2.Nurses' Experience: Impact of Hospital Information Systems on Nursing Care.

OBJECTIVE: The purposes of this study were to understand nurses' experience after the computerization of nursing care as a part of hospital information systems and to explore the change in nursing care time. METHODS: This study used a qualitative approach. The subjects were ten nurses by purposeful snowball sampling. In-depth interview was performed. All interviews were tape-recorded and transcribed verbatim. The data were analyzed using qualitative theme analysis. RESULTS: Nurses took into account the computerization as a 'benefit'. Their experiences after computerization were categorized into process and outcome impact. The emerged themes in nursing care process were 'accuracy', 'rapidness', and 'time saving in indirect nursing care'. Those in nursing care outcome included 'leisure', 'satisfaction', 'burden', and 'threatening privacy'. In addition, nurses' personal characteristics was the most important factor affecting the efficient use of saved time due to decrease of indirect nursing time to increase direct nursing care. CONCLUSION: Maximizing the positive impact of computerization requires redesign of nursing job description and work pattern, and education of efficient time management for individual nurse.

3.A Case Report of Mantle Cell Lymphoma in Leukemic Phase.

Recently, we experienced a patient with a B cell leukemia which could not be classified as chronic lymphocytic leukemia (CLL), prolymphocytic leukemia (PL) or follicular lymphoma in leukemic phase. He is a 58-year-old male and suffered from progressive lymphadenopathy at both inguinal and cervical areas. His blood film showed lymphocytosis (79%), and the cells were heterogeneous in size and shape. Twenty-five percent of the lymphocytes had nuclear irregularities or cleavages unlike the lymphocytes of CLL In which the cells often shows monomorphic features, small size with scanty cytoplasm and round nucleus. The bone marrow trephine biopsy specimen showed a diffuse pattern of infiltration of atypical Iymphoid cells. The lymph node histology showed atypical lymphoid colls proliferated as wide mantles around non-neoplastic appearing germinal centers. The immunophenotype of circulating lymphocytes in peripheral blood showed strong reactivity with CD5, CDl9 and CD20 without expression of CD2, CD3, CD7, CD10, CD22 and CD23. Although this case resembled CLL, the laboratory features showed major differences, notably in the Peripheral blood morphology, histology Patterns and the membrane Phenotype. By combining these data, we diagnosed this case as a leukemic phase of mantle cell lymphoma.

4.Effects of a Daily Life-Based Physical Activity Enhancement Program for Middle-Aged Women at Risk for Cardiovascular Disease

PURPOSE:
The purpose of this study was to examine the effects of a daily life-based physical activity enhancement program performed by middle-aged women at risk for cardiovascular disease.
METHODS:
This study used a randomized control group pretest-posttest design. Middle-aged women aged 45 to 64 were recruited from two outpatient cardiology departments, and randomly assigned to an experimental group (n=28) and a control group (n=30). For the experimental group, after providing one-on-one counseling and education, we provided customized text messages to motivate them in daily life. To monitor the practice of physical activity, they also used an exercise diary and mobile pedometer for 12 weeks. Subjects' physical activities (MET-min/week) were measured using the International Physical Activity Questionnaire (IPAQ). Their physiological data were obtained by blood tests using a portable analyzer, and the data were analyzed using the SPSS 21.0/WIN program.
RESULTS:
There were significant differences in exercise self-efficacy, health behavior, IPAQ score, body fat, body muscle, and fasting blood sugar between the two groups. However, there were no significant differences in total cholesterol, hemoglobin A1c, high-density lipoprotein cholesterol, and waist-to-hip ratio.
CONCLUSION
Strengthening physical activity in daily life without being limited by cost burden and time and space constraints. Therefore, it is essential to motivate middle-aged women at risk for cardiovascular disease to practice activities that are easily performed in their daily lives.

6.Barriers to the Operation of Patient Safety Incident Reporting Systems in Korean General Hospitals.

OBJECTIVES: This study aimed to explore the barriers to and factors facilitating the operation of patient safety incident reporting systems. METHODS: A qualitative study that used a methodological triangulation method was conducted. Participants were those who were involved in or responsible for managing incident reporting at hospitals, and they were recruited via a snowballing sampling method. Data were collected via interviews or emails from 42 nurses at 42 general hospitals. A qualitative content analysis was performed to derive the major themes related to barriers to and factors facilitating incident reporting. RESULTS: Participants suggested 96 barriers to incident reporting in their hospitals at the organizational and individual levels. Low reporting rates, especially for near misses, were the most commonly reported issue, followed by poorly designed incident reporting systems and a lack of adequate patient safety leadership by mid-level managers. To resolve and overcome these barriers, 104 recommendations were suggested. The high-priority recommendations included introducing reward systems; improving incident reporting systems, by for instance implementing a variety of reporting channels and ensuring reporter anonymity; and creating a strong safety culture. CONCLUSIONS: The barriers to and factors facilitating incident reporting include various organizational and individual factors. As an important way to address these challenging issues and to improve the incident reporting systems in hospitals, we suggest several feasible methods of doing so.

