1.Study on dehydroepiandrosteron permeation from Transdermal Therapeutic Systems

To study in vitro usability of Transdermal Therapeutic Systems (TTS) dehydroepiandrosteron (DHEA) on pig's ear skin. Results showed that: there was correlation about DHEA permeation between human and pig's ear skin. DHEA permeation speed was linear proportional with square root of time and the DHEA content in TTS. DHEA permeation speed via human skin had linear correlation with permeation speed via pig's ear skin with angular co-efficient 0.74; this showed that pig's ear skin can be used for exploratory research on establishing formulation of transdermal drugs. Pig skins are available material and easy to collect with large amount. However, this result also showed that permeation ability of human skin is lower than pig’s ear skin

2.The significance of the magnetic resonance image after the performance of the percutaneous laser disc decompression (PLDD)

The magnetic resonance image is compulsory indication for patients with the suspected clinical symptoms of the disc dislocation and treated by the percutaneous laser disc decompression (PLDD). The magnetic resonance image should be taken care after PLDD in the indication and implementation. The parameters of the first performance must be the same as these of the second performance. An evaluation in the results of image must be based on the principles of technique which is the reduction of the pressure but not reduction of volume of ectopia

3.A study in the cutaneous conductivity at 12 couples of Yuan points in adults

120 healthy subjects were selected and divided into two groups to study the cutaneous conductivity at 12 couples of Yuan point, group 1 including 60 subjects aged between 21 to 25 (30 males, 30 females), group 2 including 60 subjects aged between 50 to 67 (30 males, 30 females). The results: there was no difference in cutaneous conductivity at 12 couple of Yuan point of the lest and right body in both groups of ages and sexes. In female, cutaneous conductivity at 12 couple of Yuan point were diferent depending on menstrual cycle, considerably lower in the latter haft of the cycle compared to the first half of the cycle. In the same age group, cutaneous conductivity at 12 couple of Yuan point in male was lower than that in female. In the same gender group, cutaneous conductivity at 12 couple of Yuan point of the group 1 was lower that of the group 2

4.Cutaneous protothecosis: A case report

Introduction:
Cutaneous protothecosis usually presents as pyoderma-like lesions or infiltrating papules and plaques
on the extensor side of the extremities. It can be misdiagnosed as eczema, pyoderma, or a fungal infection. Although
it has been isolated from a swimming pool, sewers and rivers in the Philippines, there has been no reported case of
cutaneous protothecosis in the country.
Case summary:
A 78-year-old Taiwanese male farmer visited the dermatology clinic due to a six-month history of a
large, pruritic erythematous plaque studded with papulopustules on his left forearm. A potassium hydroxide (KOH)
examination showed negative for hyphae or spores. And a skin biopsy showed morula-like bodies, which were
highlighted by the Periodic acid-Schiff stain.
Conclusion
We report a case of cutaneous protothecosis from Taiwan so Filipino dermatologists will be aware of the
clinical and histopathologic manifestations and management of cutaneous protothecosis.

5.Treatment of trigeminal neuralgia by percutaneous injection of hot saline into the trigeminal cavum (meckel fossa)

New method of treatment of trigeminal neuralgia by percutaneous injection of boiling saline into the cavum trigeminale in replacement of the former Frazier’s operation. 71 cases with aged of 20-80 in which 69 cases the territories of the 2nd and the 3rd division of the 5th nerve were involved. The technique of the procedure is described in detail. It did not require stereotactic facilities or X-rays control of the position of the needle during its introduction intothe Meckel’s cave through the Foramen Ovale. One or two injections of 2 ml of boiling saline were necessary for an instant relief of pain. The major complications, well accept by the patients were: hypoesthesia of the affected hemiface and motor hemiparalysis of muscles of mastication, but in a lot of patients they were improving or subsided with the passing of time

Microneedles have been developed rapidly in the field of transdermal administration in the past few decades. In recent years, the development of microelectronics technology has expanded the applications of microneedles by combining with microelectronic systems, especially in biological diagnosis and treatment. Different types of microneedles have been designed to extract blood and tissue fluids for detection, or as electrodes to directly detect blood sugar, melanoma and pH in real-time in vivo, both show good prospects for real-time detection applications. In this paper, we review the design of materials and structure of microelectronic-based microneedles, and discuss their advances in biological diagnosis.

8.In vitro transdermal permeation of main compositions in Baimai Ointment.

