1.A Fibroma with Cystic Change Developing in an Accessory Ovary: A Brief Case Report.

Accessory ovaries are rare entities defined as small portions of ovarian tissue situated near, and connected to, the normally placed ovary. Tumors arising in accessory ovaries are extremely rare. In particular, a fibroma arising from an accessory ovary has not been reported in the literature. We describe such a case with a fibroma developing in an accessory ovary. A 53-year-old multiparous woman presented with abdominal pain for 2 months. Pelvic computed tomography revealed 11.0x8.0x6.0 cm sized cystic mass with a thick septal wall in right adnexa. The preoperative diagnosis was a borderline ovarian tumor. Furthermore, a laparotomy showed a cystic mass connected to the right ovary by stalk. The bilateral eutopic ovaries were completely normal. Histologically, an accessory ovary was replaced by a fibroma accompanied by extensive cystic change.

3.Relationship of Nutritional Status at the Time of Admission to Length of Hospital Stay ( LOS ) and Mortality : A Prospective Study Based on Computerized Nutrition Screening.

This study was prospectively conducted to investigate any relationship of nutritional status at the time of admission to length of hospital stay and mortality. All patients admitted to the Asan Medical Center between October 13 and November 12, 1997 who met the study criteria were included in the study. Patients were classified as Not-at-risk, At-risk I or At-risk II based on the levels of serum albumin and total lymphocyte count in a computerized nutrition screening program. Sixty three percent of the patients were classified as Not-at-risk Group, 29% as At-risk Group I and 8% as At-risk Group II. Significant correlation was observed between nutritional status and LOS (P<0.01) as well as mortality rate (P<0.05). The more the patient had the nutritional risk factors, the longer the LOS and the higher the mortality rate were. Further studies have to be done in order to demonstrate cost-effectiveness of medical therapy for the malnourished hospitalized patients.

4.Imprint Cytology of Hepatic Angiomyolipoma: A Case Report.

Angiomyolipoma of the liver is a rare benign tumor that's composed of variable mixtures of adipose tissue, smooth muscle and thick-walled blood vessels. We report here on the imprint cytologic features of a hepatic angiomyolipoma in a 47-year-old man. The smears showed spindle and epithelioid tumor cells in clusters, trabeculae and single cells. The spindle cells had elongated, cigar-shaped nuclei with finely granular chromatin and fibrillary cytoplasm. The epithelioid cells had round nuclei with a moderate amount of cytoplasm. Any adipose tissue was not found. Immunohistochemically, both the spindle and epithelioid cells revealed cytoplasmic positivity for smooth muscle actin and HMB-45.

5.Mechanical properties and antibacterial effects of glass ionomer cement containing gallium phosphate glass (GPG)

This study investigated the mechanical properties and antibacterial activity of glass ionomer cement (GIC) containing 1 mol% or 5 mol% gallium phosphate glass (GPG) powder. GPG were added to conventional GIC powder in a range of 0~20 wt%. The specimens for compressive strength (4 mmØ×6 mm) and diametral tensile strength (6 mmØ×4 mm) were prepared by mixing with the cement liquid and kept in an water of 37±1℃. Mechanical properties were evaluated at an interval of 1 h, 24 h, and 7 days. Antibacterial activity of specimens against to S. mutans and S. sobrinus was evaluated using agar diffusion methods. Data were statistically analyzed by one-way ANOVA and Tukey HSD post-hoc test (p<0.05). Diametral tensile strength of GIC containing GPG were significantly higher than control GIC while compressive strength showed no statistical difference. There was an increasing tendency of antibacterial activity for GPG-GICs. Results indicated that GPG can be considered as potential reinforcing agent for increasing antibacterial and mechanical properties for conventional GIC.
6.Relationships between Sedation Knowledge and Nurse Practice Competences in Intensive Care Unit

PURPOSE: For this study an examination was done of relationships between intensive care unit (ICU) nurses' knowledge related to sedation and their clinical competencies in sedation practice. METHODS: Ninety one ICU nurses were recruited from two tertiary hospitals in South Korea. A self-report questionnaire was used to examine the levels of knowledge related to sedation and nursing practice competence based on Ajzen's theory of planned behavior. Descriptive statistics, independent t-tests, one-way ANOVA, and Pearson correlations were performed using the IBM SPSS 21.0 RESULTS: ICU nurses are more knowledgeable about general information on sedation rather than up-to-date information. Continuing education on sedation was related to difference in knowledge levels. However, the levels of knowledge were not related to competence in sedation practice. Instead, a positive attitude toward sedation practice was significantly related to the subjective norms; orders and goals, perceived behavioral control, practice of sedation, and intention to use. CONCLUSION: The results of this study provide fundamental information on levels of knowledge related to sedation practices of Korean ICU nurses. Continuing education should emphasize up-to-date information on sedation practice and need for positive attitudes which influence all other competencies towards sedation practices to achieve optical care of sedation in the ICU.

