 cases were male and 276 cases were female. There were female preponderance in atrial septal defect and patent ductus arteriosus, whille in male tetralogy of Fallot, pulmonary stenosis, and ventricular septal defect. 2) The incidence of congenital heart disease were as folliws;ventricular septal defect 35.7% tetralogy of Fallot 17.5%, patient ductus arteriousus 13.9%, arterial septal defect 7.5% and pulmonary stenosis 2.9%. 3) Most of the patients were visited hospital for the first time between the age of 2 and 10 years. 4) Assosiated anomalies of major congenital heart disease were as follows:the most commonly associated anomaly in ventricular septal defect were pulmonary stenosis and patent ductus arteriosus. Pulmonary stenosis was frequently associated with atrial septal defect. Right sided aortic arch and pentalogy were assosiated with tetralogy of Fallot. The mitral regurgitation was associated with patent ductus arteriosus mostly. 5) The overall surgical mortality was 6.8%. The mortality was high in tetralogy of Fallot(16.1%), transposition of great vessels(28.5%).

4.Measurements of Mid-arm Circumference(MAC) and Mid-arm Circumference/Head Circumference(MAC/HC) Ratio as Indices of Nutritional Status in Newborn Infants.

Mid-arm circumference and mid-arm circumference/head circumference ratio(MAC/HC) were measured in 207 AGA(appropriate for gestational age) infants delivered at 26 to 42 weeks of gestation from January 1990 to December 1993 in Yeungnam University Hospital, Taegu, Korea. There were linear relationships between MACs and MAC/HC ratios and gestational age(MAC : y=03181x -2.2069, r=0.81, p<0.001 ; MAC/HC ratio : y=0.049x+0.1128, r=0.62, ; < 0.001). Using standard curves of MAC and MAC/HC ratio according to the gestational age, measurement of MAC or MAC/HC ratio can be a noninvasive, simple method to evaluate the intrauterine growth of newborn infants and the nutritional status of growing premature infants.

5.Low bone mineral density in children and adolescents with cancer

Maximizing accumulation of bone mass during childhood and adolescence is essential to attaining optimal peak bone mass. Childhood cancer survivors (CCS) have lower bone mineral density (BMD) than the general population. Chemotherapeutic agents including steroids and radiotherapy can affect BMD. Cancer itself, hormonal insufficiency, a poor nutritional state, and a deficit of physical activities during or after treatment also influence BMD in CCS, resulting in failure to achieve appropriate peak bone mass. Low BMD in childhood and adolescence can lead to osteoporosis in adult life and complications such as bone pain, bone deformity, and fractures. Thus, BMD in CCS should be monitored with appropriate intervention. Adequate intake of calcium and vitamin D and an increase in physical activity are recommended. Timely supplements of hormones are needed in some cases. Some publications have reported that bisphosphonate therapies using pamidronate or alendronate were well tolerated in CCS and helped increase BMD.
6.Diagnostic Stewardship on the Extra-Analytical Phase to Secure Quality Assurance of Diagnostic Tests for Clostridioides difficile Infection

Background:
Clostridioides difficile infection (CDI) is the most essential cause of antimicrobial-related diarrhea in hospitalized patients. It is also considered as an important hospital-acquired infection. Adequately performing the laboratory diagnostic tests for CDI is critical for the control and treatment of CDI in healthcare facilities. This study focused on the management of the extra-analytical phase to secure quality assurance of the diagnostic tests for CDI.
Methods:
We analyzed the volume and pattern of requested C. difficile - related tests from patients in 2018. The culture-positive rate was compared with the stool quality. Finally, the clinical characteristics of antibiotic-related diarrhea were compared based on whether toxigenic C. difficile was isolated.
Results:
Culture plus toxin enzyme immunoassay was the most common pattern; however, it showed low positive rates for toxigenic C. difficile . The culture-positive rates were not different according to the Bristol stool form scale. The significant risk factors for the detection of toxigenic C. difficile in patients with antibiotic-associated diarrhea were inhibitor-combined penicillin or cephalosporin (odds ratio [OR], 5.060; 95% confidence interval [CI], 1.317–19.447; P =0.0183) and extended-spectrum cephalosporin (OR, 16.224; 95% CI, 3.166–83.134; P =0.0008).
Conclusions
The pre-pre-analytical errors are easy to overlook in routine laboratory procedures. Our findings could provide a good example of the management of the extra-analytical phase to secure quality assurance of the diagnostic tests for CDI.
7.An Epidemic of Aseptic Meningitis in Summer 1996 and Global Analysis and Comparision of It with 1993.