To establish a determination method for the contents of ammonium glycyrrhetate,nardosinone,and curcumin in transdermal receptor liquid of Baimai Ointment,and investigate the percutaneous permeability of Baimai Ointment and the effects of two kinds of penetration enhancers on percutaneous absorption of three components. The contents of ammonium glycyrrhetate,nardosinone,and curcumin in transdermal receptor liquid were determined by high pressure liquid chromatography( HPLC). The vertical modified Franz diffusion cell was used to perform a transdermal experiment in vitro with the abdominal skin of mice( treated and untreated). The transdermal receptor liquid was preferably used to investigate the transdermal absorption rule of the Baimai Ointment and the effect of the penetration enhancer. The results showed that the comprehensive solubility of PEG-ET-NS( 3 ∶3 ∶4) was best among three types of receptor liquid PG-ET-NS( 3 ∶3 ∶4),PEG-ET-NS( 3 ∶3 ∶4),ET-NS( 3 ∶7). PEG-ET-NS was used as the receptor liquid for in vitro transdermal experiments. The cumulative permeation area of ammonium glycyrrhetate,nardosinone and curcumin within 24 h was 5. 73,18. 99,0. 38 μg·cm~(-2)respectively. Taking QEFand ER as comprehensive evaluation indicators of permeation performance,the comprehensive penetration-promoting performance of ammonium glycyrrhizinate: 3% PEG 400-ethanol-normal saline ≈ 1. 19 times( 3%azone) = 1. 94 times( blank); comprehensive penetration-promoting performance of nardosinone: 3% PEG 400-ethanol-normal saline≈1. 28 times( 3% azone) = 1. 37 times( blank); the comprehensive penetration performance of curcumin: 3% PEG 400-ethanol-normal saline≈1. 77 times( 3% azone) ≈3. 42 times( blank). The comprehensive penetration enhancement properties of the two penetration enhancers were as follows: 3% PEG 400-ethanol-normal saline>3%azone>blank. The transdermal absorption curve of ammonium glycyrrhetate,nardosinone and curcumin in Baimai Ointment were consistent with the zero-order equation,indicating that the transdermal absorption process was irrelevant to the concentration of three components,and its was a diffusion process. This experiment provides reference for the study of ointment transdermal preparations.

9.Preparation of Huoluo Xiaoling gel plaster and its transdermal penetration in vitro.

Huoluo Xiaoling Dan is a classical prescription commonly used for blood circulation and pain relief in clinic with obvious effects. To make it directly treat lesion and improve the effect, this research optimized the preparation process of Huoluo Xiaoling gel paste and further evaluated its in vitro transdermal absorption performance, so as to provide a scientific basis for its development and utilization. Using primary viscosity, holding viscosity, and sensory score as evaluation indexes, the matrix amount of gel paste was determined by the single factor test and Box-Behnken response surface method. The ultra-performance liquid chromatography(UPLC) method was established to determine the content of eight active ingredients, including Danshensu, ferulic acid, salvianolic acid B, salvianolic acid A, ligustilide, tanshinone Ⅱ_A, 11-keto-β-boswellic(KBA), and 3-acetyl-11-keto-β-boswellic acid(AKBA). A mo-dified Franz diffusion cell method was used to evaluate and compare the absorption properties of the gel paste without volatile oil and with volatile oil microemulsion. The results showed that the optimal prescription for Huoluo Xiaoling gel paste matrix was NP700(1.35 g), glycerol(7.00 g), micropowder silica gel(1.25 g), sodium carboxymethyl cellulose(0.20 g), tartaric acid(0.06 g), and glyceryl aluminum(0.04 g). The mass fractions of eight active ingredients in the paste were successively 0.48, 0.014, 0.95, 0.39, 0.57, 0.055, 0.35, and 0.97 mg·g~(-1). The results of the in vitro transdermal absorption test showed that the addition of the volatile oil or the volatile oil microemulsion promoted the transdermal absorption of the active ingredients, and the law of drug penetration conformed to the zero equation or the Higuchi equation. The gel paste prepared by the optimal prescription has good appearance and adhesion, with no residue, and has the characteristics of skeletal slow-release preparation, which is easy to reduce the number of administration, la-ying a foundation for the development of new external dosage forms of Huoluo Xiaoling Dan.

Microneedle percutaneous immunization is achieved by puncturing the stratum corneum of the skin with microneedles so that the vaccine is efficiently recognized by antigen-presenting cells to induce a specific immune response. Due to the advantages of efficient induction of immune response, low pain and easy storage, transdermal immunization by microneedles has been widely used for immunization of various vaccines in recent years. This review summarizes the materials of microneedles, application for transcutaneous immunization, as well as the challenges that need to be addressed.