7.Fine Needle Aspiration Cytology of Hepatic Hydatid Cyst: A Case Study.

Hydatid cysts (echinococcosis) are caused by an infestation with larval tapeworms of the genus Echinococcus. The disease is extensively distributed worldwide, and it has been rarely reported in Korea. We describe the cytologic features of a case of hepatic hydatid cyst in a 28-year-old male. Computed tomography revealed a cystic mass in the right lobe of the liver. A right hemihepatectomy was performed. The aspirated fluid from the hepatic cystic mass was clear. The smears showed protoscolices, hooklets, and a laminated membrane.

Strumal carcinoid of ovary is an unusual form of monodermal ovarian teratoma with thyroid-like follicles admixed with typical carcinoid tumor pattern. Most of them arise in dermoid cyst or in mature solid teratoma. Recently we experienced a case of strumal carcinoid postoperatively and report this case with a brief review of the concerned literatures.

9.An Analysis on Prescribing Patterns of Antidepressants and Their Associated Factors in Lung Cancer Patients.

BACKGROUND: Depression is the leading cause of lowering the quality of life of cancer patients and lung cancer is the most likely to cause depression. It is necessary to find out depression-related factors in lung cancer patients. METHODS: The study was a retrospective cohort study using medical records, and was a non-equivalent comparison group design. It involved patients diagnosed of lung cancer at the Konkuk University Medical Center from January to December 2012. Between antidepressants prescription group and non prescription group, socio-demographic factors, clinical factors, treatment-related factors and other factors were analyzed statistically. RESULTS: Antidepressant prescription group consisted of 23 people and non-prescription group of 206 people. Prescription rate of quetiapine was the highest 47.8% (11/23), followed by escitalopram (43.5%, 10/23), amitryptyline and trazodone (30.4%, 7/23). The prescription group was prescribed with an average of 1.9 antidepressants. Antidepressants were prescribed after average of 248 days from lung cancer diagnosis and prescription period per patient was average 177.5 days. According to the result of univariate logistic regression analysis between 2 groups, factors such as number of outpatient visit, number of admission, days of hospitalization, sleep disorder, and comorbidity were found to be statistically significant (p < 0.05). However, According multivariate logistic regression analysis showed that number of admission, days of hospitalization and sleep disorder were statistically significant (p < 0.05) excluding comorbidity. CONCLUSION: About 10% of lung cancer patients had received a prescription for antidepressants after lung cancer diagnosis. A sleep disorder, number of hospitalization and length of stay were identified as factors influencing the prescribing antidepressants.

10.Relation between pH in the Trunk and Face: Truncal pH Can Be Easily Predicted from Facial pH.

BACKGROUND: The clinical symptoms of facial and truncal acne differ. Skin surface acidity (pH), which is affected by sebum secretions, reflects the different clinical characteristics of the face and trunk. However, no studies have been conducted on truncal sebum production and skin pH. OBJECTIVE: We evaluated the differences and relationship between pH values of the face and trunk. We also evaluated the relationship between pH and the quantity of sebum produced in the trunk. METHODS: A total of 35 female patients clinically diagnosed with truncal acne were included. We measured pH on the face and truncal area using the Skin-pH-Meter PH 905®. We measured truncal sebum secretions using the Sebumeter SM 815®. Statistical analysis was performed to evaluate the correlations and differences between pH and sebum. RESULTS: Facial pH was significantly higher than chest and back pH values. The correlation between pH on the trunk and the face was significant. We used linear regression equations to estimate truncal pH using only measured pH from the chin. There was no significant relationship between truncal sebum secretion and pH. CONCLUSION: This was the first study that evaluated the differences and correlations between facial and truncal pH. We found that facial pH can predict truncal pH. In addition, we conclude that differences in pH and sebum secretion between the face and trunk are one of the reasons for differences in acne symptom at those sites.
