1.The feasibility of shear wave elastography for diagnosing superficial benign soft tissue masses.

PURPOSE: The purpose of this study was to investigate the feasibility of shear wave ultrasound elastography for differentiating superficial benign soft tissue masses through a comparison of their shear moduli. METHODS: We retrospectively analyzed 48 masses from 46 patients from February 2014 to May 2016. Surgical excision, fine-needle aspiration, and clinical findings were used for the differential diagnosis. The ultrasonographic examinations were conducted by a single musculoskeletal radiologist, and the ultrasonographic findings were reviewed by two other radiologists who were blinded to the final diagnosis. Conventional ultrasonographic features and the median shear modulus were evaluated. We compared the median shear moduli of epidermoid cysts, ganglion cysts, and lipomatous tumors using the Kruskal-Wallis test. Additionally, the Mann-Whitney U test was used to compare two distinct groups. RESULTS: Significant differences were found in the median shear moduli of epidermoid cysts, ganglion cysts, and lipomatous tumors (23.7, 5.8, and 9.2 kPa, respectively; P=0.019). Epidermoid cysts showed a greater median shear modulus than ganglion cysts (P=0.014) and lipomatous tumors (P=0.049). CONCLUSION: Shear wave elastography may contribute to the differential diagnosis of superficial benign soft tissue masses through a direct quantitative analysis.

2.The Association between the Depression and the Perception of Obesity by Adolescent Girls.

BACKGROUND: This study was performed to find the association between the depression and perception of obesity by adolescent girls in Jung Eup City, Chollapukdo. The data were collected by questionnaire from July to October, 1996. RESULTS: 1. While the obesity rate of the girls by BMI criteria was 6.0%, 41.1% of girls perceived that they were obese. 2. Forty-eight percent of respondents tried weight control already, and as the method of weight control 33.3% tried exercise, 27.2% reduced food, 23.6% avoided snack before going to bed. 3. while 39.3% among those whose BMI was lower than 20 and 54.2% among those with BMI 20-24 tried weight control, only 5.1% among those who perceived their bodies as slim tried weight control. 4. By the analysis of self-rating depression scale (SDS) only 14.8% was normal without depression and the rest showed high SDS scale; 36.9% as mild, 38.5% as moderate, and 9.7% as severe state of depression. 5. While no statistically significant difference of SDS scores by the size of BMI was shown, significant different of SDS scores was shown among groups with different perception of their obesity and practice of weight control. 6. As a result of stepwise multiple regression analysis, dieting, perceived body image, and body mass index were significantly associated with depression of the adolescent girls. CONCLUSION: Adolescent girls who perceived themselves overweight regardless of actual weight were more likely to diet than other girls. And these characteristics had an influence on the depression of the adolescents. It is suggested that appropriate education should be required to prevent thoughtless weight control of the adolescents and to maintain mental health of the adolescents.

3.Comparison of Methods for Collecting Saliva for Pepsin Detection in Patients with Laryngopharyngeal Reflux.

BACKGROUND AND OBJECTIVES: To compare the simple spitting method and the Salivette® method of collecting saliva for detecting pepsin in patients with laryngopharyngeal reflux disease (LPRD). SUBJECTS AND METHOD: Thirty-two patients diagnosed with LPRD by 24 hour multichannel intraluminal impedance and pH monitoring were enrolled prospectively. The amounts of pepsin in saliva determined by the simple spitting method and the Salivette® method were compared. RESULTS: Simple spitting showed higher sensitivity, specificity, accuracy, positive predictive value and negative predictive value. There was no statistically significant difference between the amount of pepsin detected by simple spitting (10.07±11.68 ng/mL) versus that detected using the Salivette® method (7.09±7.27 ng/mL) (p=0.258). CONCLUSIONS: The simple spitting method has higher sensitivity, specificity and accuracy than the Salivette® method for detecting pepsin in patients with LPRD.