PURPOSE: In the Summer 1996, there was an outbreak of aseptic meningitis without sequele. 116 cases who were admitted to our Department during a six month period from May to October, 1996. Therefore, we report epidemic of aseptic meningitis in 1996 and analyze and compare it with a epidemic of aseptic meningitis in 1993. METHODS: We studies about clinical manefestation and laboratory finding by retrospective study on 116 patients diagnosed as aseptic meningitis who had been admitted at Department of Pediatrics, Kangnam Sacred Heart Hospital, College of Medicine, Hallym University. RESULTS: 1) Aseptic meningitis affected mostly children between the ages of 1 to 5 in 1996. Male-to-female ratio was 1.2 : 1 in aseptic meningitis in 1996. 2) The outbreak of aseptic meningitis was from middle to late August in 1996.3) Common chief complaints were fever, headache, vomiting, abdominal pain, upper respiratory tract infection, diarrhea, and irritibility in order. On physical examination, pharyngeal injection, neck stiffness, Kernig's sign were detected in decreasing frequency. 4) Values of WBC on peripheral blood were 5000-10,000/mm3 (60 cases, 51.7%) and values of ESR were more than 20mm/Hr (70 cases, 60.3%) and values of CRP positivity were 58 cases (50.0%) in 1996. In comparision with its in 1996, more than 10,000/mm3 in values of WBC and less than 20mm/hr in values of ESR and negativity in CRP values were detected mostly in 1993. 5) 176 cerebrospinal fluid studies were done. WBC count in cerebrospinal fluid were 6-75,000/mm3 (754+/-2351/mm3) and peaked on eighth days of onset of illness and decreased afterwards. Pleocytosis of cerebrospinal fluid were had of mean value 27.2% and peaked on first days of onset of aseptic meningitis. On virus culture, six cases were cultured in that one case of coxsakie virus, two cases of echo virus and six cases of nonspecific enterovirus and on enterovirus PCR, the positivity were 49 cases (41.0%). 6) 76 cases of who were diagnosed as aseptic meningitis were treated with antibiotics and 18 cases treated with mannitol for severe headache. None cases reported with seizure or decreased consciousness. The disease course mostly were three to seven days. CONCLUSIONS: We concluded that the causative agents of outbreak of aseptic meningitis in our hospital in 1996 were enterovirus and on virus culture, echo virus 9 and coxsackie virus A24 were cultured. Recently in our state, outbreak of aseptic meningitis epidemically in summer season and the causative viruses are cultured variably and various clinical symptoms and signs are manafested as well as.

8.Expression and localization of human papillomavirus type 16 E6 and E7 open reading frame proteins in human epidermal keratinocyte.

Over 60 different types of human papillomavirus (HPV) have been identified, and they are classified into high and low risk groups based on the risk for malignant progression of HPV associated lesions. HPVs belonging to a high risk group have been shown to express two major transforming proteins, E6 and E7. With respect to the transforming activity of these proteins, many investigators have reported the location of these proteins in the cell, but their results are still controversial. In the present study, HPV type 16 E6 or E7 open reading frame (ORF) proteins were expressed and localized in human epidermal keratinocytes (RHEK-1) using the vaccinia virus as an expression vector. Immunofluorescence detection using monoclonal antibodies against E6 or E7 ORF proteins revealed that E6 or E7 proteins of HPV type 16 were located in the cytoplasm of RHEK-1 cells. These results suggest that E6 and E7 proteins bind to the tumor suppressor counterparts, thereby preventing transport of these proteins into the nucleus. These antioncogene products that fail to be rapidly transported out of the cytosol may be degraded by certain proteases such as the ubiquitin dependent system. In this way, the precise function of antioncogene products in the regulation of cell growth could be destroyed, and abnormal cell growth could occur.

9.Motor Imagery and Action Observation.

In the rehabilitation, the real executive movement has been considered the main tool for motor recovery and motor learning. Many studies reported that active exercising creates a flow of sensory information which can change neuroplasticity. Recently the numerous studies report that both the imagery and the observation of a movement increase corticospinal excitability of primary motor cortex and activation of M1 area similar to the real execution of the movement. And the studies also report that these central activations occur in a specific manner. Now many people consider the imagery and observation can be a new therapeutic tool in rehabilitation especially for who can not execute a real motion at all due to neurologic deficit. We will review the articles reported the central nerve system changes by the imagery and observation of an action compared with the executive motion. And most of these experimental studies were based on the results of the healthy subjects, we discussed about the limitation of these therapeutic strategies according to the aging process and to the brain dysfunction.
10.A Case of Horizontal Partial Laryngectomy for Laryngeal Trauma.

Laryngeal trauma is rare compared to other head and neck traumas, but it occurs, it can be life threatening. As for treatment, a laryngeal fracture that involves displacement of cartilage or extensive injury requires appropriate surgical treatments. For severe laryngeal fractures, conservative management is usually preferred with placing a stent to prevent laryngeal stenosis. But the downside of placing stents in the larynx includes the risk of granulation and infection. In this report, the authors describe a 35-year-old patient, who was diagnosed with blunt laryngeal trauma and treated by Horizontal partial laryngectomy. The patient's post-operative breathing and voice were fair, and airway stenosis did not occur afterwards.