7.Review of Semantically Interoperable Electronic Health Records for Ubiquitous Healthcare.

In order to provide more effective and personalized healthcare services to patients and healthcare professionals, intelligent active knowledge management and reasoning systems with semantic interoperability are needed. Technological developments have changed ubiquitous healthcare making it more semantically interoperable and individual patient-based; however, there are also limitations to these methodologies. Based upon an extensive review of international literature, this paper describes two technological approaches to semantically interoperable electronic health records for ubiquitous healthcare data management: the ontology-based model and the information, or openEHR archetype model, and the link to standard terminologies such as SNOMED-CT.

8.An Analysis of Articles for International Marriage Immigrant Women Related to Health.

OBJECTIVES: This study was for analyzing the research about international marriage immigrant women and a trial to find the right direction for future research. METHODS: Sixty articles published from June, 2004 to June, 2009 were reviewed and analyzed according to the general characteristics, major of author, and theme of health domains. RESULTS: Most of them were master's thesis(71.7%) and journals(21.7%) and doctoral dissertation(6.7%) have been published mostly after thesis. Among 83.3% for quantitative research, descriptive(33.3%) and descriptive correlation(41.7%) methods were the most used and there were some qualitative researches(16.7%). The most frequently used data gathering method was questionnaire(81.7%) and the next was interview(16.7%). The major rates of the author were 61.7% for social welfare and 2.1% for nursing. The investigated variables in social health domain were adaptation(28.3%), and communication(1.7%). In psychological health domain, marriage satisfaction(16.7%), life satisfaction(11.7%), and depression(10.0%) were most researched. Utilization of medical center(5.0%) and health promotion behavior(1.7%) were investigated in physical health domain. CONCLUSIONS: Above this, most articles were researched about the adaptation of international marriage immigrant women. But the life in foreign countries can cause physical and psychosocial unhealthy conditions, so many-sided health related researches are supposed to be conducted for adaptation and prevention health problems of international marriage immigrant women.

9.Benefits of lateral cephalogram during landmark identification on posteroanterior cephalograms.

OBJECTIVE: Precise identification of landmarks on posteroanterior (PA) cephalograms is necessary when evaluating lateral problems such as facial asymmetry. The aim of the present study was to investigate whether the use of lateral (LA) cephalograms can reduce errors in landmark identification on PA cephalograms. METHODS: Five examiners identified 16 landmarks (Cg, N, ANS, GT, Me, RO, Lo, FM, Z, Or, Zyg, Cd, NC, Ms, M, and Ag) on 32 PA cephalograms with and without LA cephalograms at the same time. The positions of the landmarks were recorded and saved in the horizontal and vertical direction. The mean errors and standard deviation of landmarks location according to the use of LA cephalograms were compared for each landmark. RESULTS: Relatively small errors were found for ANS, Me, Ms, and Ag, while relatively large errors were found for N, GT, Z, Or, and Cd. No significant difference was found between the horizontal and vertical errors for Z and Or, while large vertical errors were found for N, GT, and Cd. The value of identification error was lower when the landmarks were identified using LA cephalograms. Statistically significant error reductions were found at N and Cd with LA cephalograms, especially in the vertical direction. CONCLUSIONS: The use of LA cephalograms during identification of landmarks on PA cephalograms could help reduce identification errors.

This study examined all the research published in Journal of Korean Academy of Nursing (JKAN) for last 30 years, from its beginning year to year 1999, based on an objective frame of evaluation. The purpose of this study is to reflect the trend of nursing research in Korea to date, and to provide an optimal direction for future research. The total number of 959 studies were analyzed with the following results. 1. Since 1990s, the number of non-degree based research has been larger than that of degree-based research compared to 1970s and 1980s. Both collaborative and funded research has been particularly increased in a gradual rate. 2. Research on nurses or nursing students has decreased while research on more diverse populations such as patients, patient families, or community-dwelling healthy clients has increased. 3. While essential nursing concepts such as human, nursing, and health have been consistently dealt as main research topics, research related to the concept of environ- ment has been insufficiently conducted. 4. It was remarkable that the quality of nursing research has been improved due to the acknowledgement of the importance of the empirical research method within the nursing community. 5. Qualitative research has appeared in the journal since the late 1990s, but it is still regarded to be in its novice stage. 6. Although the utilization of research findings for expanding the nursing body of knowledge has been wide in diverse specialties, most studies are limited to focus on concept development or evolution yet. also, in some of these research finding are utilization already considerably as to develop nursing interventions. Based on these findings, conclusions are extended to the following discussions: For the last 30 years, nursing research in Korea has shown an acute methodological development in both quantity and quality. However, there has been a lack of research on theory development although it is one of the ultimate goals in nursing. Further research should be empirical enough to be utilized in an actual nursing care context, and thus to be basis of developing culturally competent nursing theories in Korea.