4.Standard digit-based versus 90° rotation technique for supraglottic airway device insertion: a meta-analysis of randomized controlled trials

Background:
Prompt insertion and placement of supraglottic airway (SGA) devices in the correct position are required to secure the airway. This meta-analysis was performed to validate the usefulness of the 90° rotation technique as compared with the standard digit-based technique for the insertion of SGA devices in anesthetized patients in terms of insertion success rate, insertion time, and postoperative complications.
Methods:
A literature search of PubMed, EMBASE, CENTRAL, CINAHL, Scopus, and Web of Science was conducted. Randomized controlled trials, without limitations on publication period, language, journal, or region, until July 2021, that compared the 90° rotation and the standard digit-based techniques for insertion of SGA devices in anesthetized patients were included.
Results:
The first-attempt (risk ratio [RR]: 1.16, 95% CI [1.09, 1.25], P < 0.001) and overall success rates (RR: 1.06, 95% CI [1.03, 1.09], P < 0.001) were significantly higher in the 90° rotation group. The insertion time was shorter in the 90° rotation group (mean difference: −4.42 s, 95% CI [−6.70, −2.15 s], P < 0.001). The incidences of postoperative sore throat (RR: 0.63, 95% CI [0.49, 0.83], P < 0.001) and blood staining (RR: 0.28, 95% CI [0.20, 0.39], P < 0.001) were lower in the 90° rotation group.
Conclusions
The use of the 90° rotation technique increases the success rate of SGA device insertion and decreases postoperative complications as compared with that of the standard digit-based technique in anesthetized patients.
5.Aspergillosis of Central Nervous System in Immunocompetent Host.

Aspergillosis of the central nervous system from sinonasal origin is rare in immunocompetent hosts. Due to the rarity of the cases in immunocompetent hosts, only few cases are reported in the previous literature. But, some cases reported diversity of clinical presentations in immunocompetent hosts. Aspergillosis of the central nervous system from sinonasal origin, bony invasion with only orbit or cranial base was more common than intracerebral aspergillosis in immunocompetent hosts. In this type of disease, although maxillary sinus is more commonly involved, sphenoid sinus and clivus are unusual sites for aspergillosis of central nervous system. Because of the anatomical position of sphenoid sinus, it is associated with poor prognosis with intracranial extension. We reported a case of the aspergillosis of central nervous system involving clivus in the immunocompetent host who was misdiagnosed as metastatic tumor or plasmacytoma due to the unusual location and host factor.

6.Multiple Symmetric Lipomatosis (Madelung's Disease) Presenting as Bilateral Huge Gynecomastia.

Multiple symmetric lipomatosis (MSL), or Madelung's disease, is a rare disease of unknown etiology. It is characterized by the presence of loose adipose tissue deposits localized in the cervical region and upper body. MSL presenting as bilateral huge gynecomastia is an extremely rare phenomenon. The present report describes a case of MSL in a 66-year-old man. The patients presented with bilateral breast bulging. He had a history of cigarette and alcohol use. His condition was treated with a bilateral nipple-sparing mastectomy. MSL can present as a form of gynecomastia, for its accurate diagnosis and proper treatment of MSL, increasing awareness of the clinical characteristics of the disease is required, especially amongst breast surgeons. Herein, we review the literature and discuss the clinical characteristics, pathology, and surgical treatment of MSL.

7.Transvginal Sonography in the Prediction of Preterm Labor.

OBJECTIVE: Preterm labor and delivery is probably the largest problem in modern perinatology. The early diagnosis of preterm labor is crucial for prevention of preterm birth. To predict the onset of preterm labor, we examined the diagnostic performance of serial cervical assessment by transvaginal sonography. METHODS: In this prospective study, we performed transvaginal sonography at approximately 30 and 34 weeks of gestation in women with singleton pregnancies. Cervical parameters evaluated included endocervical length, the presence of funneling, funnel length and funnel width. We then assessed the relation between cervical parameters and the risk of spontaneous preterm labor. RESULTS: We examined 258 women at approximately 30 weeks of gestation and 247 of these women again at approximately 34 weeks. Spontaneous preterm labor occurred in 9 of women examined at 30 weeks(Group 1) and in 13 at 34 weeks(Group 2). The endocervical length was normally distributed at 30 and 34 weeks(mean +/-SD, 38.26+/-6.82mm and 35.63+/-7.35mm, respectively). The endocervical length decreased significantly from 30weeks to 34 weeks(p=0.0001). Both groups showed significantly shorter endocervical length(p <0.05) and groupl more presence of funneling than group of term pregnancy(p<0.05). Receiver-operator characteristic curve and multiple logistic regression analyses indicated that endocervical length <-30mm at 30 weeks and <-25mm at 34 showed highest diagnostic index in predicting the onset of preterm labor(p=0.0001). Conclusions: Serial transvaginal ultrasound assessment of endocervical length during early third trimester is a useful predictor of preterm labor and delivery in low-risk patients.

8.A case of pheochromocytoma manifested diabetic ketoacidosis.

A 45-year-old woman was admitted with the diagnosis of diabetic ketoacidosis (DKA). During the hospital course, fluctuation of blood pressure accompanying with headache and palpitation was noted. The diagnosis of pheochromocytoma was proven by elevated urinary catecholamine and its metabolite, and confirmed histologically after right adrenalectomy. Mild glucose intolerance is known to be associated with pheochromocytoma in many cases. However, acute diabetic complication such as DKA has been reported in only 4 cases, mainly in Japanese. We think this is the first case of pheochromocytoma manifested DKA in Korea, so we report this with literature review.

9.Characteristics of Personality Factors in Patients with Posttraumatic Stress Disorder : Survivors of Daegu Subway Fire Accident.

OBJECTIVES: The purpose of this study was to assess the personality factors of posttraumatic stress disorder (PTSD) patients to investigate the contribution of personality to the development of PTSD. METHODS : The PTSD group and Non-PTSD group with no other sympton met DSM-IV Diagnostic Criteria for major psychiatric disorders were selected among 129 survivors of Daegu subway fire accident. Of 129 survivors of the accident, 95 survivors who had faithfully resonded to the psychiatric evaluation were assessed with Catell's 16 Personality Factor Questionnaire (16PF) and Spielberger's State-Trait Anxiety Inventory (STAI). Of 95 survivors, 52 survivors were diagnosed to have PTSD. The results were compared among PTSD group, non-PTSD group and normal group. RESULTS : In 16PF measurement, the scores of Anxiety and Tender-mindedness were significantly higher in PTSD group than normal and Non-PTSD groups. And the scores of Liberalism was significantly higher in PTSD group than normal group. The scores of Warmth, Stableness, Dominance, Surgency were significantly lower in PTSD and Non-PTSD than normal group. The scores of Guilty-proness and Anxiety were significantly higher in PTSD and Non-PTSD groups than normal group. In STAI measurement, PTSD group showed higher scores than Non-PTSD and normal group. CONCLUSION : These results suggested that patients with PTSD group were disposed to tension, anxiety, inpatience, fatigue, dependence, concern and compassion. It indicates that development of PTSD is associated with personality factors such as Anxiety and Tender-mindedness. Further prospective studies are necessary to clarify how personality factors were involved in the development of PTSD.

10.Protective Effect of Heat Shock Protein 70 Against Oxidative Stresses in Human Corneal Fibroblasts.

We evaluated DNA protection effect of heat shock protein (HSP) against cytotoxic effects of exogenous nitric oxide (NO) and reactive oxygen intermediate (ROI). Cultured human corneal fibroblasts were divided into 4 groups. Control (Group I) was not exposed to a sub-lethal heat treatment. Other 3 groups were exposed to 43 degrees C for 1 hr, then incubated at 37 degrees C during different duration (1, 6, 24 hr, Group II, III, IV, respectively). Expression pattern of HSP 70 was analyzed by Western blot. Cell viability was measured by MTT assay and the relationship between HSP 70 expression and DNA damage was examined by terminal deoxyribonucleotidyl transferase mediated dUTP-digoxigenin nick and labeling (TUNEL) stain and single cell gel electrophoresis. Expression pattern of HSP 70 was dependent on recovery times. Cell viability following heat treatment was significantly increased and the TUNEL positive cell number was decreased at 6 hr. In single cell gel electrophoresis, tail moments were increased in a dose-dependent manner by SNAP and X/XO. Following heat treatment, tail moments showed decreased significantly at 6 hr. These results suggest that induction of HSP 70 by sub-lethal heat treatment is closely related with cytoprotective effects against oxidative stresses in human corneal fibroblasts.
